OBJECTIVE: To describe the epidemiology, the surgical treatment, the microbiology, the antibiotic prophylaxis and the outcome of patients with the most severe type of open fractures. METHODS: Retrospective chart reviews of patients with Gustilo type III open fracture admitted to an university hospital in Switzerland between January 2007 and December 2011. The patient's and fracture's characteristics, surgery, antibiotic prophylaxis, and microbiology findings at the initial and at the revision surgery were described. RESULTS: Thirty patients were included (83% male, mean age 41 years). More than half of the patients had polytrauma. In all patients, debridement and stabilization surgery (70% using external fixation) were performed at admission. Soft tissue reconstruction was performed in 87% and in 23% immediate bone graft was performed. Antibiotic prophylaxis were given in all patients for a median duration of 9 days (60% received amoxicillin/clavulanic acid). Positive bacterial culture was found in 53% of the patients at initial surgery and in 88% at revision surgery. At initial and revision surgery, 47% and 88% of the pathogens were amoxicillin/clavulanic acid-resistant. Treatment outcome was favorable in 24 of 30 patients (80%) and in six cases (20%) an amputation had to be performed. None of the patients had chronic bone infection. CONCLUSIONS: Positive cultures were found often in open fractures. Amoxicillin/clavulanic acid which is often mentioned in many guidelines as prophylaxis in open fractures does not cover the most common isolated organisms. The combination of surgery and antibiotic prophylaxis leads to good outcome in Gustilo type III fracture.

OBJECTIVES: Transapical transcatheter valve procedures are performed through a left minithoracotomy and require apical sutures to seal the apical access site. The use of large-calibre devices compromises any attempt to fully perform the procedure with a thoracoscopic approach or percutaneously. We report our preliminary experience in animals with a new sutureless self-expandable apical occluder, engineered to perform transapical access site closure in a minimally invasive setting with large-size introducer sheaths. METHODS: The apical occluder with extendable waist was implanted in six young pigs during an acute animal study. Under general anaesthesia, animals (mean weight: 62 ± 8 kg) received full heparinization (heparin: 100 UI/kg; activated clotting time above 250 s). Through a median sternotomy, a 21-Fr Certitude? introducer sheath (outer diameter: 25 Fr) was placed over the wire into the cardiac apex. The delivery catheter carrying the constrained apical plug was inserted into the sheath and deployed under fluoroscopic control, whereas the Certitude? was retrieved. After protamine infusion, we observed and recorded the 1-h bleeding with standard haemodynamic parameters. Animals were sacrificed, and hearts analysed. RESULTS: Six apical closure devices were successfully introduced and deployed in six pig hearts through large-size apical sheaths at first attempt. In all animals, the plugs guaranteed immediate apical sealing and traces of blood were collected in the pericardium during the 1-h observational period (mean of 16 ± 3.4 ml of blood loss per animal). Haemodynamic parameters remained stable during the entire study period and no plug dislodgement was detected with normal systemic blood pressure (mean arterial mean blood pressure: 65 ± 7 mmHg). Post-mortem analysis confirmed the full deployment and good fixation of all plugs, without macroscopic damages to the surrounding myocardium. CONCLUSIONS: This sutureless self-expandable apical occluder is a simple device capable of sealing large-size apical access sites (20-35 Fr) in an acute animal study. This approach is a step further towards less invasive transapical valve procedures in the clinical setting, and further animal tests will be performed to confirm the long-term efficacy and safety of this device.

Post-testicular sperm maturation occurs in the epididymis. The ion concentration and proteins secreted into the epididymal lumen, together with testicular factors, are believed to be responsible for the maturation of spermatozoa. Disruption of the maturation of spermatozoa in the epididymis provides a promising strategy for generating a male contraceptive. However, little is known about the proteins involved. For drug development, it is also essential to have tools to study the function of these proteins in vitro. One approach for screening novel targets is to study the secretory products of the epididymis or the G protein-coupled receptors (GPCRs) that are involved in the maturation process of the spermatozoa. The modified Ca2+ imaging technique to monitor release from PC12 pheochromocytoma cells can also be applied to monitor secretory products involved in the maturational processes of spermatozoa. PC12 pheochromocytoma cells were chosen for evaluation of this technique as they release catecholamines from their cell body, thus behaving like endocrine secretory cells. The results of the study demonstrate that depolarisation of nerve growth factor -differentiated PC12 cells releases factors which activate nearby randomly distributed HEL erythroleukemia cells. Thus, during the release process, the ligands reach concentrations high enough to activate receptors even in cells some distance from the release site. This suggests that communication between randomly dispersed cells is possible even if the actual quantities of transmitter released are extremely small. The development of a novel method to analyse GPCR-dependent Ca2+ signalling in living slices of mouse caput epididymis is an additional tool for screening for drug targets. By this technique it was possible to analyse functional GPCRs in the epithelial cells of the ductus epididymis. The results revealed that, both P2X- and P2Y-type purinergic receptors are responsible for the rapid and transient Ca2+ signal detected in the epithelial cells of caput epididymides. Immunohistochemical and reverse transcriptase-polymerase chain reaction (RTPCR) analyses showed the expression of at least P2X1, P2X2, P2X4 and P2X7, and P2Y1 and P2Y2 receptors in the epididymis. Searching for epididymis-specific promoters for transgene delivery into the epididymis is of key importance for the development of specific models for drug development. We used EGFP as the reporter gene to identify proper promoters to deliver transgenes into the epithelial cells of the mouse epididymis in vivo. Our results revealed that the 5.0 kb murine Glutathione peroxidase 5 (GPX5) promoter can be used to target transgene expression into the epididymis while the 3.8 kb Cysteine-rich secretory protein-1 (CRISP-1) promoter can be used to target transgene expression into the testis. Although the visualisation of EGFP in living cells in culture usually poses few problems, the detection of EGFP in tissue sections can be more difficult because soluble EGFP molecules can be lost if the cell membrane is damaged by freezing, sectioning, or permeabilisation. Furthermore, the fluorescence of EGFP is dependent on its conformation. Therefore, fixation protocols that immobilise EGFP may also destroy its usefulness as a fluorescent reporter. We therefore developed a novel tissue preparation and preservation techniques for EGFP. In addition, fluorescence spectrophotometry with epididymal epithelial cells in suspension revealed the expression of functional purinergic, adrenergic, cholinergic and bradykinin receptors in these cell lines (mE-Cap27 and mE-Cap28). In conclusion, we developed new tools for studying the role of the epididymis in sperm maturation. We developed a new technique to analyse GPCR dependent Ca2+ signalling in living slices of mouse caput epididymis. In addition, we improved the method of detecting reporter gene expression. Furthermore, we characterised two epididymis-specific gene promoters, analysed the expression of GPCRs in epididymal epithelial cells and developed a novel technique for measurement of secretion from cells.

Tuotantotehokkuus näyttelee yhä suurempaa roolia teollisuudessa, minkä vuoksi myös pakkauslinjastoille joudutaan asettamaan suuria vaatimuksia. Usein leikkaus- ja kappaleensiirtosovelluksissa käytetään lineaarisia ruuvikäyttöjä, jotka voitaisiin tietyin edellytyksin korvata halvemmilla ja osittain suorituskykyisimmillä hammashihnavetoisilla johteilla. Yleensä paikkasäädetty työsolu muodostuu kahden tai kolmen eri koordinaatistoakselin suuntaan asennetuista johteista. Tällaisen työsolun paikoitustarkkuuteen vaikuttavat muun muassa käytetty säätörakenne, moottorisäätöketjun viiveet, sekä laitteiston eri epälineaarisuudet, kuten kitka. Tässä työssä esitetään lineaarisen hammashihnaservokäytön dynaamista käytöstä kuvaava matemaattinen malli ja laaditaan mallin pohjalta laitteen simulointimalli. Mallin toimivuus varmistetaan käytännön identifiointitesteillä. Lisäksi työssä tutkitaan, kuinka hyvään suorituskykyyn lineaarinen hammashihnaservokäyttö kykenee, jos teollisuudessa paikoitussäätörakenteena tyypillisesti käytetty kaskadirakenne tai PID-rakenne korvataan kehittyneemmällä mallipohjaisella tilasäädinrakenteella. Säädön toimintaa arvioidaan simulointien ja koelaitteistolla suoritettavien mittausten perusteella.

OBJETIVO: Este estudo tem como objetivo descrever apresentações incomuns do pseudotumor do hemofílico no diagnóstico por imagem. MATERIAIS E MÉTODOS: Estudo retrospectivo com avaliação de cinco pseudotumores ósseos do hemofílico em dois pacientes. Os achados de imagem em dois pacientes hemofílicos tipo A foram avaliados em consenso por dois radiologistas musculoesqueléticos. Foram estudados exames de radiografia simples, tomografia computadorizada e ressonância magnética. RESULTADOS: Em uma das lesões analisadas a fase pós-contraste intravenoso da tomografia computadorizada mostrou áreas de reforço heterogêneo e de aspecto sólido no interior da lesão da coxa direita. Este aspecto foi confirmado no exame anatomopatológico da lesão em questão. Outro achado raro foi a identificação de dois pseudotumores intraósseos no úmero, separados por segmento de osso normal. E, por fim, também um pseudotumor do fêmur com extensão para partes moles e transarticular, com conseqüente acometimento da tíbia e patela. CONCLUSÃO: Os achados de diagnóstico por imagem acima descritos não são comumente relatados para os pseudotumores ósseos do hemofílico. É importante que o radiologista tenha conhecimento dessas apresentações mais raras.

Introduction Les Echelles Lausannoises d'Auto-Evaluation des Difficultés et des Besoins ELADEB, est un outil utilisé en réhabilitation psychiatrique permettant de dresser le profil des problèmes rencontrés par les patients psychiatriques dans les principaux domaines de la vie quotidienne, ainsi que le besoin d'aide supplémentaire à celle déjà existante. Cet outil a déjà été validé dans une étude1 portant sur 94 patients et recueille un intérêt grandissant. Afin de compléter les données de validation recueillies lors de la première étude, nous présentons ici une seconde étude visant à comprendre d'avantage cet outil en le couplant à l'Eye tracker qui permet une étude du regard et ainsi observer des phénomènes visuels des sujets qui pratiquent l'auto-évaluation des difficultés et des besoins d'aide. Objectifs Cette étude pilote et exploratrice a pour but de mesurer l'exploration visuelle des cartes lors de l'évaluation d'ELADEB au travers des trois variables suivantes d'oculométrie récoltées au travers de l'outil de mesure de l'Eye tracker : (1) la variation de la taille pupillaire (pixels), (2) le rapport des nombres de points de fixation/temps, et (3) de la durée totale du temps de traitement de l'information de la carte (ms). Hypothèses. Le traitement visuel des cartes sélectionnées comme problèmes versus non problèmes sera différent chez tout le monde d'une part au niveau de : (1) la variation pupillaire, (2) du nombre de points de fixation de l'oeil/temps ainsi que (3) de la durée totale du traitement de l'information de la carte en fonction de notre compréhension. On peut poser l'hypothèse que l'émotion sera différente selon que la personne rencontre un problème ou non et ainsi, on peut s'attendre à ce que les cartes sélectionnées comme problèmes soient associées à une réaction émotionnelle plus importante que les autres, ce qui nous permettrait d'observer une corrélation entre le balayage visuel et ELADEB. Méthodes Cette étude exploratoire porte sur un échantillon de 10 sujets passant un test d'évaluation des difficultés et des besoins ELADEB, couplé à un appareil d'Eye tracking. Les critères d'inclusion des sujets sont : (1) hommes entre 30 et 40 ans, (2) dépourvu de symptômes psychotiques ou de psychoses. Les critères d'exclusion des sujets sont : (1) consommation d'alcool ou de drogues, (2) troubles psycho-organiques, (3) retard mental, (4) difficulté de compréhension de la langue française, (5) état de décompensation aiguë empêchant la réalisation de l'étude. Trois instruments sont utilisés comme supports d'analyses pour cette étude : (1) ELADEB, (2) l'Eye tracking et (3) le Gaze tracker. Le dispositif (2) d'Eye tracking se présente sous forme de lunettes particulières à porter par le sujet et qui permet de suivre les mouvements de l'oeil. L'une des caméras suit et enregistre les mouvements oculaires du sujet et permet de « voir en temps réel ce que voit le sujet et où il regarde » tandis que la seconde caméra scrute et analyse sa pupille. Les données sont ensuite répertoriées par le logiciel de mesures (3) « Gaze tracker », qui les analyse avec, à la fois une évaluation quantitative des points de fixation2 en se basant sur leurs nombres et leurs durées, ainsi qu'une mesure de la variation pupillaire fournie en pixels par le logiciel. Résultat(s) escompté(s) Il s'agit d'une étude exploratoire dans laquelle on espère retrouver les variations moyennes intra-sujets selon les hypothèses carte problème versus non problèmes. On peut s'attendre à trouver une retraduction des mesures de l'échelle ELADEB au travers de l'Eye tracking, ce qui concorderait avec les hypothèses énoncées et ajouterait encore d'avantage de validation à ELADEB au travers d'un outil pointu de mesure physiologique.

INTRODUCTION: Early surgical management is often advocated for fractures of the tooth-bearing portion of the mandible. A 6-hour delay has been mentioned for the fixation of these fractures. Our aim was to bring this paradigm into question. METHODS: All patients referred to our department from September 2012 to May 2014 for fractures of the tooth-bearing portion of the mandible were retrospectively included. For each patient, age, gender, aetiology of the fracture, and characteristics of the fractures were recorded. Tobacco and/or alcohol addictions, diabetes and mandibular dental condition were taken into account. We also noticed the preoperative delay and the occurrence of complications such as: haematoma, infection, wound dehiscence, osteosynthesis failure and pseudarthrosis. RESULTS: Among the 47 patients referred, 36 were treated with a delay of more than 6hours (76.6%). In 88.8% of the cases, the reason for this delay was unavoidable. The mean delay time from trauma to surgery was 52hours (range: 7-312). Forty-nine percent of the patients had comorbidities. Complications occurred in 6 patients leading to an overall complication rate of 16.67%. A statistically significant higher complication rate was observed among smokers (P=0.006). No statistical relationship was found between the delay and the occurrence of complications (P=0.994). This study suggests that fractures of the tooth-bearing portion of the mandible should no longer be considered as an emergency that must be treated within a 6-hour delay.

Covert spatial attention produces biases in perceptual and neural responses in the absence of overt orienting movements. The neural mechanism that gives rise to these effects is poorly understood. Here we report the relation between fixational eye movements, namely eye vergence, and covert attention. Visual stimuli modulate the angle of eye vergence as a function of their ability to capture attention. This illustrates the relation between eye vergence and bottom-up attention. In visual and auditory cue/no-cue paradigms, the angle of vergence is greater in the cue condition than in the no-cue condition. This shows a top-down attention component. In conclusion, observations reveal a close link between covert attention and modulation in eye vergence during eye fixation. Our study suggests a basis for the use of eye vergence as a tool for measuring attention and may provide new insights into attention and perceptual disorders.

Background: Neonatal brain injuries are the main cause of visual deficit produced by damage to posterior visual pathways.While there are several studies of visual function in low-risk preterm infants or older children with brain injuries, research in children of early age is lacking. Aim: To assess several aspects of visual function in preterm infants with brain injuries and to compare them with another group of low-risk preterm infants of the same age. Study design and subjects: Forty-eight preterm infants with brain injuries and 56 low-risk preterm infants. Outcome measures: The ML Leonhardt Battery of Optotypes was used to assess visual functions. This test was previously validated at a post-menstrual age of 40 weeks in newborns and at 30-plus weeks in preterm infants. Results: The group of preterminfants with brain lesions showed a delayed pattern of visual functions in alertness, fixation, visual attention and tracking behavior compared to infants in the healthy preterm group. The differences between both groups, in the visual behaviors analyzed were around 30%. These visual functions could be identified from the first weeks of life. Conclusion: Our results confirm the importance of using a straightforward screening test with preterminfants in order to assess altered visual function, especially in infants with brain injuries. The findings also highlight the need to provide visual stimulation very early on in life.

Plusieurs études populationnelles ont montré l'existence d'une association entre des taux sanguins élevés de transferrine et le syndrome métabolique (SM). Bien que cette association soit bien établie, restent encore à être décrites les associations entre le SM et les autres marqueurs sanguins du métabolisme du fer, tels que le fer, la transferrine (Tsf), la capacité totale de fixation de la transferrine (CTF) ou la saturation de la transferrine (SaTsf) sanguins. Le but de notre étude a été d'identifier les associations entre les différents marqueurs du métabolisme du fer (fer, ferritine, Tsf, CTF et SaTsf) et le SM. Les données de l'étude CoLaus, récoltées entre 2003 et 2006, ont été utilisées. Le SM était défini selon les critères du National Cholesterol Education Program Adult Panel III. L'analyse statistique a été faite en stratifiant selon le genre ainsi que le status ménopausal chez les femmes. Des 6733 participants, 1235 (18%) ont été exclus de fait d'absence de données concernant les variables qui nous intéressaient, ou chez qui nous avons soupçonné une possible hémochromatose non diagnostiquée (SaTsf> 50%). Des 5498 participants restant (âge moyen ± écart-type: 53 ± 11 ans), 2596 étaient des hommes, 1285 des femmes pré- et 1617 des femmes postménopausées. La prévalence du SM était de 29,4% chez les hommes, 8,3% et 25,5% chez les femmes pré- et postménopausées, respectivement. Dans les trois groupes, la prévalence du SM était la plus haute dans les quartiles les plus élevés de ferritine, Tsf et CTF, ainsi que dans le quartile le plus bas de SaTsf. Après ajustement sur l'âge, l'indice de masse corporelle, la protéine C réactive, la consommation de tabac et/ou d'alcool, la prise de suppléments en fer et les marqueurs hépatiques, l'appartenance au quartile le plus élevé de ferritine, Tsf ou CTF était associée à un risque plus important de SM chez les hommes et les femmes postménopausées : Odds ratio (OR) et [intervalle de confiance à 95%] pour la ferritine 1.44 [1.07-1.94] et 1.47 [0.99-2.17]; pour la Tsf et la CTF, OR=1.43 [1.06-1.91] et 2.13 [1.44-3.15] pour les hommes et les femmes postménopausées, respectivement. Au contraire, l'appartenance au quartile le plus élevé de la SaTsf était associé à un risque moins important de SM: OR=0.77 [0.57-1.05] et 0.59 [0.39-0.90] pour les hommes et les femmes postménopausées, respectivement. Il n'y avait aucune association entre les marqueurs sanguins du métabolisme du fer et le SM chez les femmes préménopausées, ni entre le fer sanguin et le SM chez les trois groupes. En conclusion, la majorité des marqueurs sanguins du métabolisme du fer, mais pas le fer lui-même, sont associés de manière indépendante au SM chez les hommes et les femmes postménopausées. -- Context: Excessive iron storage has been associated with metabolic syndrome (MS). Objective: To assess the association between markers of iron metabolism and MS in a healthy population. Design: Cross-sectional study conducted between 2003 and 2006. Setting: Population-based study in Lausanne, Switzerland. Patients: 5,498 participants aged 35-75 years, stratified by sex and menopausal status. Participants with transferrin saturation (TSAT) >50% were excluded. Intervention: None. Main Outcome Measures: serum iron, ferritin, transferrin, total iron binding capacity (TIBC) and TSAT. MS was defined according to ATP-III criteria. Results: Prevalence of MS was 29.4% in men, 8.3% in premenopausal and 25.5% in postmenopausal women. On bivariate analysis, the highest prevalence of MS occurred in the highest quartiles of serum ferritin, transferrin and TIBC, and in the lowest quartile of TSAT. After multivariate adjustment for age, body mass index, C-reactive protein, smoking, alcohol, liver markers and iron supplementation, men and postmenopausal women in the highest quartile of serum ferritin, transferrin and TIBC had a higher risk of presenting with MS: for ferritin, Odds ratio and [95% CI]=1.44 [1.07-1.94] for men and 1.47 [0.99-2.17] for postmenopausal women; for transferrin and TIBC, OR=1.43 [1.06-1.91] and 2.13 [1.44-3.15], Participants in the highest quartile of TSAT had a lower risk of MS: OR=0.77 [0.57-1.05] for men and 0.59 [0.39-0.90] for postmenopausal women. No association was found between iron and MS and between markers of iron metabolism and MS in premenopausal women. Conclusion: Ferritin, transferrin, TIBC are positively and TSAT is negatively associated with MS in men and postmenopausal women.

In recent years correlative microscopy, combining the power and advantages of different imaging system, e.g., light, electrons, X-ray, NMR, etc., has become an important tool for biomedical research. Among all the possible combinations of techniques, light and electron microscopy, have made an especially big step forward and are being implemented in more and more research labs. Electron microscopy profits from the high spatial resolution, the direct recognition of the cellular ultrastructure and identification of the organelles. It, however, has two severe limitations: the restricted field of view and the fact that no live imaging can be done. On the other hand light microscopy has the advantage of live imaging, following a fluorescently tagged molecule in real time and at lower magnifications the large field of view facilitates the identification and location of sparse individual cells in a large context, e.g., tissue. The combination of these two imaging techniques appears to be a valuable approach to dissect biological events at a submicrometer level. Light microscopy can be used to follow a labelled protein of interest, or a visible organelle such as mitochondria, in time, then the sample is fixed and the exactly same region is investigated by electron microscopy. The time resolution is dependent on the speed of penetration and fixation when chemical fixatives are used and on the reaction time of the operator for cryo-fixation. Light microscopy can also be used to identify cells of interest, e.g., a special cell type in tissue or cells that have been modified by either transfections or RNAi, in a large population of non-modified cells. A further application is to find fluorescence labels in cells on a large section to reduce searching time in the electron microscope. Multiple fluorescence labelling of a series of sections can be correlated with the ultrastructure of the individual sections to get 3D information of the distribution of the marked proteins: array tomography. More and more efforts are put in either converting a fluorescence label into an electron dense product or preserving the fluorescence throughout preparation for the electron microscopy. Here, we will review successful protocols and where possible try to extract common features to better understand the importance of the individual steps in the preparation. Further the new instruments and software, intended to ease correlative light and electron microscopy, are discussed. Last but not least we will detail the approach we have chosen for correlative microscopy.

The aim of this study was to prospectively assess the prevalence of orthoptic anomalies following conservative management of pure blowout orbital fractures and to evaluate their clinical relevance. Clinical and radiologic data of patients with unilateral conservatively managed pure blowout orbital fractures with a minimum follow-up of 6 months were reviewed. Eligible patients were contacted and invited to undergo an extended ophthalmologic examination as follows: distance and near visual acuities, Hertel exophthalmometry, corneal light reflex (Hirschberg test), ductions and versions in the 6 cardinal fields of gaze, eye deviation with prisms and alternate cover test in all of the 9-gaze directions with Maddox rod, degrees of incyclo/excyclotorsion with right and left eye fixation, horizontal and vertical deviation with Hess-Weiss coordimetry, degree of horizontal/vertical and incyclo/excyclotorsion deviation with Harms wall deviometry, and vertical deviation with Bielschowsky head-tilt test. Of the 69 patients contacted, 49 declined to participate given that they were asymptomatic. Twenty patients agreed to undergo the examination. One patient complained of minimal double vision limited to the extreme downgaze. Four patients had asymptomatic ocular motility disturbances limited to the extreme gaze. Seven patients had asymptomatic horizontal heterophoria. These disturbances did not interfere with daily or professional activities in any of the patients. The current study demonstrated that conservative management of pure orbital blowout fractures can result in orthoptic anomalies. These sequelae were restricted to a very limited portion of the binocular field of the vision and were not found to be clinically relevant.

Ce diplôme présente les marchés prédictifs, qui sont un moyen d'agréger l'information en utilisant les processus à la base des marchés financiers et en proposant des contrats à termes représentant la probabilité de survenance d'une proposition. Des publications dans le domaine de la finance, du management ou de l'informatique font référence à l'utilisation des marchés prédictifs et démontrent les bons résultats obtenus par cette méthode. Nous nous sommes demandé s'il était possible de concevoir et utiliser un tel marché dans un milieu académique, pour prédire l'émergence de nouvelles théories et applications dans le domaine des communications mobiles. Pour supporter notre réflexion, nous avons mis en place un prototype qui devrait pouvoir nous permettre de tester le concept, d'étudier les fonctions de calcul de prix et les mécanismes d'agrégation de l'information. Pour débuter, nous présentons l'état de l'art en nous appuyant sur la littérature parue depuis 1988 lorsque l'University of Iowa a lancé sa plate-forme Iowa Electronic Market (IEM) dans le cadre de l'élection présidentielle Bush-Dukakis. Depuis, plusieurs chercheurs se sont penchés sur les marchés prédictifs comme instrument de prévision dans de nombreux domaines: politique, sport, cinéma, marketing, ... Nous allons comparer les marchés prédictifs à d'autres outils de prévision puis nous présenterons en détail les marchés prédictifs ainsi qu'un essai d'en formaliser les spécifications. Nous illustrerons alors ceci par des exemples tirés des principales plates-formes en activité pour le moment. S'ensuit une discussion sur l'orientation ludique des plates-formes actuelles et une présentation des utilisations que l'on pourrait en faire et qui apporteraient un réel bénéfice. Nous nous penchons alors sur l'analyse des besoins tant de l'expérimentateur que des utilisateurs. Cherchant des raisons à l'absence totale de telles plates-formes dans le cadre de la recherche, nous avons mené une série d'entretiens avec des professeurs de l'EPFL engagés dans la direction du projet MICS. Leurs réticences et craintes ont été prises en compte pour la définition des spécifications du prototype. Nous utilisons la méthode de (Spann and Skiera 2003) pour définir les spécifications de la plate-forme. Ces spécifications comprennent le choix du but prévisionnel, qui dans notre cas découle directement de notre proposition de recherche, des mécanismes financiers à la base des transactions, ainsi que des moyens incitatifs mis en place pour amener les utilisateurs à utiliser la plate-forme et surtout, pour amener les plus éclairés d'entre eux à transmettre leurs informations au marché. Ces spécifications débouchent sur la présentation de quatre cas d'utilisation. Notre objectif étant de développer un prototype pour y tester nos hypothèses, nous faisons un tour d'horizon des plates-formes à disposition et présentons l'architecture de ce dernier, basé sur les développements à la base d'USIFEX, qui n'est plus en activité depuis quatre ans. Le prototype utilise la souplesse de la programmation objet en se basant sur le langage Python dans sa version 2.3. Nous nous attardons ensuite sur la présentation de l'interface du prototype et des interactions avec les utilisateurs. Il faudra par la suite s'attacher à refondre l'interface pour le faire évoluer vers plus de convivialité, et nous permettre de guider l'utilisateur lors de ses sessions sur la plate-forme. Les parties théoriques et logicielles sous toit il nous reste encore à décrire les contrats et la manière de les formuler. A nouveau nous nous penchons sur les marchés actuels pour y tirer des enseignements qui pourraient nous être utiles. N'ayant malheureusement pas fait l'objet de publications, la définition des contrats est intimement liée avec les auteurs qui les rédigent. Nous tentons de tirer quelques enseignements provenant des contrats actifs sur les marchés des matières premières pour proposer un cadre nous permettant de formaliser la rédaction d'un contrat. Ne s'agissant que d'une proposition, il faudra encore tester et compléter ce cadre pour pouvoir s'assurer que la manière de rédiger les contrats n'influence pas sur la pertinence des prédictions. Finalement, nous esquissons les travaux qu'il reste à accomplir avant de pouvoir mener les premiers tests réels de la plate-forme. De la formulation des contrats à l'étude des mécanismes combinatoires d'agrégation de l'information en passant par les fonctions de fixation du prix. Nous revenons sur les travaux effectués dans le cadre de ce diplôme et présentons quelques idées de recherche futures.

AbstractThe present essay is aimed at getting the radiologist familiar with the basic histological skin structure, allowing for a better correlation with sonographic findings. A high-frequency (22 MHz) ultrasonography apparatus was utilized in the present study. The histological analysis was performed after the skin specimens fixation with formalin, inclusion in paraffin blocks and subsequent staining with hematoxylin-eosin. The authors present a literature review showing the relationship between sonographic and histological findings in normal cutaneous tissue, and discuss the technique for a better performance of the sonographic scan. High-frequency ultrasonography is an excellent tool for the diagnosis of different skin conditions. However, as this method is operator-dependent, it is crucial to understand the normal skin structure as well as the correlation between histological and sonographic findings.
Concerted changes in N and C primary metabolism in alfalfa (Medicago sativa) under water restriction

Although the mechanisms of nodule N2 fixation in legumes are now well documented, some uncertainty remains on the metabolic consequences of water deficit. In most cases, little consideration is given to other organs and, therefore, the coordinated changes in metabolism in leaves, roots, and nodules are not well known. Here, the effect of water restriction on exclusively N2-fixing alfalfa (Medicago sativa L.) plants was investigated, and proteomic, metabolomic, and physiological analyses were carried out. It is shown that the inhibition of nitrogenase activity caused by water restriction was accompanied by concerted alterations in metabolic pathways in nodules, leaves, and roots. The data suggest that nodule metabolism and metabolic exchange between plant organs nearly reached homeostasis in asparagine synthesis and partitioning, as well as the N demand from leaves. Typically, there was (i) a stimulation of the anaplerotic pathway to sustain the provision of C skeletons for amino acid (e.g. glutamate and proline) synthesis; (ii) re-allocation of glycolytic products to alanine and serine/glycine; and (iii) subtle changes in redox metabolites suggesting the implication of a slight oxidative stress. Furthermore, water restriction caused little change in both photosynthetic efficiency and respiratory cost of N2 fixation by nodules. In other words, the results suggest that under water stress, nodule metabolism follows a compromise between physiological imperatives (N demand, oxidative stress) and the lower input to sustain catabolism.
