Biologic agents (also termed biologicals or biologics) are therapeutics that are synthesized by living organisms and directed against a specific determinant, for example, a cytokine or receptor. In inflammatory and autoimmune diseases, biologicals have revolutionized the treatment of several immune-mediated disorders. Biologicals have also been tested in allergic disorders. These include agents targeting IgE; T helper 2 (Th2)-type and Th2-promoting cytokines, including interleukin-4 (IL-4), IL-5, IL-9, IL-13, IL-31, and thymic stromal lymphopoietin (TSLP); pro-inflammatory cytokines, such as IL-1β, IL-12, IL-17A, IL-17F, IL-23, and tumor necrosis factor (TNF); chemokine receptor CCR4; and lymphocyte surface and adhesion molecules, including CD2, CD11a, CD20, CD25, CD52, and OX40 ligand. In this task force paper of the Interest Group on Biologicals of the European Academy of Allergy and Clinical Immunology, we review biologicals that are currently available or tested for the use in various allergic and urticarial pathologies, by providing an overview on their state of development, area of use, adverse events, and future research directions.

ABSTRACT Dual-trap optical tweezers are often used in high-resolution measurements in single-molecule biophysics. Such measurements can be hindered by the presence of extraneous noise sources, the most prominent of which is the coupling of fluctuations along different spatial directions, which may affect any optical tweezers setup. In this article, we analyze, both from the theoretical and the experimental points of view, the most common source for these couplings in dual-trap optical-tweezers setups: the misalignment of traps and tether. We give criteria to distinguish different kinds of misalignment, to estimate their quantitative relevance and to include them in the data analysis. The experimental data is obtained in a, to our knowledge, novel dual-trap optical-tweezers setup that directly measures forces. In the case in which misalignment is negligible, we provide a method to measure the stiffness of traps and tether based on variance analysis. This method can be seen as a calibration technique valid beyond the linear trap region. Our analysis is then employed to measure the persistence length of dsDNA tethers of three different lengths spanning two orders of magnitude. The effective persistence length of such tethers is shown to decrease with the contour length, in accordance with previous studies.

É crescente o uso de alta taxa de dose em braquiterapia para tratamento do câncer. O número de pacientes tratados em relação ao número de equipamentos existentes no Brasil é muito grande, sendo constantes os fatores tempo e pressão na rotina de funcionamento. Por isso, é comum, até mesmo nos grandes serviços, a falta de uma reflexão mais cuidadosa voltada à resolução de situações de emergência. Este trabalho faz uma análise prática das situações de emergência mais freqüentes e propõe um manual de conduta básico quando erros impossibilitam o retorno da fonte radioativa ao cofre, causando possível irradiação adicional à paciente e ao "staff" em unidades remotas de braquiterapia. Sugerem-se treinamentos, cuidados adicionais e controle de qualidade como forma de minimizar doses indesejáveis, bem como atenuar transtornos causados nas situações de emergência.

Estamos viviendo desde la entrada al siglo XXI una explosión acelerada de las tecnologías, de las TIC’s, las redes sociales en internet, etc. que están modificando enormemente los hábitos de consumo y los principios de la sociedad. A todo ello, hay que añadir la situación de crisis económica general que existe en el mundo, y que afecta enormemente a las empresas y al tejido empresarial de un país. En este entorno, las compañías deben basar sus estrategias en estar cerca del cliente y ser capaces de visualizar necesidades no cubiertas. Sobre todo, las empresas deben transformarse para crear organizaciones humanizadas capaces de generar innovación y adaptarse rápidamente a los nuevos entornos que van surgiendo. Para poder hacer frente a esta nueva situación las empresas deben poner en valor el talento de todas las personas que forman parte de la organización. El activo más importante y escaso, para poder desarrollar estrategias triunfadoras. El trabajo pretende resumir la situación actual para las empresas, recoger todas las ideas y nuevas tendencias que existen en cuanto a la gestión del talento, la organización del trabajo y la creación de estructuras que permitan innovar. A lo largo del trabajo se van desarrollando las diferentes herramientas, sistemas y disciplinas que están a disposición de las empresas para crear un entorno que favorezca la innovación, como camino para mantenerse en el mercado global.

The Commission on Classification and Terminology and the Commission on Epidemiology of the International League Against Epilepsy (ILAE) have charged a Task Force to revise concepts, definition, and classification of status epilepticus (SE). The proposed new definition of SE is as follows: Status epilepticus is a condition resulting either from the failure of the mechanisms responsible for seizure termination or from the initiation of mechanisms, which lead to abnormally, prolonged seizures (after time point t1 ). It is a condition, which can have long-term consequences (after time point t2 ), including neuronal death, neuronal injury, and alteration of neuronal networks, depending on the type and duration of seizures. This definition is conceptual, with two operational dimensions: the first is the length of the seizure and the time point (t1 ) beyond which the seizure should be regarded as "continuous seizure activity." The second time point (t2 ) is the time of ongoing seizure activity after which there is a risk of long-term consequences. In the case of convulsive (tonic-clonic) SE, both time points (t1 at 5 min and t2 at 30 min) are based on animal experiments and clinical research. This evidence is incomplete, and there is furthermore considerable variation, so these time points should be considered as the best estimates currently available. Data are not yet available for other forms of SE, but as knowledge and understanding increase, time points can be defined for specific forms of SE based on scientific evidence and incorporated into the definition, without changing the underlying concepts. A new diagnostic classification system of SE is proposed, which will provide a framework for clinical diagnosis, investigation, and therapeutic approaches for each patient. There are four axes: (1) semiology; (2) etiology; (3) electroencephalography (EEG) correlates; and (4) age. Axis 1 (semiology) lists different forms of SE divided into those with prominent motor systems, those without prominent motor systems, and currently indeterminate conditions (such as acute confusional states with epileptiform EEG patterns). Axis 2 (etiology) is divided into subcategories of known and unknown causes. Axis 3 (EEG correlates) adopts the latest recommendations by consensus panels to use the following descriptors for the EEG: name of pattern, morphology, location, time-related features, modulation, and effect of intervention. Finally, axis 4 divides age groups into neonatal, infancy, childhood, adolescent and adulthood, and elderly.

La constante évolution des biotechnologies de la procréation médicalement assistée (PMA) introduit des nouveautés qui perturbent les représentations de la famille et du « naturel » de la procréation notamment. Ces nouveautés engendrent des préoccupations aussi bien sociales qu'individuelles sur la légitimité et les conditions du recours à la PMA. Partant d'une approche dialogique de la communication, du langage et de la cognition ainsi que de la théorie des représentations sociales, nous faisons l'hypothèse que ces perturbations sont traitées différemment selon l'activité communicative dans laquelle les individus sont engagés. Nous avons alors travaillé à partir de deux corpus de données relevant d'un type d'activité communicative différent : un corpus de presse, de l'ordre d'un discours générique, portant sur la grossesse dite « tardive » (post-ménopause) et un corpus d'entretiens de recherche, de l'ordre d'un discours singulier, avec des couples qui ont recouru à la PMA, et portant sur la cryoconservation des zygotes. Nous appuyant sur les méthodes de l'analyse thématique et de l'analyse de discours, nous centrons notre examen sur les représentations sociales de la maternité (corpus de presse) et du « naturel » (corpus d'entretiens). Nous analysons ce que ces discours permettent d'accomplir socialement, étudions le rapport qu'ils établissent à l'ordre social et symbolique et interrogeons leurs fonctions en termes de processus de « naturalisation » de la PMA. Nos résultats montrent que les nouveautés de la PMA forcent les individus à se (re)positionner dans le champ des valeurs et des normes, lis montrent aussi que, dans ce contexte, les représentations sociales existantes sont mobilisées de différentes manières en fonction des visées argumentatives poursuivies. Ceci impliquant, par ailleurs, si ce n'est une transformation, du moins un renouvellement des représentations. Ils indiquent en outre que, bien que le discours de presse et celui des entretiens ne servent pas exactement les mêmes objectifs, leurs effets de « naturalisation » de la PMA sont comparables en termes de (re)construction de la norme. D'un point de vue dialogique, ils permettent aussi d'avancer que les polémiques sociales et individuelles sont non seulement articulées mais se co-constituent. Ils témoignent aussi du fait que la référence au « naturel » renvoie, plus qu'à un problème de perturbation de la « nature », à l'idée d'une menace de l'ordre social et symbolique. De ce point de vue, les réflexions sur les pratiques de PMA méritent d'être menées par la psychologie sociale et les sciences humaines et sociales plus largement. -- The constant evolution of biotechnologies of medically assisted procreation (MAP) introduces novelties that disturb representations of the family and, notably, of what is considered « natural » in procreation. These novelties give rise to social and individual questions about the legitimacy and the conditions of the use of MAP. Drawing upon a dialogical approach to communication, language and cognition, as well as social representations theory, a hypothesis is advanced that these disturbances are handled differently, depending on the communicative activity individuals are engaged in. Two corpuses of data, representing different communicative activity types, were used: a press corpus, reflecting a generic discourse on "late" pregnancy (post-menopause), and a research interviews corpus, reflecting particular discourses developed by couples who used MAP and have cryopreserved zygotes. Using methods of thematic and discourse analysis, the study focuses on social representations of pregnancy (press corpus) and of the "natural" (interviews corpus). The analysis questions what these discourses enable to achieve socially, how they relate to the social and symbolic order, as well as their function in "naturalising" MAP. The results show that MAP novelties force individuals to (re)position themselves in the field of values and norms. They also show that, in this context, existing social representations are mobilised in different ways depending on the argumentative aims that are being pursued. This implies, if not a transformation, at minimum a renewal of representations. Additionally the results indicate that, although press and interviews discourses might not serve the same aims, their effects on "naturalising" MAP are comparable in terms of (re)constructing the norm. From a dialogical perspective, the findings also allow us to argue that social and individual polemics are not only articulated with one another but are effectively co-constituted. They also evidence the fact that the reference to the "natural" brings forth, more than a problem of disturbing "nature", a threat to the social and symbolic order. From this perspective, reflections on MAP practices deserve further attention within social psychology and social and human sciences at large.

The quality of sample inoculation is critical for achieving an optimal yield of discrete colonies in both monomicrobial and polymicrobial samples to perform identification and antibiotic susceptibility testing. Consequently, we compared the performance between the InoqulA (BD Kiestra), the WASP (Copan), and manual inoculation methods. Defined mono- and polymicrobial samples of 4 bacterial species and cloudy urine specimens were inoculated on chromogenic agar by the InoqulA, the WASP, and manual methods. Images taken with ImagA (BD Kiestra) were analyzed with the VisionLab version 3.43 image analysis software to assess the quality of growth and to prevent subjective interpretation of the data. A 3- to 10-fold higher yield of discrete colonies was observed following automated inoculation with both the InoqulA and WASP systems than that with manual inoculation. The difference in performance between automated and manual inoculation was mainly observed at concentrations of >10(6) bacteria/ml. Inoculation with the InoqulA system allowed us to obtain significantly more discrete colonies than the WASP system at concentrations of >10(7) bacteria/ml. However, the level of difference observed was bacterial species dependent. Discrete colonies of bacteria present in 100- to 1,000-fold lower concentrations than the most concentrated populations in defined polymicrobial samples were not reproducibly recovered, even with the automated systems. The analysis of cloudy urine specimens showed that InoqulA inoculation provided a statistically significantly higher number of discrete colonies than that with WASP and manual inoculation. Consequently, the automated InoqulA inoculation greatly decreased the requirement for bacterial subculture and thus resulted in a significant reduction in the time to results, laboratory workload, and laboratory costs.

OBJETIVO: Avaliar, radiograficamente, o efeito da tração manual sobre o comprimento da coluna cervical. MATERIAIS E MÉTODOS: Cinqüenta e cinco participantes de ambos os gêneros - 12 masculinos (22%) e 43 femininos (78%) - sem história de distúrbios cervicais contituíram a amostra deste estudo. Eles foram submetidos a dois procedimentos radiológicos, um antes e outro durante a tração manual sustentada por 120 segundos. As distâncias entre as bordas anteriores e posteriores da segunda à sétima vértebras cervicais foram mensuradas e comparadas antes e durante a tração manual. RESULTADOS: A mediana da distância anterior antes da tração foi de 8,40 cm e durante a tração aumentou para 8,50 cm (p=0,002). A mediana da distância posterior antes da tração foi de 8,35 cm e durante a tração aumentou para 8,50 cm (p<0,001). CONCLUSÃO: Os resultados demonstraram que a aplicação da tração manual promoveu aumento estatisticamente significante do comprimento da coluna cervical em indivíduos assintomáticos.

The objective of this study is to show that bone strains due to dynamic mechanical loading during physical activity can be analysed using the flexible multibody simulation approach. Strains within the bone tissue play a major role in bone (re)modeling. Based on previous studies, it has been shown that dynamic loading seems to be more important for bone (re)modeling than static loading. The finite element method has been used previously to assess bone strains. However, the finite element method may be limited to static analysis of bone strains due to the expensive computation required for dynamic analysis, especially for a biomechanical system consisting of several bodies. Further, in vivo implementation of strain gauges on the surfaces of bone has been used previously in order to quantify the mechanical loading environment of the skeleton. However, in vivo strain measurement requires invasive methodology, which is challenging and limited to certain regions of superficial bones only, such as the anterior surface of the tibia. In this study, an alternative numerical approach to analyzing in vivo strains, based on the flexible multibody simulation approach, is proposed. In order to investigate the reliability of the proposed approach, three 3-dimensional musculoskeletal models where the right tibia is assumed to be flexible, are used as demonstration examples. The models are employed in a forward dynamics simulation in order to predict the tibial strains during walking on a level exercise. The flexible tibial model is developed using the actual geometry of the subject’s tibia, which is obtained from 3 dimensional reconstruction of Magnetic Resonance Images. Inverse dynamics simulation based on motion capture data obtained from walking at a constant velocity is used to calculate the desired contraction trajectory for each muscle. In the forward dynamics simulation, a proportional derivative servo controller is used to calculate each muscle force required to reproduce the motion, based on the desired muscle contraction trajectory obtained from the inverse dynamics simulation. Experimental measurements are used to verify the models and check the accuracy of the models in replicating the realistic mechanical loading environment measured from the walking test. The predicted strain results by the models show consistency with literature-based in vivo strain measurements. In conclusion, the non-invasive flexible multibody simulation approach may be used as a surrogate for experimental bone strain measurement, and thus be of use in detailed strain estimation of bones in different applications. Consequently, the information obtained from the present approach might be useful in clinical applications, including optimizing implant design and devising exercises to prevent bone fragility, accelerate fracture healing and reduce osteoporotic bone loss.

Newly generated olfactory receptor axons grow from the peripheral to the central nervous system aided by olfactory ensheathing cells (OECs). Thus, OEC transplantation has emerged as a promising therapy for spinal cord injuries and for other neural diseases. However, these cells do not present a uniform population, but, instead, a functionally heterogeneous population that exhibits a variety of responses including adhesion, repulsion and crossover during cell-cell and cell-matrix interactions. Some studies report that the migratory properties of OECs are compromised by inhibitory molecules and potentiated by chemical gradients. Here, we demonstrated that rodent OECs express all the components of the Nogo Receptor complex and that their migration is blocked by Myelin. Next, we used cell tracking and traction force microscopy to analyze OEC migration and its mechanical properties over Myelin. Our data relate the absence of traction force of OEC with lower migratory capacity, which correlates with changes in the F-Actin cytoskeleton and focal adhesion distribution. Lastly, OEC traction force and migratory capacity is enhanced after cell incubation with the Nogo Receptor inhibitor NEP1-40.

OBJECTIVE: To develop disease-specific recommendations for the diagnosis and management of eosinophilic granulomatosis with polyangiitis (Churg-Strauss syndrome) (EGPA). METHODS: The EGPA Consensus Task Force experts comprised 8 pulmonologists, 6 internists, 4 rheumatologists, 3 nephrologists, 1 pathologist and 1 allergist from 5 European countries and the USA. Using a modified Delphi process, a list of 40 questions was elaborated by 2 members and sent to all participants prior to the meeting. Concurrently, an extensive literature search was undertaken with publications assigned with a level of evidence according to accepted criteria. Drafts of the recommendations were circulated for review to all members until final consensus was reached. RESULTS: Twenty-two recommendations concerning the diagnosis, initial evaluation, treatment and monitoring of EGPA patients were established. The relevant published information on EGPA, antineutrophil-cytoplasm antibody-associated vasculitides, hypereosinophilic syndromes and eosinophilic asthma supporting these recommendations was also reviewed. DISCUSSION: These recommendations aim to give physicians tools for effective and individual management of EGPA patients, and to provide guidance for further targeted research.
