INTRODUCTION: The co-infection Trypanosoma cruzi/HIV has been described as a clinical event of great relevance. The objective of this study wasto describe clinical and epidemiological aspects published in literature. METHODS: It is a systematic review of a descriptive nature from the databases Medline, Lilacs, SciELO, Scopus, from 1980 to 2010. RESULTS: There were 83 articles (2.8 articles/year) with a total of 291 cases. The co-infection was described in 1980 and this situation has become the defining AIDS clinical event in Brazil. This is the country with the highest number of publication (51.8%) followed by Argentina (27.7%). The majority of cases are amongst adult men (65.3%) native or from endemic regions with serological diagnosis in the chronic stage (97.9%) and indeterminate form (50.8%). Both diseases follow the normal course, but in 41% the reactivation of the Chagas disease occurs. The most severe form is the meningoencephalitis, with 100% of mortality without specific and early treatment of the T. cruzi. The medication of choice was the benznidazole on doses and duration normally used for the acute phase. The high parasitemia detected by direct or indirect quantitative methods indicated reactivation and its elevation is the most important predictive factor. The lower survival rate was related to the reactivation of the Chagas disease and the natural complications of both diseases. The role of the antiretroviral treatment on the co-infection cannot yet be defined by the knowledge currently existent. CONCLUSIONS: Despite the relevance of this clinical event there are still gaps to be filled.

This paper analyzes the in-, and out-of sample, predictability of the stock market returns from Eurozone’s banking sectors, arising from bank-specific ratios and macroeconomic variables, using panel estimation techniques. In order to do that, I set an unbalanced panel of 116 banks returns, from April, 1991, to March, 2013, to constitute equal-weighted country-sorted portfolios representative of the Austrian, Belgian, Finish, French, German, Greek, Irish, Italian, Portuguese and Spanish banking sectors. I find that both earnings per share (EPS) and the ratio of total loans to total assets have in-sample predictive power over the portfolios’ monthly returns whereas, regarding the cross-section of annual returns, only EPS retain significant explanatory power. Nevertheless, the sign associated with the impact of EPS is contrarian to the results of past literature. When looking at inter-yearly horizon returns, I document in-sample predictive power arising from the ratios of provisions to net interest income, and non-interest income to net income. Regarding the out-of-sample performance of the proposed models, I find that these would only beat the portfolios’ historical mean on the month following the disclosure of year-end financial statements. Still, the evidence found is not statistically significant. Finally, in a last attempt to find significant evidence of predictability of monthly and annual returns, I use Fama and French 3-Factor and Carhart models to describe the cross-section of returns. Although in-sample the factors can significantly track Eurozone’s banking sectors’ stock market returns, they do not beat the portfolios’ historical mean when forecasting returns.

INTRODUCTION: Methicillin-resistant Staphylococcus aureus (MRSA) is spread out in hospitals across different regions of the world and is regarded as the major agent of nosocomial infections, causing infections such as skin and soft tissue pneumonia and sepsis. The aim of this study was to identify risk factors for methicillin-resistance in Staphylococcus aureus bloodstream infection (BSI) and the predictive factors for death. METHODS: A retrospective cohort of fifty-one patients presenting bacteraemia due to S. aureus between September 2006 and September 2008 was analysed. Staphylococcu aureus samples were obtained from blood cultures performed by clinical hospital microbiology laboratory from the Uberlândia Federal University. Methicillinresistance was determined by growth on oxacillin screen agar and antimicrobial susceptibility by means of the disk diffusion method. RESULTS: We found similar numbers of MRSA (56.8%) and methicillin-susceptible Staphylococcus aureus (MSSA) (43.2%) infections, and the overall hospital mortality ratio was 47%, predominantly in MRSA group (70.8% vs. 29.2%) (p=0.05). Age (p=0.02) was significantly higher in MRSA patients as also was the use of central venous catheter (p=0.02). The use of two or more antimicrobial agents (p=0.03) and the length of hospital stay prior to bacteraemia superior to seven days (p=0.006) were associated with mortality. High odds ratio value was observed in cardiopathy as comorbidity. CONCLUSIONS: Despite several risk factors associated with MRSA and MSSA infection, the use of two or more antimicrobial agents was the unique independent variable associated with mortality.

INTRODUCTION: The laboratory diagnosis of schistosomiasis is based mainly on the detection of parasite eggs in stool samples through the Kato-Katz (KK) technique, reading one slide by test. However, a widely known limitation of parasitological methods is reduced sensitivity, particularly in low endemic areas. METHODS: To increase sensitivity, we conducted further slide readings from the same stool sample using the parasitological method associated with a serological test. We used the KK method (three slides) and the IgG anti-Schistosoma mansoni-enzyme-linked immunosorbent assay (ELISA) technique to diagnose schistosomiasis in low endemic areas in the Brazilian State of Ceará. Fecal samples and sera from 250 individuals were analyzed. RESULTS: Sixteen percent and 47.2% of samples were positive in parasitological tests and serological tests, respectively. Parasitological methods showed that 32 (80%) individuals tested positive on the first slide, 6 (15%) on the second slide, and 2 (5%) on the third. The performance of the ELISA test in the diagnosis, using the KK method as diagnostic reference, showed a negative predictive value of 100%, with specificity and positive predictive values of 62.8% and 33.9%, respectively. CONCLUSIONS: In this study, the increase from one to three slides analyzed per sample using the KK technique was shown to be a useful procedure for increasing the diagnostic sensitivity of this technique.

RESUMO:Introdução: O conhecimento acerca da prática da Fisioterapia, bem como a influência das características dos indivíduos com Osteoartrose do joelho (OA) no prognóstico dos resultados alcançados com a Fisioterapia, tem sido alvo de estudo em vários países. No entanto em Portugal, estudos desta natureza são escassos ou inexistentes. Objetivo: Este trabalho teve dois principais objetivos: Por um lado pretendeu caracterizar a prática da Fisioterapia e determinar os seus resultados em indivíduos com OA do joelho ao nível da intensidade da dor, incapacidade funcional e perceção global de melhoria; Por outro, identificar fatores preditivos para os resultados de sucesso após a intervenção da Fisioterapia. Metodologia: Trata-se de um estudo de coorte prospetivo com 77 participantes que foram avaliados antes e após a intervenção (8 semanas), com um follow-up de 3 meses. Procedeu-se à caracterização da prática da Fisioterapia quanto às modalidades utilizadas, número de sessões de tratamento e duração do episódio de cuidados. Os resultados obtidos foram medidos após intervenção (8 semanas) e num follow-up (3 meses), através da Escala Numérica da Dor (END), da versão Portuguesa do Questionário Knee Injury and Osteoarthritis Outcome Score (KOOS-PT), e da versão Portuguesa da Patient Global Impression of Change Scale (PGIC-PT). As características sociodemográficas e clínicas foram incluídas como potenciais fatores de prognóstico com base no critério de pontuação ≥5 na PGIC-PT e a análise dos dados foi realizada através do método de regressão logística multivariado no sentido de identificar as associações entre as variáveis na baseline e as variáveis de resultado (p<0.05). Resultados: Os resultados obtidos revelaram que existe uma grande diversidade de modalidades e procedimentos terapêuticos realizados pelos fisioterapeutas no tratamento da OA do joelho (em média 4 modalidades por utente), sendo os exercícios, a eletroterapia e a terapia manual, as intervenções mais frequentemente realizadas. Os resultados da intervenção revelaram uma redução significativa da intensidade da dor e da incapacidade funcional (medida pelas diferentes dimensões da KOOS-PT) às 8 semanas e aos 3 meses (p=0,001). A probabilidade de obter bons resultados na percepção de melhoria para realizar actividades da vida diária, está significativamente associada com a pontuação na dimensão KOOS AVD (OR=1,106, 95% IC 1,056-1,159) e com a intensidade da dor, (OR=0,543, 95% IC 0,300-0,983), na baseline. A probabilidade de obter bons resultados na percepção de melhoria relativa à dor, está significativamente associada com a pontuação na dimensão KOOS DOR (OR=1,116, 95% IC 1,030-1,209), e dimensão KOOS ADL (OR=1,123, 95% IC 1,014-1,243), na baseline. Conclusões: Os resultados do presente estudo revelam que a intervenção em Fisioterapia em casos de OA do joelho, apesar de utilizar uma grande diversidade de modalidades terapêuticas, proporciona uma redução significativa dos níveis de dor e incapacidade funcional em utentes com OA do joelho. Os resultados do presente estudo sugerem ainda que o sucesso da intervenção pode ser predito a partir de variáveis clinicas na baseline.------------ABSTRACT:Introduction: The knowledge about the practice of physical therapy, as well as the influence of the characteristics of subjects with knee osteoarthritis (OA) in the prognosis of the results achieved with physiotherapy, have been studied in several countries. However in Portugal, such studies are scarce or nonexistent. Objective: This study had two main aims: on one hand sought to characterize the practice of physical therapy and determine their outcomes in subjects with OA of the knee joint at the level of pain intensity, functional disability and global perception of improvement; Secondly, to identify predictive factors for successful outcomes after the intervention of physiotherapy. Methods: This was a prospective cohort study with 77 participants who were evaluated before and after the intervention (8 weeks), with a follow-up of 3 months. Proceeded to characterize the practice of physical therapy as modalities used, number of treatment sessions and duration of the period of care. The results were measured after intervention (8 weeks) and a follow-up (3 months) by a Numerical Rating Scale (NRS), the Portuguese version of the Knee Injury and Osteoarthritis Questionnaire Outcome Score (KOOS-PT) and the Patient Global Impression of Change Scale- Portuguese Version (PGIC-PT). The sociodemographic and clinical characteristics were included as potential prognostic factors based on scoring criteria≥5 na PGIC-PT and data analysis was performed using the method of multivariate logistic regression to identify associations between variables at baseline and outcome variables (p<0.05). Results: The results revealed that there is a great diversity of modalities and therapeutic procedures performed by physiotherapists in the treatment of knee OA (average 4 per user modes), with exercises, electrotherapy and manual therapy interventions most frequently performed. The results of intervention showed a significant reduction in the severity of pain and functional disability (measured by the different dimensions of the KOOS-PT) at 8 weeks and at 3 months (p = 0.001). The probability of getting good results in the perception of improvement to perform activities of daily living, is significantly associated with scores on the dimension KOOS ADL (OR=1,106, 95% CI 1,056-1,159) and the intensity of pain (OR = 0.543, 95% CI 0.300 to 0.983) at baseline. The probability of getting good results in the perception of improvement of the pain is significantly associated with scores on the KOOS PAIN (OR = 1.116, 95% CI 1.030 to 1.209) and KOOS ADL (OR = 1.123, 95% CI 1.014 to 1.243) at the baseline. Conclusions: The results of this study show that Physiotherapy treatment in cases of knee OA, provide a significant reduction of pain and disability among users with knee OA, despite the wide variety of therapeutic modalities. The results of this study also suggest that the success of the intervention can be predicted from clinical variables at baseline.

RESUMO: Introdução/ Objetivo: Segundo a revisão sistemática de Chester e colaboradores (2013b)apenas dois fatores de prognóstico demonstraram uma associação consistente com o resultado que foram a duração dos sintomas e a funcionalidade na avaliação inicial. O objetivo do estudo é identificar indicadores de bom e mau prognóstico em utentes com disfunção do complexo articular do ombro (DCAO), tendo por base, aspetos da avaliação inicial do utente e critérios de alta de abolição da dor, aumento da funcionalidade e da estabilidade dinâmica considerando uma intervenção terapêutica direcionada para o aumento da estabilidade dinâmica da escápulo-torácica. Metodologia: Efetuou-se um estudo de coorte clínico retrospetivo. Para tal, aplicou-se um protocolo de intervenção terapêutica e analisou-se os resultados. A amostra foi constituída por 82 indivíduos com DCAO [53 com síndrome do conflito subacromial (SCSA) e 29 com instabilidade da glenoumeral (IGU)], residentes nos distritos de Lisboa, Setúbal e Santarém com o intuito de iniciar tratamento de fisioterapia. A análise dos dados foi efetuada tendo em consideração dois procedimentos: análise univariada (através do método de Kaplan-Meier para cada CVP) e análise multifatorial (pela análise de regressão de Cox e regressão logística nos grupos de utentes com SCSA, IGU e DCAO). Resultados: O tempo mediano de continuação no tratamento em fisioterapia foi de 7 semanas para os utentes com SCSA e 6 semanas para utentes com IGU. Segundo o teste de Logrank, na análise univariada, existem sete e oito covariáveis preditoras (CVP) com associação estatisticamente significativa (p<0,05) para o subgrupo SCSA e IGU, respectivamente. De acordo com estes resultados, a primeira parte da DASH e a SPADI são as únicas CVP com associação comuns às duas disfunções. Pela análise multifatorial e, em congruência com o teste de Wald, nenhuma das CVP contribui estatisticamente para o modelo preditivo de continuidade do tratamento de fisioterapia em qualquer um dos três modelos estudados: subgrupo SCSA, subgrupo IGU e utentes com DCAO. Conclusão: Por uma análise univariada verificou-se que existem CVP associadas à alta dos tratamentos em fisioterapia e estas não são as mesmas em ambas as DCAO. Contudo, a magnitude do efeito de cada CVP nos modelos multifatoriais definidos para os grupos de utentes com SCSA, IGU e DCAO não demonstraram valor estatisticamente significativo pelo que não foi possível determinar modelos de prognóstico em utentes com DCAO.-------------ABSTRACT: Background/ Purpose: According with the systematic review from Chester and collaborators (2013b) just two prognostic factors demonstrated a consistent association with the outcome: the duration of symptoms and functionality in the initial assessment. The purpose of the study is to identify indicators of good and poor prognosis in patients with shoulder’s dysfunctions, based on aspects of the initial assessment and discharge criteria of absence of pain, increased functionality and dynamic stability considering a therapeutic intervention used to increase the dynamic stability of scapulo-thoracic. Methodology: It was conducted a retrospective study of clinical cohort. For this purpose it was applied a protocol with therapeutic intervention and the results were analyzed. The sample consisted of 82 individuals with shoulder’s dysfunction (53 with subacromial impingement (SIMP) and 29 with shoulder instability (SINS) residing in the districts of Lisbon, Setúbal and Santarém in order to start physiotherapy. Data analysis was performed taking into account two procedures: univariate analysis [using the Kaplan-Meier method for each co-variant predictor variable (CVP)] and multifactorial analysis [analysis by Cox regression and logistic regression on groups of patients with SIMP, SINS and shoulder’s dysfunction (SD)]. Results: The median time of follow-up treatment at physical therapy was 7 weeks for patients with SIMP and 6 weeks for patients with SINS. According to the Logrank test in the univariate analysis, there are seven and eight CVP with a statistically significant association (p<0.05) for the patients with SIMP and SINS, respectively. According to these results, the first part of the DASH and SPADI are the only CVP common to both disorders association. By multifatorial analyses, and in agreement with the Wald test, none of the CVP contributes statistically to the predictive model of continuity of physiotherapy treatment in any of the three studied models: patients with SIMP, patients with SINS and patients with SD. Conclusion: In an univariate analysis, it was verified that there are CVP associated with discharge from treatments of physical therapy and these are not the same in both SD. However, the magnitude of effect of each CVP in multifactorial models for defined patients groups with SIMP, SINS and SD showed no statistically significant. Therefore, it was not possible to determine prognostic models for patients with SD.

INTRODUCTION: Operational classification of leprosy based on the number of skin lesions was conceived to screen patients presenting severe forms of the disease to enable their reception of a more intense multidrug regimen without having to undergo lymph smear testing. We evaluated the concordance between operational classification and bacilloscopy to define multibacillary and paucibacillary leprosy. METHODS: We selected 1,213 records of individuals with leprosy, who were untreated (new cases) and admitted to a dermatology clinic in Recife, Brazil, from 2000 to 2005, and who underwent bacteriological examination at diagnosis for ratification of the operational classification. RESULTS: Compared to bacilloscopy, operational classification demonstrated 88.6% sensitivity, 76.9% specificity, a positive predictive value of 61.8%, and a negative predictive value of 94.1%, with 80% accuracy and a moderate kappa index. Among the bacilloscopy-negative cases, 23% had more than 5 skin lesions. Additionally, 11% of the bacilloscopy-positive cases had up to 5 lesions, which would have led to multibacillary cases being treated as paucibacillary leprosy if the operational classification had not been confirmed by bacilloscopy. CONCLUSIONS: Operational classification has limitations that are more obvious in borderline cases, suggesting that in these cases, lymph smear testing is advisable to enable the selection of true multibacillary cases for more intense treatment, thereby contributing to minimization of resistant strain selection and possible relapse.

INTRODUCTION: Some viruses of the Herpesviridae family are frequently the etiologic agents of oral lesions associated with HIV. The aim of this study was to identify the presence of herpes simplex virus types 1 and 2 (HSV-1, HSV-2), Varicella Zoster virus (VZV), Epstein-Barr virus (EBV), human cytomegalovirus (HCMV), human herpesvirus type 6, type 7 and type 8 (HHV-6, HHV-7 and HHV-8) in the oral cavity of HIV-infected children/adolescents and verify the association between viral subtypes and clinical factors. METHODS: The cells of oral mucosa were collected from 50 HIV infected children/adolescents, 3-13 years old (mean age 8.66). The majority (66%) of selected were girls, and they were all outpatients at the pediatric AIDS clinic of a public hospital in Rio de Janeiro. Nested-PCR was used to identify the viral types. RESULTS: Absence of immunosuppression was observed in 66% of the children. Highly active antiretroviral therapy (HAART) was used by 72.1% of selected and moderate viral load was observed in 56% of the children/adolescents. Viral types were found in 86% of the children and the subtypes were: HSV-1 (4%), HSV-2 (2%), VZV (4%), EBV (0%), HCMV (24%), HHV6 (18%), HHV-7 (68%), HHV8 (0%). CONCLUSIONS: The use of HAART has helped to reduce oral lesions, especially with herpes virus infections. The health professionals who work with these patients should be aware of such lesions because of their predictive value and the herpes virus can be found circulating in the oral cavity without causing lesions.

Introduction Toxoplasmosis may be life-threatening in fetuses and in immune-deficient patients. Conventional laboratory diagnosis of toxoplasmosis is based on the presence of IgM and IgG anti-Toxoplasma gondii antibodies; however, molecular techniques have emerged as alternative tools due to their increased sensitivity. The aim of this study was to compare the performance of 4 PCR-based methods for the laboratory diagnosis of toxoplasmosis. One hundred pregnant women who seroconverted during pregnancy were included in the study. The definition of cases was based on a 12-month follow-up of the infants. Methods Amniotic fluid samples were submitted to DNA extraction and amplification by the following 4 Toxoplasma techniques performed with parasite B1 gene primers: conventional PCR, nested-PCR, multiplex-nested-PCR, and real-time PCR. Seven parameters were analyzed, sensitivity (Se), specificity (Sp), positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (PLR), negative likelihood ratio (NLR) and efficiency (Ef). Results Fifty-nine of the 100 infants had toxoplasmosis; 42 (71.2%) had IgM antibodies at birth but were asymptomatic, and the remaining 17 cases had non-detectable IgM antibodies but high IgG antibody titers that were associated with retinochoroiditis in 8 (13.5%) cases, abnormal cranial ultrasound in 5 (8.5%) cases, and signs/symptoms suggestive of infection in 4 (6.8%) cases. The conventional PCR assay detected 50 cases (9 false-negatives), nested-PCR detected 58 cases (1 false-negative and 4 false-positives), multiplex-nested-PCR detected 57 cases (2 false-negatives), and real-time-PCR detected 58 cases (1 false-negative). Conclusions The real-time PCR assay was the best-performing technique based on the parameters of Se (98.3%), Sp (100%), PPV (100%), NPV (97.6%), PLR (∞), NLR (0.017), and Ef (99%).

Introduction Rapid diagnostic tests (RDTs) may improve the early detection of visceral leishmaniasis (VL), but their real-world performance requires additional study. Therefore, we evaluated the performance of an rK39-based RDT (Kalazar Detect™) for the detection of VL in an endemic, large urban area. Methods Data were collected from a registry of rK39 RDT performed at 11 emergency care units in Belo Horizonte, Brazil, and from a national database of reportable communicable diseases of the Sistema de Informação de Agravos de Notificação (SINAN). Results The rapid rK39 test was performed in 476 patients, with 114 (23.9%) positive results. The analysis of rK39 RDT performance was based on 381 (80%) cases reported to the SINAN database, of which 145 (38.1%) were confirmed cases. Estimates for sensitivity and specificity were 72.4% (95% CI: 64.6-79%) and 99.6% (95%CI: 97.6-99.9%), respectively. Positive and negative predictive values were estimated at 99.1% (95%CI: 94.9-99.8%) and 85.5% (95%CI: 80.8-89.1%), respectively. In addition, close agreement between the rK39 RDT and indirect immunofluorescence was observed. Conclusions In summary, the rK39 RDT showed a high specificity but only moderate sensitivity. In endemic areas for VL, treatment may be considered in cases with clinical manifestations and a positive rK39 RDT, but those with a negative test should be subjected to further investigation.

Introduction: Epidemiological studies on amebiasis have been reassessed since Entamoeba histolytica and E. dispar were first recognized as distinct species. Because the morphological similarity of these species renders microscopic diagnosis unreliable, additional tools are required to discriminate between Entamoeba species. The objectives of our study were to compare microscopy with ELISA kit (IVD®) results, to diagnose E. histolytica infection, and to determine the prevalence of amebiasis in a sample of students from southeastern Brazil. Methods: In this study, diagnosis was based on microscopy due to its capacity for revealing potential cysts/trophozoites and on two commercial kits for antigen detection in stool samples. Results: For 1,403 samples collected from students aged 6 to 14 years who were living in Divinópolis, Minas Gerais, Brazil, microscopy underestimated the number of individuals infected with E. histolytica/E. dispar (5.7% prevalence) compared with the ELISA kit (IVD®)-based diagnoses (15.7% for E. histolytica/E. dispar). A comparison of the ELISA (IVD®) and light microscopy results returned a 20% sensitivity, 97% specificity, low positive predictive value, and high negative predictive value for microscopy. An ELISA kit (TechLab®) that was specific for E. histolytica detected a 3.1% (43/1403) prevalence for E. histolytica infection. Conclusions: The ELISA kit (IVD®) can be used as an alternative screening tool. The high prevalence of E. histolytica infection detected in this study warrants the implementation of actions directed toward health promotion and preventive measures.

Release of chloroethene compounds into the environment often results in groundwater contamination, which puts people at risk of exposure by drinking contaminated water. cDCE (cis-1,2-dichloroethene) accumulation on subsurface environments is a common environmental problem due to stagnation and partial degradation of other precursor chloroethene species. Polaromonas sp. strain JS666 apparently requires no exotic growth factors to be used as a bioaugmentation agent for aerobic cDCE degradation. Although being the only suitable microorganism found capable of such, further studies are needed for improving the intrinsic bioremediation rates and fully comprehend the metabolic processes involved. In order to do so, a metabolic model, iJS666, was reconstructed from genome annotation and available bibliographic data. FVA (Flux Variability Analysis) and FBA (Flux Balance Analysis) techniques were used to satisfactory validate the predictive capabilities of the iJS666 model. The iJS666 model was able to predict biomass growth for different previously tested conditions, allowed to design key experiments which should be done for further model improvement and, also, produced viable predictions for the use of biostimulant metabolites in the cDCE biodegradation.

Research literature and regulators are unconditional in pointing the disclosure of operating cash flow through direct method a section of unique information. Besides the intuitive facet, it is also consistent in forecasting future operating cash flows and a cohesive piece to financial statement puzzle.
