A new synthesis of (+)-brefeldin A is reported via Padwa allenylsulfone [3+2]-cycloadditive elimination. Cycloadduct 13 was initially elaborated into iodide
27, which, following treatment with Zn, gave aldehyde 28 whose C(9) stereocenter was epimerized. Further elaboration into enoate 38 and Julia−Kocienski olefination with 5 subsequently afforded 39, which was deprotected at C(1) and O(15). Yamaguchi macrolactonization of the seco-acid thereafter afforded a macrocycle that underwent O-desilylation and inversion at C(4) to give (+)-brefeldin A following deprotection

BACKGROUND AND AIMS: Although it has become clear that aneurysmal and occlusive arterial disease represent two distinct etiologic entities, it is still unknown whether the two vascular pathologies are prognostically different. We aim to assess the long-term vital prognosis of patients with abdominal aortic aneurysmal disease (AAA) or peripheral artery disease (PAD), focusing on possible differences in survival, prognostic risk profiles and causes of death. METHODS: Patients undergoing elective surgery for isolated AAA or PAD between 2003 and 2011 were retrospectively included. Differences in postoperative survival were determined using Kaplan-Meier and Cox regression analysis. Prognostic risk profiles were also established with Cox regression analysis. RESULTS: 429 and 338 patients were included in the AAA and PAD groups, respectively. AAA patients were older (71.7 vs. 63.3 years, p < 0.001), yet overall survival following surgery did not differ (HR: 1.16, 95% CI: 0.87-1.54). Neither was type of vascular disease associated with postoperative cardiovascular nor cancer-related death. However, in comparison with age- and gender-matched general populations, cardiovascular mortality was higher in PAD than AAA patients (48.3% vs. 17.3%). Survival of AAA and PAD patients was negatively affected by age, history of cancer and renal insufficiency. Additional determinants in the PAD group were diabetes and ischemic heart disease. CONCLUSIONS: Long-term survival after surgery for PAD and AAA is similar. However, overall life expectancy is significantly worse among PAD patients. The contribution of cardiovascular disease towards mortality in PAD patients warrants more aggressive secondary prevention to reduce cardiovascular mortality and improve longevity.

China's water pollution control law stipulates the Water Pollution Discharge Permit (WPDP) institution and authorizes the State Council to draft the regulations for its implementation and enforcement. However, until today, national regulations have not been established and the permitting system has been operating according to provincial regulations. in contrast to USA, the effluents permit system has been operated for more than 40 years and received relatively successful results. The CWA/NPDES experience offers a valuable reference for China’s water permit system.

O cancro é um dos maiores causadores globais de mortalidade e morbilidade, ocorrendo cerca de 14 milhões de novos casos por ano e 8,2 milhões de mortes anuais com esta patologia, números que tendem a aumentar 70% nas próximas duas décadas. A característica tumoral mais nefasta é a sua capacidade de metastização para outros órgãos, um mecanismo que pode ser despoletado pela falha dos mecanismos normais de controlo de crescimento, proliferação e reparação celulares, que facilita o processo de transformação de células normais em células cancerígenas. A oncogénese processa-se em três etapas, a iniciação, a promoção e a progressão e pode ter origem em células estaminais cancerígenas, que regulam as capacidades de propagação e recidiva do tumor. As neoplasias hematológicas resultam de alterações genéticas e /ou epigenéticas que conduzem à desregulação da proliferação, ao bloqueio da diferenciação e/ou à resitência à apoptose. Para além dos fatores de risco exógenos, como agentes carcinogénicos físicos, químicos e biológicos, existem também fatores endógenos, incluindo características genéticas, que podem alterar a predisposição para o aparecimento de neoplasias, bem como influenciar a resposta à terapêutica. Uma das terapêuticas aplicadas no tratamento do cancro é a quimioterapia. Os fármacos administrados a doentes oncológicos seguem normalmente o percurso de absorção, distribuição, metabolização e eliminação. Este curso pode sofrer alterações caso as proteínas transportadoras e metabolizadoras necessárias não atuem corretamente. Para um melhor conhecimento da influência das alterações provocadas por variações nos genes que codificam proteínas transportadoras de efluxo (MDR1, MRP1), proteínas de influxo (OCTN2) e proteínas metabolizadoras (UCK2), o objetivo deste trabalho consistiu na avaliação de polimorfismos nos genes MDR1, MRP1, OCTN2 e UCK2 e da sua relação com a predisposição para o desenvolvimento de neoplasias hematológicas. Para isto, foram utilizadas amostras de 307 doentes com neoplasias hematológicas, 83 de Síndrome Mielodisplásica (SMD), 63 Leucemia Mieloide Aguda (LMA), 16 de Síndrome Mielodisplásica/Neoplasias Mieloproliferativas (SMD/NMP), 77 de Mieloma Múltiplo (MM) e 68 de Gamapatia Monoclonal de Significado Indeterminado (MGUS) e 164 de controlos não neoplásicos e/ou indivíduos saudáveis. As amostras de ADN foram extraídas do sangue periférico com protocolo adequado. De forma a determinar os genótipos correspondentes a cada amostra, realizaram-se técnicas de RFLP-PCR e ARMS-PCR. Posteriormente, calcularam-se estatisticamente as frequências alélicas e genotípicas relativas às variantes polimórficas dos genes MDR1, MRP1, OCTN2 e UCK2 e verificou-se se estavam em Equilíbrio de Hardy-Weinberg. De seguida, avaliou-se a força de associação entre as formas polimórficas e o risco de desenvolvimento de neoplasias hematológicas, através do cálculo do risco relativo por análise de regressão logística. Avaliaram-se ainda os perfis genéticos e a possível relação com o desenvolvimento e progressão da neoplasia com recurso a regressão logística e análise de Kaplan-Meier. De um modo geral as frequências alélicas e genotípicas não se revelaram alteradas comparativamente ao esperado. A análise do odds ratio associado ao polimorfismo rs1045642 do gene MDR1 revelou que o genótipo CT pode constituir um fator de risco aumentado de 1,84x para o desenvolvimento de Gamapatias Monoclonais e 2,27x para o desenvolvimento de Mieloma Múltiplo. Por outro lado, a presença de genótipos portadores do alelo T têm um efeito protetor no desenvolvimento de MM (OR=0,41). O cálculo do risco associado ao polimorfismo rs4148330 do gene MRP1 revela que o genótipo AG é um fator protetor (OR=0,50) para o desenvolvimento de LMA, assim como o alelo G (OR=0,50). Além disso, verificámos que existe uma associação de risco de desenvolver neoplasia com o polimorfismo rs2185268 do gene UCK2. De facto, a presença dos genótipos CC e AC representam um fator de risco 4,59x aumentado para o desenvolvimento de SMD/NMP. O polimorfismo rs274561 do gene OCTN2 não apresenta relação com o risco relativo de desenvolvimento neoplásico. Da avaliação da influência dos polimorfismos em estudo na sobrevivência global dos doentes, podemos assumir que a presença do genótipo GG relativo ao polimorfismo rs2185268 do gene UCK2 representa uma diminuição da sobrevivência em 11 meses. Os resultados obtidos a partir do nosso estudo permitem-nos concluir que os polimorfismos podem ser fatores relevantes na predisposição para o desenvolvimento de neoplasias hematológicas e na progressão destas doenças.

Peripheral blood mononuclear cells (PBMCs) play quite diverse and important roles in monitoring immune homeostasis. Thus, these subset of blood cells may provide access to potential physiological relevant biomolecules, namely proteins. For this reason, PBMCs represent a promising biological sample in scientific research, particularly as a source of potential biological markers discovery of the most diverse diseases. Prior studies of proteomic characterization of PBMCs from healthy individuals lack either the identification of a large number of proteins or its quantification in a way that is compatible with the search of potential biomarker candidates. Therefore, this study aimed to provide a comprehensive PBMCs proteome characterisation as well as to create a SWATH library. It was also evaluated if by using the BD Vacutainer® CPT™ tubes for PBMCs isolation, it would be possible to identify a larger number of immunologically relevant proteins in comparison to plasma samples. The enrichment test assay revealed that it is possible to identify more immune-related proteins from isolated PBMCs than from plasma. Moreover, the majority of the quantified proteins with an “immune system” GO term assigned is present in higher amounts in PBMCs samples. 2D LC-MS/MS proved to be the best approach to use in qualitative analysis of PBMCs and in the construction of a SWATH library, since it resulted in an increase of both identified and quantified proteins (66.3% and 16.9%, respectively) in comparison to 1D LC-MS/MS. A total of 2071 proteins were identified and it was possible to quantify 922 different proteins among six distinct samples. From these proteins, 445 were commom between all individuals. In conclusion, this work provides a comprehensive PBMCs proteome dataset that will be useful in further studies that focus on the search for potential biological markers of various pathologies in these cells. Additionally, SWATH-MS proved to be a reproducible and effective acquisition method to quantify PBMCs proteins.

Western Pacific hydrothermal vents will soon be subjected to deep-sea mining and peripheral sites are considered the most practical targets. The limited information on community dynamics and temporal change in these communities makes it difficult to anticipate the impact of mining activities and recovery trajectories. We studied community composition of peripheral communities along a cline in hydrothermal chemistry on the Eastern Lau Spreading Center and Valu Fa Ridge (ELSC-VFR) and also studied patterns of temporal change. Peripheral communities located in the northern vent fields of the ELSC-VFR are significantly different from those in the southern vent fields. Higher abundances of zoanthids and anemones were found in northern peripheral sites and the symbiont-containing mussel Bathymodiolus brevior, brisingid seastars and polynoids were only present in the northern peripheral sites. By contrast, certain faunal groups were seen only in the southern peripheral sites, such as lollipop sponges, pycnogonids and ophiuroids. Taxonomic richness of the peripheral communities was similar to that of active vent communities, due to the presence of non-vent endemic species that balanced the absence of species found in areas of active venting. The communities present at waning active sites resemble those of peripheral sites, indicating that peripheral species can colonize previously active vent sites in addition to settling in the periphery of areas of venting. Growth and mortality were observed in a number of the normally slow-growing cladorhizid stick sponges, indicating that these animals may exhibit life history strategies in the vicinity of vents that differ from those previously recorded. A novel facultative association between polynoids and anemones is proposed based on their correlated distributions.

Introduction: Early diagnosis and treatment of Kawasaki disease as the most common cause of acquired heart disease in childhood, may significantly improve the prognosis. Diagnosing infantile Kawasaki (younger than a year) is difficult because of obscure symptoms; at the same time they are at the higher risk of coronary abnormalities. Case Presentation: We report three infants with prolonged (more than 5 days) fever and peripheral gangrene without any other clinical manifestations of Kawasaki disease. Kawasaki was diagnosed due to dilation of coronary artery and other aortic branches, thrombocytosis, and rising of ESR and CRP. All patients were treated with high dose aspirin, IVIG and pulse therapy with methylprednisolone. Additionally, cytotoxic drugs or infliximab were used for two of them because of severe aneurysms in the aortic branches. All 3 patients received aspirin with anti-platelet aggregation dose and 2 patients heparin as an anti-coagulant agent for longtime. After adequate treatment, peripheral gangrene, arterial dilations and aneurysms improved, but during 12 months follow-up coronary aneurysms did not improve completely. Conclusions: Peripheral gangrene must be regarded as an important sign of infantile Kawasaki disease early treatment of which can prevent severe permanent coronary involvements and sequels.

A simple method has been recently proposed to assess acute hydration status in humans; however, several questions remain regarding its reliability, validity, and practicality. Objective: Establish reliability of a simple method to assess euhydration, that is, to analyze whether this method can be used as a consistent indicator of a person´s hydration status. In addition, the study sought to assess the effect exercise has on urine volume when euhydration is maintained and a standardized volume of water is ingested. Methods: Five healthy physically active men and five healthy physically active women, 22.5 ± 2.3 years of age (mean ± standard deviation) reported to the laboratory after fasting for 10 hours or more on three occasions, each one week apart. During the two identical resting euhydration conditions (EuA and EuB), participants remained seated for 45 minutes. During the exercise condition (EuExer), participants exercised intermittently in an environmental chamber (average temperature and relative humidity = 32 ± 3°C and 65 ± 7%, respectively) for a period of 45 minutes and drank water to offset loss due to sweating. The order of treatments was randomized. Upon finishing the treatment period, they ingested a volume of water equivalent to 1.43% body mass (BM) within 30 minutes. Urine was collected and measured henceforth every 30 minutes for 3 hours. Results: Urine volume eliminated during EuExer (1205 ± 399.5 ml) was not different from EuB (1072.2±413.1 ml) or EuA (1068 ± 382.87 ml) (p-value = 0.44). Both resting conditions were practically identical (p-value = 0.98) and presented a strong intraclass correlation (r = 0.849, p-value = 0.001). Conclusions: This method, besides simple, proved to be consistent in all conditions; therefore, it can be used with the certainty that measurements are valid and reliable.
