INTRODUCTION: Fungemia corresponds to the isolation of fungi in the bloodstream and occurs mostly in immunosuppressed patients. The early diagnosis and treatment of these infections are relevant given the serious threat to the affected patients and possible spread to other organs, often becoming fatal. The growing number of fungemia associated with poor prognosis resulted in this research aiming to diagnose and assess the epidemiological aspects of hematogenous infections by fungi. METHODS: The study included 58 blood samples collected within a 1-year period, from patients at the Hospital das Clinicas, Federal University of Pernambuco, by venipuncture in vacuum tubes. Blood samples were processed for direct examination and culture and identification, conducted by observing the macroscopic and microscopic characteristics, as well as physiological characteristics when necessary. RESULTS: Eight (13.8%) episodes of fungemia were identified, accounting for the total sample, and these pathogens were Candida, Histoplasma, Trichosporon, Cryptococcus, and a dematiaceous fungus. C. albicans was the prevalent species, accounting for 37.5% of the cases. Most affected patients were adult males. There was no predominance for any activity, and the risk of acquired immunodeficiency syndrome was the underlying pathology most often cited. CONCLUSIONS: The isolation of fungi considered as emergent species, such as C. membranifaciens and dematiaceous species, highlights the importance of epidemiological monitoring of cases of fungemia in immunocompromised patients, as the therapy of choice depends on the knowledge of the aethiological agent.

Introduction Vascular access in patients undergoing hemodialysis is considered a critical determinant of bloodstream infection (BSI) and is associated with high morbidity and mortality. The purpose of this study was to investigate the occurrence of BSI in patients with end-stage renal disease using central venous catheters for hemodialysis. Methods A cohort study was conducted in a public teaching hospital in central-western Brazil from April 2010 to December 2011. For every patient, we noted the presence of hyperemia/exudation upon catheter insertion, as well as fever, shivering, and chills during hemodialysis. Results Fifty-nine patients were evaluated. Thirty-five (59.3%) patients started dialysis due to urgency, 37 (62.7%) had BSI, and 12 (20%) died. Hyperemia at the catheter insertion site (64.9%) was a significant clinical manifestation in patients with BSI. Statistical analysis revealed 1.7 times more cases of BSI in patients with hypoalbuminemia compared with patients with normal albumin levels. The principal infective agents identified in blood cultures and catheter-tip cultures were Staphylococcus species (24 cases), non-fermentative Gram-negative bacilli (7 cases of Stenotrophomonas maltophilia and 5 cases of Chryseobacterium indologenes), and Candida species (6). Among the Staphylococci identified, 77.7% were methicillin-resistant, coagulase-negative Staphylococci. Of the bacteria isolated, the most resistant were Chryseobacterium indologenes and Acinetobacter baumannii. Conclusions Blood culture was demonstrated to be an important diagnostic test and identified over 50% of positive BSI cases. The high frequency of BSI and the isolation of multiresistant bacteria were disturbing findings. Staphylococcus aureus was the most frequently isolated microorganism, although Gram-negative bacteria predominated overall. These results highlight the importance of infection prevention and control measures in dialysis units.

Introduction We analyze how infectious disease physicians perceive and manage invasive candidosis in Brazil, in comparison to intensive care unit specialists. Methods A 38-question survey was administered to 56 participants. Questions involved clinicians' perceptions of the epidemiology, diagnosis, treatment and prophylaxis of invasive candidosis. P < 0.05 was considered statistically significant. Results The perception that candidemia not caused by Candida albicans occurs in less than 10% of patients is more commonly held by intensive care unit specialists (p=0.018). Infectious disease physicians almost always use antifungal drugs in the treatment of patients with candidemia, and antifungal drugs are not as frequently prescribed by intensive care unit specialists (p=0.006). Infectious disease physicians often do not use voriconazole when a patient's antifungal treatment has failed with fluconazole, which also differs from the behavior of intensive care unit specialists (p=0.019). Many intensive care unit specialists use fluconazole to treat candidemia in neutropenic patients previously exposed to fluconazole, in contrast to infectious disease physicians (p=0.024). Infectious disease physicians prefer echinocandins as a first choice in the treatment of unstable neutropenic patients more frequently than intensive care unit specialists (p=0.013). When candidemia is diagnosed, most infectious disease physicians perform fundoscopy (p=0.015), whereas intensive care unit specialists usually perform echocardiograms on all patients (p=0.054). Conclusions This study reveals a need to better educate physicians in Brazil regarding invasive candidosis. The appropriate management of this disease depends on more drug options being available in our country in addition to global coverage in private and public hospitals, thereby improving health care.

Introduction: There are more than 300,000 extractors using the babaçu coconut as a source of income in the States of Maranhão, Pará, Tocantins and Piauí, and this activity is associated with fungal infections. The objective of this study was to examine the occurrence of emergent fungi in the conjunctiva, nails and surface and subcutaneous injuries of female coconut breakers in Esperantinópolis, Maranhão. Additionally, soil samples and palm structures were collected. Methods: The obtained samples were cultured in Petri dishes containing potato-dextrose-agar and chloramphenicol. The etiological agent was confirmed by a direct mycological exam and growth in culture. Results: In total, 150 domiciles were visited, and samples were collected from 80 patients. From the ground, the most frequently isolated fungus was Aspergillus niger (53. 8%). the most frequently detected fungus in babaçu coconut was Aspergillus niger (66.7%). Conjunctival fungal growth occurred in 76.3% of the women. The ocular fungal microbiota consisted of filamentous fungi (80.6%), and yeasts were present in 19.4% of cases. Onychomycosis was diagnosed in 44% (11/25) of the women. Conclusions: The identification of the genera Neosartorya, Rhizopus and Curvularia in onychomycoses shows that emergent filamentous fungi can be isolated. Aspergillus sp., Penicillium sp. and Scedosporium sp. were the predominant genera found in the babaçu coconut. From ocular conjunctiva, Candida spp. were the most prevalent species isolated, and Fusarium sp. was present only in one woman. The nearly permanent exposure of coconut breakers to the external environment and to the soil is most likely the reason for the existence of a mycotic flora and fungal infections, varying according to the individual's practices and occupation.

Introduction There are few studies reporting the antifungal activities of Lippia alba extracts. Methods A broth microdilution assay was used to evaluate the antifungal effects of Lippia alba extracts against seven yeast species of Candida and Cryptococcus. The butanol fraction was investigated by gas chromatography-mass spectrometry. Results The butanol fraction showed the highest activity against Candida glabrata. The fraction also acted synergistically with itraconazole and fluconazole against C. glabrata. The dominant compounds in the butanol fraction were 2,2,5-trimethyl-3,4-hexanedione, 3,5-dimethyl-4-octanone and hexadecane. Conclusions The butanol fraction may be a good candidate in the search for new drugs from natural products with antifungal activity.

IntroductionThis is the first study to examine the in vitrosusceptibility and the expression of virulence factors in Candida species in the presence of Pimenta pseudocaryophyllus (Gomes) L.R. Landrum (Myrtaceae), a Brazilian plant known as paucravo. Additionally, the mechanisms of action of the crude ethanol extract and the ethyl acetate and aqueous fractions of this plant were investigated.MethodsThe in vitro susceptibility of Candida was tested using the broth microdilution method, whereas an XTT reduction assay was used for biofilms. Adherence was determined by counting the number of yeast cells that adhered to 100 oral epithelial cells, and hyphal formation was verified in the hyphal induction medium M199. Flow cytometry with propidium iodide and FUN-1 was performed to assess the mechanism of action.ResultsThe results revealed that the crude ethanol extract and the ethyl acetate and aqueous fractions of P. pseudocaryophyllusinhibited the growth of Candida isolates at a minimal inhibitory concentration (MIC) ranging from 64 to 256µg/mL, whereas the 50% sessile minimal inhibitory concentration (SMIC50) ranged from 512 to >1,024µg/mL. Adherence and hyphal formation were significantly reduced in the presence of the crude ethanol extract and both fractions. Although cell membrane injury was detected, the predominant mechanism of action appeared to be the alteration of yeast metabolism, as demonstrated by flow cytometry.ConclusionsOur results indicated that antifungal activity reduced the expression of virulence factors in yeast via the alteration of yeast metabolism, suggesting that the crude extract of P. pseudocaryophyllus and its fractions may contain novel antifungal agents.

INTRODUCTION: To evaluate predictive indices for candidemia in an adult intensive care unit (ICU) and to propose a new index. METHODS: A prospective cohort study was conducted between January 2011 and December 2012. This study was performed in an ICU in a tertiary care hospital at a public university and included 114 patients staying in the adult ICU for at least 48 hours. The association of patient variables with candidemia was analyzed. RESULTS: There were 18 (15.8%) proven cases of candidemia and 96 (84.2%) cases without candidemia. Univariate analysis revealed the following risk factors: parenteral nutrition, severe sepsis, surgical procedure, dialysis, pancreatitis, acute renal failure, and an APACHE II score higher than 20. For the Candida score index, the odds ratio was 8.50 (95% CI, 2.57 to 28.09); the sensitivity, specificity, positive predictive value, and negative predictive value were 0.78, 0.71, 0.33, and 0.94, respectively. With respect to the clinical predictor index, the odds ratio was 9.45 (95%CI, 2.06 to 43.39); the sensitivity, specificity, positive predictive value, and negative predictive value were 0.89, 0.54, 0.27, and 0.96, respectively. The proposed candidemia index cutoff was 8.5; the sensitivity, specificity, positive predictive value, and negative predictive value were 0.77, 0.70, 0.33, and 0.94, respectively. CONCLUSIONS: The Candida score and clinical predictor index excluded candidemia satisfactorily. The effectiveness of the candidemia index was comparable to that of the Candida score.

Recentemente tem-se observado um crescente interesse pelo uso de plantas aromáticas, devido às inúmeras propriedades benéficas que lhes têm sido associadas, e a sua aplicação em diversas indústrias, como a indústria alimentar e farmacêutica. Este estudo incidiu sob três ervas aromáticas utilizadas comummente em Portugal: coentros, salsa e segurelha. O objetivo do mesmo foi contribuir para um melhor conhecimento das propriedades bioativas das plantas estudadas, através da determinação do seu teor em compostos fenólicos totais e em flavonoides, e avaliação das suas atividades antioxidante, antimicrobiana e anti-inflamatória. A atividade antioxidante foi determinada através de diferentes tipos de ensaios, nomeadamente avaliação da capacidade redutora, com os ensaios FRAP e CUPRAC, avaliação da capacidade de sequestro do radical DPPH• e avaliação da capacidade de sequestro do radical anião superóxido. A atividade antimicrobiana foi determinada utilizando três estirpes microbianas, Staphylococcus aureus, Escherichia coli e Candida albicans. Por sua vez, a ação anti-inflamatória foi determinada através de ensaios in-vivo apenas para a segurelha, utilizando dois modelos, um para a inflamação aguda (modelo de edema da pata induzido pela carragenina) e outro para a inflamação crónica (modelo da colite ulcerativa). Os resultados demonstraram que todos os extratos de plantas aromáticas possuem atividade antioxidante, sendo que o extrato de segurelha foi aquele que apresentou um maior teor de compostos fenólicos e flavonoides, bem como o que demonstrou possuir uma maior atividade antioxidante relativamente aos restantes extratos estudados, o que sugere o envolvimento destes compostos neste tipo de atividade antioxidante. Entre o extrato de coentros e salsa, o extracto de coentros apresentou um teor em compostos fenólicos e uma actividade antioxidante significativamente superior relativamente ao extracto de salsa. O extrato de segurelha foi o único que apresentou ação antimicrobiana, o que poderá indicar que os compostos fenólicos se encontram também interligados com esta propriedade. A atividade anti-inflamatória foi determinada apenas para o extrato de segurelha, sendo que se observou que este possui efeitos benéficos para a inflamação aguda e crónica, o que poderá estar novamente relacionado com o seu teor em fenóis e flavonoides. O efeito anti-inflamatório observado com o extrato de segurelha no modelo de colite pode ser muito útil do ponto de vista de profilaxia pois o extrato foi administrado aos animais logo a seguir à indução da colite e os resultados demonstram haver uma diminuição significativa da extensão e violência dos sinais da inflamação crónica observados no grupo controlo positivo.

Este trabalho trata da determinação da atividade antibiótica dos extratos de Gnetum paniculatum e G. schwackeanum e dos constituintes químicos isolados deste último como resperatrol, gnetina C e E, os quais foram testados contra várias bactérias e fungos. O extrato de Gnetum schwackeanum e dos constituintes químicos isolados deste último como resveratrol, gnetina C e E, os quais foram testados contra várias bactérias e fungos. O extrato de Gnetum schwackeanum e todas as substâncias dele isoladas foram ativos a Staphylococcus aureus. S. epidermis e Mycobacterium smegmatis. Resveratrol e gnetina C são ativos contra a Candida albicans, mas somente gnetina C possui atividade em relação a Candida parapsilosis e Saccharomyces cervisiae. O derivado sintético de gnetina E não mostrou nenhuma atividade. O extrato de G. paniculatum, é completamente inativo à bactéria e fungos o que sugere que a atividade de G. paniculatum, é completamente inativo à bactéria e fungos o que sugere que a atividade de G. schwackeanum deve-se à presença dos hidroxiestilbenos e seus derivados, uma vez que G. paniculatum não contêm esses tipos de substâncias.

O extrato de G. paniculatum, é completamente inativo à bactéria e fungos o que sugere que a atividade de G. schwackeanum deve-se à presença dos hiroxiestilbenos e seus derivados, uma vez que G. paniculatum não contém esses tipos de substâncias.

The objective of the work presented in this thesis was the development of an innovative approach for the separation of enantiomers of secondary alcohols, combining the use of an ionic liquid (IL) - both as solvent for conducting enzymatic kinetic resolution and as acylating agent - with the use of carbon dioxide (CO2) as solvent for extraction. Menthol was selected for testing this reaction/separation approach due to the increasing demand for this substance, which is widely used in the pharmaceutical, cosmetics and food industries. With a view to using an ionic ester as acylating agent, whose conversion led to the release of ethanol, and due to the need to remove this alcohol so as to drive reaction equilibrium forward, a phase equilibrium study was conducted for the ehtanol/(±)-menthol/CO2 system, at pressures between 8 and 10 MPa and temperatures between 40 and 50 oC. It was found that CO2 is more selective towards ethanol, especially at the lowest pressure and highest temperature tested, leading to separation factors in the range 1.6-7.6. The pressure-temperature-composition data obtained were correlated with the Peng-Robinson equation of state and the Mathias-Klotz-Prausnitz mixing rule. The model fit the experimental results well, with an average absolute deviation (AAD) of 3.7 %. The resolution of racemic menthol was studied using two lipases, namely lipase from Candida rugosa (CRL) and immobilized lipase B from Candida antarctica (CALB), and two ionic acylating esters. No reaction was detected in either case. (R,S)-1-phenylethanol was used next, and it was found that with CRL low, nonselective, conversion of the alcohol took place, whereas CALB led to an enantiomeric excess (ee) of the substrate of 95%, at 30% conversion. Other acylating agents were tested for the resolution of (±)-menthol, namely vinyl esters and acid anhydrides, using several lipases and varying other parameters that affect conversion and enantioselectivity, such as substrate concentration, solvent and temperature. One such acylating agent was propionic anhydride. It was thus performed a phase equilibrium study on the propionic anhydride/CO2 system, at temperatures between 35 and 50 oC. This study revealed that, at 35 oC and pressures from 7 MPa, the system is monophasic for all compositions. The enzymatic catalysis studies carried out with propionic anhydride revealed that the extent of noncatalyzed reaction was high, with a negative effect on enantioselectivity. These studies showed also that it was possible to reduce considerably the impact of the noncatalyzed reaction relative to the reaction catalyzed by CRL by lowering temperature to 4 oC. Vinyl decanoate was shown to lead to the best results at conditions amenable to a process combining the use of supercritical CO2 as agent for post-reaction separation. The use of vinyl decanoate in a number of IL solvents, namely [bmim][PF6], [bmim][BF4], [hmim][PF6], [omim][PF6], and [bmim][Tf2N], led to an enantiomeric excess of product (eep) values of over 96%, at about 50% conversion, using CRL. De 1981 a 1985, foram atendidos 96 acientes nas unidades de saúde: Instituto de Dermatologia Troppical e Venereologia "Alfredo da Matta" IDTV-AM e Serviço Médico do Instituto nacional de Pesquisas da Amazônia (INPA), Manaus-AM.Os portadores de diagnóstico clínico de Tiena pedis (pé-de-atleta), foram submetidos ao exame micológico, os quais revelaram que dos segmentos podais mais afetados foram os da Região plantar (42,0%) e interdigital (26,0%). Das 99 espécies fúngicas, 74% eram dermatófitos e 26% de leveduras. Dentre as espécies observadas, verificou-se que as mais frequentes por número de ocorrência foram: Trichophyton rubrum (335,5%), Trichopthyton mentagrophytes (25,00% e Candida sp. (25,0%).

O objetivo deste trabalho foi identificar 50 leveduras isoladas a partir de diferentes substratos da Região Amazônica e selecionar uma espécie de maior atividade proteolítica. Entre as 26 espécies identificadas predominaram Candida aquatica (12%) e Candida tropicalis (10%). A fermentação submersa foi realizada em Extrato de Malte suplementado com gelatina 1%, Candida intermedia foi a que expressou maior atividade proteolítica, halo = 25 mm e 273 U/mL, crescimento máximo a 30 ºC, viabilidade celular 6,2 x 10(6) UFC, em 48 horas, não demonstrou características de patogenicidade e a melhor densidade do inóculo foi 3%, utilizando-se culturas com 24 horas de crescimento em ágar Malte.

O nome vulgar adotado nos inventários florestais tem agrupado espécies distintas. Para exemplificar este problema, foram avaliadas as identificações de indivíduos comercialmente determinados como "tauari" em inventários de duas áreas manejadas de 100 ha nos pólos madeireiro central e leste do Estado do Pará. Características dendrológicas de cada espécie foram anotadas para diferenciá-las. Dados sobre a distribuição geográfica dessas espécies, as propriedades tecnológicas de sua madeira e a legislação atual para o manejo florestal são discutidos. O inventário feito no pólo madeireiro central registrou 112 indivíduos de "tauari" nominados como Couratari guianensis, e seis indivíduos de "tauari-cachimbo" determinados como Couratari sp. Depois de uma revisão botânica com material coletado de cada árvore, foi constatado que os indivíduos determinados como Couratari guianensis agrupavam três espécies: Couratari guianensis, C. oblongifolia e C. stellata, esta última com maior densidade relativa. O que antes constava como Couratari sp. agrupava as espécies Cariniana micrantha e Cariniana decandra. No pólo leste, o inventário contava 33 indivíduos, listados como "tauari" ou Couratari guianensis. Para estes, a identificação botânica mostrou o agrupamento de duas espécies: C. guianensis com maior densidade relativa e C. oblongifolia. Fora da área de estudo, foi registrada a ocorrência de C. tauari. Este estudo mostra que é possível separar as espécies utilizando aspectos dendrológicos (folhas, ramos e tronco). O inventário botânico é demonstrado como base para o conhecimento da diversidade e indispensável para assegurar o sucesso dos planos de manejo. No contexto jurídico, o agrupamento inviabiliza o cumprimento das leis brasileiras referentes ao manejo.

Este trabalho objetiva avaliar a composição florística e a fitossociologia de espécies arbóreas do parque fenológico da Embrapa Amazônia Ocidental no Distrito Agropecuário da Suframa (DAS), Manaus-AM, a fim de subsidiar seleções futuras de árvores matrizes visando estudos fenológicos e a implantação de áreas de coleta de sementes. Foram alocadas aleatoriamente 20 parcelas de 10m x 50 m ao longo de um transecto, amostrando-se todos os indivíduos com diâmetro a 1,30 m do solo, (DAP) ≥ 20,0 cm. Foram registrados 240 indivíduos, distribuídos em 100 espécies, 70 gêneros e 29 famílias. As famílias de maior importância ecológica são, em ordem decrescente, Lecythidaceae, Sapotaceae, Mimosaceae, Caesalpiniaceae, Chrysobalanaceae, Fabaceae, Humiriaceae, Moraceae, Vochysiaceae e Apocynaceae. Essas famílias contribuem com 67% da riqueza local de espécies e com 75,8% do número de indivíduos, sugerindo que a diversidade vegetal da área está concentrada em poucas famílias. A família Lecythidaceae possui os maiores valores de dap e número de indivíduos, sendo Sapotaceae a que possui a maior riqueza de espécies na área. As espécies mais importantes, segundo o Índice de Valor de Importância-IVI, são Eschweilera coriacea (DC) S.A. Mori; Qualea paraensis Ducke; Vantanea macrocarpa Ducke; Eschweilera atropetiolata< S.A. Mori; Couratari stellata A.C. Sm.; Lecythis usidata Miers; Eperua duckeana R.S. Cowan; Eschweilera amazonica R. Knuth e Chrysophyllum manaosense (Aubr.) T.D. Penn. O valor do índice de diversidade de Shannon de 2,71, aparentemente baixo, deve ser interpretado com cuidado ao se avaliar a diversidade do parque fenológico, uma vez que o diâmetro mínimo de 20,0 cm adotado neste trabalho pode ter restringido a identificação de um maior número de espécies. O índice de similaridade de Sorensen indicou uma alta dissimilaridade florística entre as parcelas. A primeira classe de diâmetro medindo de 20cm -30cm foi a que apresentou maior número de indivíduos, contendo 45% dos indivíduos amostrados. Acima do centro de classe de 55cm de dap, cerca de 90% das espécies estão representadas por apenas um indivíduo.
