A presente investigação situa-se na área da integração da Educação Profissional com Educação Básica na Modalidade de Jovens e Adultos (PROEJA) e busca a compreensão das vivências dos estudantes do curso no Câmpus Rio Grande que integra o Instituto Federal de Educação, Ciência e Tecnologia do Rio Grande do Sul (IFRS-RG). Faz parte também dos objetivos identificar as contribuições do curso para a socialização desses estudantes e os significados atribuídos ao curso. Em função da elevada taxa de evasão, a pesquisa foi desenvolvida com estudantes que não concluíram o curso. A hermenêutica filosófica constitui-se na epistemologia que ancora teoricamente a abordagem metodológica. A Análise Textual Discursiva foi a metodologia qualitativa adotada para a análise do material empírico constituído centralmente pelas histórias contadas pelos estudantes. Eles contam sua passagem pelo PROEJA, desde o ingresso até a sua evasão. Foram produzidos dados quantitativos além de levantamentos para conhecer o contexto da trajetória escolar dos estudantes. Esses elementos subsidiaram a análise qualitativa das informações. Como resultados foram encontradas: uma taxa de evasão geral da ordem de 57%. Nos dois primeiros módulos ocorreu 87 % de toda a evasão do curso. Em todo o período de oferecimento do curso, ou seja sete anos, apenas três estudantes concluíram o mesmo. Pela análise qualitativa, emergiram cinco categorias que tratam: a) do ingresso no PROEJA e dos desencontros nesse processo; b) do estudante com suas peculiaridades, dificuldades e significados; c) do ser professor no PROEJA; d) do curso pelo viés da formação integrada; e) dos componentes constituintes da evasão. Como conclusões a pesquisa aponta para a inadequação do curso em seus diversos aspectos. Reforça que a construção curricular adaptada de um curso existente foi um equívoco. A formação dos professores e gestores promoveu a proposta de um curso repleto de contradições. Mesmo com todos os distanciamentos, o curso promoveu um ensino de qualidade na formação humana e por isso os estudantes desenvolveram sentimentos positivos quanto ao curso. Promoveu os estudantes a novas oportunidades na sua vida social, inclusive com ingresso na universidade. A tese geral aponta para a potencialidade do PROEJA do Câmpus Rio Grande, na transformação da realidade do sujeito, apesar do alto índice de evasão e da inadequação do curso.

Abstract The developmental changes during adolescence may affect subsequent risk for diseases and health-related behaviors. Traditionally, professionals assume that knowledge is sufficient for behavioral changes; however adolescents continue to engage in unhealthy behaviors despite clearly knowing what they should do and how to change. "What is lacking is the motivation to apply that knowledge". Motivational Interview (MI) may be taken as an essential tool in the provision of nursing care to adolescents, being itself a workspace with possible therapeutic effects. Objectives The objective of this scoping is to examine and map the use of MI by nurses in their clinical practice with adolescents to promote health behaviors. The review will focus on knowing what is the current extent of the use of nurse-led MI; which adolescent populations were included; in which contexts nurses use MI; which MI techniques/strategies have been used and what outcomes on health behaviors promotion have been reported. Methodology This scoping review will be informed by JBI methodology. The population of this study is adolescents aged 10 to 19 years participating in nurse-led MI. The concept of MI include MI done by personal or telephone call, with any number of sessions, brief interventions and other motivational interventions grounded but not limited to the principles described by Miller & Rollnick (2008). All geographical and all clinical practice contexts where nurses' undertake MI with adolescents such as hospitals, primary health care, health care centers, community or schools will be contemplated. English, Spanish and Portuguese published studies will be considered for inclusion. Results An initial limited search of MEDLINE and CINAHL was undertaken followed by analysis of the text words contained in the title and abstract, and of the index terms used to describe the concepts, synonyms (with truncations), MeSH Terms and Cinhal headings of this study. It was identified 5 synonyms for "Adolescents", 7 for "MI" and 2 for "nurse". A first search using the all 14 identified keywords and index terms was made at Medline (Title/Abstract) and brought up 125 articles. Other 16 databases referenced at the protocol will be searched to identify additional studies. Articles identified from the final search will be assessed for relevance to the review, based on information provided in the title and abstract. The full article will be retrieved for all studies that meet the inclusion criteria of the review. It is expected that findings from this Scoping Review provide needed information to nurses related to the use of MI to promote health behaviors in adolescents. Conclusions There is little knowledge of what works for whom (which adolescent subpopulation) under what circumstances (in which setting, for what problem) in relation to nurse-led MI. There is a need for scoping or mapping the nurse-led MI with adolescents to identify evidence gaps and to inform opportunities for future development in nursing practice. Moreover, information regarding implemented and evaluated interventions, techniques used, contexts of application and adolescents groups is dispersed in the literature which impedes the formulation of questions about the outcomes and effectiveness of those interventions. The practical implication of this mapping will be clarifying all these aspects.

Introdução The hospitalization of a child is an experience that causes big changes in child and his family life. The parents often suffer from stress and anxiety. This can affect their relationship with the child. Because of the closeness to the parents, nurses have an important role in giving parents support so they can reduce their stress and have more energy to support and take care of their children and in the inclusion of the family in the process of care Objectivos The aim of this study was to define what family-centered care is, to define the needs of hospitalized children's parents and to identify the strategies and methods that the nurses use to give parental support adapted to the parent's needs. It was also a goal to identify and understand the main differences between parental support given by nurses in Belgium and Portugal. Metodologia The study exists out of two parts. First is an integrative review of literature. The search was performed using the databases MEDLINE, CINAHL , PubMed and Science Direct. 18 Articles were selected based on inclusion and exclusion criteria. They had to involve nurses, hospitalized children between 0 and 18 years and their parents. Second part was a focus group. The participants were pediatric nurses from Portugal and Belgium. The goal was to understand different perspectives related to the parental needs of hospitalized children and nursing interventions to answer that needs. Resultados family-centered care can be considered as a partnership between family and nurses. It has some general principles: information sharing, respect differences, negotiation and care in the context of the family. Parent's participation is important to reduce the parental stress and it is essential for meeting the needs of the children. Parents have different needs: knowledge and communication, support, comfort, proximity and assurance. Parents cope with stress in different ways and nurses can support them while they are in the hospital. It is a nurse task to identify the stressors and know methods of emotional support, so she can protect the family structure. Nurses should always see the family as a path to the child, with whom the nurses should worry about taking care and meeting their needs. This is crucial to ensure the family's well-being, adaption to hospitalization and the child's recovery. Conclusões Nurses should collect information about the family which includes family relationships, cultural and religious habits and familiar dynamic. Parents need interpersonal emotional support. It is important for parents to be close to their children but they also need to take care of themselves. When nurses have enough information they can use it to the identification of parental needs and the planning of nursing interventions. It is important that nurses create an environment where parents feel safe and that they have privacy. To create a therapeutic and professional relationship efficient communication is needed. Parents will experience less stress and anxiety.

Introdução O transporte de crianças em ambulâncias terrestres é um tema pouco estudado em todo o mundo. A ambulância apresenta um ambiente singular com problemas únicos como a proteção dos ocupantes transportados em diferentes posições. Ao contrário das diretrizes de transporte da criança em veículos automóveis, faltam especificações consistentes no transporte de crianças em ambulâncias. Os enfermeiros fazem diariamente transferências de crianças entre serviços de saúde, por isso, a gestão da segurança deste transporte deve ser motivo de preocupação. Objetivos Com este trabalho pretende-se conhecer as medidas de segurança utilizadas no transporte de crianças em ambulâncias terrestres por enfermeiros e bombeiros/tripulantes portugueses e identificar o conhecimento que estes profissionais têm acerca das medidas de segurança ideais para este tipo de transporte; e verificar possíveis associações entre as práticas de transporte e as caraterísticas sociodemográficas dos profissionais que o efetuam. Metodologia Estudo exploratório descritivo de abordagem quantitativa. O questionário usado foi construído tendo por base as recomendações da NHTSA (National Highway Traffic Safety Administration) para 4 possíveis situações de transporte e abrangendo 5 faixas etárias pediátricas e permite avaliar as medidas de segurança usadas no transporte de crianças em ambulâncias terrestres. Obteve-se uma amostra de 135 enfermeiros e bombeiros/tripulantes de ambulâncias portuguesas, 51,1% do género feminino e 71,1% com idade inferior a 40 anos. Dos inquiridos, 53,3% são Enfermeiros (n = 72) e destes 56,9% trabalham em serviços de pediatria. Resultados Os resultados mostram uma grande variedade de medidas de segurança utilizadas na prática dos inquiridos e uma diferença significativa entre a prática de transporte e a forma que consideram ser a ideal. Além disso, as formas de transporte avaliadas situam-se mais próximas dos níveis aceitáveis de transporte do que dos níveis recomendados como ideais pela NHTSA. As situações em que o transporte é feito do modo menos seguro são o transporte de uma criança que não está doente nem ferida mas que tem de acompanhar uma pessoa que precisa de cuidados, pois não pode ficar sozinha e o transporte de uma criança doente e/ou ferida mas cuja condição não requer monitorização e/ou intervenção contínua e/ou intensiva. Variáveis, como o género, as habilitações literárias, profissão e serviço dos profissionais parecem influenciar opções de transporte mais seguras. Assim, as mulheres, profissionais com habilitações literárias superiores e enfermeiros que exercem nos serviços de pediatria parecem transportar com mais segurança as crianças em ambulâncias. Conclusões Apesar de demonstrados os riscos neste tipo de transporte continua a haver escassas diretrizes e regulamentos de segurança no transporte de crianças nas ambulâncias. Muitos profissionais referiram não saber qual a possibilidade de transporte mais seguro para as crianças nem conhecer recomendações para este tipo de transporte. A própria dispersão apurada na forma como o transporte é efetuado e os resultados obtidos sugerem a necessidade de regulamentação deste transporte, de investimento na formação dos profissionais e de sensibilização das instituições de saúde para a importância do uso de Sistemas de Retenção para Crianças durante o transporte de crianças.

Adolescents - defined as young people between 10 and 19 years of age1 - are, in general, a relatively healthy segment of the population.2 However, the developmental changes that take place during adolescence may affect their subsequent risk for diseases and for a variety of health-related behaviors. In fact, early onset of preventable health problems (e.g. obesity, malnutrition, STDs) and the engagement in health risk behaviors (e.g., sedentary life style, excessive alcohol consumption, unprotected sex) during adolescence, are likely to put them at greater risk for physical and mental health problems at a later stage in life. Moreover, health related problems and health risk behaviors may disrupt adolescents' physical and cognitive development and therefore may affect their ability to think and act in relation to decisions about their health in the future.1 In summary, health-related behaviors in adolescence, apart from their influence on the continuum of "health-disease", they also have the potential to influence future behaviors. In fact, several studies have shown that past behaviors are good predictors of future behaviors .3,4 Thus, promoting healthy practices during adolescence and taking measures to better protect young people from health risks are essential for the prevention of health problems in adulthood.5 According to the World Health Organization, the main problems affecting young people include mental health problems (such as behavioral disorders, eating disorders, suicide, anxiety or depression), the use of substances (illegal substances, alcohol and tobacco), interpersonal violence, nutrition (a proper nutrition consists of healthy eating habits and physical exercise), unintentional injuries (which are a leading cause of death and disability among young people, with road traffic injuries accounting for about 700 deaths per day), sexual and reproductive health (for example, risky sexual behaviors, early pregnancy and childbirth) and HIV (resulting from sexual transmission and drug injection).5,6 On the other hand, the number of children and youth with chronic health conditions has increased dramatically in the past four decades7 as larger numbers of chronically ill children survive beyond the age of 10.8 Despite the lack of data on adolescents' health making it difficult to determine the prevalence of chronic illnesses in this age group9, it is known that one in ten adolescents suffers from a chronic condition worldwide.10 In fact, national population based studies from Western countries show that 20-30% of teenagers have a chronic illness, defined as one that lasts longer than six months.8 The most prevalent chronic illness among adolescents is asthma and the one with the highest incidence is diabetes mellitus, particularly type II.9 Traditionally, healthcare professionals have been mainly investing in health education activities, through the transmission of knowledge with a view to creating habits, customs and behaviors, and promoting healthy lifestyles. However, empowering people does not only consist of giving them the right information11 , i.e. good information is not enough to cause people to make changes.12 The motivation or desire to change unhealthy behaviors and habits depends on many factors, namely intrinsic motivation, control over personal decisions, self-confidence and perception of effectiveness, personal ambivalence, and individualized assistance.12 Many professionals assume that supplying knowledge is sufficient for behavioral changes; however, even very good advice often fails to generate behavioral change. After all, people continue to engage in unhealthy behaviors despite clearly knowing what they should do and how to change. "What is lacking is the motivation to apply that knowledge".13, p.1233 In fact, behavioral change is a complex phenomenon with multiple determinants that also includes motivational variables. It is associated with ambivalent processes expressed in the dilemma between keeping the current status and moving on to new ways of acting. For example, telling adolescents that if they keep on engaging in a certain behavior, they are increasing the risk of developing a long-term condition such as cardiovascular disease, stroke or diabetes is rarely enough to trigger the desired behavioral change; people are more likely to change when they believe that the change is really effective and that they are able to implement it.12 Therefore, it is essential to provide specific training for "healthcare professionals to master motivational techniques, avoid confrontation with the users, and facilitate behavioral changes".14 In this context, motivating patients to make behavioral changes is also an important nursing task where change in lifestyle is a major element of patients' treatment and preventive interventions.15 One of the nurse's goals is to help improve a patient's health or help them to manage existing health conditions. Once nurses are in a position where they have to focus on accomplishing tasks and telling patients what needs to be accomplished16, the role of the nurse is expanding even more into the use of motivational strategies.17 MI is bringing nurses back to therapeutic communication and moving them closer to successful health promotion and disease management, by promoting behavior change and empowering their patients. As the nursing profession evolves, MI is seen as a challenge and the basis of nurse's interactions with individuals, families and communities.16, 17 In the same way, MI may be taken as an essential tool in the provision of nursing care to adolescents, being itself a workspace with possible therapeutic effects regarding problems, clarification of doubts, and development of skills.18 In fact, MI may be particularly applicable in work with adolescents because of their specific developmental stage. Adolescents attempt to establish their own autonomy and identity while struggling with social interactions and moral issues, which leads to ambivalence.19 Consistent with the developmental challenges during adolescence, "MI explicitly honors autonomy, people's right and irrevocable ability to decide about their own behavior"20 while allowing the person to explore possibilities for change of risky or maladaptive behaviours.19 MI can be defined as a directive, client-centred counselling style for eliciting behavior change by helping clients to explore and resolve ambivalence. It is most centrally defined not by technique but by its spirit as a facilitative style of interpersonal relationship.21 It is a set of strategies and techniques widely used in clinical practice based on the transtheoretical model of change. The Stages of Change model describes five stages of readiness—precontemplation, contemplation, preparation, action, and maintenance—and provides a framework for understanding behavior change.22 The MI has been widely tested and applied in different areas, such as modification of addictive behaviors, interventions with offenders in the context of justice, eating disorders, promotion of therapeutic adherence among chronic patients, promotion of learning in school settings or intervention with adolescents at risk.18,23 In general, clinical practice has been adopting the perspective of motivation as something relatively immutable, i.e., the adolescent is either motivated for change/treatment and, in these conditions, the professional's role is to help him/her, or the adolescent is not motivated and then change/treatment is not feasible. Alternatively the theoretical model underlying the MI technique postulates that the individual's adherence to change/treatment depends on his/her motivation, which can change throughout the therapeutic intervention. As several studies found positive results for effects of MI24-26 and its use by health professionals is encouraged23,27 nurses may play an important role in patients' process of change. As nurses have a crucial role in clinical contexts, they can facilitate the process of ending risk behaviors and/or adopting positive health behaviors through some motivational techniques, namely with adolescents. A considerable number of systematic reviews about MI already exist pointing to some benefits of its use in the treatment of a broad range of behavioral problems and diseases.13,28,29 Some of the current reviews focus on examining the effectiveness of MI for adolescents with diverse health risks/problems 30-32. However, to date there are no reviews that present and assess the evidence for the use of nurse-led MI in adolescents. Therefore, we have little knowledge of what works for whom (which adolescent subpopulation) under what circumstances (in which setting, for what problem) in relation to motivational interviewing by nurses. There is a clear need for scoping or mapping the use of MI by nurses with adolescents to identify evidence gaps and to inform opportunities for future development in nursing practice. On the other hand, information regarding nurse-led implemented and evaluated interventions, techniques and/or strategies used, contexts of application and adolescents subpopulation groups is dispersed in the literature33-36 which impedes the formulation of precise questions about the effectiveness of those interventions conducted by nurses and therefore the realization of a systematic review. In other words, it is known that different kind of motivational interventions have been implemented in different contexts by nurses, however does not exist a map about all the motivational techniques and/or strategies used. Furthermore the literature does not clarify which is the role of nurses at cross professional motivational intervention implemented programs and finally the outcomes and evaluation of interventions are unclear. Thus, the practical implication of this mapping will be clarifying all these aspects. Without this clarification is not possible to proceed to the realization of a systematic review about the effectiveness of the use of motivational interviews by nurses to promote health behaviors in adolescents, in a particular context and/or health risk behavior; or regarding the effectiveness of certain technique and/or strategy of MI. Consequently, there are important questions about the nature of the evidence in this area that need to be answered before formulating a precise question of effectiveness. This scoping review aims to respond to these questions. An initial search of the JBI Database of Systematic Reviews & Implementation Reports, Cochrane Database of Systematic Reviews, , Database of promoting health effectiveness reviews (DoPHER), The Campbell Library, Medline and CINAHL, has revealed that currently there is no Scoping Review (published or in progress) on the subject. In this context, this scoping review will examine and map the published and unpublished research around the use of MI by nurses implemented and evaluated to promote health behaviors in adolescents; to establish its current extent, range and nature and identify its feasibility, outcomes and gaps in the evidence defining research priorities in this field. This scoping review will be informed by the JBI methodology37 that suggests a five stage methodological framework for conducting scoping reviews which includes: identifying the research question, searching for relevant studies, selecting studies, charting data, collating, summarizing and reporting the results.

Background: The transport of children in ground ambulances is a rarely studied topic worldwide. The ambulance vehicle is a unique and complex environment with particular challenges for the safe, correct and effective transportation of patients. Unlike the well developed and readily available guidelines on the safe transportation of a child in motor vehicles, there is a lack on consistent specifications for transporting children in ambulances. Nurses are called daily to transfer children to hospitals or other care centers, so safe transport practices should be a major concern. Purpose: to know which are the safety precautions and specific measures used in the transport of children in ground ambulances by nurses and firefighters and to identify what knowledge these professionals had about safe modes of children transportation in ground ambulances. Methods: In this context, an exploratory - descriptive study and quantitative analysis was conducted. A questionnaire was completed by 135 nurses and firefighters / ambulance crew based on 4 possible children transport scenarios proposed by the NHTSA (National Highway Traffic Safety Administration) and covered 5 different children´s age groups (new born children, 1 to 12 months; 1 to 3 years old; 4 to 7 years old and 8 to 12 years old). Results: The main results showed a variety of safety measures used by the professionals and a significant difference between their actual mode of transportation and the mode they consider to be the ideal considering security goals. In addition, findings showed that achieved scores related to what ambulance crews do in the considered scenarios reflect mostly satisfactory levels of transportation rather than optimum levels of safety, according to NHTSA recommendations. Variables as gender, educational qualifications, occupational group and local where professionals work seem to influence the transport options. Female professionals and nurses from pediatric units appear to do a safer transportation of children in ground ambulances than other professionals. Conclusion: Several professionals refereed unawareness of the safest transportation options for children in ambulances and did not to know the existence of specific recommendations for this type of transportation. The dispersion of the results suggests the need for investment in professional training and further regulation for this type of transportation.

Objetives A scoping review has been currently undertaking to find out what is the current extent of the use of nurse-led MI; which adolescent populations were included; in which contexts nurses use MI; which MI techniques/strategies have been used and what outcomes on health behaviors promotion have been reported. The first task was to develop a search strategy to identify relevant studies for this review. It is described here the experience in constructing a search strategy for this review. Methodology This scoping review will be informed by JBI methodology. An initial search of MEDLINE and CINAHL was undertaken. An analysis of the text words contained in the title and abstract, and of the index terms used to describe the searched articles were retrieved and these were then used to construct a search strategy for use in Medline and Cinahl. Results Analysis of the text words and the index terms generated 19 keywords: It was identified 5 synonyms for "Adolescents", 12 for "MI" and 2 for "nurse". A new research formula was designed using the text words identified. Ao contrário das diretrizes de transporte da criança em veículos automóveis, faltam especificações consistentes no transporte de crianças em ambulâncias. Os enfermeiros fazem diariamente transferências de crianças entre serviços de saúde, por isso, a gestão da segurança deste transporte deve ser motivo de preocupação. Objetivos Com este trabalho pretende-se conhecer as medidas de segurança utilizadas no transporte de crianças em ambulâncias terrestres por enfermeiros e bombeiros/tripulantes portugueses e identificar o conhecimento que estes profissionais têm acerca das medidas de segurança ideais para este tipo de transporte; e verificar possíveis associações entre as práticas de transporte e as caraterísticas sociodemográficas dos profissionais que o efetuam. Metodologia Estudo exploratório descritivo de abordagem quantitativa. O questionário usado foi construído tendo por base as recomendações da NHTSA (National Highway Traffic Safety Administration) para 4 possíveis situações de transporte e abrangendo 5 faixas etárias pediátricas e permite avaliar as medidas de segurança usadas no transporte de crianças em ambulâncias terrestres. Obteve-se uma amostra de 135 enfermeiros e bombeiros/tripulantes de ambulâncias portuguesas, 51,1% do género feminino e 71,1% com idade inferior a 40 anos. Dos inquiridos, 53,3% são Enfermeiros (n = 72) e destes 56,9% trabalham em serviços de pediatria. Resultados Os resultados mostram uma grande variedade de medidas de segurança utilizadas na prática dos inquiridos e uma diferença significativa entre a prática de transporte e a forma que consideram ser a ideal. Além disso, as formas de transporte avaliadas situam-se mais próximas dos níveis aceitáveis de transporte do que dos níveis recomendados como ideais pela NHTSA. As situações em que o transporte é feito do modo menos seguro são o transporte de uma criança que não está doente nem ferida mas que tem de acompanhar uma pessoa que precisa de cuidados, pois não pode ficar sozinha e o transporte de uma criança doente e/ou ferida mas cuja condição não requer monitorização e/ou intervenção contínua e/ou intensiva. Variáveis, como o género, as habilitações literárias, profissão e serviço dos profissionais parecem influenciar opções de transporte mais seguras. Assim, as mulheres, profissionais com habilitações literárias superiores e enfermeiros que exercem nos serviços de pediatria parecem transportar com mais segurança as crianças em ambulâncias. Conclusões Apesar de demonstrados os riscos neste tipo de transporte continua a haver escassas diretrizes e regulamentos de segurança no transporte de crianças nas ambulâncias. Muitos profissionais referiram não saber qual a possibilidade de transporte mais seguro para as crianças nem conhecer recomendações para este tipo de transporte. A própria dispersão apurada na forma como o transporte é efetuado e os resultados obtidos sugerem a necessidade de regulamentação deste transporte, de investimento na formação dos profissionais e de sensibilização das instituições de saúde para a importância do uso de Sistemas de Retenção para Crianças durante o transporte de crianças.

Enquadramento: O intercâmbio académico internacional possibilita novos conhecimentos científicos e culturais, incluindo tecnologias em saúde, partilha de saberes e experiências e ampliação das oportunidades profissionais. Objetivos: Identificar os benefícios do intercâmbio para a formação do enfermeiro, na perspetiva dos estudantes. Metodologia: Pesquisa exploratório-descritiva de cariz qualitativo. Participaram no estudo 17 estudantes que realizaram programas de mobilidade internacional no Centro Universitário Franciscano - Santa Maria/RS/Brasil e na Escola Superior de Enfermagem de Coimbra/Portugal. Os dados foram colhidos através de entrevistas semiestruturadas e submetidos à análise temática de conteúdo. Resultados: A análise dos dados resultou em 3 categorias: A saúde no país de origem vs. país de intercâmbio - contextualizando saberes e vivências profissionais, Intercâmbio académico - possibilidade de desenvolvimento da autonomia dos estudantes e, Ampliando horizontes - contribuições culturais no processo do intercâmbio académico. Conclusão: Os processos de intercâmbio permitem a aproximação dos estudantes a novas realidades práticas e de investigação, o que contribui para a formação do estudante e um melhor desempenho profissional como enfermeiro, no futuro.

os Objetivos de Desarrollo Sostenible (ODS) guiarán las decisiones que adoptemos durante los próximos 15 años y en ellos se lograron avances en el marco de los Objetivos de Desarrollo del Milenio (ODM) cuyos objetivos con la salud materna, neonatal e infantil y con la salud reproductiva distan de haber sido alcanzados. La mortalidad infantil es importante porque es un indicador de la utilización, accesibilidad y disponibilidad del sistema de salud de los habitantes, especialmente de los niños. La tasa de mortalidad infantil ha reducido a la mitad en 25 años (1990 a 2015) más la meta mundial del ODM de una reducción de 2/3 no se alcanzó. En 2015 la mayoría de las muertes en niños menores de cinco años fueron causadas por enfermedades neonatales, infecciosas o de nutrición. Estas muertes se pueden prevenir y tratar fácilmente en casa o en los centros de salud. Las enfermeras y parteras están en la primera línea de la prestación de estos servicios. En muchos países son líderes o actores clave en los equipos de salud interdisciplinarios. Ofrecen una amplia gama de servicios de enfermería y partería en todos los niveles del sistema de salud. Las Naciones Unidas piden la equidad en salud a través de la cobertura de salud universal, de modo que todos los niños puedan acceder a los servicios de salud esenciales. Pero esto sólo se puede lograr con el apoyo de los recursos humanos de salud, y de enfermería en particular. La investigación ha demostrado que más de la mitad de los casi 11 millones de muertes de niños/año, podrían salvarse mediante medidas como vacunas, mejora de la atención de la familia y las prácticas de lactancia, promover la planificación familiar y la educación de las mujeres. Para la lograr; la calidad, cantidad y relevancia de la fuerza de trabajo de enfermería y obstetricia tendrán que ser asegurado. La práctica de enfermería, a través de su experiencia directa y intervenciones terapéuticas centradas en el cliente y capaz de proporcionar cuidados eficaces de acuerdo con los contextos de salud y enfermedad es fundamental. Para alcanzar los ODS se necesitan datos sobre los recursos humanos de salud. La investigación de enfermería es decisiva en la creación y explicación de datos que incluyen los números, competencias, lugares de práctica y calidad de los resultados en el plano local, nacional e internacional. También es necesario documentación sobre la función y el trabajo de las enfermeras en la creación, soporte y mantenimiento de los sistemas de salud. Por fin, las políticas relacionadas con los recursos humanos deben estar basadas en la evidencia, y las enfermeras no se pueden olvidar su papel clave en las mesas de política.

, las Naciones Unidas nos dicen que la salud es un bien que debe alcanzarse por el valor que representa en sí mismo, no obstante, la Agenda nos recuerda que la salud es también un medio para alcanzar los otros objetivos y un indicador fiable de los progresos logrados a escala mundial en favor del desarrollo sostenible. Por esta razón, podemos decir que las 169 METAS de los 17 objetivos de desarrollo sostenible están conectadas y son de carácter integrado e indivisible - El objetivo relativo a la salud incluye nuevas metas para cuestiones importantes en las que se lograron grandes avances en el marco de los Objetivos de Desarrollo do Milenio. Se han reducido las epidemias mundiales de tuberculosis y paludismo y la causada por el VIH. A nivel mundial, la mortalidad infantil y la mortalidad materna han disminuido drásticamente desde 1990, con descensos del 53% y el 40%, respectivamente. Pero aún queda mucho camino por recorrer. A menudo, los informes sobre los progresos realizados a nivel mundial no reflejan las distintas velocidades con que avanzan los países y las regiones dentro de un mismo país. En la actualidad se reconoce la necesidad no solo de que las personas logren sobrevivir, sino también de que prosperen en la vida.Las enfermeras son vistos como profesionales de confianza que tienen la capacidad de influir en los elementos de la reforma de salud . Por esso, los ODS representan un desafío y una oportunidad para fortalecer la contribución de la enfermería para alcanzarlos.

The discovery of antibiotics was a major breakthrough in medicine. However, short after their introduction in clinical practice resistant bacteria were detected. Nowadays, antibiotic resistance constitutes a serious public health problem. In hospital settings, with high resistance levels, reducing drastically the therapeutic options. Carbapenems are last-resort antibiotics used in Portugal, only in hospitals, to treat serious infections. Bacterial resistance towards this class of antibiotics has increased during last years. In Gram-negative bacteria the production of carbapenemases is a common resistance mechanism. OXA-48 is a carbapenemase of Ambler class D and represents a major concern for human health. It is frequently detected in clinical isolates of Enterobacteriaceae. There are few studies suggesting that genes encoding for OXA-48 variants originated from genes present in the chromosome of members of genus Shewanella, and have disseminated to Enterobacteriaceae members, associated with mobile genetic elements. The aim of this study was to characterize strains from different sources of Shewanella to confirm its role as OXA-48 progenitor. For this, the phylogenetic affiliation of 33 strains of Shewanella was performed by 16SrDNA and gyrB sequencing. The most common species were S. hafniensis and S. xiamenensis, but also S. aestuarii, S. baltica, S. indica, S. haliotis, S. putrefaciens, S. algidipiscicola, S. irciniae, S. algae and S. fodinae were identified. blaOXA-48-like genes were detected in 21 isolates: S. hafniensis (8/8), S. xiamenensis (5/5), S. baltica (4/4), S. algae (1/1), S. fodinae (1/1), S. putrefaciens (1/2) and S. algidipiscicola (1/2). Sequence analysis revealed that genes encoded enzymes identical to OXA-48, OXA-181 and OXA-204 but also new variants differing from OXA-48 from 2 to 81 aminoacids. Genetic context analysis revealed the C15 gene upstream and lysR gene downstream, identical to what has been identified so far flanking blaOXA-48-like genes in Shewanella spp. The assessment of antibiotic susceptibility was performed for all isolates using the disk diffusion method. In general, it was observed a great sensitivity for all antibiotics except to amoxicillin and aztreonam. Multidrug resistance was detected in only 1 isolate. Other resistance genes and the presence of integrons were not identified. Plasmids were detected in 30.3% isolates (10/ 33). These results reinforce the role of Shewanella spp. as origin of blaOXA-48-like genes.
