Intérêt des faibles doses de rt-PA dans les syndromes inflammatoires et hémorragiques du segment antérieur: étude clinique et revue de la littérature [Treatment of anterior segment fibrinous reactions and hemorrhage with intracameral low dose rt-PA: clinical study and review of the literature].

PURPOSE: to evaluate the efficacy and the safety of low dose intraocular tissue plasminogen activator (rt-PA) in the treatment of traumatic hyphema and postoperative fibrinous membrane. METHODS: Six microg to 10 microg of rt-PA was injected into the anterior chamber to treat severe fibrinous postoperative membranes and total traumatic hyphemae. RESULTS: Thirteen eyes of 13 patients were treated. Four cases of traumatic hyphema and 9 cases of fibrinous membranes were included. Complete fibrinolysis within 24 hours was observed in 4 cases (30.8%). A partial success was noted in 7 eyes (53.8%). No beneficial effect was observed in two cases of traumatic hyphema associated with intravitreal hemorrhage after penetrating trauma. No side effect attributable to rt-PA occurred. CONCLUSION: Low dose intraocular rt-PA appears to be safe and effective in the treatment of postoperative fibrinous membrane and endocular hemorrhage limited to the anterior chamber.

CoLaus: diabète et dyslipidémie, il y a toujours du nouveau [CoLaus: diabetes and dyslipidemia, there is always something new!].

Cardiovascular diseases remain the first cause of mortality in our country. They are associated with well known risk factors such as diabetes and dyslipidemia. Herein we summarize main results of the CoLaus study regarding, first the prevalence and characteristics of the treatment of these risk factors. Then we present recent discoveries of new genetic determinants associated with these risk factors. Finally, we discuss whether this knowledge changes our current clinical management of our patients.

Dépistage organisé du cancer colorectal: défis et enjeux pour un essai pilote en Suisse. [Organized screening for colorectal cancer: challenges and issues for a Swiss pilot study].

Colorectal cancer is among the three most common and lethal neoplasms in Switzerland (about 4,000 new cases and 1,600 deaths per year, respectively). Several screening modalities are available. The effectiveness of colorectal cancer screening on mortality is established. The implementation of a successful screening intervention shall follow specified phases and conditions, and abide with global principles: pooling and coordinating efforts from all partners, setting goals and priorities, and identifying action plans. A feasibility (pilot) study is needed to verify requirements for quality, effectiveness, efficiency and innocuousness. Endeavours towards implementing a pilot programme for colorectal cancer screening in the Vaud canton (Switzerland) are presented and discussed.

La liberté des Modernes à l'épreuve de la finitude

Les problèmes environnementaux et les réponses qui pourraient leur être apportées font-ils peser une menace nouvelle sur la liberté individuelle ? La question mérite d'être posée à l'heure où l'on prend de plus en plus conscience des limites de la planète, tant au niveau des ressources que de la capacité de charge des écosystèmes. La conception moderne de la liberté, telle que définie par le projet libéral, est faite d'indépendance, de « jouissance paisible » et de droit à la poursuite de son intérêt personnel. Mais est- elle compatible avec la finitude de la planète et l'impossibilité qui en découle de poursuivre une croissance universelle des flux de matière et d'énergie ? Cet article tente de dégager les tensions qui apparaissent entre le concept libéral de la liberté (hérité de Hobbes et de Locke), ainsi que ses développements libertariens, et la question environnementale, sous deux aspects : celui du progrès technique et celui de la propriété privée.
