INTRODUCTION: The prevalence and intensity of geohelminth infections and schistosomiasis remain high in the rural areas of Zona da Mata, Pernambuco (ZMP), Brazil, where these parasites still represent a significant public health problem. The present study aimed to spatially assess the occurrences of schistosomiasis and geohelminthiasis in the ZMP. METHODS: The ZMP has a population of 1,132,544 inhabitants, formed by 43 municipalities. An ecological study was conducted, using secondary data relating to positive human cases and parasite loads of schistosomiasis and positive human cases of geohelminthiasis that were worked up in Excel 2007. We used the coordinates of the municipal headquarters to represent the cities which served as the unit of analysis of this study. The Kernel estimator was used to spatially analyze the data and identify distribution patterns and case densities, with analysis done in ArcGIS software. RESULTS: Spatial analysis from the Kernel intensity estimator made it possible to construct density maps showing that the northern ZMP was the region with the greatest number of children infected with parasites and the populations most intensely infected by Schistosoma mansoni. In relation to geohelminths, there was higher spatial distribution of cases of Ascaris lumbricoides and Trichuris trichiura in the southern ZMP, and greater occurrence of hookworms in the northern/central ZMP. CONCLUSIONS: Despite several surveys and studies showing occurrences of schistosomiasis and geohelminthiasis in the ZMP, no preventive measures that are known to have been effective in decreasing these health hazards have yet been implemented in the endemic area.

Introduction This study evaluates the factors associated with the development of severe periportal fibrosis in patients with Schistosoma mansoni. Methods A cross-sectional study was conducted from April to December 2012 involving 178 patients infected with S. mansoni who were treated in the Hospital das Clínicas of Pernambuco, Brazil. Information regarding risk factors was obtained using a questionnaire. Based on the patients' epidemiological history, clinical examination, and upper abdomen ultrasound evaluation, patients were divided into 2 groups: 137 with evidence of severe periportal fibrosis and 41 patients without fibrosis or with mild or moderate periportal fibrosis. Univariate and multivariate analyses were conducted using EpiInfo software version 3.5.5. Results Illiterate individuals (30.1%) and patients who had more frequent contact with contaminated water in towns in the Zona da Mata of Pernambuco (33.2%) were at greater risk for severe periportal fibrosis. Based on multivariate analysis, it was determined that an education level of up to 11 years of study and specific prior treatment for schistosomiasis were preventive factors for severe periportal fibrosis. Conclusions The prevailing sites of the severe forms of periportal fibrosis are still within the Zona da Mata of Pernambuco, although there has been an expansion to urban areas and the state coast. Specific treatment and an increased level of education were identified as protective factors, indicating the need for implementing social, sanitary, and health education interventions aimed at schistosomiasis to combat the risk factors for this major public health problem.

Different aspects of hepatosplenic schistosomiasis are revisited here. Manson's schistosomiasis causes periportal fibrosis and portal hypertension in approximately 6% of infected subjects, usually with preservation of their hepatic function. The assessment of liver involvement is of major importance in determining the prognosis and risk of complications from schistosomiasis, such as upper digestive bleeding secondary to variceal rupture. For many years, the diagnosis of hepatosplenic schistosomiasis and liver fibrosis was made by abdominal palpation and the finding of liver and/or spleen enlargement. However, there is no consensus regarding the clinical parameters of the liver and spleen to be considered in this physical evaluation. For the last three decades, abdominal ultrasound (US) has become the best imaging technique to evaluate liver fibrosis caused by schistosomiasis mansoni. However, US is a subjective procedure and is therefore examiner-dependent. Magnetic resonance imaging (MRI) findings have provided valuable information in addition to ultrasound and clinical examination. The combination of a comprehensive history and physical examination, basic laboratory tests (a stool examination for Schistosoma mansoni eggs and a blood cell count), biomarkers for liver fibrosis/portal hypertension and imaging methods seem to offer the best approach for evaluating patients with this disease. In situations where research is involved or in patients with severe disease, MRI may be considered.

Introduction Schistosomiasis is a parasitic disease of public health concern in Brazil, and the construction of hydroelectric dams, in addition to increasing permanent human settlement and tourism, has created conditions suitable for the establishment of mollusks that can transmit schistosomiasis. Such areas require a number of actions to prevent the establishment of schistosomiasis. This paper reports on a freshwater malacological survey carried out in the geographical area of the Manso Power Plant. Methods Mollusks were collected in 18 municipalities in the State of Mato Grosso between February 2002 and February 2004 (qualitative study) and from April 2009 to February 2011 (quantitative study). Results Thirty-one species of mollusks were collected, including newly recorded species (Antillorbis nordestensis and Burnupia ingae). In addition, the geographic distributions of known species, including Biomphalaria straminea, a snail vector of Schistosoma mansoni, were expanded. A total of 4,507 specimens were collected in the APM Manso reservoir (Usina Hidrelétrica de Aproveitamento Múltiplo de Manso) during the quantitative study, and Biomphalaria amazonica was found in six of the 10 localities analyzed. The Afroasiatic species Melanoides tuberculata, introduced after February 2009, was the dominant species (relative abundance 94.96%). Conclusions The study area is epidemiologically important due to the occurrence of B. straminea and B. amazonica, which are vectors of schistosomiasis, and M. tuberculata, a snail host of Centrocestus formosanus, which is responsible for centrocestiasis transmission. Observations of M. tuberculata and the exotic freshwater clams Corbicula fluminea and Corbicula largillierti raise concerns about biodiversity.

Introduction This study evaluated the presence of pathogenic human parasites on field-grown strawberries in the Federal District of Brazil. Methods A total of 48 samples of strawberries and 48 soil samples from 16 properties were analyzed. Results Contaminated strawberries were detected in 56% of the properties. Schistosoma mansoni, Ascaris lumbricoides or Ascaris suum, Balantidium coli, Endolimax nana, and Entamoeba spp. were detected. Soil was contaminated with Entamoeba spp., Entamoeba coli, Strongyloides spp., Ancylostomatidae, and Hymenolepis nana. Conclusions Producers should be instructed on the safe handling of strawberries in order to reduce the incidence of strawberries that are contaminated with enteroparasites.

INTRODUCTION : We hypothesized higher mannose-binding lectin level and classic factors (i.e., age, sex, alcohol consumption, exposure, and specific treatment) are associated with the severity of periportal fibrosis in schistosomiasis. METHODS : This cross-sectional study involved 79 patients infected with Schistosoma mansoni with severe or mild/moderate periportal fibrosis. Serum concentrations of mannose-binding lectin were obtained by enzyme-linked immunosorbent assay (ELISA). RESULTS: Higher serum level of mannose-binding lectin was significantly associated with advanced periportal fibrosis. CONCLUSIONS: Mannose-binding lectin may contribute to liver pathology in schistosomiasis and may represent a risk factor for advanced periportal fibrosis in the Brazilian population studied.

AbstractDespite its infrequent occurrence, testicular schistosomiasis forming pseudo-tumors can be considered in the differential diagnosis of testicular tumors, especially in areas where the parasitic disease is endemic. In this report, we present a case of testicular schistosomiasis caused by Schistosoma mansoni and mimicking a testicular neoplasm. We describe the patterns of a testicular nodule on ultrasonography and magnetic resonance images in a 46-year-old man. The nodule was removed after a pre-operative diagnosis of a non-malignant lesion. Histology demonstrated granulomas with epithelioid macrophages and eosinophils around S. mansoni eggs within a fibrous tissue that formed a nodular structure.

Os hemócitos do caramujo Biomphalaria glabrata, um importante transmissor do trematódeo Schistosoma mansoni no Brasil, foram coletados de especimens na região Bragantina, localizada a oeste do estado do Pará. Os hemócitos foram examinados por meio de microscopia eletrônica de transmissão. As células foram fixadas pelo método de rotina com o uso do tampão PHEM (PIPES, HEPES, EGTA e Magnésio). Foram descritos os aspectos ultra-estruturais celulares como inclusões citoplasmáticas limitadas por membranas, mitocôndrias, retículos endoplasmáticos e outros. As observações mostram que esse tampão possui a propriedade de preservação do citoesqueleto celular, apresentando bons resultados na preservação das estruturas dos hemócitos e suas organelas.

Em levantamento malacológico realizado no município de Mariana, estado de Minas Gerais, Brasil, entre abril de 2003 e fevereiro de 2004 foram encontrados moluscos Biomphalaria glabrata (Say, 1818) e Physa marmorata Guilding, 1828 albergando larvas de trematódeos digenéticos. Em B. glabrata foram observadas além de cercária de Schistosoma mansoni Sambon, 1907, estrigeocercária, dois tipos de xifidiocercária, cercária ocelífera e dois tipos de gimnocéfala, enquanto em P. marmorata a única forma larvar emergente foi a cercária equinóstoma. Algumas das cercárias encontradas diferem daquelas descritas na literatura.

O A. apresenta os resultados do inquérito realizado no Município de Catendo, Estado de Pernambuco, afim de verificar o índice de infestação de moluscos do gênero Australorbis, pelas cercarias de Schistosoma mansoni, e experimentar a ação da cal extinta e do sulfato de cobre como recursos para a erradicação dêsses moluscos. O alto índice de infestação do homem, pelo Schistosoma mansoni (50% em mais de 300 exames) deve-se ao contato frequente da população com a água dos rios, para banhos e outros misteres. A cal extinta dá melhores resultados que o sulfato de cobre; além disso oferece vantagens de preço e facilidade de aquisição. Empregou-se a solução de cal de 4 a 5%.

Examination of 267.107 liver specimens obtained in Brazil by viscerotomy during from 1937 to 1946 inclusive revealed 5,953 Schistosoma mansoni infe¬ctions. This represents 2.23% ± 0.019 of the total number of livers studied. Data on the incidence of the disease is tabulated by states and municipios. Infected livers were found in all of the states and territories except the Territory of Amapá. Schistosomiasis is widespread in Brazil with highest incidence in the states of the Northeast. The disease is quite common in Espírito Santo and Minas Gerais as well. A study of the age distribution of cases of intestinal schistosomiasis observed among liver specimens obtained in the year 1938 showed a low inci¬dence on young children with a peak of prevalence in the 10 to 19 year age group. The purpose of this contribution is to call attention of the health autho¬rities to the extent and gravity of the problem of intestinal schistosomiasis in Brazil.

Visando à aplicação sistemática de parâmetros comportamentais como indicadores da ação tóxica de moluscicidas empregados no combate aos hospedeiros intermediários do Schistosoma mansoni, um procedimento para avaliação quantitativa do efeito de dose sobre o comprotamento de Biomphalaria glabrata foi desenvolvido, com base no paradigma recomendado pela O.M.S. para ensaios biológicos e envolvendo registro comportamental por cenematografia ocm lapso de tempo: caramujos com 5 7/8 ± 1/8 giros eram subemtidos a diferentes concentrações subletais de sulfato de cobre durante 24 horas e em seguida transferidos para recuperação em água destilada desionizada; a partir da análise dos registros foram computados (a) a freqüência total de subidas à superfície por indivíduo, (b) a frqüência total de saídas da água por indivíduo e (c) a proporção média de indivíduos no terço superior do recipiente de teste. O método Litchfield-Wilcoxon foi empregaod para determinação de índices de referência (denominados "concentrações de efieto comportamental de 50%" ou CEC50) em relação a cada parâmetro, e os valores obtidos - (a) 0,010, (b) 0,006 e (c) 0,029 ppm de cobre - não só evidenciaram a exeqüibilidade da aplicação sistemática de critérios comportamentais de toxidade, como se revelaram capazes de detectar o efieto tóxico do produto em concentrações muito infereiores às obtidas nas determinações convencionais de letalidade. Os dados também mostraram alterações na atividade dos caramujos em decorrência do ciclo de iluminação dia-noite. Embora o esclarecimento dos aspectos etológicos envolvidos no problema do controle químico do vetor dependa da análise das relações entre o indivíduo e seu ambiente natural, estudos de laboratório com mensurações acuradas de parâmetros relacionados a comportamentos de proteção podem fornecer subsídios relevantes a respeito.

É registrado o primeiro encontro do molusco planorbideo Biomphalaria glabrata, hospedeiro intermediário do Schistosoma mansoni, no Estado do Piauí, coletado em vários criadouros na cidade de Parnaíba. O exame de 694 exemplares revelou a presença de formas evolutivas de algumas espécies de trematodeos, mas não de Schistosomatidea. Nenhum caso autoctone de xistosomose foi até agora identificado na população humana da cidade. A presença da B. glabrata em Parnaíba amplia em 20 km para leste a área de sua distribuição na Região Litoral Norte da Grande Região Nordeste do Brasil onde era conhecida até em Avaioses no extremo leste da parte maranhense da referida Região. Outros moluscos também coletados nos mesmos criadouros foram Biomphalaria straminea, Drepanotrema lucidum. D. cimex, D. depressissimum, Physidae e Ampullarriidae.

The process of light orientation by the snail Biomphalaria glabrata was studied using the selection technique in a Y-shaped aquarium under vertical or horizontal lighting schemes. Snail behavior was measured on the basis of distance (cm) covered per hour, direction of locomotion, and localization of the animal in the aquarium. A comparison was made of the action of the light stimulus on young and adult animals of albino populations from Santa Luzia (State of Minas Gerais, Brazil) and of a melanic population from Touros (State of Rio Grande do Norte) studied in groups and separately. All groups studied were attracted to light. Analysis of the data suggests the exitence of two orientation mechanisms with respect to light in these animals, i.e. high photo-orthokinesia and positive phototaxis, which influence their motion in the environment. This evidence permitted us to discuss features of the distribution dynamics of these mollusks in the environment and their relationship with the larval phases of Schistosoma mansoni, for which they act as intermediated hosts.

In addition to previous records of Biomphalaria glabrata in the Dominican Republic, the southern central communities of Haina Arriba and Boca Chica, in the National District, are reported as new localities for that species; other species collected were Biomphalaria obstructa, B. helophila, Drepanotrema lucidum and Lymnaea viatrix. Biomphalaria straminea, a potential vector of Schistosoma mansoni, was found for the first time in the country, in the River Iguamo, just outside of the community of San Pedro de Macorís.
