The brown crab (Cancer pagurus) fishery in Ireland is one of the most important financially and socio-economically, with the species worth approximately €15m per year in the first half of the decade. Only mackerel (Scomber scombrus) and Dublin Bay prawn (Nephrops norvegicus) are of greater value. Despite this, very little research has been conducted to describe the stock structure of brown crab on a national scale. In this study a country-wide assessment of genetic population structure was carried out. Sampling was conducted from commercial fishing boats from 11/06 to 04/08 at seven sample sites representing the central Irish brown crab fisheries, with one sample site from the UK also included in the study. Six microsatellite markers, specifically developed for brown crab, were used to assess genetic diversity and estimate population differentiation parameters. Significant genetic structuring was found using F-statistics (Fst = 0.007) and exact tests, but not with Bayesian methods. Samples from the UK and Wexford were found to be genetically distinct from all other populations. Three northern populations from Malm Head and Stanton Bank were genetically similar with Fst estimates suggesting connectivity between them. Also, Stanton Bank, again on the basis of Fst estimates, appeared to be connected to populations down the west coast of Ireland, as far south as Kerry. Two Galway samples, one inside and one outside of Galway Bay, were genetically differentiated despite their close geographic proximity. It is hypothesised that a persistent northerly summer current could transport pelagic larvae from populations along the southwest and west coasts of Ireland towards Stanton Bank in the North, resulting in the apparent connectivity observed in this study.

Mussel populations on the Irish Atlantic coast comprise an interbreeding mixture of the blue mussel, Mytilus edulis (L.) and the Mediterranean mussel, Mytilus galloprovincialis (Lmk.). The occurrence of hybrid genotypes varies between sites but can be as high 80%. This study compares the reproductive cycle of M. edulis, M. galloprovincialis and their hybrids to determine if the extensive hybridisation observed at Irish Atlantic coast sites is linked to spawning synchrony between the two taxa. Mussels (40-45 mm size class) were collected monthly from a sheltered shore in Galway Bay from January to December 2005. Two major spawning events (March- June and September-October) were observed and gametogenesis took place throughout the year. The spawning cycles of the three taxa were largely overlapping. Small differences were observed in the timing of peak spawning which occurred in March and October in M. galloprovincialis and in May-June and September in M.edulis. Spawning of hybrid individuals was intermediate between the parental genotypes. Fecundity was slightly higher in M. galloprovincialis females compared to the other taxa (up to 30% difference, p<0.05). This apparent advantage is not shared by the sexes and is likely being offset by high numbers of hybrid genotypes releasing gametes during peak spawning of M. galloprovincialis. There was no evidence for increased mortality in hybrid males; sex ratios did not deviate from the 1:1 ratio. The results show that in this region of the hybrid zone the timing of reproduction does not present a barrier to gene flow between M. edulis and M. galloprovincialis. Nonetheless, small differences in the timing of peak spawning may increase the likelihood of conspecific fertilisation at certain times of the year. Hybrids outnumber the parental genotypes, undergo complete gametogenesis and show no evidence of depressed fitness (i.e. hybrids are reproductively competent suggesting a high degree of introgression.

FUNDAMENTO: Na insuficiência cardíaca, o teste ergométrico (TE) avalia capacidade funcional, um determinante de prognóstico. A cintilografia com I¹²³ MIBG mostra a ativação simpática cardíaca. OBJETIVO: Avaliar a associação entre as variáveis do TE e as alterações da cintilografia com I¹²³ MIBG. MÉTODOS: Foram submetidos 23 pacientes (FEVE < 45%) à cintilografia com I¹²³ MIBG e separados em: G1) taxa de Washout < 27%; G2) > 27%. Esses pacientes realizaram TE, onde foram analisadas: pressão arterial sistólica no pico do esforço (PASP), frequência cardíaca no pico do esforço (FCP), variação da pressão arterial sistólica intraesforço (Δ PAS), reserva cronotrópica e capacidade funcional em METs. Utilizaram-se para a análise estatística, o teste t de Student ou o teste U de Mann-Whitney, o coeficiente de Spearman e o coeficiente de regressão linear. RESULTADOS: A PASP (G1: 181,00 ± 28,01; G2: 153,27 ± 27,71, p = 0,027), a Δ PAS [G1: 64 (47,5-80,5); G2: 36 (25-47) mmHg, p = 0,015], a FCP (G1: 136,91 ± 19,66; G2: 118,45 ± 13,98 bpm, p = 0,018), a reserva cronotrópica (G1: 70,42 ± 17,94; G2: 49,47 ± 14,89%, p = 0,006) e a capacidade funcional [G1: 8,37 (6,47-10,27); G2: 4,42 (2,46-6,38) METs, p = 0,003] foram menores no G2. Houve correlação negativa entre taxa de Washout com PASP (r = -0,505, p = 0,014), Δ PAS (r = -0,493, p = 0,017) e capacidade funcional (r = -0,646, p = 0,001). Após regressão linear, PASP (r = -0,422, p = 0,016) e capacidade funcional (r = -0,804, p = 0,004) foram associadas com taxa de Washout. CONCLUSÃO: Em pacientes com insuficiência cardíaca, PASP e capacidade funcional foram as varáveis mais associadas com a taxa de Washout.

FUNDAMENTO: A síndrome metabólica representa um conjunto de fatores de risco, associados a doenças cardiovasculares e a diabete melito tipo 2. O tratamento inclui mudanças no estilo de vida, dieta, atividade física e medicamentos. A adesão do paciente é crucial ao tratamento da doença. Objetivo: Avaliar dois modelos de intervenção dietoterápica e a relação com a adesão ao tratamento e o impacto na melhora clínica de pacientes com síndrome metabólica. MÉTODOS: Ensaio clínico randomizado, com duração de quatro meses. Os pacientes foram randomizados em grupos intervenção e controle. Todos seguiram dieta específica por quatro meses. O grupo Intervenção recebeu pacote de intervenção, com dieta individualizada, manual de orientação, aconselhamento via telefone e material educativo. Foram realizadas anamnese nutricional, avaliação antropométrica, avaliação dietética, orientação dietética individualizada e exames bioquímicos. RESULTADOS: Os pacientes que chegaram mais motivados foram aqueles que tiverem maior redução nos valores do índice de massa corporal (p < 0,001), que reduziu de 31,7 kg/m² (DP ± 3,9) para 30,9 kg/m² (DP ± 3,8), na circunferência abdominal a redução foi de 108,1 cm (DP ± 9,8) para 105,9 cm (DP ± 9,5). As associações estatisticamente significativas se deram nas correlações entre índice de massa corporal, glicemia e triglicerídeos, redução do consumo de leite integral (p = 0,002), aumento no consumo de cereais integrais (p = 0,008) e de leite desnatado (p = 0,010), e entre o aumento no consumo de vegetais e a redução dos triglicerídeos. CONCLUSÃO: Ambos os grupos mostraram melhora significativa nos parâmetros clínicos, que foi significativamente associada a motivação prévia. Os pacientes que chegaram mais motivados foram aqueles que responderam melhor ao tratamento.

FUNDAMENTO: A Insuficiência Cardíaca (IC) é uma síndrome complexa, marcada pela intolerância ao esforço e perda da capacidade funcional. OBJETIVO: Avaliar a capacidade funcional dos pacientes com IC suplementados com creatina. MÉTODOS: Estudo prospectivo, randomizado, duplo cego. Foram randomizados em dois grupos 33 pacientes com IC, com idade > 18 anos, sexo masculino, classes funcionais II a IV. O grupo experimental (G CRE, n = 17) foi suplementado com creatina com dose de 5 g por dia durante seis meses. E o grupo placebo (G PLA, n = 16) recebeu 5 g de maltodextrina por dia durante o mesmo período de tempo. Ambos os grupos foram submetidos a avaliação da capacidade funcional por meio da ergoespirometria e teste de caminhada de seis minutos (TC6) pré e pós-intervenção. O modelo estatístico Ancova e a correlação de Pearson foram utilizados na análise dos grupos e nas formas de tratamento. RESULTADOS: Das variáveis avaliadas na ergoespirometria, o volume de oxigênio pico (VO2 pico), limiar anaeróbio (LA) e o pulso de oxigênio (pulso de O2) não apresentaram diferenças significativas entre os grupos (P>0,05). No TC6 não houve diferença significativa na distância percorrida. CONCLUSÃO: A suplementação de creatina em pacientes com IC não promoveu melhora significativa na capacidade funcional.

Background:Isolated congenital atrioventricular block (CAVB) is a rare condition with multiple clinical outcomes. Ventricular remodeling can occur in approximately 10% of the patients after pacemaker (PM) implantation.Objectives:To assess the functional capacity of children and young adults with isolated CAVB and chronic pacing of the right ventricle (RV) and evaluate its correlation with predictors of ventricular remodeling.Methods:This cross-sectional study used a cohort of patients with isolated CAVB and RV pacing for over a year. The subjects underwent clinical and echocardiographic evaluation. Functional capacity was assessed using the six-minute walk test. Chi-square test, Fisher's exact test, and Pearson correlation coefficient were used, considering a significance level of 5%.Results:A total of 61 individuals were evaluated between March 2010 and December 2013, of which 67.2% were women, aged between 7 and 41 years, who were using PMs for 13.5 ± 6.3 years. The percentage of ventricular pacing was 97.9 ± 4.1%, and the duration of the paced QRS complex was 153.7 ± 19.1 ms. Majority of the subjects (95.1%) were asymptomatic and did not use any medication. The mean distance walked was 546.9 ± 76.2 meters and was strongly correlated with the predicted distance (r = 0.907, p = 0.001) but not with risk factors for ventricular remodeling. Abstract Hypertension affects 25% of the world's population and is considered a risk factor for cardiovascular disorders and other diseases. The aim of this study was to examine the evidence regarding the acute effect of exercise on blood pressure (BP) using meta-analytic measures. Sixty-five studies were compared using effect sizes (ES), and heterogeneity and Z tests to determine whether the ES were different from zero. The mean corrected global ES for exercise conditions were -0.56 (-4.80 mmHg) for systolic BP (sBP) and -0.44 (-3.19 mmHg) for diastolic BP (dBP; z ≠ 0 for all; p < 0.05). The reduction in BP was significant regardless of the participant's initial BP level, gender, physical activity level, antihypertensive drug intake, type of BP measurement, time of day in which the BP was measured, type of exercise performed, and exercise training program (p < 0.05 for all). ANOVA tests revealed that BP reductions were greater if participants were males, not receiving antihypertensive medication, physically active, and if the exercise performed was jogging. A significant inverse correlation was found between age and BP ES, body mass index (BMI) and sBP ES, duration of the exercise's session and sBP ES, and between the number of sets performed in the resistance exercise program and sBP ES (p < 0.05). Regardless of the characteristics of the participants and exercise, there was a reduction in BP in the hours following an exercise session. However, the hypotensive effect was greater when the exercise was performed as a preventive strategy in those physically active and without antihypertensive medication.
