O estudo buscou identificar as variáveis de posicionamento e pega, durante a amamentação, relacionadas aos traumas mamilares. Estudo caso-controle que investigou o aparecimento do trauma mamilar entre mulheres internadas em um hospital Universitário de São Paulo, em 2004 e 2005. Os casos foram puérperas com diagnóstico de trauma mamilar uni ou bilateral. Para a análise dos dados, foram aplicados os testes qui-quadrado, t de Student, razão de chances (IC= 95%) e análise de correspondência. Foram estudadas 146 puérperas e seus recém-nascidos, sendo 73 casos e 73 controles. As variáveis de posicionamento e pega, estatisticamente significativas para a ocorrência da lesão, foram: criança com pescoço torcido, queixo longe da mama e lábio inferior virado para dentro. A prevenção do trauma, no início da amamentação, é decisiva para a continuidade desta prática. O acompanhamento do posicionamento adequado é determinante para o estabelecimento da amamentação efetiva e prolongada.

O objetivo foi caracterizar os idosos que sofreram trauma e foram internados nos três hospitais terciários, da cidade de Londrina - Paraná. Pesquisa quantitativa através de um estudo observacional, transversal, com 121 idosos de 60 a 74 anos e seus cuidadores, realizada durante um período de seis meses, por meio de entrevistas e visita domiciliar. Os resultados evidenciaram que a maioria dos idosos era do sexo masculino, com média de idade de 67,7 anos. Os traumas sofridos foram: 62,0% quedas, 25,6% acidentes de transporte e 10,4% outras causas. Como conseqüência ao trauma houveram de uma a três lesões, e 11 idosos (9,1%), foram a óbito. A queda foi a principal causa de trauma, a qual pode ser evitada observando os fatores intrínsecos e os extrínsecos que a predispõem, com o intuito de estabelecer estratégias de prevenção desse tipo de trauma.

BACKGROUND: The usual treatment of blunt aortic injury (BAI) is prompt surgery. Frequently severe injuries to the brain or lungs exclude further surgical treatment. The purpose of this study is to assess the feasibility of placing endovascular stent-grafts. METHODS: From 1992 through 1999, in our primary and referral trauma center, 26 acute BAI, 21 males and 5 females, mean age 40.2+/-16.3 yrs were diagnosed. The last 4 patients underwent prospectively endovascular repair with Talent endograft. Endoprosthesis parameters were measured on three-dimensional spiral CT reconstruction. While waiting for devices, blood pressure was aggressively lowered and aortic lesions were monitored by transesophageal echography. RESULTS: Stent-graft deployment was successful in all 4 patients. There were no complications of endoleak, stent migration, paraplegia or death. Angiographic exclusion was complete in all 4 patients. CT scans at a mean follow-up of 11+/-5 months showed complete healing of the aortic wall in all patients. CONCLUSIONS: For stable acute BAI, endovascular stent-graft repair is feasible and safe, and is an effective therapeutic alternative to open surgery. Because of the normal proximal and distal wall in aortic injuries, endoluminal treatment might be the therapy of choice in the near future.

Orphan receptors of the FTZ-F1-related group of nuclear receptors (xFF1r) were identified in Xenopus laevis by isolation of cDNAs from a neurula stage library. Two cDNAs were found, which encode full length, highly related receptor proteins, xFF1rA and B, whose closet relative known so far is the murine LRH-1 orphan receptor. xFF1rA protein expressed by a recombinant vaccinia virus system specifically binds to FTZ-F1 response elements (FRE; PyCAAGGPyCPu). In cotransfection studies, xFF1rA constitutively activates transcription, in a manner dependent on the number of FREs. The amounts of at least four mRNAs encoding full-length receptors greatly increase between gastrula and early tailbud stages and decrease at later stages. At early tailbud stages, xFTZ-F1-related antigens are found in all nuclei of the embryo.

Stress can cause damage and atrophy of neurons in the hippocampus by deregulating the expression of neurotrophic factors that promote neuronal plasticity. The endocannabinoid system represents a physiological substrate involved in neuroprotection at both cellular and emotional levels. The lack of CB1 receptor alters neuronal plasticity and originates an anxiety-like phenotype in mice. In the present study, CB1 knockout mice exhibited an augmented response to stress revealed by the increased despair behavior and corticosterone levels showed in the tail suspension test and decreased brain derived neurotrophic factor (BDNF) levels in the hippocampus. Interestingly, local administration of BDNF in the hippocampus reversed the increased despair behavior of CB1 knockout mice, confirming the crucial role played by BDNF on the emotional impairment of these mutants. The neurotrophic deficiency seems to be specific for BDNF since no differences were found in the levels of NGF and NT-3, two additional neurotrophic factors. Moreover, BDNF impairment is not related to the activity of its specific receptor TrkB or the activity of the transcription factor CREB. These results suggest that the lack of CB1 receptor originates an enhanced response to stress and neuronal plasticity by decreasing BDNF levels in the hippocampus that lead to impairment in the responses to emotional disturbances.
Indicadores clínicos e pré-hospitalares de sobrevivência no trauma fechado: uma análise multivariada

O objetivo do estudo foi identificar os indicadores clínicos e pré-hospitalares associados à sobrevivência de vítimas de trauma fechado. Foram utilizadas a análise de sobrevivência de Kaplan Meier, e de Riscos Proporcionais de Cox, para analisar a associação de 33 variáveis ao óbito precoce e tardio, propondo modelos multivariados. Os modelos finais até 48h pós-trauma evidenciaram altos coeficientes de risco promovidos pelas lesões abdominais, Injury Severity Score >25, procedimentos respiratórios avançados e compressões torácicas pré-hospitalares. No modelo até 7 dias, a pressão arterial sistólica na cena do acidente, se menor de 75mmHg, foi associada a maior risco de óbito e se ausente, foi associada ao mais elevado risco de óbito após 7 dias. A reposição de volume pré-hospitalar apresentou efeito protetor em todos os períodos. Os resultados sugerem que a magnitude da hipoxemia e da instabilidade hemodinâmica diante da hemorragia, influenciaram de forma significante o óbito precoce e tardio desse grupo de vítimas.

Anti-angiogenic therapies are currently in cancer clinical trials, but to date there are no established tests for evaluating the angiogenic status of a patient. We measured 11 circulating angiogenesis-associated molecules in cancer patients before and after local treatment. The purpose of our study was to screen for possible relationships among the different molecules and between individual molecules and tumor burden. We measured VEGF-A, PlGF, SCF, MMP-9, EDB+ -fibronectin, sVEGFR-2, sVEGFR-1, salphaVbeta3, sTie-2, IL-8 and CRP in the blood of 22 healthy volunteers, 17 early breast, 17 early colorectal, and 8 advanced sarcoma/melanoma cancer patients. Breast cancer patients had elevated levels of VEGF-A and sTie-2, colorectal cancer patients of VEGF-A, MMP-9, sTie-2, IL-8 and CRP, and melanoma/sarcoma patients of sVEGFR-1. salphaVbeta3 was decreased in colorectal cancer patients. A correlation between VEGF-A and MMP-9 was found. After tumor removal, MMP-9 and salphaVbeta3 significantly decreased in breast and CRP in colorectal cancer, whereas sVEGFR-1 increased in colorectal cancer patients. In sarcoma/melanoma patients treated regionally with TNF and chemotherapy we observed a rise in VEGF-A, SCF, VEGFR-2, MMP-9, Tie-2 and CRP, a correlation between CRP and IL-8, and a decreased in sVEGFR-1 levels. In conclusion, among all factors measured, only VEGF-A and MMP-9 consistently correlated to each other, elevated CRP levels were associated with tumor burden, whereas sVEGF-R1 increased after tumor removal in colorectal cancer. Treatment with chemotherapy and TNF induced changes consistent with an angiogenic switch. These results warrant a prospective study to compare the effect of surgical tumor removal vs. chemotherapy on some of these markers and to evaluate their prognostic/predictive value.

Federal and state policy makers increasingly emphasize the need to reduce highway crash rates. This emphasis is demonstrated in Iowa’s recently released draft Iowa Strategic Highway Safety Plan and by the U.S. Department of Transportation’s placement of “improved transportation safety” at the top of its list of strategic goals. Thus, finding improved methods to enhance highway safety has become a top priority at highway agencies. The objective of this project is to develop tools and procedures by which Iowa engineers can identify potentially hazardous roadway locations and designs, and to demonstrate the utility of these tools by developing candidate lists of high crash locations in the State. An initial task, building an integrated database to facilitate the tools and procedures, is an important product, in and of itself. Accordingly, the Iowa Department of Transportation (Iowa DOT) Geographic Information Management System (GIMS) and Geographic Information System Accident Analysis and Location System (GIS-ALAS) databases were integrated with available digital imagery. (The GIMS database contains roadway characteristics, e.g., lane width, surface and shoulder type, and traffic volume, for all public roadways. GIS-ALAS records include data, e.g., vehicles, drivers, roadway conditions, and the crash severity, for crashes occurring on public roadways during then past 10 years.)

Background: Few studies have used longitudinal ultrasound measurements to assess the effect of traffic-related air pollution on fetal growth.Objective: We examined the relationship between exposure to nitrogen dioxide (NO2) and aromatic hydrocarbons [benzene, toluene, ethylbenzene, m/p-xylene, and o-xylene (BTEX)] on fetal growth assessed by 1,692 ultrasound measurements among 562 pregnant women from the Sabadell cohort of the Spanish INMA (Environment and Childhood) study.Methods: We used temporally adjusted land-use regression models to estimate exposures to NO2 and BTEX. We fitted mixed-effects models to estimate longitudinal growth curves for femur length (FL), head circumference (HC), abdominal circumference (AC), biparietal diameter (BPD), and estimated fetal weight (EFW). Unconditional and conditional SD scores were calculated at 12, 20, and 32 weeks of gestation. Sensitivity analyses were performed considering time–activity patterns during pregnancy.Results: Exposure to BTEX from early pregnancy was negatively associated with growth in BPD during weeks 20–32. None of the other fetal growth parameters were associated with exposure to air pollution during pregnancy. When considering only women who spent 2 hr/day in nonresidential outdoor locations, effect estimates were stronger and statistically significant for the association between NO2 and growth in HC during weeks 12–20 and growth in AC, BPD, and EFW during weeks 20–32.Conclusions: Our results lend some support to an effect of exposure to traffic-related air pollutants from early pregnancy on fetal growth during mid-pregnancy.

On-street parking has been considered problematic by engineers for many years. In fact, numerous studies have concluded that diagonal or angle parking in particular is potentially more of a safety concern than parallel or no parking at all. It is a common position of many states, including Iowa, to discourage or completely prohibit angle parking on primary road extensions in urban areas. However, with the acceptance of “context sensitive design” and traffic calming techniques, policies for on-street parking are receiving re -consideration in many agencies including the FHWA. This study was undertaken to analyze operational and safety histories in the state of Iowa where various types of on-street parking have existed for many years, concentrating in particular on smaller communities. Specifically of interest was a comparison of diagonal parking locations to other types with regard to related crash histories. If possible, it was intended to develop guidelines to assist Iowa Department of Transportation designers in the consideration of parking requirements for road improvements through small communities. In this regard, several criteria were analyzed to determine possible contribution to crash history including road width, clearance to parked vehicles, traffic volumes, community population, and length of parking area. None of these factors, with the possible exception of population, displayed a clearly definable relationship to crash history. However, when average crash rates for various parking types were compared for non-intersection crashes, differences in rates between areas with diagonal parking and those with parallel parking were almost negligible. In fact, those observed rates were less than sample locations with no parking at all. These results seem to indicate that indeed there may exist no compelling justification for blanket prohibition of angle parking along Iowa’s primary extensions in all urban areas. Rather, a case-by-case investigation with each project design of the most applicable parking type would seem appropriate in smaller communities.

Blunt chest traumas are a clinical challenge, both for diagnosis and treatment. The use ofCardiovascular Magnetic Resonance can play a major role in this setting. We present two cases: a12-year-old boy and 45-year-old man. Late gadolinium enhancement imaging enabled visualizationof myocardial damage resulting from the trauma.

Estudo qualitativo que avaliou o conhecimento do enfermeiro acerca da dor na vítima de trauma. Desenvolvido em um hospital público, por meio da aplicação de questionário e teste de conhecimento e tratados pela Análise de Conteúdo. A amostra compreendeu 27 enfermeiros, em sua quase totalidade de mulheres (92,6%), idade média 31 ±10,3 anos e maioria com formação inferior a 1 ano (51,8%). Os resultados evidenciaram a dor como sensação desagradável, sinal de alerta e uma experiência subjetiva. A avaliação da dor é vista por aspectos subjetivos e objetivos. A maioria (59,3%) desconhece os instrumentos de avaliação e dentre os que conhecem a escala numérica foi a mais referida. As estratégias de controle de dor mencionadas foram medidas em farmacológicas, não farmacológicas e combinadas. Todos os enfermeiros consideraram a avaliação da dor importante e um caminho para o tratamento humanizado e qualificado, que orienta a conduta terapêutica e restabelece o bem-estar do paciente.

Although homology is a fundamental concept in biology and is one of the shared channels of communication universal to all biology, it is difficult to find a consensus definition. Indeed, the interpretations of homology have changed as biology has progressed. New terms, such as paramorphism, have been introduced into the literature with mixed success. In addition, different research fields operate with different definitions of homology, for example the mechanistic usage of evo-devo is not strictly historical and would not be acceptable in cladistics. This makes a global understanding of homology complex, whereas the integration of evolutionary concepts into bioinformatics and genomics is increasingly important. We propose an ontology organizing homology and related concepts and hope this solution will also facilitate the integration and sharing of knowledge among the community.
