The Comprehensive Heart Disease Action Plan for Andalusia 2005-2009 has been prepared within the framework of the presentations made both in the 3rd Andalusian Health Action Plan and in the Quality Plan of the Public Health System. Consequently, as with these two referral instruments, the improvement in the health results for the citizens of Andalusia continues to be sought without wavering, specifically with regard to meeting the needs and expectations of the population affected by health problems in this area.

This study aims to analyze the adjustment of women, victims of intimate partner violence, by applying the Prochaska and Di Clemente Stages of Change Model. An interpretative qualitative study was made in 35 domestic violence victims women detected in primary care, women who recognized their relationship as abusive (perceived maltreatment). This is a multicentric study, with participation of six health centers of Malaga city. Biographical Narration technique by audio-recorded and transcribed interview was used; about this, thematic analysis adjustment to Transtheoretic Model phases was applied. ATLAS-TI 5.0 program was used for codification. Precontemplative, maintenance and ending stages were more represented while action phases were poorly mentioned. Main phases characteristics were: "blindness" and inexplicability in precontemplative stage; pros / cons analysis in contemplative phase; making decisions difficulty in action phases; suffering and going ahead purpose in maintenance stage, and determination and analysis capacity in the ending stage. Keys for intervention according to the phase of the process were offered.

BACKGROUND Socio-economic inequalities in mortality are observed at the country level in both North America and Europe. The purpose of this work is to investigate the contribution of specific risk factors to social inequalities in cause-specific mortality using a large multi-country cohort of Europeans. METHODS A total of 3,456,689 person/years follow-up of the European Prospective Investigation into Cancer and Nutrition (EPIC) was analysed. Educational level of subjects coming from 9 European countries was recorded as proxy for socio-economic status (SES). Cox proportional hazard model's with a step-wise inclusion of explanatory variables were used to explore the association between SES and mortality; a Relative Index of Inequality (RII) was calculated as measure of relative inequality. RESULTS Total mortality among men with the highest education level is reduced by 43% compared to men with the lowest (HR 0.57, 95% C.I. 0.52-0.61); among women by 29% (HR 0.71, 95% C.I. 0.64-0.78). The risk reduction was attenuated by 7% in men and 3% in women by the introduction of smoking and to a lesser extent (2% in men and 3% in women) by introducing body mass index and additional explanatory variables (alcohol consumption, leisure physical activity, fruit and vegetable intake) (3% in men and 5% in women). Social inequalities were highly statistically significant for all causes of death examined in men. In women, social inequalities were less strong, but statistically significant for all causes of death except for cancer-related mortality and injuries. DISCUSSION In this European study, substantial social inequalities in mortality among European men and women which cannot be fully explained away by accounting for known common risk factors for chronic diseases are reported.

Cette étude porte sur la recherche biomédicale en Suisse dans une perspective interprétative. Elle s'intéresse à l'usage que font les acteurs scientifiques et institutionnels de la catégorie «biomédical», à la signification qu'ils en donnent et aux processus de structuration de la recherche biomédicale autour de ces enjeux de catégorisation. Nous avons formulé l'hypothèse que le «biomédical» pouvait être considéré comme un label, à savoir une stratégie discursive de positionnement des acteurs, ou pouvait constituer un champ, à savoir un espace social de recherche fortement structuré. Pour pouvoir vérifier la validité de ces hypothèses, trois perspectives analytiques ont été retenues: topographie, discours et pratiques. Dans un premier temps, nous avons établi une topographie de la recherche biomédicale en repérant les acteurs (et leur appartenance disciplinaire) et les institutions qui s'associent au terme «biomédical», que ce soit pour décrire des institutions ou des projets de recherche. Les résultats de cette analyse offrent une première approximation d'un espace de la recherche en donnant une image d'un domaine peu unifié. Ainsi, l'usage de la catégorie «biomédical» dans les projets des chercheurs n'est pas le fait des seuls médecins et biologistes, mais également de représentants d'autres disciplines. La physique, la chimie et les sciences de l'ingénieur occupent ainsi également une place très importante dans cet espace de recherche. Puis, dans une perspective discursive, nous avons analysé le «biomédical» non seulement comme un label, mais également comme un objet-frontière permettant d'articuler différentes significations, de produire du sens là où des univers de recherche pourraient s'opposer, ou à coordonner des politiques qui ne l'étaient pas. L'analyse des différentes définitions du «biomédical» nous a confirmé l'existence d'un espace social marqué par une grande diversité disciplinaire, toutefois articulé autour d'un coeur médical et, plus particulièrement, d'une application médicale (potentielle ou actuelle). De plus, il ne semble pas y avoir de profondes luttes pour l'établissement de limites claires au «biomédical». Finalement, nous avons étudié les différentes activités de la production des savoirs (carrières, financement, collaboration, publication, etc.). Cette analyse a permis de comprendre que la diversité des définitions et des significations que les acteurs attribuent à la catégorie «biomédical» a aussi un ancrage dans la matérialité des réseaux sociotechniques dans lesquels les chercheurs s'inscrivent. Ces éléments confirment l'idée d'une fragmentation et d'une hétérogénéité de l'espace de la recherche biomédicale. En dépit de cette fragmentation, nous avons également montré que différentes mesures et instruments d'action publique visant à organiser et réguler les pratiques des chercheurs sont mis en oeuvre. Néanmoins et paradoxalement, la recherche biomédicale ne constitue pas pour autant un objet de politique scientifique abordé par les autorités politiques, en tous les cas pas sous l'angle de la catégorie «biomédical». Ces différents niveaux d'analyse ont permis d'arriver à la conclusion que la catégorie «biomédical» n'est pas suffisamment institutionnalisée et que le degré d'interaction entre l'ensemble des chercheurs qui en font usage est trop faible pour que l'on puisse considérer le «biomédical» comme un espace social fortement organisé et structuré, à savoir un champ de la recherche biomédicale. Cela est principalement lié au fait que les acteurs ne partagent pas les mêmes définitions de ce qu'est (ou devrait être) le «biomédical», que leurs pratiques de recherche s'inscrivent dans des univers relativement séparés, et que cette diversité ne donne pas lieu à de fortes luttes pour l'imposition d'une définition légitime ou de normes d'excellence scientifiques dominantes. Par contre, les analyses ont permis de confirmer la validité du «biomédical» comme label, puisque les acteurs se servent de cette catégorie pour valoriser leurs pratiques de recherche et se positionner, même si d'autres notions ont émergé ces dernières années («translationnel», «biotech», «medtech», médecine personnalisée, etc.). On peut, in fine, considérer le «biomédical» comme un probable langage commun («objet-frontière») reposant tant sur la scientificisation du médical que sur la médicalisation des sciences («de base» et «techniques »), visant à améliorer les conditions de possibilité d'un dialogue fructueux entre chercheurs fondamentaux et cliniciens.

Després de 30 anys de la restauració de la Generalitat es desconeix gairebé tot del perfil social de l¿elit política Governamental catalana, aquest Treball fa una aproximació inicial al tema a partir dels últims quatre Governs de la Generalitat de Catalunya. Els seus membres presenten biaix social respecte la població d¿acord amb el model d¿aglutinació de les elits polítiques, tot i això el model de Desproporció Social Creixent només es valida respecte el gènere. El seu perfil és el d¿un home, de més edat que la mitjana, nascut a Catalunya, amb la carrera prèvia desenvolupada a Barcelona ciutat, amb una formació molt per sobre de la mitjana i de professió economista. Aquest perfil no és exactament el mateix que el dels Parlamentaris regionals espanyols, les diferències es centren l¿edat i la professió mentre que el gènere, l¿origen i el nivell de formació coincideixen. S¿observa també que els Governs d¿esquerra-centre esquerra presenten un percentatge lleugerament superior de dones que la resta, els membres vinculats amb PANE són més joves, incorporen menys dones, menys immigrants interiors, menys educadors i no presenten credencials educatives sensiblement diferents del membres vinculats a partits d¿àmbit estatal.

Leptospirosis in humans usually involves hypokalaemia and hypomagnesaemia and the putative mechanism underlying such ionic imbalances may be related to nitric oxide (NO) production. We previously demonstrated the correlation between serum levels of NO and the severity of renal disease in patients with severe leptospirosis. Methylene blue inhibits soluble guanylyl cyclase (downstream of the action of any NO synthase isoforms) and was recently reported to have beneficial effects on clinical and experimental sepsis. We investigated the occurrence of serum ionic changes in experimental leptospirosis at various time points (4, 8, 16 and 28 days) in a hamster model. We also determined the effect of methylene blue treatment when administered as an adjuvant therapy, combined with late initiation of standard antibiotic (ampicillin) treatment. Hypokalaemia was not reproduced in this model: all of the groups developed increased levels of serum potassium (K). Furthermore, hypermagnesaemia, rather than magnesium (Mg) depletion, was observed in this hamster model of acute infection. These findings may be associated with an accelerated progression to acute renal failure. Adjuvant treatment with methylene blue had no effect on survival or serum Mg and K levels during acute-phase leptospirosis in hamsters.

Roughly fifteen years ago, the Church of Jesus Christ of Latter-day Saints published a new proposed standard file format. They call it GEDCOM. It was designed to allow different genealogy programs to exchange data.Five years later, in may 2000, appeared the GENTECH Data Modeling Project, with the support of the Federation of Genealogical Societies (FGS) and other American genealogical societies. They attempted to define a genealogical logic data model to facilitate data exchange between different genealogical programs. Although genealogists deal with an enormous variety of data sources, one of the central concepts of this data model was that all genealogical data could be broken down into a series of short, formal genealogical statements. It was something more versatile than only export/import data records on a predefined fields. This project was finally absorbed in 2004 by the National Genealogical Society (NGS).Despite being a genealogical reference in many applications, these models have serious drawbacks to adapt to different cultural and social environments. At the present time we have no formal proposal for a recognized standard to represent the family domain.Here we propose an alternative conceptual model, largely inherited from aforementioned models. The design is intended to overcome their limitations. However, its major innovation lies in applying the ontological paradigm when modeling statements and entities.

En una època de transformacions vertiginoses que han situat les generacions joves en una situació d'exclusió social, política, econòmica i simbòlica, les polítiques de joventut han d'esdevenir un revulsiu per a l'aprenentatge i la normalització de la ciutadania activa, dins un ampli programa de reforma social orientat a la recuperació de l'Estat del benestar i de la iniciativa política per part d'unes institucions refundades sobre nocions de democràcia participativa. En aquest context, el moviment associatiu juvenil és una expressió genuïna d'apoderament, participació autònoma i capacitat d'anàlisi que pot tenir un paper protagonista, amb l'extensió del model de consells de joves arreu del territori com a espais de deliberació, decisió i participació democràtica

En los últimos años la industria camaronera ha adquirido mucha importancia en Brasil y genera presiones sobre ecosistemas y comunidades tradicionales que viven en equilibrio con su entorno. En Cumbe, una comunidad tradicional del municipio de Aracati, Ceará, Brasil, el ecosistema que rodea a su población es el manglar. Debido a esta fuerte presión, tanto el manglar como los servicios ambientales que la población local se beneficia, están transformándose. En este trabajo, la población local de Cumbe identifica, valora y caracteriza los servicios ambientales que les proporciona el manglar dándole así una vital importancia, que puede servir de precedente para una posible protección. Por otra parte, se hace una aproximación en términos económicos de la pesca artesanal y las granjas de camarón para discutir a corto y a largo plazo cuál es el modelo más sostenible a seguir.

This paper aims to provide empirical support for the use of the principal-agent framework in the analysis of public sector and public policies. After reviewing the different conditions to be met for a relevant analysis of the relationship between population and government using the principal-agent theory, our paper focuses on the assumption of conflicting goals between the principal and the agent. A principal-agent analysis assumes in effect that inefficiencies may arise because principal and agent pursue different goals. Using data collected during an amalgamation project of two Swiss municipalities, we show the existence of a gap between the goals of the population and those of the government. Consequently, inefficiencies as predicted by the principal-agent model may arise during the implementation of a public policy, i.e. an amalgamation project. In a context of direct democracy where policies are regularly subjected to referendum, the conflict of objectives may even lead to a total failure of the policy at the polls.

The first decade of the twenty-first century may be remembered for the rebirth of consensus on labour market policy. After three decades of bitter political and ideological controversy between a neo-liberal and a traditional social democratic approach, a new model, often labelled flexicurity, has emerged. This model is promoted by numerous political organisations since it promises to put an end to the old trade-off between equality and efficiency. Several countries are embracing the flexicurity model as a blueprint for labour market reform, but others, mostly belonging to the 'Mediterranean Rim', are clearly lagging behind. Why is it so difficult for these countries to implement the flexicurity model? This paper argues that the application of a flexicurity strategy in these countries is complicated by the lack of social trust between social partners and the state as well as political economy traditions that highlight the role of labour market regulation as a source of social protection.

We use panel data from the U. S. Health and Retirement Study, 1992-2002, to estimate the effect of self-assessed health limitations on the active labor market participation of older men. Self-assessments of health are likely to be endogenous to labor supply due to justification bias and individual-specific heterogeneity in subjective evaluations. We address both concerns. We propose a semiparametric binary choice procedure that incorporates nonadditive correlated individual-specific effects. Our estimation strategy identifies and estimates the average partial effects of health and functioning on labor market participation. The results indicate that poor health plays a major role in labor market exit decisions.

Entrepreneurship has been the engine propelling much of the growth of the business sector as well as a driving force behind the rapid expansion of the social sector. This article offers a comparative analysis of commercial and social entrepreneurship using a prevailing analytical model from commercial entrepreneurship. The analysis highlights key similarities and differences between these two forms of entrepreneurship and presents a framework on how to approach the social entrepreneurial process more systematically and effectively. We explore the implications of this analysis of social entrepreneurship for both practitioners and researchers.

Theories on social capital and on social entrepreneurship have mainly highlighted the attitude of social capital to generate enterprises and to foster good relations between third sector organizations and the public sector. This paper considers the social capital in a specific third sector enterprise; here, multi-stakeholder social cooperatives are seen, at the same time, as social capital results, creators and incubators. In the particular enterprises that identify themselves as community social enterprises, social capital, both as organizational and relational capital, is fundamental: SCEs arise from but also produce and disseminate social capital. This paper aims to improve the building of relational social capital and the refining of helpful relations drawn from other arenas, where they were created and from where they are sometimes transferred to other realities, where their role is carried on further (often working in non-profit, horizontally and vertically arranged groups, where they share resources and relations). To represent this perspective, we use a qualitative system dynamic approach in which social capital is measured using proxies. Cooperation of volunteers, customers, community leaders and third sector local organizations is fundamental to establish trust relations between public local authorities and cooperatives. These relations help the latter to maintain long-term contracts with local authorities as providers of social services and enable them to add innovation to their services, by developing experiences and management models and maintaining an interchange with civil servants regarding these matters. The long-term relations and the organizational relations linking SCEs and public organizations help to create and to renovate social capital. Thus, multi-stakeholder cooperatives originated via social capital developed in third sector organizations produce new social capital within the cooperatives themselves and between different cooperatives (entrepreneurial components of the third sector) and the public sector. In their entrepreneurial life, cooperatives have to contrast the "working drift," as a result of which only workers remain as members of the cooperative, while other stakeholders leave the organization. Those who are not workers in the cooperative are (stake)holders with "weak ties," who are nevertheless fundamental in making a worker's cooperative an authentic social multi-stakeholders cooperative. To maintain multi-stakeholder governance and the relations with third sector and civil society, social cooperatives have to reinforce participation and dialogue with civil society through ongoing efforts to include people that provide social proposals. We try to represent these processes in a system dynamic model applied to local cooperatives, measuring the social capital created by the social cooperative through proxies, such as number of volunteers and strong cooperation with public institutions. Using a reverse-engineering approach, we can individuate the determinants of the creation of social capital and thereby give support to governance that creates social capital.

In order to identify new compounds to treat Chagas disease during the acute phase with higher activity and lower toxicity than the reference drug benznidazole (Bz), two hydroxyphthalazine derivative compounds were prepared and their trypanocidal effects against Trypanosoma cruzi were evaluated by light microscopy through the determination of IC50 values. Cytotoxicity was determined by flow cytometry assays against Vero cells. In vivo assays were performed in BALB/c mice, in which the parasitemia levels were quantified by fresh blood examination; the assignment of a cure was determined by reactivation of blood parasitemia levels after immunosuppression. The mechanism of action was elucidated at metabolic and ultra-structural levels, by (1)H NMR and TEM studies. Finally, as these compounds are potentially capable of causing oxidative damage in the parasites, the study was completed, by assessing their activity as potential iron superoxide dismutase (Fe-SOD) inhibitors. High-selectivity indices observed in vitro were the basis of promoting one of the tested compounds to in vivo assays. The tests on the murine model for the acute phase of Chagas disease showed better parasitemia inhibition values than those found for Bz. Compound 2 induced a remarkable decrease in the reactivation of parasitemia after immunosuppression. Compound 2 turned out to be a great inhibitor of Fe-SOD. The high antiparasitic activity and low toxicity together with the modest costs for the starting materials render this compound an appropriate molecule for the development of an affordable anti-Chagas agent.
