Background The 2011 Malawi HIV guidelines promote CD4 monitoring for pre-ART assessment and considering HIVRNA monitoring for ART response assessment, while some clinics used CD4 for both. We assessed clinical ordering practices as compared to guidelines, and determined whether the samples were successfully and promptly processed. Methods We conducted a retrospective review of all patients seen in from August 2010 through July 2011,, in two urban HIV-care clinics that utilized 6-monthly CD4 monitoring regardless of ART status. We calculated the percentage of patients on whom clinicians ordered CD4 or HIVRNA analysis. For all samples sent, we determined rates of successful labprocessing, and mean time to returned results. Results Of 20581 patients seen, 8029 (39%) had at least one blood draw for CD4 count. Among pre-ART patients, 2668/2844 (93.8%) had CD4 counts performed for eligibility. Of all CD4 samples sent, 8082/9207 (89%) samples were successfully processed. Of those, mean time to processing was 1.6 days (s.d 1.5) but mean time to results being available to clinician was 9.3 days (s.d. 3.7). Regarding HIVRNA, 172 patients of 17737 on ART had a blood draw and only 118/213 (55%) samples were successfully processed. Mean processing time was 39.5 days (s.d. 21.7); mean time to results being available to clinician was 43.1 days (s.d. 25.1). During the one-year evaluated, there were multiple lapses in processing HIVRNA samples for up to 2 months. Conclusions Clinicians underutilize CD4 and HIVRNA as monitoring tools in HIV care. Laboratory processing failures and turnaround times are unacceptably high for viral load analysis. Alternative strategies need to be considered in order to meet laboratory monitoring needs.

Objetivo: Validar los criterios de CENTOR modificados (CENTOR-m) y los tests rápidos de detección del antígeno de Estreptococo del Grupo A (SGA) en la faringitis aguda. Diseño: Estudio de validación de pruebas diagnósticas. Emplazamiento y participantes: Ciento un pacientes elegibles, que consultaron al departamento de urgencias de un hospital de tercer nivel con cuadro clínico compatible con faringitis aguda. Mediciones Principales: Se obtuvieron muestras de hisopados faríngeos para la realización del test rápido antigénico para SGA (FAMR) y para cultivo, respectivamente. Se calculó en cada caso los criterios de CENTOR-m. Resultados: La edad media de los pacientes incluidos en el estudio fue de 22,6 años (DE:13,8). El 48,5 % eran varones. El SGA fue el patógeno aislado en el 20,79 % de los casos. El CENTOR-m presentó una sensibilidad del 83,3 % (50,9 %-97,1 %), especificidad del 45,5 % (30,7 %-61,0 %) valor predictivo positivo (VPP) del 29,4 % (15,7 %-47,7 %) y valor predictivo negativo (VPN) del 90,9 % (69,4 %-98,4 %). El FAMR presento una sensibilidad del 81,5 % (61,3 %-93,0 %) especificidad del 98,6 % (91,4 %-99,9 %), VPP del 95,7 % (76,0 %-99,8 %) y VPN del 93,3 % (84,5 %-97,5 %). El 49,5 % de los pacientes recibieron antibióticos basándose en el juicio médico, lo que resultó en una proporción de sobreindicación de antimicrobianos del 62 %. Conclusiones: Los criterios de CENTOR-m demostraron adecuado valor pronóstico negativo y el FAMR buena sensibilidad, especificidad y valor pronóstico positivo para faringitis por SGA. La utilización de ambos métodos en la atención urgente podría optimizar el manejo de la patología y la adecuación antibiótica.

Background: Clinical features of Clostridium difficile infection (CDI) cases diagnosed by detection of polymerase chain reaction (PCR), with negative toxin enzyme immunoassay results (EIA) have not been fully elucidated. The purpose of this study was to determine the magnitude of CDI patients who had negative EIA toxin determinations but positive PCR tests, and their differences in clinical presentation. Methods: We performed a retrospective study comparing the clinical features of CDI cases detected by EIA (toxins A + B) with cases detected by PCR (toxin negative, PCR positive) over a 16-month period. Only patients with an initial Clostridium difficile infection episode that fulfilled a standardized definition were included. Results: During the study period, 107 episodes of CDI were detected. Seventy-four patients (69%) had positive glutamate dehydrogenase (GDH) antigen and EIA determinations (EIA positive patients). Thirty-three patients (31%) had GDH positive, negative toxin EIA and positive PCR determination (PCR positive patients). PCR positive patients were younger, 57 (27) years (mean [SD]), than EIA positive patients, 71 (16) years, (p < 0.001). Fewer PCR positive patients were receiving proton pump inhibitors (21 patients, 64%) than EIA positive patients (61 patients, 82%, p = 0.034). The clinical presentation was similar in both groups. In the multivariate analysis, lower age was identified as the only independent variable associated with PCR positive patients. Conclusions: One third of Clostridium difficile infection patients present negative toxin EIA and PCR positive tests. Performing PCR determination after the negative EIA test is more relevant in younger patients.

Background: Asphyxia is considered an important cause of morbidity and mortality in neonates. This condition can affect many vital organs including the central nervous system and may eventually lead to death or developmental disorders. Objectives: Considering the high prevalence of asphyxia and its adverse consequences, the present study was conducted to evaluate the risk factors for birth asphyxia and assess their correlation with prognosis in asphyxiated infants. Patients and Methods: This two-year follow-up cohort study was conducted on 260 infants (110 asphyxiated infants and 150 healthy neonates) at Mashhad Ghaem Hospital during 2007 - 2014. Data collection tools consisted of a researcher-designed questionnaire including maternal and neonatal information and clinical/laboratory test results. The subjects were followed-up, using Denver II test for 6, 12, 18, and 24 months (after discharge). For data analysis, t-test was performed, using SPSS version 16.5. P value ≤ 0.05 was considered statistically significant. Results: Of 260 neonates, 199 (76.5%) and 61 (23.5%) cases presented with normal neonatal outcomes and with abnormal neonatal outcomes (developmental delay), respectively. Variables such as the severity of asphyxia (P = 0.000), five-minute Apgar score (P = 0.015), need for ventilation (P = 0.000), and severity of acidosis at birth (P = 0.001) were the major prognostic factors in infants with asphyxia. Additionally, prognosis was significantly poorer in boys and infants with dystocia history (P = 0.000). Conclusions: Prevalence of risk factors for developmental delay including the severity of asphyxia need for mechanical ventilation, and severity of acidosis at birth, dystocia, and Apgar score were lower in surviving infants; therefore, controlling these risk factors may reduce asphyxia-associated complications.

Objetivo: Determinar la prevalencia de anemia en escolares de 6 a 12 años del área urbana de la ciudad de Cuenca y su relación con el coeficiente intelectual. Diseño: Se realizó un estudio transversal durante los meses de noviembre y diciembre de 2007 en el área urbana de la ciudad de Cuenca del austro ecuatoriano. Se eligieron 4 escuelas aleatoriamente hasta completar el tamaño muestral de 261 niños. Se recogieron los datos antropométricos (peso y talla); se determinaron los valores de hemoglobina y se realizó un test psicológico. Resultados: La prevalencia general de anemia fue de 9,6, y de los escolares afectados el 77,77tenían anemia por déficit de hierro. El parámetro antropométrico que mostró la mayor asociación con la presencia de anemia fue la relación entre la estatura y la edad (OR: 1,35 IC: 1,33 - 9,21). La variable anemia se asoció significativamente al coeficiente intelectual en un 17. El porcentaje de niños con desnutrición crónica fue de 11,9y riesgo de desnutrición del 15,7El tipo de escuela se asoció levemente al coeficiente intelectual (p= 0.03) (OR: 1,87 IC: 1,05 - 3,39). No se encontró ninguna relación entre la prevalencia de anemia por un lado, y el sexo por el otro. Tampoco existió relación entre la variable sexo y el coeficiente intelectual. Conclusiones: La anemia en general y la anemia ferropénica en particular, no constituyen un problema de salud pública en la población estudiada. Los escolares con talla alta tienen menor riesgo de desarrollar anemia. La anemia se asoció significativamente al coeficiente intelectual

Antecedentes: Distintos estudios han demostrado que aproximadamente la mitad de los niños presentan al menos un episodio de obstrucción bronquial durante los primeros 3 años de vida y, en más del 50% de ellos el episodio se repite una o más veces. Entonces es necesario prever que niños que inician con sibilancias en una etapa temprana de la vida, van a padecer de asma en el futuro, estudios realizados muestran una sensibilidad del 16 % y especificidad del 97 % para el IPA Castro – Rodríguez y para el PIAMA una sensibilidad de 63% y una especificidad del 64% aproximadamente. Objetivo General: Determinar la validez de diversos índices predictores de asma bronquial en niños y niñas de 6 a 11 años atendidos en Consulta Externa de Neumología del Hospital Vicente Corral Moscoso. Metodología: Se trató de un estudio de validación de test predictores de asma en la totalidad de niños/as de 6 a 11 años con diagnóstico de asma y un grupo equitativo escogidos al azar de niños de las mismas edades atendidos en consulta externa de Neumología durante el periodo Enero – Diciembre 2014. Se aplicó una encuesta a sus representantes, además se revisó los hemogramas previos que mostraron eosinofilia. Los datos fueron analizados con los programas SPSS versión 20. Uso de resultados: Se mostró la validez de los Índices Predictores de Asma que podrán ser aplicados a niños en edades aún más tempranas

The current study investigated the influence of a play-based curriculum on the development of pretend play skills and oral language in children attending their first year of formal schooling. In this quasi-experimental design, two groups of children were followed longitudinally across the first 6 months of their first year at school. The children in the experimental group were attending a school with a play-based curriculum; the children in the control group were attending schools following a traditional curriculum. A total of 54 children (Time 1 M age = 5;6, range: 4;10–6;2 years) completed standardised measures of pretend play and narrative language skills upon school entry and again 6 months later. The results showed that the children in the play-based group significantly improved on all measures, whereas the children in the traditional group did not. A subset of the sample of children (N = 28, Time 1 M age = 5;7, range: 5;2 – 6;1) also completed additional measures of vocabulary and grammar knowledge, and a test of non-verbal IQ. The results suggested that, in addition to improving play skills and narrative language ability, the play-based curriculum also had a positive influence on the acquisition of grammar.

Rates of survival of victims of sudden cardiac arrest (SCA) using cardio pulmonary resuscitation (CPR) have shown little improvement over the past three decades. Since registered nurses (RNs) comprise the largest group of healthcare providers in U.S. hospitals, it is essential that they are competent in performing the four primary measures (compression, ventilation, medication administration, and defibrillation) of CPR in order to improve survival rates of SCA patients. The purpose of this experimental study was to test a color-coded SMOCK system on:1) time to implement emergency patient care measures 2) technical skills performance 3) number of medical errors, and 4) team performance during simulated CPR exercises. The study sample was 260 RNs (M 40 years, SD=11.6) with work experience as an RN (M 7.25 years, SD=9.42).Nurses were allocated to a control or intervention arm consisting of 20 groups of 5-8 RNs per arm for a total of 130 RNs in each arm. Nurses in each study arm were given clinical scenarios requiring emergency CPR. Nurses in the intervention group wore different color labeled aprons (smocks) indicating their role assignment (medications, ventilation, compression, defibrillation, etc) on the code team during CPR. Findings indicated that the intervention using color-labeled smocks for pre-assigned roles had a significant effect on the time nurses started compressions (t=3.03, p=0.005), ventilations (t=2.86, p=0.004) and defibrillations (t=2.00, p=.05) when compared to the controls using the standard of care. In performing technical skills, nurses in the intervention groups performed compressions and ventilations significantly better than those in the control groups. The control groups made significantly (t=-2.61, p=0.013) more total errors (7.55 SD 1.54) than the intervention group (5.60, SD 1.90). There were no significant differences in team performance measures between the groups. Study findings indicate use of colored labeled smocks during CPR emergencies resulted in: shorter times to start emergency CPR; reduced errors; more technical skills completed successfully; and no differences in team performance.

Introduction. Test of Everyday Attention for Children (TEA-Ch) has been validated in different countries demonstrating that it is an instrument with a correct balance between reliability and duration. Given the shortage of trustworthy instruments of evaluation in our language for infantile population we decide to explore the Spanish version of the TEA-Ch. Methods. We administered TEA-Ch (version A) to a sample control of 133 Spanish children from 6 to 11 years enrolled in school in the Community of Madrid. Four children were selected at random by course of Primary Education, distributing the sex of equivalent form. Descriptive analysis and comparison by ages and sex in each of the TEA-Ch's subtests were conducted to establish a profile of the sample. In order to analyze the effect of the age, subjects were grouped in six sub-samples: 6, 7, 8, 9, 10 and 11 years-old. Results. This first descriptive analysis demonstrates age exerted a significant effect on each measure, due to an important "jump" in children's performance between 6 and 7 years-old. The effect of sex was significant only in two visual attention measures (Sky Search & Map) and interaction age and sex exerted a significant effect only in the dual task (Score DT). Conclusions. The results suggest that the Spanish version of the TEA-Ch (A) might be a useful instrument to evaluate attentional processes in Spanish child population.

Single and continuous vertical jumping tests, as well as the Wingate anaerobic test (WAnT), are commonly used to assess the short-term muscle power of female volleyball players; however, the relationship among these tests has not been studied adequately. Thus, the aim of the present study was to examine the relationship of single and continuous vertical jumps with the WAnT in female volleyball players. Seventy adolescent (age 16.0 ˘ 1.0 years, body mass 62.5 ˘ 7.1 kg, height 170.4 ˘ 6.1 cm, body fat 24.2% ˘ 4.3%) and 108 adult female volleyball players (age 24.8 ˘ 5.2 years, body mass 66.5 ˘ 8.7 kg, height 173.2 ˘ 7.4 cm, body fat 22.0% ˘ 5.1%) performed the squat jump (SJ), countermovement jump (CMJ), Abalakov jump (AJ), 30 s Bosco test and WAnT (peak power, Ppeak; mean power, Pmean). Mean power in the Bosco test was correlated (low to large magnitude) with Pmean of the WAnT (r = 0.27, p = 0.030 in adolescents versus r = 0.56, p < 0.001 in adults). SJ, CMJ and AJ also correlated with Ppeak (0.28 ď r ď 0.46 in adolescents versus 0.58 ď r ď 0.61 in adults) and with Pmean (0.43 ď r ď 0.51 versus 0.67 ď r ď 0.71, respectively) of the WAnT (p < 0.05). In summary, the impact of the Bosco test and WAnT on muscle power varied, especially in the younger age group. Single jumping tests had larger correlations with WAnT in adults than in adolescent volleyball players. These findings should be taken into account by volleyball coaches and fitness trainers during the assessment of short-term muscle power of their athletes.

El objetivo del presente estudio fue comparar dos pruebas submáximas de capacidad aeróbica, el YMCA step test y la prueba de capacidad de trabajo físico a 170 ppm (P170) de frecuencia cardíaca (FC), en jugadoras de voleibol femenino. Se analizaron en 152 participantes variables antropométricas, el rendimiento, la frecuencia cardiaca final (Stepend) y la frecuencia cardiaca en el primer minuto de recuperación (Steprec) en la prueba YMCA step test, la P170 fue analizada expresando los resultados en valores absolutos (P170,abs, W) y relativos (P170,rel, W.kg-1). Además, un subgrupo (n=14) fue analizado otra vez después de un año. Los resultados mostraron como el YMCA step test correlacionó altamente (Steprec) y muy altamente (Stepend) con la P170 rel (r = –0.58 and r = –0.76, p < 0.001, respectively), y con la P170,abs (r = –0.54 and r = –0.68, p < 0.001, respectively). No se encontraron correlaciones entre los porcentajes de cambio en los test de capacidad aeróbica después de un año (p>0.05). La edad presentó una correlación baja a moderada con todos los índices de capacidad aeróbica (0.23 ≤ ŇrŇ ≤ 0.45, p<0.05), viendo como las mayores edades tenían una mejor capacidad aeróbica. Basándonos en estos resultados podemos concluir que la prueba YMCA step test (especialmente el parámetro Stepend) y la P170 podrian ser utilizadas indistintamente por entrenadores y preparadores físicos para monitorizar la capacidad aeróbica de jugadoras de voleibol. El paradójico incremento de la capacidad aeróbica con la edad puede ser atribuido a los métodos de evaluación utilizados que están basados en la FC y al descenso de la FC máxima con la edad. Por lo tanto, estos test deberían ser utilizados solo con edades inferiores.

BACKGROUND: Allergic reactions can result in life-threatening situations resulting in high economic costs and morbidity. Therefore, more effective reagents are needed for allergy treatment. A causal relationship has been suggested to exist between the intake of omega-3/6 fatty acids, such as docosahexanoic acid (DHA), eicosapentanoic acid (EPA), docosapentanoic acid (DPA) and arachidonic acid (AA), and atopic individuals suffering from allergies. In allergic cascades, the hallmark cytokine IL-4 bind to IL-4 receptor (IL-4R) and IL-13 binds to IL-13 receptor (IL-13R), this activates the STAT6 phosphorylation pathway leading to gene activation of allergen-specific IgE antibody production by B cells. The overall aim of this study was to characterize omega-3/6 fatty acids and their effects on STAT6 signaling pathway that results in IgE production in allergic individuals. METHODS: The fatty acids were tested in vitro with a HEK-Blue IL-4/IL-13 reporter cell line model, transfected with a reporter gene that produces an enzyme, secreted embryonic alkaline phosphatase (SEAP). SEAP acts as a substitute to IgE when cells are stimulated with bioactive cytokines IL-4 and/or IL-13. RESULTS: We have successfully used DHA, EPA and DPA in our studies that demonstrated a decrease in SEAP secretion, as opposed to an increase in SEAP secretion with AA treatment. A statistical Student's t-test revealed the significance of the results, confirming our initial hypothesis. CONCLUSION: We have successfully identified and characterised DHA, EPA, DPA and AA in our allergy model. While AA was a potent stimulator, DHA, EPA and DPA were potential inhibitors of IL-4R/IL-13R signalling, which regulates the STAT6 induced pathway in allergic cascades. Such findings are significant in the future design of dietary therapeutics for the treatment of allergies.

Todos nós, educadores, devemos prestar uma atenção aprofundada às potencialidades e inclinações das crianças que constituem os nossos grupos, para que possamos estimular e desenvolver as suas competências. É nesse sentido que desenvolvemos este Estudo Exploratório, com o qual se pretende analisar a adaptação da avaliação de potencial derivada da Teoria das Inteligência Múltiplas ao contexto da Educação Pré-Escolar, com o objectivo de contribuir para a qualidade da intervenção educativa, articulando conceitos como Inteligência, Competência, Potencial, Currículo, Avaliação Dinâmica. Participam no estudo 42 crianças que frequentam Jardim de Infância e integram dois grupos heterogéneos e as respectivas educadoras. A metodologia é de natureza qualitativa e quantitativa, apoiando-se na aplicação dos seguintes instrumentos: modelo de avaliação de potencial baseado no Spectrum; Inventário de Quociente Emocional, versão para crianças e jovens (Bar-On Emocional Quotient Inventory: Youth Version), adaptado a educadores por Candeias e Monteiro (2010); questionário de Caracterização do ambiente educativo - Sala de Actividades, adaptado por Candeias e lglésias (2010); Teste de Avaliação de Habilidades Cognitivas de Solução de Problemas lnterpessoais (EVHACOSPI). Os resultados obtidos sugerem a importância de uma avaliação do potencial, baseada na Teoria das Inteligências Múltiplas, que utiliza instrumentos adequados a cada um dos domínios do potencial e da competência humana propostos por H. Gardner: verbal, Lógico-Matemática, Musical, Corporal-Cinestésica, Visuo-espacial, lnterpessoal, Intrapessoal e Naturalista. Apontam também para o papel que este tipo de avaliação pode desempenhar na intervenção educativa que se objectiva intencional e fundamentada. ABSTRACT: Ali of us, educators, must pay a special attention to the potentialities and inclinations of children who make up our groups, so enabling the stimulation and development of their competences. ln this sense we developed this Exploratory Study, through which is intended to analyse the adaptation of potential evaluation derived from the Theory of Multiple intelligences in the range of Pre-School Education, aiming at the contribution for the educative intervention: articulating concepts such as Intelligence, Competence Potential, Curriculum, and Dynamic Evaluation. The study involves 42 children attending Infant School, belonging to two heterogeneous groups and respective educators. Methodology is of qualitative and quantitative nature, supported by the following instruments: evaluation model based on Spectrum; Inventory of Emotional Quotient, version for children and youths, adapted to educators by Candeias e Monteiro (2010), Query of Characterization of Educative Environment - Activities Room, adapted by Candeias e lglésias (2010); Test for Evaluation of Cognitive Abilities of Interpersonal Problems Solution. (EVHACOSPI). The results suggest the importance of an assessment, based on the Theory of Multiple Intelligence, which uses instruments fitted to each of the domains of human potential and competence proposed by H. Gardiner: Verbal, Logical-Maths, Musical, Corporal¬ Kinaesthetic, Visuo-Spatial, Interpersonal, Intrapersonal and Naturalist. They also refer to the role that this type of assessment can play in the educational intervention aiming to be intentional and grounded.
