We present an optimization procedure to improve the propagation properties of the depressed cladding, buried micro-structured waveguides formed in a z-cut lithium niobate (LN) crystal by high repetition rate femtosecond (fs) laser writing. It is shown that the propagation wavelength for which the confinement losses of ordinary (O) and extraordinary ordinary (E) polarizations are below 1 dB/cm can be optimized beyond 3 micro meter for hexagonal WG structures with seven rings of tracks.

We review recent progress in the research on nonlinear mechanisms of pulse generation in passively mode-locked fibre lasers. These include parabolic self-similar pulse mode-locking, a mode-locking regime featuring pulses with a triangular distribution of the intensity, and spectral compression arising from nonlinear pulse propagation. We also report on the possibility of achieving various regimes of advanced temporal waveform generation in a mode-locked fibre laser by inclusion of a spectral filter into the laser cavity.

In northern regions where observational data is sparse, lake ice models are ideal tools as they can provide valuable information on ice cover regimes. The Canadian Lake Ice Model was used to simulate ice cover for a lake near Churchill, Manitoba, Canada throughout the 2008/2009 and 2009/2010 ice covered seasons. To validate and improve the model results, in situ measurements of the ice cover through both seasons were obtained using an upward-looking sonar device Shallow Water Ice Profiler (SWIP) installed on the bottom of the lake. The SWIP identified the ice-on/off dates as well as collected ice thickness measurements. In addition, a digital camera was installed on shore to capture images of the ice cover through the seasons and field measurements were obtained of snow depth on the ice, and both the thickness of snow ice (if present) and total ice cover. Altering the amounts of snow cover on the ice surface to represent potential snow redistribution affected simulated freeze-up dates by a maximum of 22 days and break-up dates by a maximum of 12 days, highlighting the importance of accurately representing the snowpack for lake ice modelling. The late season ice thickness tended to be under estimated by the simulations with break-up occurring too early, however, the evolution of the ice cover was simulated to fall between the range of the full snow and no snow scenario, with the thickness being dependant on the amount of snow cover on the ice surface.

Advertising investment and audience figures indicate that television continues to lead as a mass advertising medium. However, its effectiveness is questioned due to problems such as zapping, saturation and audience fragmentation. This has favoured the development of non-conventional advertising formats. This study provides empirical evidence for the theoretical development. This investigation analyzes the recall generated by four non-conventional advertising formats in a real environment: short programme (branded content), television sponsorship, internal and external telepromotion versus the more conventional spot. The methodology employed has integrated secondary data with primary data from computer assisted telephone interviewing (CATI) were performed ad-hoc on a sample of 2000 individuals, aged 16 to 65, representative of the total television audience. Our findings show that non-conventional advertising formats are more effective at a cognitive level, as they generate higher levels of both unaided and aided recall, in all analyzed formats when compared to the spot.

This article considers the opportunities of civilians to peacefully resist violent conflicts or civil wars. The argument developed here is based on a field-based research on the peace community San José de Apartadó in Colombia. The analytical and theoretical framework, which delimits the use of the term ‘resistance’ in this article, builds on the conceptual considerations of Hollander and Einwohner (2004) and on the theoretical concept of ‘rightful resistance’ developed by O’Brien (1996). Beginning with a conflict-analytical classification of the case study, we will describe the long-term socio-historical processes and the organizational experiences of the civilian population, which favoured the emergence of this resistance initiative. The analytical approach to the dimensions and aims of the resistance of this peace community leads to the differentiation of O`Brian’s concept of ‘rightful resistance’.

Abstract: The first metatarsal sesamoid bones are not always taken into consideration when making a diagnosis, in pathologies that affect the region of the first metatarsal head. This is due to the insufficient knowledge of all the pathologies that can affect the sesamoids and the relative little incidence that they have. With the increment of sports activities, in particular the running, increasingly affects of the symptoms concerning this region are observed. Methods: A literature search was performed in 5 databases (Medline, PubMed, Scopus, Cochrane Library and BUCEA). The terms included in the search were: sesamoids, anatomy, biomechanics, sesamoids review and sesamoids pathology. In the initial search articles with no more than 10 years, only humans and revision texts are considered. Results: 24 articles were selected and include different pathologies with diagnosis using imaging tests and treatments, both conservative and surgical; as well as aspects from the biomechanics of the metatarsal-sesamoid joint. Conclusion: Sesamoids due of his anatomy, topography and function can be involved in a lot of pathologies; with similar signs and symptoms that can confuse the podiatry when he has to make a correct diagnosis or treatment.

Objective: The study was designed to validate use of elec-tronic health records (EHRs) for diagnosing bipolar disorder and classifying control subjects. Method: EHR data were obtained from a health care system of more than 4.6 million patients spanning more than 20 years. Experienced clinicians reviewed charts to identify text features and coded data consistent or inconsistent with a diagnosis of bipolar disorder. Natural language processing was used to train a diagnostic algorithm with 95% specificity for classifying bipolar disorder. Filtered coded data were used to derive three additional classification rules for case subjects and one for control subjects. The positive predictive value (PPV) of EHR-based bipolar disorder and subphenotype di- agnoses was calculated against diagnoses from direct semi- structured interviews of 190 patients by trained clinicians blind to EHR diagnosis. Results: The PPV of bipolar disorder defined by natural language processing was 0.85. Coded classification based on strict filtering achieved a value of 0.79, but classifications based on less stringent criteria performed less well. No EHR- classified control subject received a diagnosis of bipolar dis- order on the basis of direct interview (PPV=1.0). For most subphenotypes, values exceeded 0.80. The EHR-based clas- sifications were used to accrue 4,500 bipolar disorder cases and 5,000 controls for genetic analyses. Conclusions: Semiautomated mining of EHRs can be used to ascertain bipolar disorder patients and control subjects with high specificity and predictive value compared with diagnostic interviews. EHRs provide a powerful resource for high-throughput phenotyping for genetic and clinical research.

Question:Does continuity of care improve control of clinical risk factors in people with diabetes?Study design:Cross-sectional community-based survey.Main results:85.5% of participants had continuity of care (single care site and usual provider), 9.3% had a single care site but different providers, and 5.2% had no usual source of care. Good glycaemic control was more likely with continuity of care or single care site compared with no usual source of care (continuity of care: OR 4.62, 95% CI 2.02 to 10.60; single care site: OR 6.13, 95% CI 2.08 to 18.04). There were no significant differences between groups with a usual site. There was no increased likelihood of good control of blood pressure or lipid level among groups.Authors’ conclusionsThere is evidence that good glycaemic control is more likely among people whose diabetic care is provided from one site, regardless of whether it is provided by the same practitioner.
