OBJECTIVES: To assess the incremental cost-effectiveness ratio (ICER) and incremental cost-utility ratio (ICUR) of risedronate compared to no intervention in postmenopausal osteoporotic women in a Swiss perspective. METHODS: A previously validated Markov model was populated with epidemiological and cost data specific to Switzerland and published utility values, and run on a population of 1,000 women of 70 years with established osteoporosis and previous vertebral fracture, treated over 5 years with risedronate 35 mg weekly or no intervention (base case), and five cohorts (according to age at therapy start) with eight risk factor distributions and three lengths of residual effects. RESULTS: In the base case population, the ICER of averting a hip fracture and the ICUR per quality-adjusted life year gained were both dominant. In the presence of a previous vertebral fracture, the ICUR was below euro45,000 (pound30,000) in all the scenarios. For all osteoporotic women>or=70 years of age with at least one risk factor, the ICUR was below euro45,000 or the intervention may even be cost saving. Age at the start of therapy and the fracture risk profile had a significant impact on results. CONCLUSION: Assuming a 2-year residual effect, that ICUR of risedronate in women with postmenopausal osteoporosis is below accepted thresholds from the age of 65 and even cost saving above the age of 70 with at least one risk factor.

Objectifs: Exposer une nouvelle technique permettant le retrait de ciment extra-vertebral par fuite accidentelle lors du retrait du matériel. Matériels et méthodes: Nous injectons le ciment lorsque sa densité est celle d'une pâte dentifrice pour éviter les passages vasculaires. Lorsque la vertèbre est parfaitement remplie,nous patientons quelques minutes pour éviter la fuite de ciment le long du trajet (environ 4 a 6 cc en moyenne par vertèbre sont injectés). Malgré ces précautions,une fuite de ciment peut survenir lors du retrait du trocart. Cette complication est rare lorsque l'on prend les précautions nécessaires. Cependant, si la pressionintra-vertébrale est importante, le ciment peut être aspiré en dehors de la vertèbre. Résultats: L'aiguille du trocart est retirée pour être remplacée par une pince d'endoscopie 13 gauges. Sous guidage scopique, l'extraction se fait sous contrôle continu ; lapince retirant la fuite de ciment. Conclusion: La connaissance de cette intervention peut être très utile pour les équipes de radiologie interventionnelle , d'orthopédie et de neurochirurgie réalisant desvertébroplasties, pouvant être confrontées à ce type de problèmes.

O P tem baixa mobilidade no solo resultando em altas concentrações nas regiões adjacentes aos grânulos de fertilizantes fosfatados. Isso pode interferir na disponibilidade de P e na composição química do solo nesses locais. O objetivo deste estudo foi avaliar a composição química do solo e a mobilidade de P nas adjacências de regiões fertilizadas com fosfatos de amônio ou de cálcio, em solos ácidos. Este trabalho foi realizado em 2009, com amostras de dois solos catarinenses. Os tratamentos consistiram de combinações de superfosfato triplo (SFT) ou de fosfato diamônico (DAP) com KCl, além de um controle somente com KCl. As unidades experimentais foram constituídas por colunas de PVC com 10,0 cm de diâmetro e 12,0 cm de comprimento, preenchidas com 1,60 kg de solo (base úmida). Foram aplicadas quantidades equivalentes a 200 kg ha-1 de P2O5 e de K2O, considerando a área de contato superficial de solo de 10(4) m². Tentando simular a aplicação localizada de fertilizantes, essas quantidades foram estipuladas para contatarem com 357 m² de solo, tendo sido aplicadas 10,47; 9,56; e 7,33 g por coluna de SFT, DAP e KCl respectivamente, numa área de contato de 0,00785 m². Quarenta e cinco dias após a aplicação dos fertilizantes, as colunas foram desmontadas e o solo foi analisado a cada centímetro distante do local de aplicação dos grânulos. O P foi quantificado pelos métodos Mehlich-1 e da resina trocadora de ânions (RTA), numa sequencia de nove extrações cumulativas. O DAP aumentou o pH e diminuiu o Al nas adjacências dos grânulos, porém o SFT aumentou Ca e Mg. A quantidade cumulativa extraída de P variou com o tipo de adubo fosfatado, a distância dos grânulos, o tipo de solo e o método de extração. Na camada distante até 1,0 cm dos adubos, o SFT proporcionou os maiores valores de P, que chegaram a atingir 10.276 mg dm-3; nas duas camadas seguintes (1,0-2,0 e 2,0-3,0 cm), isso normalmente ocorreu com o DAP. O Mehlich-1 quantificou sempre mais P do que a RTA nas adjacências dos fertilizantes fosfatados. A movimentação do P nos dois solos ocorreu até o quarto centímetro distante do local de aplicação dos adubos, e a composição química nessas adjacências variou com o fertilizante fosfatado, o tipo de solo, o método de extração e a distância dos grânulos, mas necessita ser mais bem estudada.

BACKGROUND/AIM: Raloxifene is the first selective estrogen receptor modulator that has been approved for the treatment and prevention of osteoporosis in postmenopausal women in Europe and in the US. Although raloxifene reduces the risk of invasive breast cancer in postmenopausal women with osteoporosis and in postmenopausal women at high risk for invasive breast cancer, it is approved in that indication in the US but not in the EU. The aim was to characterize the clinical profiles of postmenopausal women expected to benefit most from therapy with raloxifene based on published scientific evidence to date. METHODS: Key individual patient characteristics relevant to the prescription of raloxifene in daily practice were defined by a board of Swiss experts in the fields of menopause and metabolic bone diseases and linked to published scientific evidence. Consensus was reached about translating these insights into daily practice. RESULTS: Through estrogen agonistic effects on bone, raloxifene reduces biochemical markers of bone turnover to premenopausal levels, increases bone mineral density (BMD) at the lumbar spine, proximal femur, and total body, and reduces vertebral fracture risk in women with osteopenia or osteoporosis with and without prevalent vertebral fracture. Through estrogen antagonistic effects on breast tissue, raloxifene reduces the risk of invasive estrogen-receptor positive breast cancer in postmenopausal women with osteoporosis and in postmenopausal women at high risk for invasive breast cancer. Finally, raloxifene increases the incidence of hot flushes, the risk of venous thromboembolic events, and the risk of fatal stroke in postmenopausal women at increased risk for coronary heart disease. Postmenopausal women in whom the use of raloxifene is considered can be categorized in a 2 × 2 matrix reflecting their bone status (osteopenic or osteoporotic based on their BMD T-score by dual energy X-ray absorptiometry) and their breast cancer risk (low or high based on the modified Gail model). Women at high risk of breast cancer should be considered for treatment with raloxifene. CONCLUSION: Postmenopausal women between 50 and 70 years of age without climacteric symptoms with either osteopenia or osteoporosis should be evaluated with regard to their breast cancer risk and considered for treatment with raloxifene within the framework of its contraindications and precautions.

PURPOSE: Continuous positive airway pressure (CPAP) is the gold standard treatment for obstructive sleep apnea. However, the physiologic impact of CPAP on cerebral blood flow (CBF) is not well established. Ultrasound can be used to estimate CBF, but there is no widespread accepted protocol. We studied the physiologic influence of CPAP on CBF using a method integrating arterial diameter and flow velocity (FV) measurements obtained for each vessel supplying blood to the brain. METHODS: FV and lumen diameter of the left and right internal carotid, vertebral, and middle cerebral arteries were measured using duplex Doppler ultrasound with and without CPAP at 15 cm H(2)O, applied in a random order. Transcutaneous carbon dioxide (PtcCO(2)), heart rate (HR), blood pressure (BP), and oxygen saturation were monitored. Results were compared with a theoretical prediction of CBF change based on the effect of partial pressure of carbon dioxide on CBF. RESULTS: Data were obtained from 23 healthy volunteers (mean ± SD; 12 male, age 25.1 ± 2.6 years, body mass index 21.8 ± 2.0 kg/m(2)). The mean experimental and theoretical CBF decrease under CPAP was 12.5 % (p < 0.001) and 11.9 % (p < 0.001), respectively. The difference between experimental and theoretical CBF reduction was not statistically significant (3.84 ± 79 ml/min, p = 0.40). There was a significant reduction in PtcCO(2) with CPAP (p = <0.001) and a significant increase in mean BP (p = 0.0017). No significant change was observed in SaO(2) (p = 0.21) and HR (p = 0.62). CONCLUSION: Duplex Doppler ultrasound measurements of arterial diameter and FV allow for a noninvasive bedside estimation of CBF. CPAP at 15 cm H(2)O significantly decreased CBF in healthy awake volunteers. This effect appeared to be mediated predominately through the hypocapnic vasoconstriction coinciding with PCO(2) level reduction. The results suggest that CPAP should be used cautiously in patients with unstable cerebral hemodynamics.

As correntes teóricas que conhecemos sob os rótulos de pós-estruturalismo e de pós-modernismo influenciaram profundamente, como sabemos, as teorizações e as pesquisas em diversos campos das ciências sociais e humanas nos últimos anos; uma influência que tem sido igualmente considerável na pesquisa em educação no Brasil. Os efeitos combinados dessas correntes, sintetizados talvez na chamada "virada lingüística", expressam-se naquilo que se convencionou chamar de "teorias pós-críticas em educação". Em seu conjunto, essas teorias utilizam uma série de ferramentas conceituais, de operações analíticas e de processos investigativos que as destacam tanto das teorias tradicionais como das teorias críticas que as precederam. Este artigo discute os efeitos dessas teorias sobre a pesquisa educacional brasileira. Traçando uma espécie de esboço de um mapa do campo dos estudos pós-críticos em educação no Brasil, este artigo mostra o início das discussões pós-críticas no campo educacional brasileiro, discute as principais temáticas exploradas por essas pesquisas e indica os principais traçados por elas efetuados, descrevendo as expansões, as fraturas, as conquistas e as aberturas produzidas no campo educacional brasileiro.

A comparative study of the parts played by technetium-99m diphosphonate and gallium-67 citrate bone scanning in the early diagnosis of infectious spondylodiscitis is presented. Nineteen patients were included in the study. All patients (11 men aged 19-70 years and eight women aged 18-72 years) had a history of back pain varying in duration from one to 15 weeks. A 99mTc diphosphonate bone scan was positive in 17 patients. The two patients with negative results had less than two weeks of back pain. The 67Ga citrate bone scan showed uptake in all patients.

The purpose of this study was to evaluate the results of an anatomic open stabilization procedure. Twenty-three consecutive patients with traumatic, recurrent, anterior glenohumeral instability were managed with a modified open procedure. All had a Bankart lesion. A standardized, true anatomic capsulolabral reconstruction was performed with suture anchors. Two patients were lost to follow-up, and twenty-one were evaluated after 36 months [range, 24-49 months] by an independent observer. Shoulder stability and function were the principal outcome measures. None of the patients had subsequent dislocations. All returned to full working capacity, and 19 reached their previous level of sport activities. The mean loss in active abduction and flexion was 1 degrees, in internal rotation, one vertebral level, and in external rotation, 7 degrees (arm at 90 degrees of abduction). Two patients had a positive anterior apprehension test. No sign of shoulder osteoarthritis was observed. The open anatomic capsulolabral reconstruction provides excellent results and allows the restoration of stability with good function.

BACKGROUND: Mortality is increased after a hip fracture, and strategies that improve outcomes are needed. METHODS: In this randomized, double-blind, placebo-controlled trial, 1065 patients were assigned to receive yearly intravenous zoledronic acid (at a dose of 5 mg), and 1062 patients were assigned to receive placebo. The infusions were first administered within 90 days after surgical repair of a hip fracture. All patients received supplemental vitamin D and calcium. The median follow-up was 1.9 years. The primary end point was a new clinical fracture. RESULTS: The rates of any new clinical fracture were 8.6% in the zoledronic acid group and 13.9% in the placebo group, a 35% risk reduction (P = 0.001); the respective rates of a new clinical vertebral fracture were 1.7% and 3.8% (P = 0.02), and the respective rates of new nonvertebral fractures were 7.6% and 10.7% (P = 0.03). In the safety analysis, 101 of 1054 patients in the zoledronic acid group (9.6%) and 141 of 1057 patients in the placebo group (13.3%) died, a reduction of 28% in deaths from any cause in the zoledronic-acid group (P = 0.01). The most frequent adverse events in patients receiving zoledronic acid were pyrexia, myalgia, and bone and musculoskeletal pain. No cases of osteonecrosis of the jaw were reported, and no adverse effects on the healing of fractures were noted. The rates of renal and cardiovascular adverse events, including atrial fibrillation and stroke, were similar in the two groups. CONCLUSIONS: An annual infusion of zoledronic acid within 90 days after repair of a low-trauma hip fracture was associated with a reduction in the rate of new clinical fractures and improved survival. A poetisa Cecília Meireles foi signatária do Manifesto dos Pioneiros da Educação Nova, histórico documento de 1932 redigido por Fernando de Azevedo e assinado por vinte e seis intelectuais, no qual se encontram as principais formulações do movimento educacional renovador denominado Escola Nova. O presente trabalho analisa a literatura já produzida sobre Meireles e textos da autora publicados entre 1930 e 1933 na coluna "Comentário" da "Página de Educação", seção dirigida por ela no jornal Diário de Notícias da cidade do Rio de Janeiro. O objetivo do trabalho é mostrar que a poetisa expressava ideias e posicionamentos da Escola Nova no início da década de 1930, dentre os quais figuram a relação entre família e escola e o uso das artes como recursos educativos. Tendo por base a teoria da análise retórica, e considerando a peculiaridade do veículo de comunicação utilizado por Meireles, o trabalho abre perspectivas para a investigação das estratégias argumentativas que compõem o discurso pedagógico da autora.

Este texto é uma reflexão sobre a cultura da performatividade na área de educação no Brasil, motivada pelo artigo de Kuhlmann Jr. (2014), em que o autor elenca argumentos contra a pressão por publicação que têm circulado na academia. Ele é produzido a partir da experiência da autora em comissões de avaliação, nomeadamente a de programas de pós-graduação no âmbito da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - Capes. Argumenta que a performatividade tem modificado a cultura de pesquisa e pós-graduação no país, mas discorda de que haja uma corrida produtivista na área de educação. Em diálogo com teóricos que, segundo Butler (2000), apostam na irrealizabilidade, a autora defende uma ação política agonística no sentido de operar nas fraturas da lógica performativa, opondo-se a uma ética humanista que assume como liberal.

Cervical artery dissection (CeAD), a mural hematoma in a carotid or vertebral artery, is a major cause of ischemic stroke in young adults although relatively uncommon in the general population (incidence of 2.6/100,000 per year). Minor cervical traumas, infection, migraine and hypertension are putative risk factors, and inverse associations with obesity and hypercholesterolemia are described. No confirmed genetic susceptibility factors have been identified using candidate gene approaches. We performed genome-wide association studies (GWAS) in 1,393 CeAD cases and 14,416 controls. The rs9349379[G] allele (PHACTR1) was associated with lower CeAD risk (odds ratio (OR) = 0.75, 95% confidence interval (CI) = 0.69-0.82; P = 4.46 × 10(-10)), with confirmation in independent follow-up samples (659 CeAD cases and 2,648 controls; P = 3.91 × 10(-3); combined P = 1.00 × 10(-11)). The rs9349379[G] allele was previously shown to be associated with lower risk of migraine and increased risk of myocardial infarction. Deciphering the mechanisms underlying this pleiotropy might provide important information on the biological underpinnings of these disabling conditions.

Rheumatoid arthritis is the only secondary cause of osteoporosis that is considered independent of bone density in the FRAX(®) algorithm. Although input for rheumatoid arthritis in FRAX(®) is a dichotomous variable, intuitively, one would expect that more severe or active disease would be associated with a greater risk for fracture. We reviewed the literature to determine if specific disease parameters or medication use could be used to better characterize fracture risk in individuals with rheumatoid arthritis. Although many studies document a correlation between various parameters of disease activity or severity and decreased bone density, fewer have associated these variables with fracture risk. We reviewed these studies in detail and concluded that disability measures such as HAQ (Health Assessment Questionnaire) and functional class do correlate with clinical fractures but not morphometric vertebral fractures. One large study found a strong correlation with duration of disease and fracture risk but additional studies are needed to confirm this. There was little evidence to correlate other measures of disease such as DAS (disease activity score), VAS (visual analogue scale), acute phase reactants, use of non-glucocorticoid medications and increased fracture risk. We concluded that FRAX(®) calculations may underestimate fracture probability in patients with impaired functional status from rheumatoid arthritis but that this could not be quantified at this time. At this time, other disease measures cannot be used for fracture prediction. However only a few, mostly small studies addressed other disease parameters and further research is needed. Additional questions for future research are suggested.

This nested case-control analysis of a Swiss ambulatory cohort of elderly women assessed the discriminatory power of urinary markers of bone resorption and heel quantitative ultrasound for non-vertebral fractures. The tests all discriminated between cases and controls, but combining the two strategies yielded no additional relevant information. INTRODUCTION: Data are limited regarding the combination of bone resorption markers and heel quantitative bone ultrasound (QUS) in the detection of women at risk for fracture. METHODS: In a nested case-control analysis, we studied 368 women (mean age 76.2 +/- 3.2 years), 195 with low-trauma non-vertebral fractures and 173 without, matched for age, BMI, medical center, and follow-up duration, from a prospective study designed to predict fractures. Urinary total pyridinolines (PYD) and deoxypyridinolines (DPD) were measured by high performance liquid chromatography. All women underwent bone evaluations using Achilles+ and Sahara heel QUS. RESULTS: Areas under the receiver operating-characteristic curve (AUC) for discriminative models of the fracture group, with 95% confidence intervals, were 0.62 (0.56-0.68) and 0.59 (0.53-0.65) for PYD and DPD, and 0.64 (0.58-0.69) and 0.65 (0.59-0.71) for Achilles+ and Sahara QUS, respectively. The combination of resorption markers and QUS added no significant discriminatory information to either measurement alone with an AUC of 0.66 (0.60-0.71) for Achilles+ with PYD and 0.68 (0.62-0.73) for Sahara with PYD. CONCLUSIONS: Urinary bone resorption markers and QUS are equally discriminatory between non-vertebral fracture patients and controls. However, the combination of bone resorption markers and QUS is not better than either test used alone.
