Some retrieved CoCrMo hip implants have shown that abrasive wear is one of the possible wear mechanisms invoked within such joints. To date, little work has focused on the third body abrasion of CoCrMo and therefore there is a general lack of understanding of the effect of abrasive size and volume concentration on the tribo-corrosion performance of the CoCrMo alloys. The present work assessed the tribo-corrosion behaviour of cast CoCrMo (F-75) under various abrasion-corrosion conditions by using a modified microabrasion tester incorporating a three-electrode electrochemical cell. The effects of reduced abrasive size/hardness and volume concentration, as well as the role of proteins on the tribo-corrosion performance of the cast CoCrMo alloy were addressed. The correlation between electrochemical and mechanical processes for different abrasion-corrosion test conditions has been discussed in detail. Results show that the reduction in abrasive size and volume concentration can significantly affect the abrasion-corrosion wear mechanisms and the wear-induced corrosion response of the material. The finding of this study implies that the smaller/softer third body particles generated in vivo could also result in significant wear-induced corrosion and therefore potential metal ion release, which could be potentially detrimental to both the patient health and the life span of the implants. © 2009 Elsevier Ltd. The abrasion damage on retrieved CoCrMo based hip joints is reported to be influenced by the entrainment of micron and sub-micron sized debris/hard particles. This paper represents the first attempt to look into the effects of relatively soft abrasives with micron and sub-micron dimensions on the abrasion mechanisms and the abrasion-corrosion performance of the cast CoCrMo in simulated hip joint environments. A modified micro-abrasion tester incorporating a liquid tank and a three-electrode electrochemical cell was used. Al O (300 nm and 1 μm) and sub-micron sized BaSO abrasives were chosen as being comparable in the size and hardness to the wear particles found in vivo. Results show that the specific wear rates of cast CoCrMo are dependent on the abrasive particle size, hardness and volume concentration. Larger particle size, higher hardness and greater abrasive volume fractions gave greater wear rates. The wear-induced corrosion current generally increases with increasing wear rates, and the presence of proteins seems to suppress the wear-induced corrosion current especially when abrasive volume fractions were high. This study shows that the nature of abrasives and the test solutions are both important in determining the wear mechanisms and the abrasion-corrosion response of cast CoCrMo. These findings provide new and important insights into the in vivo wear mechanisms of CoCrMo. © 2009 Elsevier B.V. All rights reserved.

Background: Differentiation between septic and aseptic loosening of joint replacements is essential for successful revision surgery, but reliable markers for the diagnosis of low-grade infection are lacking. The present study was performed to assess intra-articular and systemic levels of antimicrobial peptides and proinflammatory cytokines as diagnostic markers for periprosthetic joint infection. Methods: Fifteen consecutive patients with staphylococcal periprosthetic joint infections and twenty control patients with aseptic loosening of total hip and knee replacements were included in this prospective, single-center, controlled clinical trial. Expression of the antimicrobial peptides human β-defensin-2 (HBD-2), human β-defensin-3 (HBD-3), and cathelicidin LL-37 (LL-37) was determined by ELISA (enzyme-linked immunosorbent assay) in serum and joint aspirates. Proinflammatory cytokines were assessed in serum and joint aspirates with use of cytometric bead arrays. C-reactive protein in serum, microbiology, and histopathology of periprosthetic tissue served as the “gold standard” for the diagnosis of infection. Results: The antimicrobial peptides HBD-3 and LL-37 were significantly elevated in joint aspirates from patients with periprosthetic joint infection compared with patients with aseptic loosening, and the area under the curve (AUC) in a receiver operating characteristic curve analysis was equal to 0.745 and 0.875, respectively. Additionally, significant local increases in the proinflammatory cytokines interleukin (IL)-1β, IL-4, IL-6, IL-17A, interferon (IFN)-γ, and tumor necrosis factor (TNF)-α were observed to be associated with infection. Logistic regression analysis indicated that the combination of an antimicrobial peptide with another synovial fluid biomarker improved diagnostic accuracy; the AUC value was 0.916 for LL-37 and IL-4, 0.895 for LL-37 and IL-6, 0.972 for HBD-3 and IL-4, and 0.849 for HBD-3 and IL-6. In contrast, the only antimicrobial peptides and cytokines in serum that showed a significant systemic increase in association with infection were HBD-2, IL-4, and IL-6 (all of which had an AUC value of <0.75). Conclusions: The present study showed promising results for the use of antimicrobial peptides and other biomarkers in synovial fluid for the diagnosis of periprosthetic joint infection, and analysis of the levels in synovial fluid was more accurate than analysis of serum.

Background: The association between body size and head and neck cancers (HNCA) is unclear, partly because of the biases in case–control studies. Methods: In the prospective NIH–AARP cohort study, 218,854 participants (132,288 men and 86,566 women), aged 50 to 71 years, were cancer free at baseline (1995 and 1996), and had valid anthropometric data. Cox proportional hazards regression was used to examine the associations between body size and HNCA, adjusted for current and past smoking habits, alcohol intake, education, race, and fruit and vegetable consumption, and reported as HR and 95% confidence intervals (CI). Results: Until December 31, 2006, 779 incident HNCAs occurred: 342 in the oral cavity, 120 in the oro- and hypopharynx, 265 in the larynx, 12 in the nasopharynx, and 40 at overlapping sites. There was an inverse association between HNCA and body mass index, which was almost exclusively among current smokers (HR = 0.76 per each 5 U increase; 95% CI, 0.63–0.93), and diminished as initial years of follow-up were excluded. We observed a direct association with waist-to-hip ratio (HR = 1.16 per 0.1 U increase; 95% CI, 1.03–1.31), particularly for cancers of the oral cavity (HR, 1.40; 95% CI, 1.17–1.67). Height was also directly associated with total HNCAs (P = 0.02), and oro- and hypopharyngeal cancers (P < 0.01). Conclusions: The risk of HNCAs was associated inversely with leanness among current smokers, and directly with abdominal obesity and height. Impact: Our study provides evidence that the association between leanness and risk of HNCAs may be due to effect modification by smoking. The operational lifetime of hip replacement prostheses can be severely limited due to the occurrence of excessive wear at the load-bearing interfaces. The aim of this study was to investigate how the surface topography of articulating counterfaces evolves over the duration of a laboratory wear run. It was observed that modular stainless steel femoral heads wearing against ultrahigh molecular weight polyethylene (UHMWPE) can themselves be subject to wearing. A comparison with retrieved in vivo-aged femoral heads shows many topographical similarities: in a qualitative sense, scratching and pitting are evident on laboratory and in vivo-worn femoral heads; quantitatively, roughness comparisons between the new and worn devices are seen to increase typically by a factor of 4 after laboratory wearing. The observations suggest that a particular wear mode, namely third-body wear, is responsible for the increased roughness. It is conjectured that third bodies might arise through surface fatigue wear on the metal counterface, Wear debris is also observed to have been generated from the polymer surface, creating rounded debris with sizes predominantly in the range 0.4-0.8 microns: dimensions that are comparable to values previously reported for in vivo generated debris.

Falls are a significant threat to the safety, health and independence of older citizens. Despite the substantial evidence that is available around effective falls prevention programmes and interventions, their translation into falls reduction programmes and policies has yet to be fully realised. While hip fracture rates are decreasing, the number and incidence of fall-related hospital admissions among older people continue to rise. Given the demographic trends that highlight increasing numbers of older people in the UK, which is broadly reflected internationally, there is a financial and social imperative to minimise the rate of falls and associated injuries. Falling is closely aligned to growing older (Slips, Trips and Falls Update: From Acute and Community Hospitals and Mental Health Units in England and Wales, Department of Health, HMSO, London, 2010). According to the World Health Organization, around 30% of older people aged over 65 and 50% of those over 80 will fall each year (Falls Fact Sheet Number 344, WHO, Geneva, 2010). Falls happen as a result of many reasons and can have harmful consequences, including loss of mobility and independence, confidence and in many cases even death (Cochrane Database Syst Rev 15, 2009, 146; Slips, Trips and Falls Update: From Acute and Community Hospitals and Mental Health Units in England and Wales, Department of Health, HMSO, London, 2010; Falling Standards, Broken Promises: Report of the National
Audit of Falls and Bone Health in Older People 2010, Health Care Quality
Improvement Partnership, London, 2011). What is neither fair nor correct is the
common belief by old and young alike that falls are just another inconvenience to put up with. The available evidence justifiably supports the view that well-organised services, based upon national standards and expert guidance, can prevent future falls among older people and reduce death and disability from fractures. This paper will draw from the UK, as an exemplar for policy and practice, to discuss the strategic direction of falls prevention programmes for older people and the partnerships that need to exist between researchers, service providers and users of services to translate evidence to the clinical setting. Second, it will propose some mechanisms for disseminating evidence to healthcare professionals and other stakeholders, to improve the quality and capacity of the clinical workforce.

Body fat distribution is a heritable trait and a well-established predictor of adverse metabolic outcomes, independent of overall adiposity. To increase our understanding of the genetic basis of body fat distribution and its molecular links to cardiometabolic traits, here we conduct genome-wide association meta-analyses of traits related to waist and hip circumferences in up to 224,459 individuals. We identify 49 loci (33 new) associated with waist-to-hip ratio adjusted for body mass index (BMI), and an additional 19 loci newly associated with related waist and hip circumference measures (P < 5 × 10(-8)). In total, 20 of the 49 waist-to-hip ratio adjusted for BMI loci show significant sexual dimorphism, 19 of which display a stronger effect in women. The identified loci were enriched for genes expressed in adipose tissue and for putative regulatory elements in adipocytes. Pathway analyses implicated adipogenesis, angiogenesis, transcriptional regulation and insulin resistance as processes affecting fat distribution, providing insight into potential pathophysiological mechanisms.

CONTEXT: Minority communities are disproportionately affected by diabetes, and minority women are at an increased risk for glucose intolerance (dysglycemia) during pregnancy.

OBJECTIVES: In pregnant American Indian women, the objectives of the study were to use current criteria to estimate the prevalence of first-trimester (Tr1) dysglycemia and second-trimester (Tr2) incidence of gestational diabetes mellitus (GDM) and to explore new candidate measures and identify associated clinical factors.

DESIGN: This was a prospective cohort study. In Tr1 we performed a 75-g, 2-hour oral glucose tolerance test (OGTT) and glycated hemoglobin (HbA1c) to determine the following: fasting insulin; homeostasis model assessment of insulin resistance; serum 1,5-anhydroglucitol; noninvasive skin autofluorescence (SCOUT). We defined dysglycemia by American Diabetes Association and Endocrine Society criteria and as HbA1c of 5.7% or greater. In Tr2 in an available subset, we performed a repeat OGTT and SCOUT.

PARTICIPANTS: Pregnant American Indian women (n = 244 at Tr1; n = 114 at Tr2) participated in the study.

OUTCOMES: The prevalence of dysglycemia at Tr1 and incidence of GDM at Tr2 were measured.

RESULTS: At Tr1, one woman had overt diabetes; 36 (15%) had impaired glucose tolerance (American Diabetes Association criteria and/or abnormal HbA1c) and 59 (24%) had GDM-Tr1 (Endocrine Society criteria). Overall, 74 (30%) had some form of dysglycemia. Associated factors were body mass index, hypertension, waist/hip circumferences, SCOUT score, fasting insulin, and homeostasis model assessment of insulin resistance. At Tr2, 114 of the Tr1 cohort underwent a repeat OGTT and SCOUT, and 26 (23%) had GDM. GDM-Tr2 was associated with increased SCOUT scores (P = .029) and Tr1 body mass index, waist/hip circumferences, diastolic blood pressure, fasting insulin, and triglyceride levels. Overall, dysglycemia at Tr1 and/or Tr2 affected 38% of the women.

CONCLUSIONS: Dysglycemia at some point during pregnancy was common among American Indian women. It was associated with features of insulin resistance and may confer long-term health risks for mother and child.

BACKGROUND: Epidemiological and clinical studies suggest comorbidity between prostate cancer (PCA) and cardiovascular disease (CVD) risk factors. However, the relationship between these two phenotypes is still not well understood. Here we sought to identify shared genetic loci between PCA and CVD risk factors.

METHODS: We applied a genetic epidemiology method based on conjunction false discovery rate (FDR) that combines summary statistics from different genome-wide association studies (GWAS), and allows identification of genetic overlap between two phenotypes. We evaluated summary statistics from large, multi-centre GWA studies of PCA (n=50 000) and CVD risk factors (n=200 000) [triglycerides (TG), low-density lipoprotein (LDL) cholesterol and high-density lipoprotein (HDL) cholesterol, systolic blood pressure, body mass index, waist-hip ratio and type 2 diabetes (T2D)]. Enrichment of single nucleotide polymorphisms (SNPs) associated with PCA and CVD risk factors was assessed with conditional quantile-quantile plots and the Anderson-Darling test. Moreover, we pinpointed shared loci using conjunction FDR.

RESULTS: We found the strongest enrichment of P-values in PCA was conditional on LDL and conditional on TG. In contrast, we found only weak enrichment conditional on HDL or conditional on the other traits investigated. Conjunction FDR identified altogether 17 loci; 10 loci were associated with PCA and LDL, 3 loci were associated with PCA and TG and additionally 4 loci were associated with PCA, LDL and TG jointly (conjunction FDR <0.01). For T2D, we detected one locus adjacent to HNF1B.

CONCLUSIONS: We found polygenic overlap between PCA predisposition and blood lipids, in particular LDL and TG, and identified 17 pleiotropic gene loci between PCA and LDL, and PCA and TG, respectively. These findings provide novel pathobiological insights and may have implications for trials using targeting lipid-lowering agents in a prevention or cancer setting.

Bone Mineral Density (BMD) is a highly heritable trait, but genome-wide association studies have identified few genetic risk factors. Epidemiological studies suggest associations between BMD and several traits and diseases, but the nature of the suggestive comorbidity is still unknown. We used a novel genetic pleiotropy-informed conditional False Discovery Rate (FDR) method to identify single nucleotide polymorphisms (SNPs) associated with BMD by leveraging cardiovascular disease (CVD) associated disorders and metabolic traits. By conditioning on SNPs associated with the CVD-related phenotypes, type 1 diabetes, type 2 diabetes, systolic blood pressure, diastolic blood pressure, high density lipoprotein, low density lipoprotein, triglycerides and waist hip ratio, we identified 65 novel independent BMD loci (26 with femoral neck BMD and 47 with lumbar spine BMD) at conditional FDR < 0.01. Many of the loci were confirmed in genetic expression studies. Genes validated at the mRNA levels were characteristic for the osteoblast/osteocyte lineage, Wnt signaling pathway and bone metabolism. The results provide new insight into genetic mechanisms of variability in BMD, and a better understanding of the genetic underpinnings of clinical comorbidity.

Desenvolveu-se um novo método de processamento coloidal em meio aquoso para consolidar cerâmicos de α-Y-SiAlON, os quais foram sinterizados por prensagem isostática a quente (HIP) e por spark plasma sintering (SPS). Preparou-se com sucesso uma suspensão aquosa de pós precursores de YSiAlON (Si3N4, Al2O3, Y2O3 e AlN), temporariamente estável, usando Dolapix PC21 como dispersante, permitindo a fabricação de pós granulados pela técnica de aspersão-congelamento-liofilização e a consolidação de corpos em verde por enchimento por barbotina. Avaliou-se o efeito do excesso de oxigénio introduzido pelo processamento aquoso no desenvolvimento microestrutural e nas propriedades mecânicas. Os corpos de Y-SiAlON consolidados por enchimento por barbotina e sinterizados por HIP a 1800ºC apresentaram microestruturas e propriedades mecânicas similares a corpos consolidados por prensagem a seco. Estes materiais exibiram densificação completa, dureza Vickers máxima de 2003 e tenacidade à fractura (método SEVNB) máxima de 5.20 MPam1/2. Foi ainda possível estabelecer uma relação estreita entre o aumento do conteúdo em oxigénio das amostras sinterizadas e a diminuição da tenacidade à fractura. Na tentativa de melhorar a tenacidade à fractura dos materiais, procedeu-se à incorporação nas suspensões de sementes de Ca-α-SiAlON de geometria alongada produzidas por síntese por combustão, adicionadas como agentes de reforço. A síntese por combustão realizada em larga escala (cargas até 1 kg) não produziu efeitos negativos óbvios nos produtos da reacção. Investigara-se os efeitos da adição de 5 % em peso de sementes nas propriedades mecânicas e no desenvolvimento microestrutural de amostras densificadas por HIP. Em comparação com os materiais sem sementes, a tenacidade à fractura (método SEVNB) aumentou 13%, mas a dureza Vickers resultou 14,5% inferior. A sinterização por SPS permitiu obter densificação completa a temperaturas tão baixas como 1600ºC a partir dos pós granulados de Y-SiAlON sem a adição extra de Y2O3 para aumentar o teor de fase líquida. Os materiais sinterizados a 1700ºC e 1600ºC mostraram valores máximos de tenacidade à fractura de 4.36 e 4.20 MPam1/2 (método SEVNB), e de dureza Vickers de 2089 e 2084, respectivamente. Esta técnica de sinterização permitiu ainda obter corpos sinterizados translúcidos com uma transmitância máxima de 61% numa amostra de 2mm de espessura. Os corpos translúcidos apresentaram dureza Vickers de 2154 e tenacidade à fractura 3.74 MPam1/2 (método SEVNB).

O presente trabalho tem como objectivo o estudo, desenvolvimento e aplicações na área da biomecânica de sensores intrínsecos baseados em redes de Bragg em fibras ópticas (FBG). As aplicações são feitas em modelos biomecânicos in vitro tais como: implantes de anca, prótese de joelho, placas de osteossíntese e implantes dentários. A optimização do desenvolvimento de próteses e respectivos elementos de fixação é actualmente dependente da geração e validação experimental de seus modelos computacionais. A validação destes modelos é normalmente feita utilizando-se dados de ensaios não invasivos e invasivos em modelos sintéticos. Em ensaios in vitro os sensores convencionais têm um princípio de funcionamento eléctrico e apresentam por vezes dimensões inadequadas. Existem situações exploradas no presente trabalho, tais como sensoriamento de superfícies irregulares e junções ou ainda análises de deformações internas, onde é recomendável a utilização de sensores FBG, pois apresentam dimensões reduzidas e flexibilidade o que permite efectuar medidas localizadas. O desenvolvimento de um protocolo de utilização de FBG e a sua aplicação no contexto apresentado demonstrou-se mais adequado, pela precisão e segurança futura oferecidas. Foi desenvolvida uma metodologia experimental para medidas de deformações utilizando FBG ao longo de uma placa de osteossíntese metálica aparafusada a um fémur sintético fracturado. Foi efectuada a monitorização da cura do cimento ósseo utilizado como fixador do prato tibial na artroplastia total do joelho através da medida da sua contracção e temperatura. Foi também desenvolvido um sistema refrigerador com resposta às leituras de temperatura com vista a evitar a necrose do osso. Foram efectuados estudos de deformação nesse cimento após a sua cura, como resultado da aplicação de cargas mecânicas estáticas. Foram efectuados estudos da cura de cimento ósseo aplicado a próteses de anca e também de deformações nestas próteses. Foi ainda efectuado o estudo comparativo de vários implantes dentários através da medida da distribuição de deformações como resposta a excitações mecânicas impulsivas. Para a desmodulação das FBG foram inicialmente utilizados sistemas comerciais. Entretanto algumas aplicações não puderam ser implementadas com estes sistemas comerciais devido à baixa reflectividade das FBG utilizadas, mas fundamentalmente devido à necessidade de executar testes com uma taxa de aquisição maior do que os 5 Hz disponíveis (cerca de 15 kHz). Por estes motivos foi desenvolvido um sistema optoelectrónico completo de desmodulação de FBG baseado num filtro sintonizável e que tem como característica principal a alta taxa de aquisição (até 1,2 MHz) mas também se destaca pela facilidade na reconfiguração dos parâmetros de leitura, pela apresentação duma interface de utilizador amigável e pela capacidade de operar com até 5 FBG na mesma fibra óptica.

Efectuou-se um estudo prospectivo e observacional em 1200 doentes, maiores de 18 anos, com hipertensão essencial e sem eventos cardiovasculares (CV) prévbs que foram referenciados entre 1991 e 1998 para realização de MAPA no Serviço de Cardiologia do Hospital Infante D. Pedro, Aveiro (HIP), na Unidade de Hipertensão e Risco CV do Hospital Pedro Hispano (HPH), Matosinhos e na Clínica Cross/Moss do Porto. O objectivo principal deste estudo foi avaliar, nesta população, seguida durante 8- 12 anos, o valor preditivo de morbi-mortalidade CV da pressurometria ambulatória de 24 horas (MAPA), nomeadamente de diversos índices e valores dela extraída, tais como: (i) valores médios da pressão arterial (PA) de 24-h, diurna e nocturna; (ii) queda tensional noctuma, (iii) diferentes padrões de queda tensional nocturna em particular do padrão Non Dipper; (iv) PA diferencial de 24h, diurna e nodurna; (v) frequência cardíaca de 24 horas (vi) Ambulatory Arterial Stifness. Foram incluídos registos de MAPA de 1200 doentes, de um total de 2644, dos quais 53,8% eram mulheres e 10,2% eram diabéticos. Ao longo de um follow up médio de 8,2+3,0 anos, verificaram-se 62 óbitos e 152 eventos CV fatais e não fatais compreendendo 79 AVC, 51 eventos coronários e 22 classificados de outros eventos cardiovasculares. O estudo permitiu concluir que: 1- A MAPA foi superior a PA casual como preditor de eventos CV globais e de acidentes cerebrovasculares (AVC). 2 - De entre os valores da MAPA, o valor preditivo dos valores sistólicos (de 24 h, diurnos e nocturnos) foi superior ao dos respectivos valores diastólicos e o dos valores nocturnos foi superior ao dos valores diurnos. 3 - 0s eventos CV totais e os eventos coronários foram mais frequentes nos homens do que nas mulheres, sem diferença relativamente ao AVC. O valor preditivo independente da MAPA (sobretudo da PAS e da PP24h) foi superior nas mulheres vs homens relativamente aos eventos globais e AVC. 4- 0s padrões non-dipper e reverted-dipper de queda tensional nocturna associaram-se a pior prognóstico cardiovascular, relativamente ao padrão dipper. Contudo, dentro do padrão non-dipper, somente os doentes com queda tensional nocturna entre 0-4,9% -non-dippers 1 (mas não os doentes com queda tensional nocturna entre 5,O-9,9% -non-dippers 2) apresentaram risco CV superior ao do padrão dipper e semelhante ao do padrão reverted-dipper. Este achado poderá implicar a reclassificação do padrão non-dipper e do risco CV a ele associado. 5- O índice de rigidez arterial derivado da MAPA (AASI) correlaciona-se com outros índices (ex. a velocidade da onda de pulso) e contribui para estratificar o risco CV (eventos CV e AVC). O valor preditivo CV do AASI não é globalmente superior ao da PP 24h, embora em alguns casos o AASI possa acrescentar informação prognostica adicional a PP.

Os cimentos ósseos à base de PMMA para aplicações em artroplastia da anca apresentam como grande limitação o facto do seu constituinte principal ser um elemento bioinerte o que leva à falta de integração entre as interfaces cimento ósseo/tecido ósseo, comprometendo assim o desempenho mecânico da prótese ortopédica ao longo do tempo. Esta dissertação tem como objetivo principal a preparação de novas formulações de cimentos ósseos com a capacidade de estabelecer interações com os tecidos vivos circundantes. De modo a melhorar a bioatividade do sistema e facilitar a sua osseointegração, os cimentos ósseos comerciais foram reforçados com cargas significativas de HA. No entanto o recurso a elevadas cargas de HA (~60% m/m) no cimento ósseo promove debilidades do ponto de vista estrutural, levando a uma baixa resistência mecânica do material final. No sentido de ultrapassar esta limitação, foram inseridas nanoestruturas de carbono (GO ou CNTs) em baixas percentagens na matriz polimérica por forma a maximizar a sua performance mecânica através da perfeita integração de todos os componentes. A primeira fase deste trabalho consistiu no desenvolvimento de metodologias que permitissem a síntese de GO através da exfoliação química da grafite em solução aquosa. Os resultados obtidos demonstraram a obtenção de folhas de GO em larga escala e com número de camadas uniforme. A funcionalização orgânica superficial via ATRP do GO obtido, com cadeias de PMMA possibilitou o desenvolvimento de novos materiais nanocompósitos, no entanto alguns fatores de natureza tecnológica inviabilizaram o seu uso como agente de reforço na matriz idealizada. O desenvolvimento de novas formulações de cimentos ósseos consistiu numa matriz de PMMA/HA (1:2 (m/m)) reforçada com pequenas percentagens de GO ou CNTs (0,01, 0,1, 0,5 e 1,0% m/m). A síntese destes materiais nanocompósitos resultou da combinação de diversas técnicas: ultrassons, granulação por congelamento e liofilização. A análise estrutural dos nanocompósitos obtidos demonstrou a eficácia da metodologia desenvolvida na homogeneização de todos os elementos do sistema. Os estudos desenvolvidos após a conformação e caracterização estrutural dos novos materiais nanocompósitos permitiram verificar que as nanoestruturas de carbono apresentavam efeitos adversos na polimerização via radicalar do PMMA. A análise da fração orgânica permitiu verificar a presença de espécies oligoméricas o que reduziu significativamente o comportamento mecânico dos nanocompósitos. Através do estudo do aumento da concentração das espécies radicalares iniciais foi possível suplantar este problema e tirar o máximo rendimento dos agentes de reforço, tendo-se destacado os nanocompósitos reforçados com GO. A validação do ponto de vista mecânico das novas formulações de cimentos ósseos recaiu sobre o procedimento descrito na norma europeia ISO 5833 de 2002 – Implantes para cirurgia – cimentos acrílicos, tendo sido realizados os testes de compressão e de flexão. A avaliação biológica do comportamento dos cimentos ósseos assentou em duas abordagens complementares: estudos de mineralização em SBF e estudos de biocompatibilidade em meios celulares. Após a incubação das amostras em SBF ficou demonstrada a excelente capacidade para promoverem a integração de uma camada apatítica. Através de estudos celulares com Fibroblastos L929 e Osteoblastos Saos-2, nos quais foram avaliados a proliferação celular, viabilidade celular, espécies reativas de oxigénio, apoptose e morfologia celular, foi possível verificar bons níveis de biocompatibilidade para os materiais devolvidos.

Cette étude qualitative porte sur l’expérience des intervenants dans l’accompagnement au rétablissement dans les unités d’hébergement et de soins de longue durée en psychiatrie de l’Institut universitaire en santé mentale de Québec, avec un intérêt particulier pour les difficultés et les éléments facilitant cette expérience. Neufs intervenants de différentes professions (infirmiers, professionnels, éducateurs spécialisés) ont été rencontrés entre février et juin 2014. Chacun a participé à une entrevue semi-dirigée et complété le Recovery Attitude Questionnaire, version à 7 items (RAQ-7). Les entrevues ont fait l’objet d’une analyse de contenu thématique, complétée par une analyse des pointages provenant du RAQ-7. Résultats : Trois composantes de l’expérience d’accompagnement sont inter-reliées et s’inter-influencent : l’espoir (croire), l’implication et l’attachement envers la personne aidée. Une attitude positive envers le rétablissement, la compréhension de ce concept comme processus personnel et le fait de côtoyer des collègues qui croient en la possibilité d’un rétablissement influencent positivement l’expérience. En parallèle, le fait d’être confronté à des usagers-résidents présentant des symptômes importants, une culture d’intervention orientée vers la stabilisation des symptômes et un vécu d’intervention marqué par l’échec influencent négativement l’expérience. Conclusions et implications : L’expérience d’accompagnement au rétablissement est donc influencée par différents facteurs individuels et environnementaux (voir : environnement organisationnel). La connaissance de ces facteurs peut favoriser une implantation réussie de modes d’intervention favorables au rétablissement des usagers dans les milieux d’hébergement et de soins de longue durée en psychiatrie.
