Objetivo: determinar en qué etapas del curso de vida previo a los setenta ±os se acortan las expectativas de vida en salud. Método: análisis longitudinal retrospectivo. La población a estudio es una cohorte de 1286 individuos de entre 70 y 74 ±os de edad que viven en áreas metropolitanas no institucionalizados. La discapacidad se mide mediante el desarrollo de las actividades instrumentales de la vida diaria (AIVD) y las actividades básicas de la vida diaria (ABVD). Se calculan las esperanzas de vida libre de discapacidad y la probabilidades de supervivencia mediante Kaplan-Meier Resultados: La incidencia de discapacidad básica se incrementó a partir del tramo de 50 a 54 ±os acelerándose progresivamente hasta los 65-69 ±os donde la probabilidad de padecer discapacidad instrumental fue ligeramente superior en hombres que en mujeres (0,23 en hombres versus 0,19 en mujeres). La supervivencia sin discapacidad de las mujeres fue peor que la de los hombres tanto para cualquier tipo de discapacidad (LogRank = 5,80; p = 0,016) como para la discapacidad básica (LogRank = 4,315; p = 0,038). Conclusiones: se pone de manifiesto el importante peso que para la autonomía de la población masculina de estas edades tiene la falta de habilidad instrumental para cuestiones domésticas. Además, las mujeres ostentan un peor pronóstico de supervivencia sin discapacidad tanto para cualquier tipo de discapacidad como para la discapacidad básica. Para ambos sexos, desde los 40 hasta los 60 ±os de edad, hay un ligero descenso de la supervivencia sin discapacidad. A partir de esta edad, el descenso se acelera notablemente.

BACKGROUND: Worldwide data for cancer survival are scarce. We aimed to initiate worldwide surveillance of cancer survival by central analysis of population-based registry data, as a metric of the effectiveness of health systems, and to inform global policy on cancer control. METHODS: Individual tumour records were submitted by 279 population-based cancer registries in 67 countries for 25·7 million adults (age 15-99 years) and 75 000 children (age 0-14 years) diagnosed with cancer during 1995-2009 and followed up to Dec 31, 2009, or later. We looked at cancers of the stomach, colon, rectum, liver, lung, breast (women), cervix, ovary, and prostate in adults, and adult and childhood leukaemia. Standardised quality control procedures were applied; errors were corrected by the registry concerned. We estimated 5-year net survival, adjusted for background mortality in every country or region by age (single year), sex, and calendar year, and by race or ethnic origin in some countries. Estimates were age-standardised with the International Cancer Survival Standard weights. FINDINGS: 5-year survival from colon, rectal, and breast cancers has increased steadily in most developed countries. For patients diagnosed during 2005-09, survival for colon and rectal cancer reached 60% or more in 22 countries around the world; for breast cancer, 5-year survival rose to 85% or higher in 17 countries worldwide. Liver and lung cancer remain lethal in all nations: for both cancers, 5-year survival is below 20% everywhere in Europe, in the range 15-19% in North America, and as low as 7-9% in Mongolia and Thailand. Striking rises in 5-year survival from prostate cancer have occurred in many countries: survival rose by 10-20% between 1995-99 and 2005-09 in 22 countries in South America, Asia, and Europe, but survival still varies widely around the world, from less than 60% in Bulgaria and Thailand to 95% or more in Brazil, Puerto Rico, and the USA. For cervical cancer, national estimates of 5-year survival range from less than 50% to more than 70%; regional variations are much wider, and improvements between 1995-99 and 2005-09 have generally been slight. For women diagnosed with ovarian cancer in 2005-09, 5-year survival was 40% or higher only in Ecuador, the USA, and 17 countries in Asia and Europe. 5-year survival for stomach cancer in 2005-09 was high (54-58%) in Japan and South Korea, compared with less than 40% in other countries. By contrast, 5-year survival from adult leukaemia in Japan and South Korea (18-23%) is lower than in most other countries. 5-year survival from childhood acute lymphoblastic leukaemia is less than 60% in several countries, but as high as 90% in Canada and four European countries, which suggests major deficiencies in the management of a largely curable disease. INTERPRETATION: International comparison of survival trends reveals very wide differences that are likely to be attributable to differences in access to early diagnosis and optimum treatment. Continuous worldwide surveillance of cancer survival should become an indispensable source of information for cancer patients and researchers and a stimulus for politicians to improve health policy and health-care systems. Avaliaram-se os efeitos da aspersão hidrotérmica e da radi§Ã£o UV-C no controle pós-colheita da podridão olho-de-boi (POB) em m§Ã£s 'Fuji', após um e oito meses de armazenamento, e 'Gala', após cinco meses de armazenamento, ambas sob condição de atmosfera controlada (AC). Esses frutos foram inoculados ou mantidos com infe§Ã£o natural de Cryptosporiopsis perennans. As m§Ã£s 'Fuji' foram submetidas aos seguintes tratamentos, aplicados em uma linha comercial de seleção: sem tratamento (testemunha); aspersão hidrotérmica (água a 50ºC por 12 segundos); radi§Ã£o UV-C (0,0069 kJ m-2); e aspersão hidrotérmica + radi§Ã£o UV-C. As m§Ã£s 'Gala' também foram submetidas a estes tratamentos utilizados em 'Fuji', exceto ao tratamento com aspersão hidrotérmica + radi§Ã£o UV-C. Após os tratamentos, as m§Ã£s foram incubadas a 22ºC por 15 dias e avaliadas quanto à incidência da doença. Nas m§Ã£s 'Fuji', os tratamentos de aspersão hidrotérmica e/ou radi§Ã£o UV-C reduziram a incidência da POB nos frutos inoculados e com infe§Ã£o natural, proporcionando controle superior a 56% e 54%, em rel§Ã£o à testemunha, respectivamente. Em m§Ã£s 'Gala' inoculadas, os tratamentos com aspersão hidrotérmica e radi§Ã£o UV-C também reduziram o número de unidades formadoras de colônias (UFC) nos frutos, com controle superior a 70%, e a incidência da POB, com controle superior a 69% em rel§Ã£o à testemunha. Em m§Ã£s 'Gala', com infe§Ã£o natural, estes tratamentos apresentaram controle da POB superior a 85% em rel§Ã£o à testemunha. Os resultados obtidos mostram que os tratamentos com aspersão hidrotérmica e/ou radi§Ã£o UV-C reduzem a incidência da POB em m§Ã£s 'Fuji' e 'Gala', em linha comercial de seleção. Todavia, o uso da radi§Ã£o UV-C, em ambas as cultivares, foi o tratamento que apresentou maior benefício e retorno econômico.

Chicken is the most widely consumed meat all over the world due to chickens being easy to rear, their fast growth rate and the meat having good nutritional characteristics. The main objective of this paper was to study the effects of dietary fatty by-products in low, medium and high levels of oxidized lipids and trans fatty acids (TFAs) on the contents of cholesterol and oxycholesterols in meat, liver, and plasma of chickens. A palm fatty acid distillate, before and after hydrogenation, and a sunflower-olive oil blend (70/30, v/v) before and after use in a commercial frying process were used in feeding trials after adding 6% of the fats to the feeds. Highly oxidized lipid and TFA feeds significantly increased the contents of cholesterol and oxycholesterols in all tissues of chicken (0.01 < p <= 0.05). The contents of oxycholesterols in chicken meat, liver and plasma obtained from TFA feeding trials varied between 17 and 48 μg/100 g in meat, 19-42 μg/100 g in liver and 105-126 μg/dL in plasma. In contrast, in the oxidized lipid feeding trials, oxycholesterols varied between 13 and 75 μg/100 g in meat, 30-58 μg/100 g in liver and 66-209 μg/dL in plasma. Meat from chickens fed with feeds containing high levels of TFAs or oxidized lipids may contribute to higher ingestion of cholesterol and oxycholesterols by humans.

IMPORTANCE: Associations between subclinical thyroid dysfunction and fractures are unclear and clinical trials are lacking. OBJECTIVE: To assess the association of subclinical thyroid dysfunction with hip, nonspine, spine, or any fractures. DATA SOURCES AND STUDY SELECTION: The databases of MEDLINE and EMBASE (inception to March 26, 2015) were searched without language restrictions for prospective cohort studies with thyroid function data and subsequent fractures. DATA EXTRACTION: Individual participant data were obtained from 13 prospective cohorts in the United States, Europe, Australia, and Japan. Levels of thyroid function were defined as euthyroidism (thyroid-stimulating hormone [TSH], 0.45-4.49 mIU/L), subclinical hyperthyroidism (TSH <0.45 mIU/L), and subclinical hypothyroidism (TSH ≥4.50-19.99 mIU/L) with normal thyroxine concentrations. MAIN OUTCOME AND MEASURES: The primary outcome was hip fracture. Any fractures, nonspine fractures, and clinical spine fractures were secondary outcomes. RESULTS: Among 70,298 participants, 4092 (5.8%) had subclinical hypothyroidism and 2219 (3.2%) had subclinical hyperthyroidism. During 762,401 person-years of follow-up, hip fracture occurred in 2975 participants (4.6%; 12 studies), any fracture in 2528 participants (9.0%; 8 studies), nonspine fracture in 2018 participants (8.4%; 8 studies), and spine fracture in 296 participants (1.3%; 6 studies). In age- and sex-adjusted analyses, the hazard ratio (HR) for subclinical hyperthyroidism vs euthyroidism was 1.36 for hip fracture (95% CI, 1.13-1.64; 146 events in 2082 participants vs 2534 in 56,471); for any fracture, HR was 1.28 (95% CI, 1.06-1.53; 121 events in 888 participants vs 2203 in 25,901); for nonspine fracture, HR was 1.16 (95% CI, 0.95-1.41; 107 events in 946 participants vs 1745 in 21,722); and for spine fracture, HR was 1.51 (95% CI, 0.93-2.45; 17 events in 732 participants vs 255 in 20,328). Lower TSH was associated with higher fracture rates: for TSH of less than 0.10 mIU/L, HR was 1.61 for hip fracture (95% CI, 1.21-2.15; 47 events in 510 participants); for any fracture, HR was 1.98 (95% CI, 1.41-2.78; 44 events in 212 participants); for nonspine fracture, HR was 1.61 (95% CI, 0.96-2.71; 32 events in 185 participants); and for spine fracture, HR was 3.57 (95% CI, 1.88-6.78; 8 events in 162 participants). Risks were similar after adjustment for other fracture risk factors. Endogenous subclinical hyperthyroidism (excluding thyroid medication users) was associated with HRs of 1.52 (95% CI, 1.19-1.93) for hip fracture, 1.42 (95% CI, 1.16-1.74) for any fracture, and 1.74 (95% CI, 1.01-2.99) for spine fracture. No association was found between subclinical hypothyroidism and fracture risk. CONCLUSIONS AND RELEVANCE: Subclinical hyperthyroidism was associated with an increased risk of hip and other fractures, particularly among those with TSH levels of less than 0.10 mIU/L and those with endogenous subclinical hyperthyroidism. Further study is needed to determine whether treating subclinical hyperthyroidism can prevent fractures.
