Parkinson disease is mainly characterized by the degeneration of dopaminergic neurons in the central nervous system, including the retina. Different interrelated molecular mechanisms underlying Parkinson disease-associated neuronal death have been put forward in the brain, including oxidative stress and mitochondrial dysfunction. Systemic injection of the proneurotoxin 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) to monkeys elicits the appearance of a parkinsonian syndrome, including morphological and functional impairments in the retina. However, the intracellular events leading to derangement of dopaminergic and other retinal neurons in MPTP-treated animal models have not been so far investigated. Here we have used a comparative proteomics approach to identify proteins differentially expressed in the retina of MPTP-treated monkeys. Proteins were solubilized from the neural retinas of control and MPTP-treated animals, labelled separately with two different cyanine fluorophores and run pairwise on 2D DIGE gels. Out of >700 protein spots resolved and quantified, 36 were found to exhibit statistically significant differences in their expression levels, of at least ±1.4-fold, in the parkinsonian monkey retina compared with controls. Most of these spots were excised from preparative 2D gels, trypsinized and subjected to MALDI-TOF MS and LC-MS/MS analyses. Data obtained were used for protein sequence database interrogation, and 15 different proteins were successfully identified, of which 13 were underexpressed and 2 overexpressed. These proteins were involved in key cellular functional pathways such as glycolysis and mitochondrial electron transport, neuronal protection against stress and survival, and phototransduction processes. These functional categories underscore that alterations in energy metabolism, neuroprotective mechanisms and signal transduction are involved in MPTPinduced neuronal degeneration in the retina, in similarity to mechanisms thought to underlie neuronal death in the Parkinson’s diseased brain and neurodegenerative diseases of the retina proper.

El proceso de aprendizaje que tiene lugar durante la formación práctico de los estudiantes genera algunos interrogantes: ¿de qué depende ese aprendizaje práctico? ¿Cómo se lleva a cabo?, ¿cuáles son las vivencias de los estudiantes durante esos meses de formación?, ¿cuáles son sus mayores dificultades de aprendizaje? etc. El objetivo de la investigación fue conocer y comprender cuáles fueron las vivencias y experiencias de los estudiantes de sus periodos de formación práctica ante la proximidad de convertirse en profesionales de la salud. Metodología: investigación cualitativa con método fenomenológico. Los datos se obtuvieron del análisis de las memorias reflexivas que los estudiantes de 4º de grado realizaron al finalizar las veinte semanas de prácticas correspondientes al Practicum IV. Conclusiones: a través de los testimonios de los estudiantes se identifican tres aspectos significativos: la necesidad de la práctica reflexiva, los condicionantes de las relaciones humanas en el aprendizaje y valor de los aspectos invisibles del cuidado.

The current tendency to undertake more trips, but of shorter duration, throughout the year, has meant that the tourist industry has started to show greater interest in attracting those market segments that opt for more prolonged stays, as they are especially profitable. One of these segments is that of seniors. Given the aging demographic of the population worldwide, which is particularly noticeable in Spain, the object of this study is to identify the variables that determine the length of stay of Spanish seniors at their destination. The Negative Binomial model was adapted to the context of length of stay by Spanish seniors and the determinant factors identified were: age, travel purpose, climate, type of accommodation, group size, trip type and the activities carried out at the destination. This study is a contribution to this field from an empirical point of view, given the scarcity of studies of this type and their eminently descriptive character; as well as from a practical level, with interesting implications for the sector.

El objetivo de esta investigación fue determinar la percepción que sobre el concepto de “prescripción enfermera” tienen los profesionales enfermeros de atención especializada. Para ello se realizó un abordaje cualitativo mediante técnica Delphi, con la participación de 20 profesionales enfermeros que constituyeron el grupo de expertos. El período de estudio estuvo comprendido entre diciembre de 2008 y febrero de 2009 y se desarrolló en un hospital de tercer nivel de la Comunidad de Madrid. Se identificaron 17 actividades que fueron entendidas por el grupo de expertos como prescripción enfermera, siendo éstas agrupadas con arreglo a tres categorías: prescripción independiente, colaborativa y basándose en un protocolo. El grupo de expertos consideró que dichas actividades pueden ser desarrolladas mediante dos perfiles competenciales enfermeros: el perfil administrativo y el facultativo.

La Dirección de Operaciones y Enfermería del Hospital Universitario Fundación Alcorcón crea una plaza de Enfermera de Investigación y Formación, siendo este desempeño novedoso, tanto para el propio hospital, como para la mayor parte de los hospitales de la Comunidad de Madrid y no estando definidos los perfiles competenciales de puestos similares. Nos planteamos que, no habiéndose definido hasta ese momento las intervenciones de apoyo que dichas figuras ofertan, resulta difícil determinar el efecto que tienen sobre el personal al que brindan apoyo y asesoramiento, o conocer cuál es su repercusión en los resultados de salud de los pacientes, y por tanto, en la asistencia sanitaria que ofertan los hospitales. Por este motivo, se plantea la tesis que presenta como el objetivo general de definir el proceso de desarrollo competencial de una plaza de Enfermera de Investigación y Formación en una organización sanitaria de atención especializada, valorando la eficacia de su existencia en la mejora de la práctica asistencial enfermera, y por tanto, en la mejora de los resultados de salud de los pacientes. La obtención de los resultados se ha realizado mediante el desarrollo de un modelo de Gestión del Conocimiento basado en la Seguridad del Paciente, que permitía la valoración de las prácticas profesionales enfermeras y de los resultados en salud de los pacientes, asociando dichas evaluaciones a un diseño formativo en Seguridad del Paciente. Los resultados nos permiten concluir que el modelo desarrollado permitió la definición de las competencias de la Enfermera de Investigación y Formación, como figura administradora del modelo, y de las principales figuras impulsoras del mismo, consiguiendo difundir los contenidos en Seguridad del Paciente entre los colectivos receptores y permitiendo la valoración de aspectos de la práctica clínica enfermera y de los resultados de salud en los pacientes, que obtuvieron mejores indicadores tras el periodo de implementación del modelo de Gestión del Conocimiento en Seguridad del Paciente.
