A survey of the prevalence of Trypanosoma cruzi infection was carried out in Oitis, a small community in the State of Piaui, Brazil. Two hundred and sixty five individuals were screened by microscopic examination, hémoculture, indirect immunofluorescence (IFA), enzyme-linked immunosorbent assay (ELISA), and competitive enzyme-linked immunosorbent assay (C-ELISA) using the monoclonal antibody TCF87 against to a 25kd T. cruzi antigen. Seropositivity was 14.3% by the IFA test, 14.7% by ELISA, and 13.2% by C-ELISA. The C-ELISA using the TCF87 monoclonal antibody seems to be applicable in serodiagnosis of Chagas' disease.

A caracterização de infecção primária recente pelo Toxoplasma gondii se apoia principalmente na presença, no soro, de anticorpos específicos IgM. Para fins diagnósticos de toxoplasmose aguda, ou de contágio recente, a possibilidade de outros marcadores é altamente desejável. Um marcador de infecção recente atualmente referido é a baixa afinidade ou avidez de anticorpos específicos IgG. Para avaliação do novo marcador, titularam-se os soros contra poliantígenos do T. gondii pelo teste imunoenzimático (ELISA), antes e após tratamento dos complexos antígeno-anticorpo formados, com solução de ureia 6 M como agente dissociante. O deslocamento de anticorpos de baixa avidez foi indicado por uma queda de títulos, calculada em porcentagem em relação aos títulos iniciais. Foram estudados 69 soros, 23 de cada um dos 3 perfis sorológicos sucessivos, observados na infecção, e que a caracterizam respectivamente como recente, em fase de transição e crônica. Os perfis foram determinados segundo os resultados de uma bateria de testes, incluindo os de imunofluorescência IgG e IgM, de captura de anticorpos IgM e de hemaglutinação. Para os soros de infecção crônica a queda observada foi de 3% ± 3%, de 34% ± 12% para toxoplasmose recente e de 12% ± 9% para a fase de transição. Conclue-se que a determinação da avidez de anticorpos IgG pode ser utilizada como marcador de infecção primária recente pelo T. gondii.

Com o objetivo de determinar a prevalência da infecção pelo Citomegalovírus (CMV) em pacientes com AIDS, bem como relacionar os achados clínico-virológicos decorrentes desta infecção com as repercussões anatomopatológicas, estudamos 50 pacientes adultos atendidos entre abril de 1986 a junho de 1987, em dois hospitais públicos de São Paulo (HSP e HSPE). Estes pacientes foram acompanhados clínica e laboratorialmente, por período médio de 2 meses com coletas seriadas de sangue, urina e saliva. Foram realizados isolamento do CMV em monocamadas de fibroblastos humanos e testes sorológicos de Imunofluorescência Indireta (IFI-IgG/IgM) e Reação Imunoenzimática (ELISA-IgG). No momento da admissão no estudo 20% (10/50) dos pacientes apresentavam anticorpos IgM CMV específicos e 100% (50/50) deles anticorpos IgG (IFI). Durante o acompanhamento, 5 pacientes inicialmente IgM negativos tornaram-se IgM positivos, sugerindo reativação ou reinfecção pelo CMV. O CMV foi isolado de sangue periférico em 12,5%, da urina em 23,2%, da saliva em 21,9% dos pacientes. Exames anatomopatológicos foram realizados em 24 pacientes, correspondendo a 60% dos pacientes que evoluiram para óbito durante o período de estudo. Corpúsculos de inclusão citomegálica característicos foram observados em 50% das necrópsias, sendo o aparelho digestivo, pulmões e supra-renais os sítios mais acometidos. Não se observou uma relação estatisticamente significante entre os achados clínicos e os achados virológicos e anatomopatológicos.

Serum sample obtained from a male, 12 year old patient suffering from Guillain-Barré syndrome (GBS) was positive for human T-lymphotropic virus (HTLV-I) antibody by the enzyme-linked immunosorbent assay (ELISA) and the Western Blot analysis (WB). Attempts to isolate enteroviruses (including poliovirus) from faecal material in both tissue culture and suckling mice were unsuccessful; in addition, acute and convalescent paired serum samples did not show any evidence of recent poliovirus infection when tested against the three serotypes. Specific tests for detection of Epstein-Barr virus infection were not performed; however, the Paul-Bunnel test yielded negative results. ELISA for detection of anti-cytomegalovirus IgM was also negative. The concomitant occurrence of either adult T cell leukemia (ATL) or lymphoma was not recorded in this case.

The obtainment of monoclonal antibodies for adenovirus species 4(Ad4) is described.The specificities of selected monoclonal antibodies were determined by means of viral neutralization test in cell culture, immunofluorescence and Enzyme-Linked Immunosorbent Assay (ELISA), in the presence of the following species of human adenovirus: 1, 2, 5 (subgenus C), 4 (subgenus E), 7 and 16 (subgenus B) and 9 (subgenus D). Two monoclonal antibodies species specific to adenovirus 4 (1CIII and 3DIII) and one monoclonal antibody that cross reacted with adenovirus species 4 and 7 (2HIII) were obtained.

Registramos a ocorrência de epidemia de dengue causada pelo sorotipo 2 (DEN 2) na cidade de Araguaina, estado do Tocantins (TO) situado no Brasil central. Quatrocentos indíviduos de 74 famílias, residentes nos bairros S. João, Araguaina Sul e Neblina foram entrevistados e sangrados, independentemente de terem adoecido ou não. Os soros tanto de adultos quanto de crianças de ambos os sexos foram usados para pesquisa de anticorpos inibidores da hemaglutinação (IH) e IgM através de ensaio imunoenzimático (MAC ELISA). Nas casas onde haviam doentes no momento do inquérito, sangue total também foi colhido para tentativa de isolamento de vírus. O quadro clínico apresentado pelos pacientes foi caracterizado por febre, cefaléia, mialgias, artralgias e exantema do tipo máculo-papular não pruriginoso. A infecção foi mais frequente em mulheres (33.9%) do que nos homens (23.8%), ocorrendo em todas as faixas etárias, inclusive em crianças com menos de um ano de idade, bem como em maiores de 70 anos. Um total de 1105 mosquitos (56 fêmeas e 45 machos de Culex quinquefasciatus e 567 fêmeas e 437 machos de Aedes aegypti) foram obtidos a partir de larvas coletadas em Araguaina. As fêmeas de Ae. aegypti obtidas das larvas fizeram repasto sangüíneo em 8 pacientes febris. O diagnóstico laboratorial foi feito por isolamento de vírus (cultura de células de Aedes albopictus, clone C6/36) e por sorologia (IH e MAC ELISA). Foram isoladas 5 amostras de DEN 2 de pacientes febris e tipadas por imunofluorescência indireta usando anticorpos monoclonais de dengue. Nenhuma amostra viral foi isolada de mosquitos. Outrossim, comprovou-se infecção em 111 pessoas sangradas, o que revelou um índice de positividade de 27.75% (111 em 400), sendo que 66.2% das famílias estudadas apresentaram pelo menos um indivíduo positivo. Ocorreram ainda, 26.1% de infecções assintomáti-cas. Por outro lado, a correlação de positividade entre os dois testes usados (IH e MAC ELISA) foi de 94.6%. Estimamos que ocorreram aproximadamente 83.250 casos da doença, entre 15 de março a 31 de maio de 1991. Esta é a primeira epidemia de DEN 2 em um estado da Amazônia Brasileira, portanto em área endêmica de febre amarela, e a primeira evidência da interiorização do DEN 2, até então restrito ao Rio de Janeiro.

Laboratorial studies were carried out on 3178 patients with signs and symptoms suggestive of dengue infection from April 1986 to December 1987 in the State of Rio de Janeiro, Brazil. The epidemic had two peaks following the first virus isolation and affected the inhabitants of 17 counties. Both sex and all age groups were affected. Dengue virus type 1 was isolated from 1039 sera and the number of confirmed cases was increased to 1874 (59%) by MAC-ELISA. Isolation rate confirmed cases reached 80% in the specimens obtained until the 4th day after the onset of disease and viraemia ranged from 10 3.0 to 10(8.5) TCID50/ml.

The objective of the present study was to estimate the prevalence of herpes simplex virus type 2 (HSV 2) antibodies in child bearing women of 2 Brazilian populations with different socioeconomic status and to determine the risk of neonatal HSV exposure by means of maternal cultures at the onset of labor. The study was conducted at 2 hospitals: A, serving very low income patients and B, serving middle socioeconomic class. 173 participants from group A and 127 from B answered a questionnaire which showed that the patients had similar ages (27.7 and 26.8 years, respectively) but differed with regard to socioeconomic status, age at first intercourse (18.6 vs 20.6 years), number of sex partners (1.5 vs 1.2) and previous sexually transmitted diseases (15% vs. 1.5%). History of genital herpes was given by 11% of group A participants and by a similar number, 7%, of patients from group B. In addition, 200 serum samples from population A and 455 from B were tested by ELISA for and HSV antibodies and 92% and 86%, respectively, were found to be positive. Sixty seropositive samples from group A and 90 from B were further analyzed by Western blot, which showed the presence of type 2 specific antibodies in 46% and 36%, respectively, suggesting an overall HSV 2 prevalence of 42% in group A and 31% in B. Cervical specimens were obtained for culture from 299 asymptomatic patients of population A and 313 of B. HSV was isolated from one specimen in each group, indicating a 0.3% incidence of asymptomatic viral excretion in both populations. In conclusion, the prevalence of type 2 antibodies in childbearing women was very high, but it did not differ with the socioeconomic status. The risk of HSV perinatal transmission was also similar in the 2 study populations and it was comparable with the data from developed countries. Our findings do not indicate the need of special screening programs for asymptomatic HSV excretion in Brazilian pregnant women.

Two groups of patients undergoing hemodialysis (HD) maintenance were evaluated for their antibody response to non-structural c100/3 protein and structural core protein of hepatitis C virus (HCV). Forty-six patients (Group 1) never presented liver abnormalities during HD treatment, while 52 patients (Group 2) had either current or prior liver enzyme elevations. Prevalence rates of 32.6% and 41.3% were found for anti-c100/3 and anti-HCV core antibodies, respectively, in patients with silent infections (Group 1). The rate of anti-c100/3 in patients of Group 2 was 71.15% and reached 86.5% for anti-HCV core antibodies. The recognition of anti-c100/3 and anti-core antibodies was significantly higher in Group 2 than in Group 1. A line immunoassay composed of structural and non-structural peptides was used as a confirmation assay. HBV infection, measured by the presence of anti-HBc antibodies, was observed in 39.8% of the patients. Six were HBsAg chronic carriers and 13 had naturally acquired anti-HBs antibodies. The duration of HD treatment was correlated with anti-HCV positivity. A high prevalence of 96.7% (Group 2) was found in patients who underwent more than 5 years of treatment. Our results suggest that anti-HCV core ELISA is more accurate for detecting HCV infection than anti-c100/3. Although the risk associated with the duration of HD treatment and blood transfusion was high, additional factors such as a significant non-transfusional spread of HCV seems to play a role as well. The identification of infective patients by more sensitive methods for HCV genome detection should help to control the transmission of HCV in the unit under study.

Aiming at demonstrating a decrease of acquired immunity after chemotherapeutic cure, a group of mice was infected with 25 Schistosoma mansoni cercariae (LE strain). A part of these animals was treated with 400 mg/kg oxamniquine, at 120 days after infection. Challenge infections were carried out at 45, 90 and 170-day-intervals after treatment (185, 210 and 290 days after primoinfection, respectively). Recovery of worms at 20 days after reinfections showed that a residual immunity remains up to 90 days after treatment, and disappears at 170 days after cure. Using the ELISA method, it was possible to detect a decrease of antibody levels (total IgG) in the treated group, when antigens from different evolutive stages of S. mansoni were used. The epidemiological implications of the present results, and the possible mechanisms involved in the decrease of acquired immunity after treatment are discussed.

Total serum IgE, and Strongyloides - specific IgG and IgA antibodies were studied in 27 patients with parasitologically proven strongyloidiasis. Clinical manifestations in this case series were investigated by a restrospective study of the patient's records. Total serum IgE levels were elevated (greater than 250 IU/ml) in 59% of the patients (mean concentration = 1364 IU/ml). Parasite - specific IgG and IgA antibodies were detected by ELISA in the serum of 23 (85.2%) and 21 (77.8%) patients, respectively. Elevated serum IgE and clinical manifestations were not useful indexes of the presence of strongyloidiasis. On the other hand, our results support the view that serologic tests, particularly ELISA for detecting Strongyloides - specific IgG antibodies, can be usefully exploited for diagnostic purposes in strongyloidiasis.

Three distinct syndromes caused by schistosomiasis have been described: cercarial dermatitis or swimmer's itch, acute schistosomiasis or Katayama fever, and chronic schistosomiasis. Complications of acute schistosomiasis have also been reported. The absence of a serological marker for the acute stage has hindered early diagnosis and treatment. Recently, an ELISA test using KLH (keyhole limpet haemocyanin) as antigen, has proved useful in differentiating acute from chronic schistosomiasis mansoni. Clinical and experimental evidence indicate that steroids act synergistically with schistosomicides in the treatment of Katayama syndrome. In this paper, clinical, diagnostic and therapeutic features of acute schistosomiasis are updated.

Las primeras investigaciones realizadas a nivel de bancos de sangre, durante la década 50, indican que la seroprevalencia por infecciones a T. cruzi entre hemodadores fue de 12%. Un estudio posterior, entre 1963-64, efectuado en varios bancos de sangre, así como otros centros, registró una seroprevalencia global de 6.0% (1.1-10.1%). La donación de sangre en Venezuela es gratuita. El control de los bancos de sangre recae en el Departamento de Transfusiones y Bancos de Sangre del Ministerio de Sanidad y Asistencia Social. A partir de 1988, se emplea uniformemente la técnica de ELISA para el diagnóstico de infecciones a T. cruzi en los Bancos de Sangre. La seropositividad promedio interanual, entre 1984-1992, fue de 1.20% (1.09-1.94%). Existen variaciones geográficas entre las localidades de varias entidades federales. Los estados con mayor prevalencia se ubican en las regiones del occidente y centro del país, a saber: Portuguesa, Barinas, Lara, Trujillo, Cojedes y Carabobo. Por las dificultades en obtener tasas de incidencia para el Mal de Chagas, resulta adecuado emplear tasas de prevalencia para uso en salud pública, en función de su mayor estabilidad; y en el caso de Venezuela, dada la severidad menor y una sobrevivencia mayor por esta patologia hoy día. La especificidad, como parámetro de las pruebas serológicas, debería considerarse en función de la baja seroprevalencia detectada a nivel nacional. Convendría emplear varias pruebas diagnósticas en paralelo para buscar un equilibrio entre sensibilidad y especificidad.

La estrategia básica para el desarrollo de programas de lucha contra la hidatidosis es en la actualidad la de atención primaria de la salud. En el presente trabajo, y en ese marco, se instrumenta un sistema de detección precoz de la hidatidosis basado en el diagnóstico inmunológico mediante la técnica de Elisa, a partir de muestras de sangre capilar tomadas en papel de filtro por maestras y agentes sanitarios de los servicios oficiales de la provincia de Rio Negro. Fueron entrenadas 177 maestras y 45 agentes sanitarios correspondientes a 25 escuelas, 3 albergues y 9 hospitales, todos del medio rural. Se obtuvieron 890 muestras de sangre durante el entrenamiento. Posteriormente el personal entrenado instrumentó el sistema obteniendo 728 muestras al inicio del programa. No hubo diferencias estadísticas en la reactividad de ambas muestras. La prevalencia serológica hallada fue del 1.32%. La actividad desplegada por maestras y agentes sanitarios permitió detectar 21 casos nuevos, lo cual constituyó el 20% de los casos nuevos diagnosticados en el área en el período de trabajo. Se discute la viabilidad y la importancia de la incorporación de efectores no tradicionales en los Programas de Control de la Hidatidosis.

Here in is described the clinical and laboratorial findings of a laboratory-acquired infection caused by the virus SP H 114202 (Arenavirus, family Arenaviridae) a recently discovered agent responsible for a viral hemorrhagic fever. The patient was sick for 13 days. The disease had an abrupt onset characterized by high fever (39ºC.), headache, chills and myalgias for 8 days. In addition, on the 3rd day, the patient developed nauseas and vomiting, and in the 10th, epigastralgia, diarrheia and gengivorrhagia. Leucopenia was seen within the 1 st week of onset, with counts as low as 2,500 white cells per mm³. Counts performed after the 23th day of the onset were within normal limits. With the exception of moderate lymphocitosis, no changes were observed in differential counts. An increase in the liter of antibodies by complement fixation, neutralization and ELISA (IgM) was detected. Suckling mice and baby hamsters were inoculated intracerebrally with 0.02 ml of blood samples collected in the 2nd and 7th days of disease. Attempts to isolate the virus were also made in Vero cells. No virus was isolated. This virus was isolated before in a single occasion in São Paulo State, in 1990, from the blood of a patient with hemorrhagic fever with a fatal outcome. The manipulation of the virus under study, must be done carefully, since the transmission can occur through aerosols.
