BACKGROUND: The purpose of the optic nerve sheath diameter (ONSD) research group project is to establish an individual patient-level database from high quality studies of ONSD ultrasonography for the detection of raised intracranial pressure (ICP), and to perform a systematic review and an individual patient data meta-analysis (IPDMA), which will provide a cutoff value to help physicians making decisions and encourage further research. Previous meta-analyses were able to assess the diagnostic accuracy of ONSD ultrasonography in detecting raised ICP but failed to determine a precise cutoff value. Thus, the ONSD research group was founded to synthesize data from several recent studies on the subject and to provide evidence on the diagnostic accuracy of ONSD ultrasonography in detecting raised ICP. METHODS: This IPDMA will be conducted in different phases. First, we will systematically search for eligible studies. To be eligible, studies must have compared ONSD ultrasonography to invasive intracranial devices, the current reference standard for diagnosing raised ICP. Subsequently, we will assess the quality of studies included based on the QUADAS-2 tool, and then collect and validate individual patient data. The objectives of the primary analyses will be to assess the diagnostic accuracy of ONSD ultrasonography and to determine a precise cutoff value for detecting raised ICP. Secondly, we will construct a logistic regression model to assess whether patient and study characteristics influence diagnostic accuracy. DISCUSSION: We believe that this IPD MA will provide the most reliable basis for the assessment of diagnostic accuracy of ONSD ultrasonography for detecting raised ICP and to provide a cutoff value. We also hope that the creation of the ONSD research group will encourage further study. TRIAL REGISTRATION: PROSPERO registration number: CRD42012003072.

Per legislative requirement, attached is the Iowa Department of Transportation’s summary of project status for infrastructure projects that have been appropriated revenue from various funds including Rebuild Iowa Infrastructure, Health Restricted Capitals, Bridge Safety, Revenue Bonds Capitals, and Revenue Bonds Capitals II. Although a status report for the Bridge Safety Fund was already submitted to the directors of LSA and DOM, a status report on those projects is also included within this attachment for consistency with last year’s reporting. In addition, per request from LSA, status reports for the FY 2011 passenger rail appropriation from the Underground Storage Tank Fund and the FY 2010 Commercial Service Vertical Infrastructure appropriation from the General Fund are also listed in this report.

Action research is a useful instrument for the organization health care and the clinical governance of psychiatric institutions. What this type of research offers can be illustrated by the cohort study of migrant patients without health insurance who consulted the Department of Psychiatry of the Vaudois university medical center (CHUV) in 2008. While giving greater visibilty to the psychological suffering and social distress of these patients, the study also enabled the authors to determine which clinical procedures were actually offered to these patients and the amount these procedures cost the department. The small number of cases that were identified as well as their uneven distribution amongst the different services of the department suggest that considerable efforts must still be made to improve access for this population to public psychiatric services.

The Federal Highway Administration (FHWA) approves the selection of the Reconstruction of All or Part of the Interstate (Construction Alternative) as the Preferred Alternative to provide improvements to the interstate system in the Omaha/Council Bluffs metropolitan area, extending across the Missouri River on Interstate 80 to east of the Interstate 480 interchange in Omaha, Nebraska. The study considered long-term, broad-based transportation improvements along Interstate I-29 (I-29), I-80, and I-480, including approximately 18 mainline miles of interstate and 14 interchanges (3 system, 11 service), that would add capacity and correct functional issues along the mainline and interchanges and upgrade the I-80 Missouri River Crossing.

The Iowa Department of Transportation (Iowa DOT), Nebraska Department of Roads (NDOR), and the Federal Highway Administration (FHWA) are proposing to improve the interstate system around Council Bluffs with improvements extending across the Missouri River on I-80 to east of the I-480 interchange in Omaha, Nebraska, see Figure 1-1. The study considers long-term, broad-based transportation improvements along I-80, I-29, and I-480, including approximately 18 mainline miles of interstate and 14 interchanges (3 system1, 11 service), that would add capacity and correct functional issues along the mainline and interchanges and upgrade the I-80 Missouri River Crossing. These improvements, once implemented, would bring the segments of I-80 and I-29 up to current engineering standards and modernize the roadway to accommodate future traffic needs

Hypertension is one of the most common complex genetic disorders. We have described previously 38 single nucleotide polymorphisms (SNPs) with suggestive association with hypertension in Japanese individuals. In this study we extend our previous findings by analyzing a large sample of Japanese individuals (n=14 105) for the most associated SNPs. We also conducted replication analyses in Japanese of susceptibility loci for hypertension identified recently from genome-wide association studies of European ancestries. Association analysis revealed significant association of the ATP2B1 rs2070759 polymorphism with hypertension (P=5.3×10(-5); allelic odds ratio: 1.17 [95% CI: 1.09 to 1.26]). Additional SNPs in ATP2B1 were subsequently genotyped, and the most significant association was with rs11105378 (odds ratio: 1.31 [95% CI: 1.21 to 1.42]; P=4.1×10(-11)). Association of rs11105378 with hypertension was cross-validated by replication analysis with the Global Blood Pressure Genetics consortium data set (odds ratio: 1.13 [95% CI: 1.05 to 1.21]; P=5.9×10(-4)). Mean adjusted systolic blood pressure was highly significantly associated with the same SNP in a meta-analysis with individuals of European descent (P=1.4×10(-18)). ATP2B1 mRNA expression levels in umbilical artery smooth muscle cells were found to be significantly different among rs11105378 genotypes. Seven SNPs discovered in published genome-wide association studies were also genotyped in the Japanese population. In the combined analysis with replicated 3 genes, FGF5 rs1458038, CYP17A1, rs1004467, and CSK rs1378942, odds ratio of the highest risk group was 2.27 (95% CI: 1.65 to 3.12; P=4.6×10(-7)) compared with the lower risk group. In summary, this study confirmed common genetic variation in ATP2B1, as well as FGF5, CYP17A1, and CSK, to be associated with blood pressure levels and risk of hypertension.

Partant de deux études de cas, le projet de Frente Ribeirinha à Porto (Portugal) et celui d'Euroméditerranée à Marseille, ce projet de thèse a pour objectif l'étude des modalités de valorisation du tissu bâti (patrimonialisation) par les acteurs institutionnels dans les projets de régénération urbaine. Suivant l'hypothèse de l'entrée dans un nouveau régime de patrimonialité, l'analyse doit expliciter les stratégies à l'oeuvre ainsi que la manière dont les autorités publiques influent sur les représentations de l'espace construit. Abordant la question des échelles d'intervention des projets de régénération urbaine à partir des deux études de cas, mais également de littératures sur des projets anglo-saxons, la thèse cherche à voir, au prisme de la construction patrimoniale, comment ces projets sont directement connectés à l'économie internationale et au phénomène de globalisation et s'ils prennent en compte l'échelle du quotidien et de la domesticité. Il s'agit de montrer les limites d'une valorisation du tissu bâti dans un objectif de retombées économiques (ville consommable) au détriment des valeurs d'usage et de signifiants du quotidien. D'où l'hypothèse de l'importance d'une prise en compte des représentations de valeurs aux différentes échelles (dimensions socio-culturelles du projet de régénération) pour un équilibre qualitatif dans la fabrication du territoire et des projets sur le long terme. La volonté est sensiblement la compréhension des nouvelles constructions patrimoniales ainsi que des pratiques de conception et de mise en oeuvre du projet urbain. - Based on two case studies, the Frente Ribeirinha project in Porto (Portugal) and Euroméditerranée in Marseille, this research project aims at studying the modes of enhancement of the built fabric by institutional actors in urban regeneration projects. Posing the idea of a new heritage regime, this analysis attempts to explain the different strategies at work and how public authorities influence the built space's representations. Looking at the different scales of intervention of regeneration projects in our two case studies, as well as Anglo-Saxon literature on projects, it seeks at seeing, through heritage processes, how these projects are directly connected to the international economy and the phenomenon of globalization. Also, it aims at investigating whether policies take into account the scale of everyday life and domesticity. It attempts to show the limit of a built fabric's valuation with economic benefits objectives (consumable city) rather than taking into account values and meanings of everyday life. Hence, the thesis suggests taking into account representations and values at different scales (socio-cultural dimensions of the regeneration project) for a qualitative balance in urban planning and urban projects' manufacturing. The aim is to broaden the understanding on heritage construction, on urban design practices and on implementation of urban projects.

BACKGROUND: We present the results of EGASP, a community experiment to assess the state-of-the-art in genome annotation within the ENCODE regions, which span 1% of the human genome sequence. The experiment had two major goals: the assessment of the accuracy of computational methods to predict protein coding genes; and the overall assessment of the completeness of the current human genome annotations as represented in the ENCODE regions. For the computational prediction assessment, eighteen groups contributed gene predictions. We evaluated these submissions against each other based on a 'reference set' of annotations generated as part of the GENCODE project. These annotations were not available to the prediction groups prior to the submission deadline, so that their predictions were blind and an external advisory committee could perform a fair assessment. RESULTS: The best methods had at least one gene transcript correctly predicted for close to 70% of the annotated genes. Nevertheless, the multiple transcript accuracy, taking into account alternative splicing, reached only approximately 40% to 50% accuracy. At the coding nucleotide level, the best programs reached an accuracy of 90% in both sensitivity and specificity. Programs relying on mRNA and protein sequences were the most accurate in reproducing the manually curated annotations. Experimental validation shows that only a very small percentage (3.2%) of the selected 221 computationally predicted exons outside of the existing annotation could be verified. CONCLUSION: This is the first such experiment in human DNA, and we have followed the standards established in a similar experiment, GASP1, in Drosophila melanogaster. We believe the results presented here contribute to the value of ongoing large-scale annotation projects and should guide further experimental methods when being scaled up to the entire human genome sequence.

The "50 States Project" is the name given to President Ronald D. Reagan;s 1981 pledge to encourage the fifty governors to initiate individual state projects to review their state Codes for unequal treatment of persons based upon sex. We believe that Iowa is the first state to complete this project. Project efforts in Iowa began in June of 1981, when the Governor Robert D. ray appointed Dr. Patricia L. Geadelmann, Chairperson on the Iowa commission on the Status of Women, as Iowa's 50 State Project representative. A 50 States planning committee was formed consisting of members from the Governor Ray's staff, the Iowa Commission on the Status of Women, and the Iowa Legislature. Various alternatives for reviewing the Iowa code and the Iowa Administrative Rules were studied and recommendations of the group were reported to Governor Terry E. Branstad prior to his inauguration.

While the hospital remains an important element of the psychiatric health-care system, the fact that it is always the best place to treat acute psychotic episodes is still debated. After a brief review of the literature relative to the main existing community care models, the authors describe the development in the Department Universitaire de Psychiatrie Adulte (DUPA), of an alternative to hospitalisation for patient going through a severe acute psychiatric episode. They present three clinical situations and the aims of the research project, which will follow this pilot phase.

Objectives: Sequencing and annotation of the genome of Aspergillus fumigatus has dramatically changed our knowledge about the proteins potentially encoded by the fungus. Own analysis have resulted in at least 47 of them contain a signal for secretion. Among those list we want to characterize those enzymes that may have impact on fungal growth outside and particularly inside the host. We thereby want to learn more about their function in general and to identify possible novel drug targets suited to combat invasive aspergillosis. Methods: Four groups of secreted proteases have been chosen for further analysis: 1 Serine-carboxyl proteases (sedolisins). Four of them were expressed in yeast and partly in bacteria. Substrate-specificity studies and kinetics as well as protein characterization of the yeast derived proteases were performed according to standard methods. Enzyme specific polyclonal antibodies were raised in rabbits using the peptides expressed in bacteria. Expression of proteases in A. fumigatus was investigated with these antibodies and gene knockout mutants for each enzyme as a control. All the following mentioned proteases will be investigated accordingly. 2 Two metalloproteases from the M12-family, ADAM-A and ADAM-B. Both proteases are likely membrane associated and may have inherent sheddase function as their counterparts in mammals. 3 One metalloprotease of the M43 family. An orthologue of this protease in Coccidioides posadasii is known to posses immunomodulating activities. 4 One putative endoprotease of the S28-family. An orthologue in Aspergillus niger is known to digest proline-rich proteins. In A. fumigatus this enzyme may facilitate invasion through proline-rich proteins like collagen. Results: All sedolisins expressed in yeast were proteolytically active: Three of them were characterized as tripeptidyl-peptidases whereas one enzyme is an endoprotease. Corresponding knockout mutants did not reveal a specific phenotype. Expression and investigations on all above mentioned proteases as well as generation of corresponding knockout mutants and double knockout mutants for the ADAMs, respectively, is underway. Promising candidates will be investigated in animal studies for reduced virulence. Conclusions : The real existence of so far hypothetical proteases predicted by the genome project was already demonstrated for the sedolisins by a reverse genetic approach (from gene to protein). With the aim of improving basic knowledge on function of other proteases potentially crucial for fungal growth and thus for pathogenesis, other hypothetical enzymes will be investigated. Those enzymes may turn out to be ideal drug targets for antimycotic chemotherapy.
