microRNAs (miRNAs) are small non-coding RNAs that regulate various biological processes. Cell-free miRNAs have been proposed as biomarkers of disease, including diagnosis, prognosis, and monitoring of treatment responses. These circulating miRNAs are highly stable in several body fluids, including plasma and serum; hence, in view of their potential use as novel, non-invasive biomarkers, the profiles of circulating miRNAs have been explored in the field of anti-doping. This chapter describes the enormous potential of circulating miRNAs as a new class of biomarkers for the detection of doping substances, and highlights the advantages of measuring these stable species over other methods that have already been implemented in anti-doping regimes. Incorporating longitudinal measurements of circulating miRNAs into the Athlete Biological Passport is proposed as an efficient strategy for the implementation of these new biomarkers. Furthermore, potential challenges related to the transition of measurements of circulating miRNAs from research settings to practical anti-doping applications are presented.

Objective: The aim of the current study was to investigate the long-term cognitive effects of electroconvulsive therapy (ECT) in a sample of adolescent patients in whom schizophrenia spectrum disorders were diagnosed. Methods: The sample was composed of nine adolescent subjects in whom schizophrenia or schizoaffective disorder was diagnosed according to DSM-IV-TR criteria on whom ECT was conducted (ECT group) and nine adolescent subjects matched by age, socioeconomic status, and diagnostic and Positive and Negative Syndrome Scale (PANSS) total score at baseline on whom ECT was not conducted (NECT group). Clinical and neuropsychological assessments were carried out at baseline before ECT treatment and at 2-year follow-up. Results: Significant differences were found between groups in the number of unsuccessful medication trials. No statistically significant differences were found between the ECT group and theNECT group in either severity as assessed by the PANSS, or in any cognitive variables at baseline.At follow-up, both groups showed significant improvement in clinical variables (subscales of positive, general, and total scores of PANSS and Clinical Global Impressions-Improvement). In the cognitive assessment at follow-up, significant improvement was found in both groups in the semantic category of verbal fluency task and digits forward. However, no significant differences were found between groups in any clinical or cognitive variable at follow-up. Repeated measures analysis found no significant interaction of time · group in any clinical or neuropsychological measures. Conclusions: The current study showed no significant differences in change over time in clinical or neuropsychological variables between the ECT group and the NECT group at 2-year follow-up. Thus, ECT did not show any negative influence on long-term neuropsychological variables in our sample.

Lactate has been shown to offer neuroprotection in several pathologic conditions. This beneficial effect has been attributed to its use as an alternative energy substrate. However, recent description of the expression of the HCA1 receptor for lactate in the central nervous system calls for reassessment of the mechanism by which lactate exerts its neuroprotective effects. Here, we show that HCA1 receptor expression is enhanced 24 hours after reperfusion in an middle cerebral artery occlusion stroke model, in the ischemic cortex. Interestingly, intravenous injection of L-lactate at reperfusion led to further enhancement of HCA1 receptor expression in the cortex and striatum. Using an in vitro oxygen-glucose deprivation model, we show that the HCA1 receptor agonist 3,5-dihydroxybenzoic acid reduces cell death. We also observed that D-lactate, a reputedly non-metabolizable substrate but partial HCA1 receptor agonist, also provided neuroprotection in both in vitro and in vivo ischemia models. Quite unexpectedly, we show D-lactate to be partly extracted and oxidized by the rodent brain. Finally, pyruvate offered neuroprotection in vitro whereas acetate was ineffective. Our data suggest that L- and D-lactate offer neuroprotection in ischemia most likely by acting as both an HCA1 receptor agonist for non-astrocytic (most likely neuronal) cells as well as an energy substrate.

Le corps humain emploie le glucose comme source principale d'énergie. L'insuline, sécrétée par les cellules ß-pancreatiques situées dans les îlots de Langerhans, est l'hormone principale assurant un maintien constant du taux de glucose sanguin (glycémie). Les prédispositions génétiques, le manque d'activité physique et un régime déséquilibré peuvent entraîner une perte de sensibilité à l'insuline et des taux de glucose dans le sang élevé (hyperglycémie), une condition nommée diabète de type 2. Cette maladie est initiée par une sensibilité diminuée à l'insuline dans les tissus périphériques, entraînant une demande accrue en insuline. Cette pression continue finie par épuiser les cellules ß-pancreatiques, qui sécrètent alors des niveaux d'insuline insuffisant en trainant l'apparition du diabète. Le vieillissement est un facteur de risque important pour les maladies métaboliques dont le diabète de type 2 faits partis. En effet la majeure partie des diabétiques de type 2 ont plus de 45 ans. Il est connu que le vieillissement entraine une perte de sensibilité à l'insuline, une sécrétion altérée d'insuline, une baisse de réplication et une plus grande mort des ß-cellules pancréatiques. Le but de ma thèse était de mieux comprendre les mécanismes contribuante au dysfonctionnement des cellules ß- pancréatiques lors du vieillissement. Les travaux du « Human Genome Project » ont révélés que seulement 2% de notre génome code pour des protéines. Le reste non-codant fut alors désigné sous le nom de « ADN déchets ». Cependant, l'étude approfondie de cet ADN non-codant ces dernières deux décennies a démontré qu'une grande partie code pour des «MicroARNs », des ARNs courts (20-22 nucleotides) découverts en 1997 chez le vers C.elegans. Depuis lors ces molécules ont été intensivement étudiées, révélant un rôle crucial de ces molécules dans la fonction et la survie des cellules en conditions normales et pathologiques. Le but de cette thèse était d'étudier le rôle des microARNs dans le dysfonctionnement des cellules ß lors du vieillissement. Nos données suggèrent qu'ils peuvent jouer un rôle tantôt salutaire, tantôt nocif sur les cellules ß. Par exemple, certains microARNs réduisent la capacité des cellules ß à se multiplier ou réduisent leur survie, alors que d'autres protègent ces cellules contre la mort. Pour conclure, nous avons démontré les microARNs jouent un rôle important dans le dysfonctionnement des cellules ß lors du vieillissement. Ces nouvelles découvertes préparent le terrain pour la conception de futures stratégies visant à améliorer la résistance des cellules ß pancréatiques afin de trouver de nouveaux traitements du diabète de type 2. -- Le diabète de type 2 est une maladie métabolique due à la résistance à l'action de l'insuline des tissus cibles combinée à l'incapacité des cellules ß pancréatiques à sécréter les niveaux adéquats d'insuline. Le vieillissement est associé à un déclin global des fonctions de l'organisme incluant une diminution de la fonction et du renouvellement des cellules ß pancréatiques. Il constitue ainsi un risque majeur de développement des maladies métaboliques dont le diabète de type 2. Le but de cette thèse était d'étudier le rôle des microARNs (une classe d'ARN non- codants) dans le dysfonctionnement lié au vieillissement des cellules ß. L'analyse par microarray des niveaux d'expression des microARN dans les îlots pancréatiques de rats Wistar mâles âgés de 3 et 12 mois nous a permis d'identifier de nombreux changements d'expression de microARNs associés au vieillissement. Afin d'étudier les liens entre ces modifications et le déclin des cellules ß, les changements observés lors du vieillissement ont été reproduits spécifiquement dans une lignée cellulaire, dans des cellules ß primaires de jeune rats ou de donneurs humains sains. La diminution du miR-181a réduit la prolifération des cellules ß, tandis que la diminution du miR-130b ou l'augmentation du miR-383 protège contre l'apoptose induite par les cytokines. L'augmentation du miR-34a induit l'apoptose et inhibe la prolifération des cellules ß en réponse aux hormones Exendin-4 et prolactine et au facteur de croissance PDGF-AA. Cette perte de capacité réplicative est similaire à celle observée dans des cellules ß de rats âgés de 12 mois. Dans la littérature, la perte du récepteur au PDGF-r-a est associée à la diminution de la capacité proliférative des cellules ß observée lors du vieillissement. Nous avons pu démontrer que PDGF-r-a est une cible directe de miR- 34a, suggérant que l'effet néfaste de miR-34a sur la prolifération des cellules ß est, du moins en partie, lié à l'inhibition de l'expression de PDGF-r-a. L'expression de ce miR est aussi plus élevée dans le foie et le cerveau des animaux de 1 an et augmente avec l'âge dans les ilôts de donneurs non-diabétiques. Ces résultats suggèrent que miR-34a pourrait être non seulement impliqué dans l'affaiblissement des fonctions pancréatiques associé à l'âge, mais également jouer un rôle dans les tissus cibles de l'insuline et ainsi contribuer au vieillissement de l'organisme en général. Pour conclure, les travaux obtenus durant cette thèse suggèrent que des microARNs sont impliqués dans le dysfonctionnement des cellules ß pancréatiques durant le vieillissement. -- Type 2 diabetes is a metabolic disease characterized by impaired glucose tolerance, of the insulin sensitive tissues and insufficient insulin secretion from the pancreatic ß-cells to sustain the organism demand. Aging is a risk factor for the majority of the metabolic diseases including type 2 diabetes. With aging is observed a decline in all body function, due to decrease both in cell efficiency and renewal. The aim of this thesis was to investigate the potential role of microRNAs (short non- coding RNAs) in the pancreatic ß-cell dysfunction associated with aging. Microarray analysis of microRNA expression profile in pancreatic islets from 3 and 12 month old Wistar male rats revealed important changes in several microRNAs. To further study the link between those alterations and the decline of ß-cells, the changes observed in old rats were mimicked in immortalized ß-cell lines, primary young rat and human islets. Downregulation of miR-181a inhibited pancreatic ß-cell proliferation in response to proliferative drugs, whereas downregulation of miR-130b and upregulation of miR-383 protected pancreatic ß-cells from cytokine stimulated apoptosis. Interestingly, miR-34a augmented pancreatic ß-cell apoptosis and inhibited ß-cell proliferation in response to the proliferative chemicals Exendin-4, prolactin and PDGF-AA. This loss of replicative capacity is reminiscent of what we observed in pancreatic ß-cells isolated from 12 month old rats. We further observed a correlation between the inhibitory effect of miR-34a on pancreatic ß-cell proliferation and its direct interfering effect of this microRNA on PDGF-r-a, which was previously reported to be involved in the age-associated decline of pancreatic ß-cell proliferation. Interestingly miR-34a was upregulated in the liver and brain of 1 year old animals and positively correlated with age in pancreatic islets of normoglycemic human donors. These results suggest that miR-34a might be not only involved in the age-associated impairment of the pancreatic ß-cell functions, but also play a role in insulin target tissues and contribute to the aging phenotype on the organism level. To conclude, we have demonstrated that microRNAs are indeed involved in the age-associated pancreatic ß-cell dysfunction and they can play both beneficial and harmful roles in the context of pancreatic ß-cell aging.

DNA cytosine methylation has been demonstrated to be a central epigenetic modification that has essential roles in a myriad of cellular processes. Some examples of these include gene regulation, DNA-protein interactions, cellular differentiation, X-inactivation, maintenance of genome integrity by suppressing transposable elements and viruses, embryogenesis, genomic imprinting and tumourigenesis. This list is increasingly growing thanks to recent advances in genome-wide technologies, like Whole Genome Bisulfite Sequencing (WGBS-Seq). The development of this technology in research has allowed the identification of new features of the DNA methylation landscape that was not possible using previous technologies, like Partially Methylated Domains (PMDs). PMDs have been found in several cell lines, as well as in both healthy and cancer primary samples. They have been described as regions with high variability in methylation levels across individual CpG sites and intermediate methylation levels on average with respect to the genome. Here, we performed an extensive search of PMDs in a big dataset of different haematopoietic primary cells from both myeloid and lymphoid lineages. We found and characterized significant PMDs in plasma B cells, confirming that PMDs are a phenomenon that is restricted to certain differentiated cells. Additionally, we found loci aberrantly hypomethylated in a myeloma sample which overlapped with plasma B cell PMDs. Genome-wide comparison of the myeloma and plasma B cell sample revealed that this is probably also the case for other loci.

L’objectiu d’aquest treball és visualitzar el paper de la dona en el mercat de treball, així com també l’estudi dels assalariats d’una empresa des de la perspectiva de gènere. La part teòrica consta d’una explicació de l’evolució del paper de la dona en el món laboral, com també el seu paper dins l’empresa i els diferents sectors on s’ha desenvolupat laboralment. L’estudi realitzat a la part pràctica, tracta d’una anàlisi estadística descriptiva de diverses variables d’una base de dades dels treballadors d’una empresa, per tal de poder veure les possibles diferències salarials entre homes i dones, així com també altres aspectes importants que s’han pogut destacar. D’aquesta manera s’han pogut extreure unes conclusions de l’estudi i s’ha pogut relacionar amb el marc teòric que hem presentat.

CSL is a key transcription factor, mostly acting as a repressor, which has been shown to have a highly context-dependent function. While known as the main effector of Notch signaling, it can also exert Notch-independent functions. The downstream effects of the Notch/CSL signaling pathway and its involvement in several biological processes have been intensively studied. We recently showed that CSL is important to maintain skin homeostasis, as its specific deletion in mouse dermal fibroblasts -or downmodulation in human stromal fibroblasts- creates an inducing environment for squamous cell carcinoma (SCC) development, possibly due to the conversion of stromal fibroblasts into cancer associated fibroblasts (CAFs). Despite the wide interest in CSL as a transcriptional regulator, the mechanism of its own regulation has so far been neglected. We show here that CSL expression levels differ between individuals, and correlate among others with genes involved in DNA damage response. Starting from this finding we show that in dermal fibroblasts CSL is under transcriptional control of stress inducers such as UVA irradiation and Reactive Oxygen Species (ROS) induction, and that a main player in CSL transcriptional regulation is the transcription factor p53. In a separate line of work, we focused on individual variability, studying the differences in gene expression between human populations in various cancer types, particularly focusing on the Caucasian and African populations. It is indeed widely known that these populations have different incidences and mortalities for various cancers, and response to cancer treatment may also vary between them. We show here several genes that are differentially expressed and could be of interest in the study of population differences in cancer. -- CSL est un facteur de transcription agissant essentiellement comme répresseur, et qui a une fonction hautement dépendant du contexte. C'est l'effecteur principal de la voie de signalisation de Notch, mais il peut également exercer ses fonctions dans une façon Notch- indépendante. Nous avons récemment montré que CSL est important pour maintenir l'homéostasie de la peau. Sa suppression spécifique dans les fibroblastes dermiques de la souris ou dans les fibroblastes stromales humaines crée un environnement favorable pour le développement du carcinome épidermoïde (SCC), probablement en raison de la conversion des fibroblastes en fibroblastes associé au cancer (CAF). Malgré le grand intérêt de CSL comme régulateur transcriptionnel, le mécanisme de sa propre régulation a été jusqu'ici négligée. Nous montrons ici que dans les fibroblastes dermiques CSL est sous le contrôle transcriptionnel de facteurs de stress tels que l'irradiation UVA et l'induction des ROS dont p53 est l'acteur principal de cette régulation. Nous montrons aussi que les niveaux d'expression de CSL varient selon les individus, en corrélation avec d'autres gènes impliqués dans la réponse aux dommages de l'ADN. Dans une autre axe de recherche, concernant la variabilité individuelle, nous avons étudié les différences dans l'expression des gènes dans différents types de cancer entre les populations humaines, en se concentrant particulièrement sur les populations africaines et caucasiennes. Il est en effet bien connu que ces populations montrent des variations dans l'incidence des cancers, la mortalité, ainsi que pour les réponses au traitement. Nous montrons ici plusieurs gènes qui sont exprimés différemment et pourraient être digne d'intérêt dans l'étude du cancer au sein de différentes populations.

PURPOSE: Needs assessment is recognized to be a key element of mental health care. Patients tend to present heterogeneous profiles of needs. However, there is no consensus in previous research about how patients' needs are organized. This study investigates both general and specific dimensions of patients' needs for care. METHODS: Patients' needs were assessed with ELADEB, an 18-domain self-report scale. The use of a self-assessment scale represents a unique way of obtaining patients' perceptions. A patient-centered psychiatric practice facilitates empowerment as it is based on the patients' personal motivations, needs, and wants. Four seventy-one patients' profiles were analyzed through exploratory factor analysis. RESULTS: A four-factor bifactor model, including one general factor and three specific factors of needs, was most adequate. Specific factors were (a) "finances" and "administrative tasks"; (b) "transports," "public places," "self-care," "housework," and "food"; and (c) "family," "children," "intimate relationships," and "friendship." CONCLUSION: As revealed by the general factor, patients expressing urgent needs in some domains are also more susceptible to report urgent needs in several other domains. This general factor relates to high versus low utilizers of public mental healthcare. Patients also present specific needs in life domains, which are organized in three dimensions: management, functional disabilities, and familial and interpersonal relationships. These dimensions relate to the different types of existing social support described in the literature.
Ambient radiation levels in positron emission tomography/computed tomography (PET/CT) imaging center

Objective: To evaluate the level of ambient radiation in a PET/CT center. Materials and Methods: Previously selected and calibrated TLD-100H thermoluminescent dosimeters were utilized to measure room radiation levels. During 32 days, the detectors were placed in several strategically selected points inside the PET/CT center and in adjacent buildings. After the exposure period the dosimeters were collected and processed to determine the radiation level. Results: In none of the points selected for measurements the values exceeded the radiation dose threshold for controlled area (5 mSv/year) or free area (0.5 mSv/year) as recommended by the Brazilian regulations. Conclusion: In the present study the authors demonstrated that the whole shielding system is appropriate and, consequently, the workers are exposed to doses below the threshold established by Brazilian standards, provided the radiation protection standards are followed.

Once a country allergic to any type of preferential treatment or quota measure for women, France has become a country that applies gender quotas to regulate women's presence and representation in politics, the business sector, public bodies, public administration, and even some civil society organizations. While research has concentrated on the adoption of electoral gender quotas in many countries and their international diffusion, few studies focus on explaining the successful diffusion of gender quotas from politics to other domains in the same country. This paper proposes to fill this gap by studying the particularly puzzling case of a country that at one point strongly opposed the adoption of gender quotas in politics, but, in less than a decade, transformed into one of the few countries applying gender quotas across several policy domains. This paper argues that the legal entrenchment of the parity principle, the institutionalization of parity in several successive women's policy agencies, and key players in these newly created agencies are mainly responsible for this unexpected development. The diffusion of gender quotas in France thus offers an illuminating example of under which conditions women's policy agencies can act autonomously to diffuse and impose a new tool for gender equality

PURPOSE: Advanced Practice Lung Cancer Nurses (APLCN) are well-established in several countries but their role has yet to be established in Switzerland. Developing an innovative nursing role requires a structured approach to guide successful implementation and to meet the overarching goal of improved nursing sensitive patient outcomes. The "Participatory, Evidence-based, Patient-focused process, for guiding the development, implementation, and evaluation of advanced practice nursing" (PEPPA framework) is one approach that was developed in the context of the Canadian health system. The purpose of this article is to describe the development of an APLCN model at a Swiss Academic Medical Center as part of a specialized Thoracic Cancer Center and to evaluate the applicability of PEPPA framework in this process. METHOD: In order to develop and implement the APLCN role, we applied the first seven phases of the PEPPA framework. RESULTS: This article spreads the applicability of the PEPPA framework for an APLCN development. This framework allowed us to i) identify key components of an APLCN model responsive to lung cancer patients' health needs, ii) identify role facilitators and barriers, iii) implement the APLCN role and iv) design a feasibility study of this new role. CONCLUSIONS: The PEPPA framework provides a structured process for implementing novel Advanced Practice Nursing roles in a local context, particularly where such roles are in their infancy. Two key points in the process include assessing patients' health needs and involving key stakeholders.

Automation was introduced many years ago in several diagnostic disciplines such as chemistry, haematology and molecular biology. The first laboratory automation system for clinical bacteriology was released in 2006, and it rapidly proved its value by increasing productivity, allowing a continuous increase in sample volumes despite limited budgets and personnel shortages. Today, two major manufacturers, BD Kiestra and Copan, are commercializing partial or complete laboratory automation systems for bacteriology. The laboratory automation systems are rapidly evolving to provide improved hardware and software solutions to optimize laboratory efficiency. However, the complex parameters of the laboratory and automation systems must be considered to determine the best system for each given laboratory. We address several topics on laboratory automation that may help clinical bacteriologists to understand the particularities and operative modalities of the different systems. We present (a) a comparison of the engineering and technical features of the various elements composing the two different automated systems currently available, (b) the system workflows of partial and complete laboratory automation, which define the basis for laboratory reorganization required to optimize system efficiency, (c) the concept of digital imaging and telebacteriology, (d) the connectivity of laboratory automation to the laboratory information system, (e) the general advantages and disadvantages as well as the expected impacts provided by laboratory automation and (f) the laboratory data required to conduct a workflow assessment to determine the best configuration of an automated system for the laboratory activities and specificities.

Chlamydial infections in koalas can cause life-threatening diseases leading to blindness and sterility. However, little is known about the systemic spread of chlamydiae in the inner organs of the koala, and data concerning related pathological organ lesions are limited. The aim of this study was to perform a thorough investigation of organs from 23 koalas and to correlate their histopathological lesions to molecular chlamydial detection. To reach this goal, 246 formalin-fixed and paraffin embedded organ samples from 23 koalas were investigated by histopathology, Chlamydiaceae real-time PCR and immunohistochemistry, ArrayTube Microarray for Chlamydiaceae species identification as well as Chlamydiales real-time PCR and sequencing. By PCR, two koalas were positive for Chlamydia pecorum whereas immunohistochemical labelling for Chlamydiaceae was detected in 10 tissues out of nine koalas. The majority of these (n=6) had positive labelling in the urogenital tract related to histopathological lesions such as cystitis, endometritis, pyelonephritis and prostatitis. Somehow unexpected was the positive labelling in the gastrointestinal tract including the cloaca as well as in lung and spleen indicating systemic spread of infection. Uncultured Chlamydiales were detected in several organs of seven koalas by PCR, and four of these suffered from plasmacytic enteritis of unknown aetiology. Whether the finding of Chlamydia-like organisms in the gastrointestinal tract is linked to plasmacytic enteritis is unclear and remains speculative. However, as recently shown in a mouse model, the gastrointestinal tract might play a role being the site for persistent chlamydial infections and being a source for reinfection of the genital tract.

Comparative genomics of several strains of Erwinia amylovora, a plant pathogenic bacterium causal agent of fire blight disease, revealed that its diversity is primarily attributable to the flexible genome comprised of plasmids. We recently identified and sequenced in full a novel 65.8 kb plasmid, called pEI70. Annotation revealed a lack of known virulence-related genes, but found evidence for a unique integrative conjugative element related to that of other plant and human pathogens. Comparative analyses using BLASTN showed that pEI70 is almost entirely included in plasmid pEB102 from E. billingiae, an epiphytic Erwinia of pome fruits, with sequence identities superior to 98%. A duplex PCR assay was developed to survey the prevalence of plasmid pEI70 and also that of pEA29, which had previously been described in several E. amylovora strains. Plasmid pEI70 was found widely dispersed across Europe with frequencies of 5–92%, but it was absent in E. amylovora analyzed populations from outside of Europe. Restriction analysis and hybridization demonstrated that this plasmid was identical in at least 13 strains. Curing E. amylovora strains of pEI70 reduced their aggressiveness on pear, and introducing pEI70 into low-aggressiveness strains lacking this plasmid increased symptoms development in this host. Discovery of this novel plasmid offers new insights into the biogeography, evolution and virulence determinants in E. amylovora
