Considering the meantime of Brazilian crisis of State and redemocratization, this article looks for a description and analysis of the facts and circumstances that marked the undergraduate education of Brazilian public administration during this period, considered by the authors as one of the cycles (or stages of construction) of academic education. Methodologically, this article makes a review of academic and non-academic works which deal directly and indirectly the PA themes, including revisiting the sources and analyzing the existing laws and opinions about the undergraduate education in public administration in a continuum of time between 1983-94, through a semi-structured interview with academics who have experienced such period. Regarding the results, this article observes that the period between 1983-94, unlike the first cycle (1952-65) and second cycle (1966-82), when the academic background in public administration had an identity - adherent to the conception (and project) of State and the contours (and production) in the public administration field of knowledge - having the Ebap/FGV a model case, was a problematic stage of construction, reflecting the crisis of State in the 80's years and also the paradigmatic crisis (or discontinuity) in public administration field of knowledge in Brazil during this time.

Placing the management of childhood forms considered at risk, we discuss the operation of a welfare institution and its effects on the subjective process. Created to manage the virtues of the poor behavior of children, makes visible to the way technology features disciplinary policies: surveillance is evident of cameras, high walls, electric fences and in the eyes of educators, the penalty stands normalizing practices corrective aimed at standardization bodies, educational meetings are compared to the examination, as they produce dossiers of individual behavior, emphasizing characteristics that are considered dangerous anticipates the administration and production of docile individuals, useful, productive, applying identity models. Following the lines of knowledge and power relations that constitute this space, we used the genealogy in the analysis of discursive practices that cross society in its historical construction, the genealogy allows us to achieve these movements in the consolidation process of the government of life, and also, the deconstruction of hegemonic practices. In an attempt to escape the set, we propose the realization of artistic and cultural activities within a process of collective construction as a strategy of resistance and confrontation with the dominant logic, consolidating practices that favor the enhancement of life in its manifold possibilities.

Objectives: Epidemiological studies have shown a relationship between long-term use of proton pump inhibitors and bone metabolism. However, this relationship has not yet become established. The aim of the present study was to analyze the mechanical properties and bone mineral density (BMD) of rats that were subjected to long-term omeprazole use. Methods: Fifty wistar rats weighing between 200 and 240 g were divided equally into five groups: OMP300 (omeprazole intake at a dose of 300 moL/kg/day); OMP200 (200 moL/ kg/day); OMP40 (40 moL/kg/day); OMP10 (10 moL/kg/day); and Cont (control group; intake of dilution vehicle). The solutions were administered for 90 consecutive days. After the rats had been sacrificed, their BMD, the mechanical properties of the dissected femurs and their serum Ca++ levels were analyzed. Results: The BMD of the OMP300 group was lower than that of the controls (p = 0.006). There was no difference in comparing the OMP200, OMP40 and OMP10 groups with the controls. The maximum strength and rigidity of the femur did not differ in the experimental groups in comparison with the controls. The OMP300 group had a statistically lower serum Ca++ concentration than that of the controls (p = 0.049), but the other groups did not show any difference in relation to the controls. Conclusion: Daily intake of 300moL/kg/day of omeprazole decreased the BMD of the femur, but without changes to the rigidity and strength of the femur in adult rats. © 2

Um felino macho, sem raça definida, pesando 3,6 kg com 10 anos de idade foi atendido no Hospital Veterinário “Luiz Quintiliano de Oliveira”, apresentando obstrução uretral com indicação para uretrostomia perineal. Ao exame físico geral, observou-se freqüência cardíaca (FC) de 240 bat.min-1, taquipnéia, temperatura retal (TR) de 38,4oC, mucosas normocoradas, tempo de preenchimento capilar de um segundo e desidratação de aproximadamente 5%. Como medicação pré-anestésica, administrou-se cetamina (6mg.kg-1), midazolam (0,15mg.kg-1) e morfina (0,15mg.kg-1) pela via intramuscular. A veia cefálica foi canulada com cateter 24G para administração de Ringer com Lactato de sódio (10mL.kg-1.h-1) e a indução foi realizada por meio da administração de propofol (4,4mg.kg-1) pela via intravenosa. Seguiu-se a intubação orotraqueal com sonda no 4,0 conectando-se a um circuito sem reinalação de Baraka, sendo a manutenção anestésica efetuada com isofluorano em fluxo diluente de oxigênio a 100%. Em seguida, realizou-se a técnica de anestesia peridural com o paciente em posição esternal e membros pélvicos estendidos cranialmente. Após localização do espaço entre as vértebras L7 e S1, introduziu-se uma agulha 13x4,5 percutaneamente até atingir o espaço peridural, administrando-se morfina (0,1mg.kg-1), fentanil (3mcg.kg-1) e lidocaína a 1% com vasoconstritor (1,8mg.kg-1), perfazendo um volume total de 0,25mL.kg-1. Durante o procedimento anestésico, monitorou-se a FC, freqüência respiratória (f), pressão arterial sistólica (PAS), pressão parcial de dióxido de carbono ao final da expiração (ETCO2), temperatura esofágica (TE), saturação periférica da hemoglobina (SpO2) e plano anestésico. O tempo total de anestesia e cirurgia foi de 65 e 50 minutos, respectivamente. O plano anestésico manteve-se estável, sem a necessidade de resgate analgésico. A SpO2, concentração de isofluorano e TE mantiveram-se em 97±2%, 1,3±0,1% e 36,7±0,4oC, respectivamente. Durante a anestesia, observou-se um momento de hipotensão, provavelmente devido ao bloqueio simpático decorrente da anestesia peridural. Tal evento foi tratado com bolus de solução de Ringer com Lactato de sódio (5mL.kg-1) promovendo o retorno da PAS aos valores de normalidade. A recuperação anestésica do animal foi isenta de complicações inerentes ao procedimento anestésico. O emprego de opióides pela via peridural é recomendado em procedimentos cirúrgicos abdominal, torácico, genito-urinário e ortopédico, principalmente em pacientes de alto risco, obesos e idosos. Em humanos, a associação da morfina e fentanil pela via peridural demonstra analgesia pós-operatória superior à promovida pelo uso isolado da morfina. Conclui-se que na espécie felina, a associação de morfina e fentanil pela via peridural pode ser uma boa alternativa para realização de protocolos de anestesia balanceada para uretrostomia.

Introduction: Post-marketing surveillance of drugs aims to detect problems related to safety, effectiveness and quality. The identification of adverse drug events (ADE) is made, mainly, by health professionals´ spontaneous reporting. This method allows risk communication in pharmacovigilance and contributes for market regulation. Objective: To estimate the prevalence of adverse drug reaction (ADR) and the suspicions of therapeutic failure (TF) reported by health professionals; to verify the active principle and type of drugs related to ADE, seriousness, causality, production mechanism and clinical manifestation of the events identified. METHODS: A cross-sectional study was performed in a teaching and public hospital which integrates the Sentinel Hospital Network, in 2008. ADR seriousness was classified according to intensity (mild, moderate, serious and lethal); drugs associated with ADE were categorized according to type (brand name drugs and non-brand name drugs); causality was imputed with Naranjo algorithm and the mechanism of occurrence was analyzed according to Rawlins e Thompson definitions (A or B). Results: There were 103 ADE reports in the period, of which 39 comprised TF and 64 ADR. Nurses reported the most ADE (53.4%). The majority of ADR were classified as type A (82.8%), mild (81.3%), possible (57.8%), according to causality assessment, and related to brand name drugs (20/35). Human immunoglobulin, docetaxel and paclitaxel were the drugs frequently associated with ADR. TF arising from no-brand name drugs (26/29), regarding, mainly, midazolam and ganciclovir. Conclusion: The results of the ADE report contribute for proposition of trigger tools for intensive monitoring of drug safety, as well as for the supplier qualification and for the improvement of quality products.

The ageing process can change the pharmacodynamics and pharmacokinetics parameters. Therefore, some medications are considered potentially inappropriate (PIM) for the elderly people, since they can increase the likelihood of occurrence of adverse drug events. The objectives are to estimate the frequency of use of PIM in the elderly people, with potentially hazardous drug interactions (PHDI) and to evaluate the impact of pharmaceutical intervention (PI) for the prescription of safer therapeutic alternatives. A cross-sectional study was performed in a Health Family Strategy (region of Araraquara, SP), between January and February/2012. The medical records of patients aged ≥60 years, that use at least one drug, were consulted for identification of PIM, according to the Beers criteria. The MPI identified were classified considering the Anatomical Therapeutic Chemical Classification System (ATC) and the essentiality of the drug (safety, effectiveness, quality and cost parameters) The inclusion criteria were met by 358 elderly, being that 93 of them (26%) had taken at least one PIM. Of the 114 different drugs prescribed for elderly, ten were classified as PIM, of which four of them act on the central nervous system, four on cardiovascular system and two on the digestive tract. Seven MPI are essential medicines, belonging to national list of essential drugs (RENAME-2010). Fourteen drug interactions were identified, of which two are PHDI (fluoxetine/amitriptyline and digoxin/hydrochlorothiazide).After the PI, there was no change in medical prescriptions of patients with PIM use or with DI. Medical prescriptions of elderly attended in the Health Family Strategy show pharmacotherapeutic safety problems, of which may be responsible for health hazardous for this age group. Although the intervention carried out by letter had been ineffective for the adherence of doctors in prescribing safe alternatives, wide dissemination of the lists that contain PIM and PHDI is need, as well as the inclusion of safer equivalents in RENAME, in order to contribute for rational use of drugs.
