For successful treatment of prosthetic joint infection, the identification of the infecting microorganism is crucial. Cultures of synovial fluid and intraoperative periprosthetic tissue represent the standard method for diagnosing prosthetic joint infection. Rapid and accurate diagnostic tools which can detect a broad range of causing microorganisms and their antimicrobial resistance are increasingly needed. With newer diagnostic techniques, such as sonication of removed implants, microcalorimetry, molecular methods and mass spectrometry, the sensitivity has been significantly increased. In this article, we describe the conventional and newer diagnostic techniques with their advantages and potential future applications.

Pancreatic stone protein/regenerating protein has recently emerged as an interesting diagnostic and prognostic marker of inflammation and sepsis in the clinical field. Increased blood concentrations have been described in patients with sepsis. Moreover, a high accuracy in predicting fatal outcomes in septic patients admitted to intensive care units has been reported. In this study, we investigated pancreatic stone protein/regenerating protein in postmortem serum in a series of sepsis-related fatalities, local infections and non-infectious cases that underwent medico-legal investigations. Procalcitonin, C-reactive protein, interleukin 6, soluble triggering receptor expressed on myeloid cells-1 and pancreatic stone protein/regenerating protein were measured in the postmortem serum collected during autopsy in a group of sepsis-related deaths, local infections and non-septic intensive care unit patients. Statistically significant differences in pancreatic stone protein/regenerating protein concentrations were observed between sepsis and control patients. A significant positive correlation was found between procalcitonin and pancreatic stone protein/regenerating protein values in septic cases. Pancreatic stone protein/regenerating protein is measurable in postmortem serum from femoral blood collected during autopsy. Additionally, as in the clinical field, pancreatic stone protein/regenerating protein can be used as a postmortem biochemical marker for the diagnosis of sepsis.

Invasive candidiasis ranges from 5 to 10 cases per 1,000 ICU admissions and represents 5% to 10% of all ICU-acquired infections, with an overall mortality comparable to that of severe sepsis/septic shock. A large majority of them are due to Candida albicans, but the proportion of strains with decreased sensitivity or resistance to fluconazole is increasingly reported. A high proportion of ICU patients become colonized, but only 5% to 30% of them develop an invasive infection. Progressive colonization and major abdominal surgery are common risk factors, but invasive candidiasis is difficult to predict and early diagnosis remains a major challenge. Indeed, blood cultures are positive in a minority of cases and often late in the course of infection. New nonculture-based laboratory techniques may contribute to early diagnosis and management of invasive candidiasis. Both serologic (mannan, antimannan, and betaglucan) and molecular (Candida-specific PCR in blood and serum) have been applied as serial screening procedures in high-risk patients. However, although reasonably sensitive and specific, these techniques are largely investigational and their clinical usefulness remains to be established. Identification of patients susceptible to benefit from empirical antifungal treatment remains challenging, but it is mandatory to avoid antifungal overuse in critically ill patients. Growing evidence suggests that monitoring the dynamic of Candida colonization in surgical patients and prediction rules based on combined risk factors may be used to identify ICU patients at high risk of invasive candidiasis susceptible to benefit from prophylaxis or preemptive antifungal treatment.

A adequada disponibilidade de N durante do ciclo do feijoeiro é fundamental para garantir elevada produtividade e qualidade dos grãos produzidos. Esse nutriente pode ser absorvido pelas raízes e folhas da planta. Contudo, ainda existem dúvidas sobre a eficiência da aplicação via foliar de N no feijoeiro e sobre a influência dessa prática na qualidade dos grãos. Objetivou-se neste trabalho avaliar o efeito da adubação nitrogenada em cobertura e via foliar sobre a produtividade e qualidade dos grãos da cultura do feijão. O experimento foi conduzido durante a safra "da seca", em um Latossolo Vermelho distroférrico, no município de Botucatu-SP. O delineamento experimental utilizado foi o de blocos ao acaso com quatro repetições, num esquema fatorial 3 x 4, constituído por três doses de N (0, 45 e 90 kg ha-1) em cobertura e quatro épocas de aplicação de N via foliar (1 - sem aplicação de N via foliar, 2 - pulverização estádio R5 (pré-floração), 3 - pulverização no estádio R7 (início da formação das vagens) e 4 - pulverizações nos estádios R5 e R7). Em cada aplicação de N via foliar foram utilizados 200 L ha-1 de uma solução com 10 % de ureia. Quando foi realizada a adubação nitrogenada de cobertura, a aplicação de N via foliar, independentemente da época, não alterou os componentes da produção, a produtividade e a qualidade dos grãos do feijoeiro. Na ausência da adubação nitrogenada de cobertura, a aplicação de N via foliar na fase reprodutiva aumentou a massa e o tamanho dos grãos, a produtividade de grãos e o teor de proteínas nos grãos do feijoeiro. A aplicação de N via foliar no estádio R5 foi mais eficiente em aumentar a produtividade de grãos do feijoeiro que a aplicação em R7.

The relationships between nutrient contents and indices of the Diagnosis and Recommendation Integrated System (DRIS) are a useful basis to determine appropriate ranges for the interpretation of leaf nutrient contents. The purpose of this study was to establish Beaufils ranges from statistical models of the relationship between foliar concentrations and DRIS indices, generated by two systems of DRIS norms - the F value and natural logarithm transformation - and assess the nutritional status of cotton plants, based on these Beaufils ranges. Yield data from plots (average acreage 100 ha) and foliar concentrations of macro and micronutrients of cotton (Gossypium hirsutum r. latifolium) plants, in the growing season 2004/2005, were stored in a database. The criterion to define the reference population consisted of plots with above-average yields + 0.5 standard deviation (over 4,575 kg ha-1 seed cotton yield). The best-fitting statistical model of the relationship between foliar nutrient concentrations and DRIS indices was linear, with R² > 0.8090, p < 0.01, except for N, with R² = 0.5987, p < 0.01. The two criteria were effective to diagnose the plant nutritional status. The diagnoses were not random, but based on the effectiveness of the chi-square-tested method. The agreement between the methods to assess the nutritional status was 92.59-100 %, except for S, with 74.07 % agreement.

A água é vital para as plantas. Uma redução na disponibilidade de água pode afetar a transpiração e o crescimento foliar das culturas. O objetivo deste trabalho foi determinar a resposta da transpiração e do crescimento foliar ao conteúdo de água disponível no solo, representado pela fração de água transpirável no solo (FATS), em dois clones de batata (um antigo e um recente) adaptados ao Sul do Brasil. Foram realizados três experimentos em ambiente protegido por abrigo telado em Santa Maria, RS, dois deles na primavera (cultivo de safra), com plantios em 20/08/2008 (E1) e 20/10/2008 (E2), e um experimento no outono (cultivo de safrinha), com plantio em 25/03/2009 (E3), usando-se dois clones de batata: um antigo ('Macaca') e um recente ('SMINIA793101-3'). A água disponível, representada pela FATS, a transpiração e o crescimento foliar foram medidos diariamente durante o período de imposição da deficiência hídrica. A FATS crítica que começa a alterar a transpiração, indicativo do início do fechamento estomático, foi de 0,39, 0,47 e 0,28 no clone 'Macaca' e de 0,47, 0,49 e 0,33 no clone 'SMINIA793101-3', para os experimentos E1, E2 e E3, respectivamente, enquanto o crescimento foliar começou a ser reduzido, com valor de FATS crítica maior para o clone 'Macaca', indicando que o clone 'SMINIA793101-3' é mais tolerante ao déficit hídrico no solo que o clone 'Macaca'.

A relação entre a produtividade e a concentração dos nutrientes na planta é a premissa para o uso da análise foliar como critério diagnóstico. Existem vários métodos de diagnóstico que utilizam a análise de tecidos vegetais, e estes desempenham papel fundamental na definição e interpretação do estado nutricional da planta; entretanto, é importante entender que a concentração do nutriente na planta muda durante o ciclo da cultura, tendo de ser padronizada a melhor época para amostragem do tecido vegetal. O objetivo do presente trabalho foi determinar a melhor época de amostragem foliar para a cultura da banana, com base no critério de que o melhor momento é observado quando houver menor efeito da concentração e diluição dos nutrientes nas folhas e que seria quando a relação entre taxas de crescimento relativo de matéria seca e da absorção de nutrientes for igual a 1. Modelando dados da cultura da banana, conclui-se que a melhor época de amostragem foliar da bananeira para avaliação do estado nutricional varia de acordo com o nutriente considerado; contudo, para padronização, os dados indicaram que a amostragem deve ser feita seis meses após o plantio.

Rheumatoid arthritis is a systemic disease that can potentially affect any organ. If the articular manifestations are central to the disease; skin, ophthalmic, neurological, cardiac, pulmonary as well as renal manifestations are well recognized, the latter particularly in the context of a secondary amyloidosis. Although incidence of extraarticular manifestations appears to decrease, likely a result from our more aggressive and early management of rheumatoid arthritis, their consequences remain severe in terms of morbidity and mortality, and their treatments complicated. The new biological therapies seem to be a promising alternative to current therapies, such as cyclophosphamide and high dose prednisone, even if evidences are still limited.

The new Swiss Chronic Obstructive Pulmonary Disease (COPD) Guidelines are based on a previous version, which was published 10 years ago. The Swiss Respiratory Society felt the need to update the previous document due to new knowledge and novel therapeutic developments about this prevalent and important disease. The recommendations and statements are based on the available literature, on other national guidelines and, in particular, on the GOLD (Global Initiative for Chronic Obstructive Lung Disease) report. Our aim is to advise pulmonary physicians, general practitioners and other health care workers on the early detection and diagnosis, prevention, best symptomatic control, and avoidance of COPD as well as its complications and deterioration.

From a technical standpoint the most widely used tests for serology include the ELISA (enzyme linked immunosorbent assay), the IFA (indirect fluorescence assay), and the immunoblot. ELISA tests are widely used as screening assays since they harbor a high sensitivity. The main pitfall of serologies is the frequency of cross-reactions, especially between the different helminths. This is why positive results should be confirmed by a second test method with a higher specificity. Results need also to be put in the perspective of the patient history, clinical signs and laboratory findings. Serological tests are most appropriate when the parasite cannot be documented by direct examination (by eye or under the microscope) and during the pre-patent period. Serologies for parasites are also useful when an unexplained eosinophilia is present.

This part of the EFISG guidelines focuses on non-neutropenic adult patients. Only a few of the numerous recommendations can be summarized in the abstract. Prophylactic usage of fluconazole is supported in patients with recent abdominal surgery and recurrent gastrointestinal perforations or anastomotic leakages. Candida isolation from respiratory secretions alone should never prompt treatment. For the targeted initial treatment of candidaemia, echinocandins are strongly recommended while liposomal amphotericin B and voriconazole are supported with moderate, and fluconazole with marginal strength. Treatment duration for candidaemia should be a minimum of 14 days after the end of candidaemia, which can be determined by one blood culture per day until negativity. Switching to oral treatment after 10 days of intravenous therapy has been safe in stable patients with susceptible Candida species. In candidaemia, removal of indwelling catheters is strongly recommended. If catheters cannot be removed, lipid-based amphotericin B or echinocandins should be preferred over azoles. Transoesophageal echocardiography and fundoscopy should be performed to detect organ involvement. Native valve endocarditis requires surgery within a week, while in prosthetic valve endocarditis, earlier surgery may be beneficial. The antifungal regimen of choice is liposomal amphotericin B +/- flucytosine. In ocular candidiasis, liposomal amphotericin B +/- flucytosine is recommended when the susceptibility of the isolate is unknown, and in susceptible isolates, fluconazole and voriconazole are alternatives. Amphotericin B deoxycholate is not recommended for any indication due to severe side effects.

The Diagnosis and Recommendation Integrated System (DRIS) can improve interpretations of leaf analysis to determine the nutrient status. Diagnoses by this method require DRIS norms, which are however not known for oil content of soybean seeds. The aims of this study were to establish and test the DRIS method for oil content of soybean seed (maturity group II cultivars). Soybean leaves (207 samples) in the full flowering stage were analyzed for macro and micro-nutrients, and the DRIS was applied to assess the relationship between nutrient ratios and the seed oil content. Samples from experimental and farm field sites of the southernmost Brazilian state Rio Grande do Sul (28° - 29° southern latitude; 52° -53° western longitude) were assessed in two growing seasons (2007/2008 and 2008/2009). The DRIS norms related to seed oil content differed between the studied years. A unique DRIS norm was established for seed oil content higher than 18.68 % based on data of the 2007/2008 growing season. Higher DRIS indices of B, Ca, Mg and S were associated with a higher oil content, while the opposite was found for K, N and P. The DRIS can be used to evaluate the leaf nutrient status of soybean to improve the seed oil content of the crop.
