BACKGROUND & AIMS: n-3 fatty acids are expected to downregulate the inflammatory responses, and hence may decrease insulin resistance. On the other hand, n-3 fatty acid supplementation has been reported to increase glycemia in type 2 diabetes. We therefore assessed the effect of n-3 fatty acids delivered with parenteral nutrition on glucose metabolism in surgical intensive care patients. METHODS: Twenty-four surgical intensive care patients were randomized to receive parenteral nutrition providing 1.25 times their fasting energy expenditure, with 0.25 g of either an n-3 fatty acid enriched-or a soy bean-lipid emulsion. Energy metabolism, glucose production, gluconeogenesis and hepatic de novo lipogenesis were evaluated after 4 days. RESULTS: Total energy expenditure was significantly lower in patients receiving n-3 fatty acids (0.015+/-0.001 vs. 0.019+/-0.001 kcal/kg/min with soy bean lipids (P<0.05)). Glucose oxidation, lipid oxidation, glucose production, gluconeogenesis, hepatic de novo lipogenesis, plasma glucose, insulin and glucagon concentrations did not differ (all P>0.05) in the 2 groups. CONCLUSIONS: n-3 fatty acids were well tolerated in this group of severely ill patients. They decreased total energy expenditure without adverse metabolic effects.

El Síndrome de Sweet (SS) es la más característica de todas las dermatosis neutrofílicas sobre todo por las entidades a las que se asocia, dada su finalidad pronóstica y terapeútica. Para definir el perfil de los pacientes diagnosticados de SS en nuestro departamento y evaluar las diferencias clínico-epidemiológicas entre subgrupos, hemos realizado un estudio retrospectivo desde 2001 a 2009, ambos inclusive, donde se han incluido veinticuatro pacientes (13 mujeres y 11 hombres) que han sido reagrupados en 4 categorías según la etiología: infecciosa, paraneoplásica, parainflamatoria e idiopática. Los resultados obtenidos se han comparado con estudios previos, encontrando algunas diferencias significativas.

Recently, the importance of the interaction between wine and lees cell surface has been reported. Cell surface properties depending on wall biochemical composition are major determinants in microbial interactions, having important repercussions in several technological aspects. Sorption and flocculation are especially important in sparkling wine production, and are governed by distinct cell surface properties. The aim of the present research carried out during the four months of the stage was to know the implication of lees surface modifications occurring during surlie ageing in sparkling wine quality and elaboration. The relationship between physico-chemical properties such as hydrophobicity, charge and electron-donor characteristics, and the yeast surface sorption capacities, we determined these factors in a model system. Then, real industrial lees samples were investigated. The surface properties of sparkling wine lees from the same strain of Saccharomyces cerevisiae were characterized according to the time of surlie ageing, and their possible influence on lees sorption and flocculation capacity was evaluated. El tratamiento del carcinoma de páncreas continúa siendo uno de los grandes retos de la Oncología Médica. Un mejor conocimiento de las vías moleculares que intervienen en su desarrollo ha contribuido al uso de nuevos agentes terapéuticos dirigidos frente a las mismas. Una de esas vías es la del EGFR. Moore et al llevaron a cabo en 2007 un ensayo fase III aleatorizado en el que compararon gemcitabina frente a gemcitabina y erlotinib (un inhibidor de EGFR) y en el que demostraron de forma estadísticamente significativa por primera vez una mejoría en supervivencia global al asociar un agente a la gemcitabina en el tratamiento del adenocarcinoma pancreático avanzado. Hemos adoptado el esquema empleado en el ensayo fase III de Moore como primera línea de tratamiento del carcinoma pancreático localmente avanzado e irresecable y metastático en nuestro centro y creo necesario analizar la toxicidad y la supervivencia observada en nuestro medio. Para ello presento los resultados de eficacia, toxicidad y supervivencia global en la serie de 55 pacientes tratados con dicho esquema entre octubre de 2007 y octubre de 2010 en nuestro centro.

La ventilació mecánica no invasiva(VMNI) associada al tractament farmacològic de l’edema agut de pulmó(EAP) ha demostrat millorar de forma precoç els paràmetres clínics i gasomètrics, reduïr el nombre d’IOT, ingressos a UCI i mortalitat quan es compara amb el tractament convencional. Evaluàrem el perfil clínic, factors associats a mortalitat, comparàrem la mortalitat, temps mig d’estància hospitalària e incidència de complicacions(IAM) pels dos tipus de tractaments realizats(VMNI vs teràpia convencional) en pacients admesos a l’Hospital per EAP entre 2006-2008. El tabaquisme, pressió arterial sistòlica, xifra d’urea i radiografia de tórax amb cardiomegàlia i redistribució són predictors independents de mortalitat.

Estudi prospectiu observacional realitzat entre gener 2003-juny 2010 amb casos diagnosticats de EI. Vam estudiar l’epidemiologia i els factors de risc de l'endocarditis infecciosa associada al medi hospitalari (EIAMH). Vam incloure 212 casos, el 34,9% eren EIAMH. Observarem un augment en la incidència en els darrers 4 anys (del 45,9% al 54,1%). La EIAMH s'ha associat a pacients grans i amb més comorbiditat. Staphylococcus aureus (21,7%) va ser el més freqüent. Aquests pacients van ser rebutjats per a la cirurgia amb major freqüència (40% vs 22%) i van presentar una major mortalitat (44,6% vs 19,6%) respecte al grup d'endocarditis comuniataria.
