The central question addressed in this paper is to what extent the influence of social origin on life chances has changed over time for both men and women. In order to capture this change, intergenerational social mobility of eight different birth-cohorts, covering most of the entire twentieth century, is analysed using a unique collection of twelve Swiss national population sample surveys. The main results show that social mobility has remained constant across cohorts born in 1912 and those born in 1974. This suggests that unlike some other industrialised countries, inequality based on social origin is persistent in Switzerland.

Female lung cancer mortality increased by 50% between the mid 1960s and the early 2000s in the European Union (EU). To monitor the current lung cancer epidemic in European women, we analyzed mortality trends in 33 European countries between 1970 and 2009 and estimated rates for the year 2015 using data from the World Health Organization. Female lung cancer mortality has been increasing up to recent calendar years in most European countries, with the exceptions of Belarus, Russia, and Ukraine, with relatively low rates, and the UK, Iceland and Ireland, where high rates were reached in mid/late 1990s to leveled off thereafter. In the EU, female lung cancer mortality rates rose over the last decade from 11.3 to 12.7/100,000 (+2.3% per year) at all ages and from 18.6 to 21.5/100,000 (+3.0% per year) in middle-age. A further increase is predicted, to reach 14/100,000 women in 2015. Lung cancer mortality trends have been more favorable over the last decade in young women (20-44 years), particularly in the UK and other former high-risk countries from northern and central/eastern Europe, but also in France, Italy, and Spain where mortality in young women has been increasing up to the early 2000s. In the EU as a whole, mortality at age 20-44 years decreased from 1.6 to 1.4/100,000 (-2.2% per year). Although the female lung cancer epidemic in Europe is still expanding, the epidemic may be controlled through the implementation of effective anti-tobacco measures, and it will probably never reach the top US rates.

Background Early age at first delivery has been identified as a risk factor for high-risk HPV-type infection and cervical cancer development. Methods A cross-sectional study was carried out in a large public maternity hospital in Sao Paulo, Brazil. During June 2006 to February 2007, 301 women aged 15-24 years who gave birth to their first child were recruited between 43 and 60 days after delivery. Detection of HPV DNA in cervical specimens was performed using a standardised PCR protocol with PGMY09/11 primers. The association of selected factors with HPV infection was assessed by using a Generalised Linear Model. Results HPV DNA was detected in 58.5% (95% CI 52.7% to 64.0%) of the enrolled young women. The most common types of HPV found were: HPV16, HPV51, HPV52, HPV58 and HPV71. The overall prevalence of HPV types targeted by the HPV prophylactic vaccines was: HPV 16-12.0%, HPV 18-2.3% and HPV 6 and 11 4.3%. In the multivariate analysis, only age (inversely, p for trend=0.02) and smoking habits were independently associated with HPV infection. Conclusions The findings show that these young primiparous women had high cervical HPV prevalence, suggesting that this is a high-risk group for cervical cancer development. Nevertheless, 17.3% were positive for any of the four HPV types included in HPV vaccines (HPV6, 11, 16 or 18), with 13.3% positive for HPV 16 or 18 and only 1.0% having both vaccine related-oncogenic HPV types. Thus, young primiparous women could benefit from catch-up HPV vaccination programmes.

Aim: To determine the impact of iron therapy on the quality of life of non-anaemic iron-deficient women with substantial unexplained fatigue. Methods: Double blind randomised placebo controlled trial in 198 women aged 18 to 53 and having a ferritin level <50 ng/mL, assigned to either oral ferrous sulphate (80 mg/day of elemental iron daily; n = 102) or placebo (n = 96) for 12 weeks, by 44 general practices in France. Main outcome measures: Level of fatigue, depression and anxiety, measured by a 24-item self-administered questionnaire. Level of fatigue was also assessed with a visual analogue scale. Results: 171 (86.4%) women were eligible for efficacy analysis. Mean age, haemoglobin concentration, serum ferritin concentration, level of fatigue, depression, and anxiety were similar in both groups at baseline. Both groups were also similar for compliance and dropout rates. After 12 weeks, asthenia score decreased by &#8722;12.9 } 10.37 points (50.8%) in the iron group compared with -9.01 } 11.71 points (36.7%) in the placebo group (p = 0.02), whereas depression and anxiety scores, already low at inclusion, slightly decrease to the same extent in both groups. In an intention to treat analysis, by considering a responder to iron supplementation as having more than two points decrease on the fatigue 10-point visual analogue scale, iron group had 83,3% (85/102) responders vs. 69.8% (67/96) in the control group (p = 0.02). The number needed to treat to have a benefit was 7. Conclusion: Iron supplementation is an efficient inexpensive approach to manage unexplained fatigue in non-anaemic iron-deficient women.

This is a critical review of the medical, ethical, judicial and financial aspects of the so called "social freezing", the cryopreservation of a woman's oocytes for non-medical purposes. The possibility of storing the eggs of fertile women in order to prevent age-related fertility decline is being widely promoted by fertility centres and the lay press throughout the world. Research data has shown that social freezing should ideally be performed on women around 25 years of age in order to increase their chances of a future pregnancy. In reality, it is mostly performed after the age of 35. Unfortunately, social freezing is in general not a solution for the underlying societal problems to fit in with professionally active women and having children. It only delays the existing problems. Furthermore, it creates a lot of potential new problems. A great deal more should be undertaken to offer real solutions to the underlying societal problems which are in part: pre-school education, care in the event of childhood illness, and the many weeks of school holidays, acceptance of professionally active women having children, and more job offers with a workload &lt;100%.). Furthermore, society should be informed about the decreasing chances of pregnancy with increasing maternal (and paternal) age as well as the increasing risks of miscarriage and obstetric/neonatal complications. Detailed information for woman considering social freezing is crucial. Every doctor, proposing social freezing to his patients, should be up to date with all these details. Follow-up studies on the outcome of these children are needed.

Rsum Introduction : Le tabagisme est le facteur de risque le plus important dans 7 des 14 premires causes de dcs chez les personnes de plus de 65 ans. De nombreuses tudes ont dmontr les bnfices sur la sant d'un arrt du tabagisme mme un ge avanc. Malgr cela, peu d'actions prventives sont entreprises dans cette population. Le but de ce travail est d'analyser les caractristiques du tabagisme et de l'arrt du tabagisme spcifiquement chez les fumeuses d'ge avanc afin de mieux les aider dans leur dsir d'arrter. Mthode : Nous avons valu les caractristiques tabagiques au sein d'une tude prospective de 7'609 femmes vivant en Suisse, ges de plus de 70 ans et physiquement autonomes (tude Semof s'intressant la mesure de l'ostoporose par ultrason osseux). Un questionnaire sur les habitudes tabagiques a t envoy aux 486 fumeuses ligibles de la cohorte. Leurs stades de dpendance nicotinique et de motivation ont t valus l'aide respectivement des scores Heavy Smoking Index et Prochaska . Les participantes ayant cess de fumer pendant le suivi ont t questionnes sur, les motivations, les raisons et les mthodes de leur arrt. Rsultats : 424 femmes ont retourn le questionnaire (taux de rponse de 87%) parmi lesquelles 372 ont rpondu de faon complte permettant leur inclusion. L'ge moyen s'levait 74,5 ans. La consommation moyenne tait de 12 cigarettes par jour, sur une moyenne de 51 ans avec une prfrence pour les cigarettes dites lgres ou light . Un peu plus de la moiti des participantes avait une consommation entre 1 et 10 cigarettes par jour et la grande majorit (78%) prsentait un score de dpendance faible. Les raisons du tabagisme les plus frquemment voques taient la relaxation, le plaisir et l'habitude. Les principaux obstacles mentionns : arrter un ge avanc n'a pas de bnfice, fumer peu ou des cigarettes dites light n'a pas d'impact sur la sant et fumer n'augmente pas le risque d'ostoporose. Le dsir d'arrter tait positivement associ avec un dbut tardif du tabagisme, une ducation plutt modeste et la considration que d'arrter est difficile. Durant le suivi de 3 ans, 57 femmes sur 372 (15%) ont arrt de-fumer avec succs. Le fait d'tre une fumeuse occasionnelle (moins de 1 cigarette par jour) et de considrer que d'arrter de fumer n'est pas difficile tait associs un meilleur taux d'arrt du tabagisme. Seuls 11% des femmes ayant stopp la cigarette signalaient avoir reu des conseils de leur mdecin. Conclusion : ces donnes illustrent le comportement tabagique spcifique des fumeuses d'ge avanc (consommation et dpendance plutt faibles) et suggrent que les interventions mdicales pour l'aide l'arrt du tabagisme devraient intgrer ces caractristiques. La volont d'arrter est associe un niveau d'ducation plutt modeste. Les obstacles les plus frquemment mentionns sont bass sur des apprciations errones de l'impact du tabagisme sur la sant.

Backgroud: Household service work has been largely absent from occupational health studies. We examine the occupational hazards and health effects identified by immigrant women household service workers. Methods: Exploratory, descriptive study of 46 documented and undocumented immigrant women in household services in Spain, using a phenomenological approach. Data were collected between September 2006 and May 2007 through focus groups and semi-structured individual interviews. Data were separated for analysis by documentation status and sorted using a mixed-generation process. In a second phase of analysis, data on psychosocial hazards were organized using the Copenhagen Psychosocial Questionnaire as a guide. Results: Informants reported a number of environmental, ergonomic and psychosocial hazards and corresponding health effects. Psychosocial hazards were especially strongly present in data. Data on reported hazards were similar by documentation status and varied by several emerging categories: whether participants were primarily cleaners or carers and whether they lived in or outside of the homes of their employers. Documentation status was relevant in terms of empowerment and bargaining, but did not appear to influence work tasks or exposure to hazards directly. Conclusions:Female immigrant household service workers are exposed to a variety of health hazards that could be acted upon by improved legislation, enforcement, and preventive workplace measures, which are discussed.

Puerperium is a period of great changes in women's lives, in physiological, psychological and socio-cultural terms. Female sexuality is a broad topic that should be explored vis--vis the singularity of woman's experiences which themselves vary at different moments in time. This article is part of the project: Sexuality in the Puerperium, the objective of which was to understand what women's experience of their sexuality is in the postpartum period. Observation and semi-structured interview were used. The research approach was qualitative and narrative was used as the conceptual and methodological method. Six primiparas who gave birth in a public hospital in So Paulo took part in the study. Observation took place in the in-patients unit in the immediate post-partum period. The interviews were conducted in the womens homes. The results of the study have enabled an understanding to be formed of the meanings and changes in sexuality during the postpartum period as well as of the experiences every woman faces in her new roles, relationships and difficulties.

The timing of puberty is highly variable. We carried out a genome-wide association study for age at menarche in 4,714 women and report an association in LIN28B on chromosome 6 (rs314276, minor allele frequency (MAF) = 0.33, P = 1.5 x 10(-8)). In independent replication studies in 16,373 women, each major allele was associated with 0.12 years earlier menarche (95% CI = 0.08-0.16; P = 2.8 x 10(-10); combined P = 3.6 x 10(-16)). This allele was also associated with earlier breast development in girls (P = 0.001; N = 4,271); earlier voice breaking (P = 0.006, N = 1,026) and more advanced pubic hair development in boys (P = 0.01; N = 4,588); a faster tempo of height growth in girls (P = 0.00008; N = 4,271) and boys (P = 0.03; N = 4,588); and shorter adult height in women (P = 3.6 x 10(-7); N = 17,274) and men (P = 0.006; N = 9,840) in keeping with earlier growth cessation. These studies identify variation in LIN28B, a potent and specific regulator of microRNA processing, as the first genetic determinant regulating the timing of human pubertal growth and development.

Objective: To analyze the vascularization of the endometrium via hysteroscopy and to assess its correlation with angiogenic factor gene expression and embryo implantation rate.Design: Cross-sectional study.Setting: Public university hospital.Patient(s): Patients undergoing hysteroscopy for supposed infertility.Intervention(s): Endometrial quality assessment according to Sakumoto-Masamoto, performed in the early secretory phase of the cycle. Collection of an endometrial tissue biopsy.Main Outcome Measure(s): RNA extraction, reverse transcription, and determination of gene expression of angiogenesis- and implantation-relevant factors using quantitative polymerase chain reaction. Retrieval of pregnancy information from the medical records.Result(s): Good quantity/quality RNA with infertility history was obtained from 63 participating women. Those with a "good" endometrium and subsequent pregnancy showed increased gene expression for placenta growth factor when compared with patients with a "bad" endometrium and who did not succeed with pregnancy to date. Nonpregnant women with a "good" endometrium presented an intermediate result. No significant differences were observed for several other genes tested, but trends in the same direction were observed.Conclusion(s): This study demonstrates for the first time that endometrial PLGF expression corresponds to the hysteroscopic appearance of the endometrium, and therefore has potential as a clinically relevant prognosticator for infertility treatment success. The objective of this study is to examine the literature and identify most salient outcomes of early postnatal discharge for women, newborns and the health system. An electronic search strategy was designed including the following sources: Web of Science, Scopus, ProQuest and PubMed/MEDLINE, using the following terms: (early AND discharge) OR (length AND stay) AND (postpartum OR postnatal) AND (effect* OR result OR outcome). Content analysis was used to identify and summarise the findings and methods of the research papers. The evidence available is not enough to either reject or support the practice of early postnatal discharge; different studies have reported different outcomes for women and newborns. The need of systematic clinical research is discussed.

Visceral adiposity is increasingly recognized as a key condition for the development of obesity related disorders, with the ratio between visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) reported as the best correlate of cardiometabolic risk. In this study, using a cohort of 40 obese females (age: 25-45 y, BMI: 28-40 kg/m(2)) under healthy clinical conditions and monitored over a 2 weeks period we examined the relationships between different body composition parameters, estimates of visceral adiposity and blood/urine metabolic profiles. Metabonomics and lipidomics analysis of blood plasma and urine were employed in combination with in vivo quantitation of body composition and abdominal fat distribution using iDXA and computerized tomography. Of the various visceral fat estimates, VAT/SAT and VAT/total abdominal fat ratios exhibited significant associations with regio-specific body lean and fat composition. The integration of these visceral fat estimates with metabolic profiles of blood and urine described a distinct amino acid, diacyl and ether phospholipid phenotype in women with higher visceral fat. Metabolites important in predicting visceral fat adiposity as assessed by Random forest analysis highlighted 7 most robust markers, including tyrosine, glutamine, PC-O 44&#8758;6, PC-O 44&#8758;4, PC-O 42&#8758;4, PC-O 40&#8758;4, and PC-O 40&#8758;3 lipid species. Unexpectedly, the visceral fat associated inflammatory profiles were shown to be highly influenced by inter-days and between-subject variations. Nevertheless, the visceral fat associated amino acid and lipid signature is proposed to be further validated for future patient stratification and cardiometabolic health diagnostics.
