(c) 2012 John Wiley & Sons A/S Background and Objective: Successful root-coverage treatment depends on the thickness of the donor tissue. This study aimed to evaluate the thickness of donor tissue after augmentation of the connective tissue in the palatal area by implantation of lyophilized collagen sponge (Hemospon (R)). Material and Methods: Ten patients with an indication for root coverage, whose palate was deficient in adequate connective tissue, were recruited. The procedure was carried out in two stages. In the first stage, the palatal thickness in the donor site was measured at three standardized points (points 1, 2 and 3), from the distal of the canine to the distal of the first molar, and the lyophilized collagen sponge was inserted. In the second stage, the palatal thickness over the implant was measured (at points 1, 2 and 3), two biopsies of the palatal mucosa were collected one over the implant (experimental sample) and the other on the contralateral side (control sample) and then root-coverage treatment was performed. Analyses consisted of clinical assessment of the palatal measurements before and after sponge implantation, and histological assessment of the experimental and control biopsy samples. Data were analyzed using the Wilcoxon test. Results: Both analyses showed a significant increase in mean thickness, of 1.08 mm of neoformed tissue in the clinical analysis (the tissue at point 2 was the thickest of the three points) and of 0.53 mm in the histological analysis. Conclusion: The insertion of lyophilized collagen sponge induced a significant increase in the thickness of palatal connective tissue.

The objective of the current study was to assess histo-morphometrically the healing process of recession defects associated with scraped roots treated with subepithelial connective tissue graft (SCTG). Six dogs were used. Bone dehiscence defects (6 x 8 mm) and root planing were carried out on maxillary canine teeth. Following a split-mouth model, according to the treatment, left canines (control) were covered with coronally positioned flap (CPF). Right canines were submitted to treatments with SCTG. After a 3-month postoperative period, the animals were killed, and the blocks processed for the histomorphometric assessment. Data assessment demonstrated that the CPF group showed parameters of a new connective tissue attachment, length of new cement (NC), length of new bone (mean +/- SD: 0.95 +/- 0.53, 2.44 +/- 1.97, and 1.96 +/- 2.29 mm, respectively), which were higher than those of SCTG group (mean +/- SD: 0.71 +/- 0.36, 2.21 +/- 1.28, and 1.52 +/- 1.31 mm, respectively), although not significantly (P > 0.05). The length of both epithelial tissue and connective tissue apposition in the SCTG group (mean +/- SD: 1.70 +/- 0.53 and 2.62 +/- 1.52 mm, respectively) were higher than those of the CPF group (mean +/- SD: 1.18 +/- 0.49 and 2.03 +/- 1.03 mm, respectively), although showing no significant differences (P > 0.05). Within the limits of the current study, it was possible to conclude that there were no significant differences between the groups according to the histologic parameters assessed.

The aim of this study was to evaluate the tendency of displacement of the supporting structures of the distal extension removable partial denture (DERPD) associated to the implant with different inclinations of alveolar ridge and implant localizations through a two-dimensional finite-element method. Sixteen mandibular models were fabricated, presenting horizontal, distally descending, distally ascending, or descending-ascending ridges. All models presented the left canine and were rehabilitated with conventional DERPD or implant-retained prosthesis with the ERA system. The models were obtained by the AutoCAD software and transferred to the finite-element software ANSYS 9.0 for analysis. A force of 50 N was applied on the cusp tips of the teeth, with 5 points of loading of 10 N. The results were visualized by displacement maps. For all ridge inclinations, the assembly of the DERPD with distal plate retained by an anterior implant exhibited the lowest requisition of the supporting structures. The highest tendency of displacement occurred in the model with distally ascending ridge with incisal rest. It was concluded that the association of the implant decreased the displacement of the DERPD, and the anterior positioning of the implant associated to the DERPD with the distal plate preserved the supporting structures for all ridges.

The aim of this study was to evaluate the biomechanical behavior of a mandibular distal extension removable partial denture (DERPD) associated with an implant and different retention system, by bidimensional finite element method. Five hemimandible models with a canine and external hexagon implant at second molar region associated with DERPD were simulated: model A, hemimandible with a canine and a DERPD; model B, hemimandible with a canine and implant with a healing abutment associated to a DERPD; model C, hemimandible with a canine and implant with an ERA attachment associated to a DERPD; model D, hemimandible with a canine and implant with an O'ring attachment associated to a DERPD; and model E, hemimandible with a canine and implant-supported prosthesis associated to a DERPD. Cusp tips were loaded with 50 N of axial or oblique force (45 degrees). Finite element analysis was performed in ANSYS 9.0. model E showed the higher displacement and overload in the supporting tissues; the patterns of stress distribution around the dental apex of models B, C, and D were similar. The association between a DERPD and an osseointegrated implant using the ERA or O'ring systems shows lower stress values. Oblique forces showed higher stress values and displacement. Oblique forces increased the displacement and stress levels in all models; model C displayed the best stress distribution in the supporting structures; healing abutment, ERA, and O'ring systems were viable with RPD, but DERPD association with a single implant-supported prosthesis was nonviable.

Purpose: To verify the prevalence of malocclusion and the influence of harmful oral habits on deciduous dentition in 5- and 6-year-old children enrolled in Brazilian public elementary schools during 2010.Materials and Methods: Exams were conducted in 1385 children from 56 Brazilian elementary schools using the method recommended by the World Health Organization (WHO) for epidemiological surveys on oral health. Information about the type of arch, social and economic data and harmful oral habits of the children were collected through a structured questionnaire.Results: In relation to canine occlusion, a high prevalence of Class I (74.5%), followed by Class II (19.4%), was found. Among all participants, 22% showed high overjet, 7.8% showed edge-to-edge occlusion and 2.3% showed anterior crossbite. In relation to overbite, 13.2% had short overbite, 14.3% open bite and 16.8% high overbite. The presence of posterior crossbite occurred in 14.6% of children. Maxillae predominantly exhibited the type I arch (67.9%) and mandibles predominantly exhibited type II (51.7%). In relation to harmful oral habits, 43.4% used a pacifier, 84.8% used a bottle and finger sucking was reported by 17.2%.Conclusion: There was a high prevalence of malocclusion associated with oral habits harmful to deciduous dentition.

OBJETIVO: a reabsorção radicular apical é uma condição comumente observada durante e após o tratamento ortodôntico e fatores como tipo de aparelhagem ortodôntica utilizada, magnitude das forças aplicadas e duração do tratamento podem estar relacionados ao processo de arredondamento do ápice radicular. Buscou-se avaliar, por meio de imagens radiográficas computadorizadas, a quantidade de reabsorção no ápice radicular, quando da utilização de duas diferentes técnicas de mecânica ortodôntica fixa: Edgewise com acessórios padrão e Edgewise com acessórios totalmente programados. METODOLOGIA: a amostra constituiu-se de 20 pacientes tratados pelo mesmo profissional na Clínica de Ortodontia da Faculdade de Odontologia de Araçatuba - UNESP. Os pacientes foram divididos em dois grupos: grupo 1 (Tratados com Edgewise com acessórios padrão e fios de aço) e grupo 2 (Tratados com Edgewise com acessórios totalmente programados e fios de níquel-titânio). A avaliação radiográfica digital foi realizada por um único operador por meio de exposições radiográficas digitais feitas no início do tratamento ortodôntico, após retração de caninos e final de tratamento, utilizando o sistema de escores proposto por Levander e Malmgren (1988). RESULTADOS E CONCLUSÕES: a análise dos escores permitiu concluir que o tratamento ortodôntico empregando a técnica Edgewise com acessórios totalmente programados e fios de níquel-titânio apresentou menores graus de reabsorção radicular apical, em comparação à técnica Edgewise com acessórios padrão e fios de aço. Observou-se que, independentemente da técnica empregada, o tratamento ortodôntico como um todo apresentou um grau moderado de reabsorção radicular apical.

OBJETIVOS: comparar os valores médios normais das angulações mesiodistais dentárias, propostos por Ursi, em 1989, com as angulações mesiodistais de caninos, pré-molares e molares inferiores em indivíduos com e sem a presença dos terceiros molares inferiores e idades entre 18 e 25 anos. Além disso, foram comparados os valores das angulações mesiodistais desses dentes nessas duas situações. MÉTODOS: foram utilizadas 40 radiografias ortopantomográficas de indivíduos, de ambos os gêneros, que não receberam tratamento ortodôntico, divididos em dois grupos: Grupo I, constituído por 20 radiografias que não apresentavam os terceiros molares inferiores; e Grupo II, formado por 20 radiografias com os terceiros molares inferiores presentes. RESULTADOS E CONCLUSÕES: a análise dos resultados e a análise estatística permitiram concluir que ambos os grupos exibiram pré-molares e molares inferiores mais angulados em sentido mesial, quando comparados à oclusão normal. Por outro lado, a angulação mesiodistal de caninos inferiores mostrou-se semelhante àquela apresentada em casos de oclusão normal. Os dois grupos, quando comparados entre si, exibiram semelhantes valores angulares dos caninos, pré-molares e molares inferiores, de modo que a presença dos terceiros molares não exerceu influência sobre essas angulações mesiodistais dentárias.

OBJETIVO: avaliar as diferenças produzidas nas dimensões e forma de arco pelos tratamentos com aparelho expansor fixo tipo Hyrax e aparelho expansor removível tipo Placa de Hawley com parafuso expansor palatino centralizado. MATERIAL E MÉTODOS: foram selecionados modelos de estudo iniciais e finais de 31 crianças portadoras de mordida cruzada posterior, de ambos os gêneros, de origem étnica diversa e na fase da dentadura mista, tratadas nos cursos de Graduação e Pós-Graduação em Ortodontia do Departamento de Clínica Infantil da Faculdade de Odontologia de Araraquara - UNESP. Destas crianças, 15 foram tratadas com uso do aparelho expansor fixo tipo Hyrax e as demais 16 crianças foram tratados com expansão rápida da maxila efetuada com o aparelho expansor fixo. Foram realizadas medidas das distâncias intercaninos e intermolares, da inclinação do processo alveolar e inclinação dentária nas imagens escaneadas do arco superior e do contorno do palato, obtido com auxílio de um template ajustável e do programa de análises Radiocef Studio. RESULTADOS E CONCLUSÕES: os resultados indicam uma mudança significante nas distâncias intercaninos e intermolares em ambos os grupos e uma inclinação dentária e do processo alveolar para vestibular significante no grupo tratado com o aparelho expansor fixo. A expansão conseguida pelo aparelho expansor fixo foi aproximadamente o dobro da promovida pelo aparelho expansor removível.

Para o presente trabalho, 235 crianças pertencentes às creches da Prefeitura Municipal de Araraquara foram avaliadas e moldadas com um dispositivo confeccionado com cera utilidade e palito abaixador de língua na forma dos arcos dentários. No intervalo de um ano as mesmas crianças foram remoldadas afim de se verificar se houveram ou não mudanças nas dimensões do arco dentário decíduo. A partir da obtenção dos modelos em gesso, foram realizadas medições por meio de um dispositivo digitalizador tridimensional denominado MicroScribe-3DX nos instantes inicial (primeira moldagem) e final (moldagem após um ano). Foram avaliadas medidas referentes às distâncias inter-segundos molares, interprimeiros molares, intercaninos, perímetro, comprimento de arco e espaços primatas. Consideraram-se ainda dimorfismo sexual, tipo de arco e influência de hábitos. Concluiu-se que as dimensões transversais sofreram aumento significativo na dentadura decídua, enquanto que o perímetro, o comprimento e os espaços primatas permaneceram constantes. As distâncias intermolares apresentaram dimorfismo sexual, com dimensões maiores no gênero feminino. Com relação às medidas de perímetro, comprimento e espaços primatas não ocorreram diferenças significantes em relação ao gênero. As dimensões de comprimento nos arcos não diferem nos arcos decíduos Tipo I e Tipo II de Baume, enquanto que o diâmetro dos arcos Tipo I de Baume, são maiores do que os do Tipo II. Os hábitos de sucção de dedo e chupeta não provocaram alterações nas dimensões dos arcos decíduos no período observado, de um ano.

Objective. Pixel intensity values (PI) and fractal dimensions (FD) were compared in selected mandibular regions on digital panoramic images of normal, osteopenic, and osteoporotic perimenopausal and postmenopausal women to evaluate their relative efficacies in detecting osteoporotic-associated bone density changes.Study design. Standardized mandibular angle, body, and canine/premolar (C/PM) regions on 54 charge-coupied device (CCD) digital panoramic images of normal and potentially osteoporotic postmenopausal women were analyzed for PI and FD. Lumbar spine and femoral neck dual-energy x-ray absorptiometry QXA) on each patient served as the reference standard examination. Pearson correlation coefficients and analysis of variance (ANOVA) were performed.Results. There was significant correlation among PI measurements (P < 0.01), and no significant correlation between FD. C/PM had significantly lower PI than control C/PM (P = 0.049).Conclusions. Osteoporotic changes in mandibular C/PM cancellous bone were detected in our study population on CCD digital panoramic images by using a robust image analysis paradigm. Future automated application of such image analysis could enable widespread, cost effective screening for osteoporosis in dental settings.

Purpose: To evaluate the biomechanical fixation, bone-to-implant contact (BIC), and bone morphology of screw-type root-form implants with healing chambers with as-machined or dual acid-etched (DAE) surfaces in a canine model. Materials and Methods: The animal model included the placement of machined (n = 24) and DAE (n = 24) implants along the proximal tibiae of six mongrel dogs, which remained in place for 2 or 4 weeks. Following euthanasia, half of the specimens were subjected to biomechanical testing (torque to interface failure) and the other half were processed for histomorphologic and histomorphometric (%BIC) assessments. Statistical analyses were performed by one-way analysis of variance at the 95% confidence level and the Tukey post hoc test for multiple comparisons. Results: At 4 weeks, the DAE surface presented significantly higher mean values for torque to interface failure overall. A significant increase in %BIC values occurred for both groups over time. For both groups, bone formation through the classic appositional healing pathway was observed in regions where intimate contact between the implant and the osteotomy walls occurred immediately after implantation. Where contact-free spaces existed after implantation (healing chambers), an intramembranous-like healing mode with newly formed woven bone prevailed. Conclusions: In the present short-term evaluation, no differences were observed in BIC between groups; however, an increase in biomechanical fixation was seen from 2 to 4 weeks with the DAE surface. This study investigated the effect of an Argon-based atmospheric pressure plasma (APP) surface treatment operated chairside at atmospheric pressure conditions applied immediately prior to dental implant placement in a canine model. Surfaces investigated comprised: rough titanium surface (Ti) and rough titanium surface + Argon-based APP (Ti-Plasma). Surface energy was characterized by the Owens-Wendt-Rabel-Kaelble method and chemistry by X-ray photoelectron spectroscopy (XPS). Six adult beagles dogs received two plateau-root form implants (n = 1 each surface) in each radii, providing implants that remained 1 and 3 weeks in vivo. Histometric parameters assessed were bone-to-implant contact (BIC) and bone area fraction occupancy (BAFO). Statistical analysis was performed by Kruskall-Wallis (95% level of significance) and Dunn's post-hoc test. The XPS analysis showed peaks of Ti, C, and O for the Ti and Ti- Plasma surfaces. Both surfaces presented carbon primarily as hydrocarbon (C?C, C?H) with lower levels of oxidized carbon forms. The Ti-Plasma presented large increase in the Ti (+11%) and O (+16%) elements for the Ti- Plasma group along with a decrease of 23% in surface-adsorbed C content. At 1 week no difference was found in histometric parameters between groups. At 3 weeks significantly higher BIC (>300%) and mean BAFO (>30%) were observed for Ti-Plasma treated surfaces. From a morphologic standpoint, improved interaction between connective tissue was observed at 1 week, likely leading to more uniform and higher bone formation at 3 weeks for the Ti-Plasma treated implants was observed. Objectives: To evaluate the use of the center of the incisive papilla as a guide for the selection of the proper width of maxillary dentures in 4 racial groups. Method and Materials: One hundred sixty stone casts were obtained from impressions of the maxillary arch of white, black, mixed, and Asian subjects. The occlusal surfaces of the casts were photocopied and the images placed on a digitizer. The most anterior and posterior points of the papilla and cusp tips of the canines were digitized. Dentofacial Planner Plus software was used to calculate the distance from a line passing through the cusp tips of the canines to the center of the papilla, defined as the midpoint of the anterior and posterior points of the papilla. The selection error (in millimeters) due to the clinical application of the method of the incisive papilla was calculated and analyzed. Results: In all studied racial groups, there was no coincidence between the center of the incisive papilla and the canine line. The utilization of the center of the papilla would lead to the selection of wider artificial teeth. In 24.9% of the white, 19.3% of the mixed, 32.9% of the black, and 15.5% of the Asian populations, errors greater than 4 mm would be present with the utilization of the papilla. Conclusion: The method of the center of the incisive papilla is not accurate, but may aid in initial artificial teeth selection for the racial groups studied. (Quintessence Int 2008;39:841-845)
