第二次大戦時に大量のユダヤ人避難民を受け入れたイスラエルは、1946年の建国時には共産主義的な社会改革思想に基づくキブツ運動などの左翼的思潮を国家建設の支柱にしていたが、その後の政治過程のなかで一貫して右傾化の方向をたどり、現在では国際的にみても最も保守的な軍事主義的思想傾向が国民のあいだで広く共有され、国内のアラブ系住民の経済的従属が永く固定化するに至った。現在のイスラエル国家を思想的にも実体経済的にも支えている基本的な理念は、建国時のそれとは全く対極的な新保守主義とグローバル化された「新自由主義」的な資本主義であり、それは当然ながら国内における安価な労働力としてのアラブ系住民の存在を所与の前提条件として組み込んでいる。これは具体的にどのような経緯によるものであり、またイスラエル国家のどのような性格から導き出されるものなのか。本論稿では政治的シオニズムがイスラエル建国後から現在までにたどってきた思想的な系譜を改めて確認し、現在のイスラエルが国際的に置かれている特異な立場とその背後にある諸要因を説明する。

The present work covers the first validation efforts of the EVA Tracking System for the assessment of minimally invasive surgery (MIS) psychomotor skills. Instrument movements were recorded for 42 surgeons (4 expert, 22 residents, 16 novice medical students) and analyzed for a box trainer peg transfer task. Construct validation was established for 7/9 motion analysis parameters (MAPs). Concurrent validation was determined for 8/9 MAPs against the TrEndo Tracking System. Finally, automatic determination of surgical proficiency based on the MAPs was sought by 3 different approaches to supervised classification (LDA, SVM, ANFIS), with accuracy results of 61.9%, 83.3% and 80.9% respectively. Results not only reflect on the validation of EVA for skills? assessment, but also on the relevance of motion analysis of instruments in the determination of surgical competence.

Residents learning nontechnical skills in Europe face two problems: (1) the difficulty to fit learning time in their overloaded schedules; and (2) the lack of standard pedagogical models for all countries. Online video-based repositories such as WeBSurg or WebOP provide ubiquitous access to surgical contents. However, their pedagogical facets have not been fully exploited and they are often seen as quick-reference repositories rather than full e-learning alternatives. We present a new pedagogically-supported Technology Enhanced Learning (TEL) solution, MISTELA, designed by surgeons, pedagogical experts and engineers. MISTELA aims at building a common European pedagogical model supported by ICT technologies and elearning. The solution proposes a pedagogical model based on a framework for pedagogically-informed design of e-learning platforms. It is composed of (1) an authoring tool for editing and augmenting videos; (2) a media asset management system; and (3) a virtual learning environment. Support of the European Association for Endoscopic Surgery (EAES) and validation of the solution, will help to determine its full potential.

Este estudo teve como finalidade avaliar cefalometricamente, por meio de telerradiografias em norma lateral, as alterações dento-esqueléticas em pacientes Classe III submetidos a tratamento ortodôntico-cirúrgico. A amostra experimental constituiu-se de 16 pacientes Brasileiros, dos sexos masculino e feminino, na faixa etária pré-cirúrgica média de 21 anos e 11 meses, apresentando má oclusão de Classe III com indicação de tratamento cirúrgico representado por recuo mandibular isolado. Para cada paciente foram realizadas telerradiografias nas fases inicial, pré-cirúrgica e pós-cirúrgica, sendo comparadas a um grupo controle, constituído de telerradiografias de indivíduos com oclusão normal. Segundo a metodologia empregada e pela análise dos resultados obtidos, avaliados estatisticamente, constatou-se que os pacientes Classe III com indicação de recuo mandibular foram caracterizados por um mau relacionamento entre as bases esqueléticas representado por um bom posicionamento da maxila associado a prognatismo mandibular, aumento da altura facial ântero-inferior, incisivos inferiores e sínfise mandibular lingualizados e incisivos superiores vestibularizados. A partir do preparo ortodôntico pré-cirúrgico, observou-se uma rotação mandibular no sentido horário, descompensação dentária representada por lingualização e extrusão dos incisivos superiores e vestibularização dos incisivos inferiores, acompanhada por uma remodelação da cortical óssea vestibular da sínfise mandibular. Esta descompensação ortodôntica definiu características dento-alveolares semelhantes às dos indivíduos com oclusão normal. O comportamento das variáveis dento-esqueléticas após a cirurgia ortognática, a partir do deslocamento póstero-superior das estruturas dento-esqueléticas da mandíbula, proporcionou um equilíbrio destas estruturas, em relação à oclusão normal.

The respiratory effects of dexmedetomidine were retrospectively examined in 33 postsurgical patients involved in a randomised, placebo-controlled trial after extubation in the intensive care unit (ICU). Morphine requirements were reduced by over 50% in patients receiving dexmedetomidine. There were no differences in respiratory rates, oxygen saturations, arterial pH and arterial partial carbon dioxide tension (PaCO2) between the groups. Interestingly the arterial partial oxygen tension (PaO2) : fractional inspired oxygen (FIO2) ratios were statistically significantly higher in the dexmedetomidine group. Dexmedetomidine provides important postsurgical analgesia and appears to have no clinically important adverse effects on respiration in the surgical patient who requires intensive care.

Objective: To evaluate the visual and refractive outcomes after phacoemulsification surgery in eyes with isolated lens coloboma. Design: Prospective, consecutive case series. Participants: Eighteen eyes with isolated lens coloboma of 13 patients were included in the study. Mean patient age was 13.9 ± 6.5 years. Methods: Patients underwent phacoemulsification surgery, with combined implantation of capsular tension ring (CTR) and intraocular lens. In colobomas of less than 120°, a CTR was used, whereas in colobomas of more than 120°, a Cionni-modified single eyelet CTR was used to achieve better capsular centration. The main outcome measures were uncorrected distance visual acuity, corrected distance visual acuity, refraction, and keratometry. Results: Mean logMAR uncorrected distance visual acuity and corrected distance visual acuity improved significantly from 1.53 ± 0.35 and 1.02 ± 0.47 before surgery to 0.67 ± 0.51 and 0.52 ± 0.49 at the last visit of the follow-up (p < 0.001). Mean refractive cylinder and spherical equivalent decreased significantly from –6.73 ± 1.73 and –6.72 ± 4.07 D preoperatively to –1.40 ± 1.39 and –0.83 ± 1.31 D at the end of the follow-up (p = 0.001 and p = 0.01, respectively). Mean keratometric astigmatism at preoperative and postoperative visits were 1.58 ± 0.97 and 1.65 ± 0.94 D, respectively (p = 0.70). Conclusions: Phacoemulsification with CTR and intraocular lens implantation is an effective and safe option for providing a refractive correction and a significant visual improvement in eyes with isolated lens coloboma.
