INTRODUCTION: Morbid obesity has grown enormously in recent decades, representing a serious public health problem. It is characterized by the accumulation of body fat and the presence of diseases associated with it, which affects the physical, psychological and social level. It has been considered bariatric ciguría as the most effective treatment for weight loss, getting the welfare of the obese person in the above-described drawings. OBJECTIVE: To evaluate the impact on the quality of life of obese people before and after bariatric surgery be tapped using the technique of laparoscopic gastrectomy (GVL) in a follow short, medium and long term. METHOD: The study population are all people with morbid obesity and are candidates for surgery, presenting at Torrecárdenas Bariatric Surgery Hospital (Almería). The design is a descriptive, longitudinal study, prospective. RESULTS: After evaluation of the obese patients a year, two years and five years after surgery, there has been decrease in weight, therefore decrease in BMI and the degree of obesity, improvement or disappearance of comorbidities and increased CV variables. DISCUSSION: The GVL gets reduce excess weight and therefore BMI and the degree of obesity, as well as the number of obesity-associated diseases, thus increasing the CV.

BACKGROUND The use of remote follow-up (RF) of people with pacemakers (PM) is limited in comparison to the hospital modality (HS), being still poor the scientific evidence that shows their comparative effectiveness. The aim of this study was to compare the quality of life in individuals with different modalities of follow-up. METHODS Controlled, not randomized nor masked clinical trial, with data collection at pre and post-implantation of pacemakers during the 6 months follow-up. All patients over 18 years-old who were implanted a PM during the study period were selected (n = 83), and they were assigned to RF (n = 30) or HF (n = 53) groups according to their personal characteristics and patient's preferences. Baseline characteristics and number of visits to the hospital were analysed, the EuroQol-5D (EQ5D) questionnaire was administered to evaluate the health-related quality of life, and Duke Activity Status Index (DASI) to assess the functional capacity. RESULTS There were no significant differences between both groups in relation to the baseline analysis, EQ5D (RF:0.7299; HF:0.6769) and DASI (RF:21.41; HF:19.99). At 6 months the quality of life was improved in both groups (EQ5D RF:0.8613; HF:0.8175; p = 0,439) still without significant differences between them. DASI score was similar to baseline (20.51 vs 21.80). RF group performed less transmissions/visits per patient (1.57) than hospital group (1.96; relative reduction 31%; p = 0.015). CONCLUSIONS Remote follow-up of people with pacemakers might be considered as an equivalent option to the hospital follow-up in relation to the quality of life and it reduces the number of hospital visits.

OBJECTIVE We aimed to analyze health-related quality of life (HRQOL) in adults with newly diagnosed respiratory allergy according to the sensitization profile for relevant aeroallergens in their usual area of residence. METHODS We performed a cross-sectional, epidemiological, observational, descriptive, multicenter study in allergy clinics in Spain. The sample comprised adults diagnosed with rhinitis, asthma, or both caused by significant allergens in their residential area (olive and/or grass pollen or house dust mite). Allergic rhinitis was classified according to the Allergic Rhinitis and its Impact on Asthma guidelines; asthma was classified according to the Guía Españiola para el Manejo del Asma (Spanish Guideline on the Management of Asthma). HRQOL was studied according to the ESPRINT-15 questionnaire and Mini Asthma Quality of Life Questionnaire. Control of asthma was measured using the Asthma Control Questionnaire 5. RESULTS We studied 1437 patients. Rhinitis was the most common respiratory disease. The HRQOL of rhinitis patients was lower in those sensitized to olive pollen only and in those with combined sensitization to olive and grass pollens. HRQOL associated with rhinitis was worse in patients diagnosed with both rhinitis and asthma than in patients diagnosed with rhinitis only. Asthma patients sensitized to olive pollen or olive and grass pollens had worse HRQOL. CONCLUSIONS In our study population, the HRQOL of patients with respiratory allergies varied with the allergen responsible for symptoms. In patients with rhinitis, the presence of asthma significantly worsened rhinitis-associated HRQOL.

The development of the economic evaluation of health care interventions has become a support tool in making decisions on pricing and reimbursement of new health interventions. The increasingly extensive application of these techniques has led to the identification of particular situations in which, for various reasons, it may be reasonable to take into account special considerations when applying the general principles of economic evaluation. In this article, which closes a series of three, we will discuss, using the Metaplan technique, about the economic evaluation of health interventions in special situations such as rare diseases and end of life treatments, as well as consideration of externalities in assessments, finally pointing out some research areas to solve the main problems identified in these fields.

Objectives: To measure the health-related quality of life (HRQoL) of multiple sclerosis (MS) patients and their caregivers, and to assess which factors can best describe HRQoL. Methods: A cross-sectional multicenter study of nine hospitals enrolled MS patients and their caregivers who attended outpatient clinics consecutively. The instruments used were the SF-36 for patients and the SF-12 and GHQ-12 for caregivers. Classification and regression tree analysis was used to analyze the explanatory factors of HRQoL. Results: A total of 705 patients (mean age 40.4 years, median Expanded Disability Status Scale 2.5, 77.8% with relapsing-remitting MS) and 551 caregivers (mean age 45.4 years) participated in the study. MS patients had significantly lower HRQoL than in the general population (physical SF-36: 39.9; 95% confidence interval [CI]: 39.1–40.6; mental SF-36: 44.4; 95% CI: 43.5–45.3). Caregivers also presented lower HRQoL than general population, especially in its mental domain (mental SF-12: 46.4; 95% CI: 45.5–47.3). Moreover, according to GHQ-12, 27% of caregivers presented probable psychological distress. Disability and co-morbidity in patients, and co-morbidity and employment status in caregivers, were the most important explanatory factors of their HRQoL. Conclusions: Not only the HRQoL of patients with MS, but also that of their caregivers, is indeed notably affected. Caregivers’ HRQoL is close to population of chronic illness even that the patients sample has a mild clinical severity and that caregiving role is a usual task in the study context

Trata-se de um estudo qualitativo realizado a partir do conceito de qualidade de vida adotado por MEEBERG (1993) relacionado com a opinião de um grupo de professoras de enfermagem. Observou-se que este conceito está presente na fala das entrevistadas de forma bem clara, especialmente nos atributos relacionados a satisfação com a própria vida em geral e com o estado aceitável de saúde física, mental, social e emocional.

Estudo descritivo e transversal, desenvolvido com o objetivo de avaliar a Qualidade de Vida (QV) de pacientes com câncer, submetidos à quimioterapia. Para a coleta de dados, utilizou-se o instrumento European Organization for Research and Treatment of Cancer - Quality of Life Questionnaire Core-30 (EORTC QLQ-C30). A amostra constitui-se de 30 pacientes que assinaram o consentimento informado. Os dados foram analisados pelo software SPSS. O QLQ-C30 mostrou que a pontuação média das funções física, cognitiva e social, e desempenho de papel, variou de 71,26 a 75,12, demonstrando um nível satisfatório. Na função emocional, a média foi baixa, de 55,46. Nas escalas de sintomas, houve o predomínio da insônia com uma média de 34,44, seguida de dor (23,33) e fadiga (22,31). A QV foi satisfatória em todos os domínios, exceto a função emocional, que foi baixa, demonstrando que os efeitos colaterais da quimioterapia influenciam negativamente a QV dos pacientes.

O artigo visa a apresentar o processo de construção e validação do Índice de Qualidade de Vida de Ferrans & Powers - Versão Feridas (IQVFP-VF) para emprego em pessoas com feridas agudas e crônicas de diferentes etiologias. O estudo metodológico desenvolveu-se por meio de procedimentos teóricos, empíricos e analíticos. Os resultados indicaram que o instrumento possui consistência interna e estabilidade satisfatórias; confirmaram a validade de conteúdo, concorrente (com o item sua satisfação) e convergente (com o WHOQOL-breve), bem como sua capacidade de discriminar os indivíduos conforme o número e duração das feridas, intensidade de dor e idade. A análise fatorial confirmatória mostrou que o instrumento manteve-se razoavelmente ajustado ao modelo original. Em conclusão, pode-se considerar que o IQVFP-VF é válido e tem confiabilidade atestada nos aspectos mais importantes da Qualidade de Vida para a população com feridas - geral, saúde e aspectos psicológicos e espirituais.

Este estudo teve como objetivos avaliar a qualidade de vida relacionada à saúde (QVRS) e o bem-estar espiritual (BEE) de pacientes com doença pulmonar obstrutiva crônica (DPOC) e analisar as relações entre QVRS e BEE. As versões em português do Seattle Obstructive Lung Disease Questionnaire (SOLDQ) e do Spiritual Well-Being Scale (SWBS) foram aplicadas por entrevista a 70 pacientes com DPOC em tratamento ambulatorial. Os domínios do SOLDQ com menor e maior escores foram: Função Física (37,29±17,19) e Satisfação com o Tratamento (68,75±28,05). O escore médio de 94,87±13,56 indica um nível moderado de BEE. O escore total do SWBS e o da subescala Bem-Estar Religioso correlacionaram-se positivamente com o domínio Satisfação com o Tratamento (p=0,007 e p=0,002, respectivamente). Correlação negativa foi encontrada entre Bem-Estar Religioso e Função Física (p=0,05). Pacientes com maior bem-estar religioso estavam mais satisfeitos com o tratamento e tinham pior funcionamento físico.
Correlação entre qualidade de vida e morbidade do cuidador de idoso com acidente vascular encefálico

Este estudo objetivou descrever as características sociodemográficas e a qualidade de vida dos cuidadores de idosos com histórico de acidente vascular cerebral e correlacionar o número de morbidades com os escores de qualidade de vida do cuidador. Inquérito domiciliar transversal que entrevistou cuidadores de idosos utilizando-se de instrumentos: semi-estruturado; World Health Organization Quality of Life-abreviado e Brazilian Multidimensional Functional Assessment Questionnaire. Foi realizada análise descritiva e correlação de Pearson (p<0,05). Prevaleceu o sexo feminino (93,5%), idade média de 55,4±14,17 anos, casados ou moram com companheiro (58,7%), com 4| - 8 anos de estudo (28,3%) e renda de 1 salário mínimo (34,8%). O maior escore de qualidade de vida foi no domínio relações sociais (67,57) e, menor no meio ambiente (54,82). O maior número de morbidades do cuidador correlacionou-se com menores escores em todos os domínios de qualidade de vida.

BACKGROUND: Pediatric rheumatic diseases have a significant impact on children's quality of life and family functioning. Disease control and management of the symptoms are important to minimize disability and pain. Specialist clinical nurses play a key role in supporting medical teams, recognizing poor disease control and the need for treatment changes, providing a resource to patients on treatment options and access to additional support and advice, and identifying best practices to achieve optimal outcomes for patients and their families. This highlights the importance of investigating follow-up telenursing (TN) consultations with experienced, specialist clinical nurses in rheumatology to provide this support to children and their families. METHODS/DESIGN: This randomized crossover, experimental longitudinal study will compare the effects of standard care against a novel telenursing consultation on children's and family outcomes. It will examine children below 16 years old, recently diagnosed with inflammatory rheumatic diseases, who attend the pediatric rheumatology outpatient clinic of a tertiary referral hospital in western Switzerland, and one of their parents. The telenursing consultation, at least once a month, by a qualified, experienced, specialist nurse in pediatric rheumatology will consist of providing affective support, health information, and aid to decision-making. Cox's Interaction Model of Client Health Behavior serves as the theoretical framework for this study. The primary outcome measure is satisfaction and this will be assessed using mixed methods (quantitative and qualitative data). Secondary outcome measures include disease activity, quality of life, adherence to treatment, use of the telenursing service, and cost. We plan to enroll 56 children. DISCUSSION: The telenursing consultation is designed to support parents and children/adolescents during the course of the disease with regular follow-up. This project is novel because it is based on a theoretical standardized intervention, yet it allows for individualized care. We expect this trial to confirm the importance of support by a clinical specialist nurse in improving outcomes for children and adolescents with inflammatory rheumatisms. Estudo descritivo, transversal desenvolvido com objetivo de associar aspectos socio demográficos e clínicos aos domínios de qualidade de vida relacionada à saúde (QVRS), para avaliar pacientes onco-hematológicos submetidos à quimioterapia. Na coleta de dados utilizou-se um instrumento sociodemográfico e clínico e o European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC) QLQ-C-30. A amostra foi constituída de 32 pacientes, sendo oito (25%) com diagnóstico de linfoma de Hodgkin, nove (28,12%) linfoma não Hodgkin e 15 (46,87%) leucemia. Os dados foram analisados pelo software Statistical Package for Social Science (SPSS). O QLQ-C-30 mostrou média das funções física, cognitiva, emocional, social e desempenho de papel de 54,81 a 41,18, demonstrando um nível pouco satisfatório. Nas escalas de sintomas, houve predomínio de fadiga média 64,57 seguida de insônia (56,90) e perda de apetite (50,71). Esses sintomas interferiram nas funções físicas, emocionais e cognitivas demonstrando que efeitos colaterais do tratamento influenciam negativamente na QVRS dos pacientes.

Este estudo teve por objetivos traduzir e adaptar o Newcastle Satisfaction with Nursing Scales para a cultura brasileira, bem como verificar seu uso prático. O instrumento contém duas escalas e tem como objetivo avaliar as experiências vivenciadas pelo paciente e a satisfação com o cuidado de enfermagem. Para o procedimento metodológico de adaptação cultural foram seguidas as etapas de tradução, síntese, retro-tradução, avaliação pelo comitê de especialistas e pré-teste. O processo de tradução e adaptação cultural foi considerado apropriado. A avaliação pelo comitê de especialistas resultou em modificações gramaticais simples para a maioria dos itens e, no pré-teste, participaram 40 sujeitos. A versão brasileira do Newcastle Satisfaction with Nursing Scales demonstrou adequada validade de conteúdo e facilidade de compreensão pelos sujeitos. Contudo, este é um estudo que antecede o processo de avaliação das propriedades psicométricas do instrumento, cujos resultados serão apresentados em publicação posterior.

Inquérito domiciliário, transversal e analítico que objetivou descrever as características sociodemográficas, de saúde e a qualidade de vida de homens idosos e verificar os fatores socioeconômicos e de saúde associados à qualidade de vida. Participaram 804 homens idosos. Os dados foram coletados pelos instrumentos: Older Americans Resources and Services(OARS), World Health Organization Quality of Life - Bref (WHOQOL-BREF) e Health Organization Quality of Life Assessment for Older Adults(WHOQOL-OLD). Foram realizados análise descritiva, teste t-Student, correlação de Pearson e regressão linear múltipla (p <0,05). Predominaram idosos com 60├ 70 anos, casados, 4├ 8 anos de estudo e renda de um salário mínimo. Os menores escores de qualidade de vida foram no domínio físico e na faceta autonomia e estiveram associados a ausência de companheira e de escolaridade, baixa renda, maior número de morbidades e incapacidade funcional. A incapacidade funcional foi o que mais influenciou a qualidade de vida, excetuando-se o domínio físico e a faceta intimidade.

By taking care of cancer patients in their process of end of life, nursing experience situations of suffering before the anguish of others. This study aimed to understand the meaning and significance attributed by the nurses from the palliative care cancer hospital. This is a phenomenological research, grounded in Heidegger’s thinking, performed with 13 nurses, who work at Oncology hospitalward, through semi-structured interviews, which were analyzed according to the steps recommended by Josgrilberg. From understanding the statementsof the subjects, two ontological themesemerged: Feeling satisfaction and love in the care offered and Feeling anger and inabilitytowards terminally ill patients.We inferred that working in Oncology Ward is something rewarding for these professionals, but it entails physical and mental suffering, from feeling helpless before the death-dying process. Thus, we showedthat nursing professionals need to be recognized as human beings and as such, also deserving of care.
