Este estudo teve como objetivo identificar e comparar a prevalência da síndrome metabólica entre hipertensos e normotensos. Os aspectos adotados para a definição de síndrome metabólica foram os definidos pelo National Cholesterol Education Program/Adults Treatment Painel (NCEP-ATP III). Os dados referentes aos perfis clínicos e bioquímicos foram processados no programa SPSS para cálculo de frequências absolutas e porcentagens. Utilizou-se o teste t de Student para comparações das médias, sendo as diferenças consideradas estatisticamente significantes para p<0,05. A amostra foi composta por 93 participantes normotensos e 168 hipertensos. Identificou-se a presença de síndrome metabólica em 60,7% dos hipertensos e 18,3% dos normotensos. Os portadores de hipertensão arterial apresentam diferença significante nos valores de pressão arterial (p<0,001), circunferência abdominal (p<0,001), glicemia (p<0,05) e triglicérides plasmáticos (p<0,05). A frequência dos riscos cardiometabólicos associados à síndrome metabólica é maior na presença de doença hipertensiva.

Objetivou-se com este trabalho analisar a utilização da tecnologia das relações na produção do cuidado a usuários com hipertensão arterial. Trata-se de estudo qualitativo, realizado em oito Unidades de Saúde da Família de Jequié-BA. Os sujeitos do estudo foram formuladores da política, profissionais de saúde e usuários, totalizando dezesseis informantes. Em 2010, realizou-se entrevista semiestruturada, observação sistemática e análise documental. Para apreciação dos dados utilizou-se análise de conteúdo temática. Os resultados evidenciaram que os profissionais, sobretudo enfermeiros e agentes comunitários de saúde, utilizam tecnologias leves na busca da integralidade. Por outro lado, alguns profissionais demonstraram prática biologicista, com ênfase em normas e procedimentos. Conclui-se que há necessidade de mudança na interação entre trabalhador e usuário, considerando-se a singularidade e a autonomia do usuário e família na construção de novas formas de cuidado.

Estudo descritivo e de corte transversal que teve como objetivo avaliar e autoavaliar o conhecimento de enfermeiros de Unidades de Terapia Intensiva adulto sobre medida direta e indireta da pressão arterial. Foram abordados 54 enfermeiros de três Unidades de Terapia Intensiva. Foi aplicado um questionário auto respondido com 65 questões (40 relacionadas à pressão arterial). Os enfermeiros apresentaram desempenho insuficiente no teste que avaliou conhecimento (nota média 4,6). Metade da amostra sentiu-se pouco satisfeita com relação ao que sabe sobre pressão arterial. Após responder o questionário os sujeitos autoavaliaram seu conhecimento como regular (48,2%), ruim (27,8%) e péssimo (9,3%), manifestando estarem conscientes em relação à importância do assunto para a prática. Os resultados do estudo mostram a necessidade de realização urgente de atividades de educação continuada para esta amostra, visto que são profissionais que atuam diretamente com um sinal vital prioritário no cuidado ao paciente crítico.

OBJECTIVE: To evaluate the thickness of cartilage at the posterior aspect of the medial and lateral condyle in Osteoarthritis (OA) knees compared to non-OA knees using computed tomography arthrography (CTA). DESIGN: 535 consecutive knee CTAs (mean patient age = 48.7 ± 16.0; 286 males), were retrospectively analyzed. Knees were radiographically classified into OA or non-OA knees according to a modified Kellgren/Lawrence (K/L) grading scheme. Cartilage thickness at the posterior aspect of the medial and lateral femoral condyles was measured on sagittal reformations, and compared between matched OA and non-OA knees in the whole sample population and in subgroups defined by gender and age. RESULTS: The cartilage of the posterior aspect of medial condyle was statistically significantly thicker in OA knees (2.43 mm (95% confidence interval (CI) = 2.36, 2.51)) compared to non-OA knees (2.13 mm (95%CI = 2.02, 2.17)) in the entire sample population (P < 0.001), as well as for all subgroups of patients over 40 years old (all P ≤ 0.01), except for females above 60 years old (P = 0.07). Increase in cartilage thickness at the posterior aspect of the medial condyle was associated with increasing K/L grade in the entire sample population, as well as for males and females separately (regression coefficient = 0.10-0.12, all P < 0.001). For the lateral condyle, there was no statistically significant association between cartilage thickness and OA (either presence of OA or K/L grade). CONCLUSIONS: Cartilage thickness at the non-weight-bearing posterior aspect of the medial condyle, but not of the lateral condyle, was increased in OA knees compared to non-OA knees. Furthermore, cartilage thickness at the posterior aspect of the medial condyle increased with increasing K/L grade.

Este estudo tem por finalidade revisar os componentes do diagnóstico de enfermagem Estilo de vida sedentário (EVS) propostos pela NANDA-I em indivíduos com hipertensão arterial. A revisão foi desenvolvida a partir da análise de conceito, com o auxílio do método da Revisão Integrativa da Literatura a partir de 43 artigos pesquisados em cinco bases de dados (LILACS, CINAHL, PUBMED, SCOPUS E COCHRANE). Foram utilizadas as seguintes combinações de descritores e equivalentes nas línguas inglesa e espanhola: Estilo de vida sedentário and Hipertensão e Sedentarismo and Hipertensão. O processo de revisão conduziu aos seguintes resultados: modificação da definição do EVS, de nomeações de alguns indicadores clínicos e acréscimo de outros. Considera-se que o estudo subsidiou uma direção para a eficiência diagnóstica de indicadores clínicos do EVS, contribuindo para o refinamento e o aprimoramento desse diagnóstico e seus componentes.

The objective of the study was to validate the content of the dimensions that constituted nonadherence to treatment of arterial systemic hypertension. It was a methodological study of content validation. Initially an integrative review was conducted that demonstrated four dimensions of nonadherence: person, disease/treatment, health service, and environment. Definitions of these dimensions were evaluated by 17 professionals, who were specialists in the area, including: nurses, pharmacists and physicians. The Content Validity Index was calculated for each dimension (IVCi) and the set of the dimensions (IVCt), and the binomial test was conducted. The results permitted the validation of the dimensions with an IVCt of 0.88, demonstrating reasonable systematic comprehension of the phenomena of nonadherence.

To target pharmacological prevention, instruments giving an approximation of an individual patient's risk of developing postoperative delirium are available. In view of the variable clinical presentation, identifying patients in whom prophylaxis has failed (that is, who develop delirium) remains a challenge. Several bedside instruments are available for the routine ward and ICU setting. Several have been shown to have a high specificity and sensitivity when compared with the standard definitions according to DSM-IV-TR and ICD-10. The Confusion Assessment Method (CAM) and a version specifically developed for the intensive care setting (CAM-ICU) have emerged as a standard. However, alternatives allowing grading of the severity of delirium are also available. In many units, the approach to delirium follows a three-step strategy. Initially, non-pharmacological multicomponent strategies are used for primary prevention. As a second step, pharmacological prophylaxis may be added. Perioperative administration of haloperidol has been shown to reduce the severity, but not the incidence, of delirium. Perioperative administration of atypical antipsychotics has been shown to reduce the incidence of delirium in specific groups of patients. In patients with delirium, both symptomatic and causal treatment of delirium need to be considered. So far symptomatic treatment of delirium is primarily based on antipsychotics. Currently, cholinesterase inhibitors cannot be recommended and the data on dexmedetomidine are inconclusive. With the exception of alcohol-withdrawal delirium, there is no role for benzodiazepines in the treatment of delirium. It is unclear whether treating delirium prevents long-term sequelae.

Nitric oxide (NO) plays a major role in the regulation of cardiovascular and metabolic homeostasis, as evidenced by insulin resistance and arterial hypertension in endothelial NO synthase (eNOS) null mice. Extrapolation of these findings to humans is difficult, however, because eNOS gene deficiency has not been reported. eNOS gene polymorphism and impaired NO synthesis, however, have been reported in several cardiovascular disease states and could predispose to insulin resistance. High-fat diet induces insulin resistance and arterial hypertension in normal mice. To test whether partial eNOS deficiency facilitates the development of insulin resistance and arterial hypertension during metabolic stress, we examined effects of an 8-week high-fat diet on insulin sensitivity (euglycemic clamp) and arterial pressure in eNOS(+/-) mice. When fed a normal diet, these mice had normal insulin sensitivity and were normotensive. When fed a high-fat diet, however, eNOS(+/-) mice developed exaggerated arterial hypertension and had fasting hyperinsulinemia and a 35% lower insulin-stimulated glucose utilization than control mice. The partial deletion of the eNOS gene does not alter insulin sensitivity or blood pressure in mice. When challenged with nutritional stress, however, partial eNOS deficiency facilitates the development of insulin resistance and arterial hypertension, providing further evidence for the importance of this gene in linking metabolic and cardiovascular disease.

Injury of an arterial vessel wall acutely triggers a multifaceted process of thrombus formation, which is dictated by the high-shear flow conditions in the artery. In this overview, we describe how the classical concept of arterial thrombus formation and vascular occlusion, driven by platelet activation and fibrin formation, can be extended and fine-tuned. This has become possible because of recent insight into the mechanisms of: (i) platelet-vessel wall and platelet-platelet communication, (ii) autocrine platelet activation, and (iii) platelet-coagulation interactions, in relation to blood flow dynamics. We list over 40 studies with genetically modified mice showing a role of platelet and plasma proteins in the control of thrombus stability after vascular injury. These include multiple platelet adhesive receptors and other junctional molecules, components of the ADP receptor signalling cascade to integrin activation, proteins controlling platelet shape, and autocrine activation processes, as well as multiple plasma proteins binding to platelets and proteins of the intrinsic coagulation cascade. Regulatory roles herein of the endothelium and other blood cells are recapitulated as well. Patient studies support the contribution of platelet- and coagulation activation in the regulation of thrombus stability. Analysis of the factors determining flow-dependent thrombus stabilization and embolus formation in mice will help to understand the regulation of this process in human arterial disease.
