FUNDAMENTOS: Síndrome Metabólica (SM) está associada com maior risco cardiovascular, porém não está claro se as alterações miocárdicas presentes nessa condição, como a disfunção diastólica, são consequência de mecanismos sistêmicos ou de efeitos diretos no miocárdio. OBJETIVOS: Comparar função diastólica, biomarcadores de atividade da Matriz Extracelular (MEC), inflamação e estresse hemodinâmico, em pacientes com SM e controles saudáveis. MÉTODOS: Pacientes com SM (n = 76) e controles saudáveis (n = 30) foram avaliados clinicamente e submetidos a exame ecocardiográfico e mensuração dos níveis plasmáticos de metaloproteinase-9 (MMP9), inibidor tecidual da metaloproteinase-1 (TIMP1), proteína C reativa ultrassensível (PCR-us), resistência insulínica (HOMA-RI) e NT-proBNP. RESULTADOS: O grupo SM apresentou menor onda E' (10,1 ± 3,0 cm/s vs. 11,9 ± 2,6 cm/s, p = 0,005), maiores valores para onda A (63,4 ± 14,1 vs. 53,1 ± 8,9 cm/s, p < 0,001), razão E/E'(8,0 ± 2,2 vs. 6,3 ± 1,2; p < 0,001), MMP9 (502,9 ± 237,1 vs. 330,4 ± 162,7 ng/mL, p < 0,001), PCR-us (p = 0,001) e HOMA-RI (p < 0,001), sem diferença nos níveis de TIMP1 e NT-proBNP. Na análise multivariada, apenas MMP9 foi independentemente associada a SM. CONCLUSÃO: Pacientes com SM apresentaram diferenças em medidas ecocardiográficas de função diastólica, na atividade da MEC, PCR-us e HOMA-RI em relação aos controles. Porém, somente MMP9 foi independentemente associada com SM. Esses achados sugerem que os efeitos precoces da SM sobre a atividade da MEC podem não ser detectados nas medidas ecocardiográficas de função diastólica usuais.
Incidência de Arritmias Ventriculares após Terapia Celular em Pacientes com Cardiomiopatia Chagásica

Fundamento: O tratamento com células-tronco nas diversas cardiomiopatias pode estar relacionado ao aumento nas arritmias. Objetivos: Determinar se a injeção intracoronária de células-tronco em portadores de cardiomiopatia chagásica está associada ao aumento da incidência de arritmias ventriculares, comparado ao Grupo Controle. Métodos: Estudo de coorte retrospectivo, que avaliou o prontuário de 60 pacientes que participaram de estudo transversal anterior. Foram coletados os seguintes dados: idade, sexo, medicamentos utilizados e variáveis do Holter, que demonstraram presença de arritmia complexa. O Holter foi realizado em quatro momentos: randomização, 2, 6 e 12 meses de seguimento. O Grupo Controle recebeu tratamento medicamentoso e injeção intracoronaria de placebo e o Grupo Estudo tratamento medicamentoso e implante autólogo de células-tronco. Resultados: Não houve diferença entre os Grupos Controle e Estudo nos critérios de arritmia analisados. Na análise intragrupo, foi encontrada diferença com significância entre os exames de Holter do Grupo Estudo na variável total de extrassístoles ventriculares comparada à basal, sendo entre de p = 0,014 entre Holter na randomização e Holter aos 2 meses, p = 0,004 entre Holter na randomização e Holter aos 6 meses, e p = 0,014 entre Holter na randomização e Holter aos 12 meses. A variável taquicardia ventricular não sustentada entre Holter na randomização e Holter aos 6 meses apresentou p = 0,036. Conclusão: A injeção intracoronária de células-tronco não aumentou a incidência de arritmia ventricular complexa em pacientes com cardiomiopatia chagásica, comparada ao Grupo Controle.

Background: The activation of the beta-adrenergic system promotes G protein stimulation that, via cyclic adenosine monophosphate (cAMP), alters the structure of protein kinase A (PKA) and leads to phospholamban (PLB) phosphorylation. This protein participates in the system that controls intracellular calcium in muscle cells, and it is the primary regulator of sarcoplasmic reticulum calcium pump activity. In obesity, the beta-adrenergic system is activated by the influence of increased leptin, therefore, resulting in higher myocardial phospholamban phosphorylation via cAMP-PKA. Objective: To investigate the involvement of proteins which regulate the degree of PLB phosphorylation due to beta-adrenergic activation in obesity. In the present study, we hypothesized that there is an imbalance between phospholamban phosphorylation and dephosphorylation, with prevalence of protein phosphorylation. Methods: Male Wistar rats were randomly distributed into two groups: control (n = 14), fed with normocaloric diet; and obese (n = 13), fed with a cycle of four unsaturated high-fat diets. Obesity was determined by the adiposity index, and protein expressions of phosphatase 1 (PP-1), PKA, PLB, phosphorylated phospholamban at serine16 (PPLB-Ser16) were assessed by Western blot. Results: Obesity caused glucose intolerance, hyperinsulinemia, hypertriglyceridemia, hyperleptinemia and did not alter the protein expression of PKA, PP-1, PLB, PPLB-Ser16. Conclusion: Obesity does not promote an imbalance between myocardial PLB phosphorylation and dephosphorylation via beta-adrenergic system.

Background: Patients with diabetes are in extract higher risk for fatal cardiovascular events. Objective: To evaluate major predictors of mortality in subjects with type 2 diabetes. Methods: A cohort of 323 individuals with type 2 diabetes from several regions of Brazil was followed for a long period. Baseline electrocardiograms, clinical and laboratory data obtained were used to determine hazard ratios (HR) and confidence interval (CI) related to cardiovascular and total mortality. Results: After 9.2 years of follow-up (median), 33 subjects died (17 from cardiovascular causes). Cardiovascular mortality was associated with male gender; smoking; prior myocardial infarction; long QTc interval; left ventricular hypertrophy; and eGFR <60 mL/min. These factors, in addition to obesity, were predictors of total mortality. Cardiovascular mortality was adjusted for age and gender, but remained associated with: smoking (HR = 3.8; 95% CI 1.3-11.8; p = 0.019); prior myocardial infarction (HR = 8.5; 95% CI 1.8-39.9; p = 0.007); eGFR < 60 mL/min (HR = 9.5; 95% CI 2.7-33.7; p = 0.001); long QTc interval (HR = 5.1; 95% CI 1.7-15.2; p = 0.004); and left ventricular hypertrophy (HR = 3.5; 95% CI 1.3-9.7; p = 0.002). Total mortality was associated with obesity (HR = 2.3; 95% CI 1.1-5.1; p = 0.030); smoking (HR = 2.5; 95% CI 1.0-6.1; p = 0.046); prior myocardial infarction (HR = 3.1; 95% CI 1.4-6.1; p = 0.005), and long QTc interval (HR = 3.1; 95% CI 1.4-6.1; p = 0.017). Conclusions: Biomarkers of simple measurement, particularly those related to target-organ lesions, were predictors of mortality in subjects with type 2 diabetes.

Background: In pathological situations, such as acute myocardial infarction, disorders of motility of the proximal gut can trigger symptoms like nausea and vomiting. Acute myocardial infarction delays gastric emptying (GE) of liquid in rats. Objective: Investigate the involvement of the vagus nerve, α 1-adrenoceptors, central nervous system GABAB receptors and also participation of paraventricular nucleus (PVN) of the hypothalamus in GE and gastric compliance (GC) in infarcted rats. Methods: Wistar rats, N = 8-15 in each group, were divided as INF group and sham (SH) group and subdivided. The infarction was performed through ligation of the left anterior descending coronary artery. GC was estimated with pressure-volume curves. Vagotomy was performed by sectioning the dorsal and ventral branches. To verify the action of GABAB receptors, baclofen was injected via icv (intracerebroventricular). Intravenous prazosin was used to produce chemical sympathectomy. The lesion in the PVN of the hypothalamus was performed using a 1mA/10s electrical current and GE was determined by measuring the percentage of gastric retention (% GR) of a saline meal. Results: No significant differences were observed regarding GC between groups; vagotomy significantly reduced % GR in INF group; icv treatment with baclofen significantly reduced %GR. GABAB receptors were not conclusively involved in delaying GE; intravenous treatment with prazosin significantly reduced GR% in INF group. PVN lesion abolished the effect of myocardial infarction on GE. Conclusion: Gastric emptying of liquids induced through acute myocardial infarction in rats showed the involvement of the vagus nerve, alpha1- adrenergic receptors and PVN.

Background:Diabetes affects approximately 250 million people in the world. Cardiovascular autonomic neuropathy is a common complication of diabetes that leads to severe postural hypotension, exercise intolerance, and increased incidence of silent myocardial infarction.Objective:To determine the variability of heart rate (HR) and systolic blood pressure (SBP) in recently diagnosed diabetic patients.Methods:The study included 30 patients with a diagnosis of type 2 diabetes of less than 2 years and 30 healthy controls. We used a Finapres® device to measure during five minutes beat-to-beat HR and blood pressure in three experimental conditions: supine position, standing position, and rhythmic breathing at 0.1 Hz. The results were analyzed in the time and frequency domains.Results:In the HR analysis, statistically significant differences were found in the time domain, specifically on short-term values such as standard deviation of NN intervals (SDNN), root mean square of successive differences (RMSSD), and number of pairs of successive NNs that differ by more than 50 ms (pNN50). In the BP analysis, there were no significant differences, but there was a sympathetic dominance in all three conditions. The baroreflex sensitivity (BRS) decreased in patients with early diabetes compared with healthy subjects during the standing maneuver.Conclusions:There is a decrease in HR variability in patients with early type 2 diabetes. No changes were observed in the BP analysis in the supine position, but there were changes in BRS with the standing maneuver, probably due to sympathetic hyperactivity.

Abstract Background: Although the beneficial effects of resistance training (RT) on the cardiovascular system are well established, few studies have investigated the effects of the chronic growth hormone (GH) administration on cardiac remodeling during an RT program. Objective: To evaluate the effects of GH on the morphological features of cardiac remodeling and Ca2+ transport gene expression in rats submitted to RT. Methods: Male Wistar rats were divided into 4 groups (n = 7 per group): control (CT), GH, RT and RT with GH (RTGH). The dose of GH was 0.2 IU/kg every other day for 30 days. The RT model used was the vertical jump in water (4 sets of 10 jumps, 3 bouts/wk) for 30 consecutive days. After the experimental period, the following variables were analyzed: final body weight (FBW), left ventricular weight (LVW), LVW/FBW ratio, cardiomyocyte cross-sectional area (CSA), collagen fraction, creatine kinase muscle-brain fraction (CK-MB) and gene expressions of SERCA2a, phospholamban (PLB) and ryanodine (RyR). Results: There was no significant (p > 0.05) difference among groups for FBW, LVW, LVW/FBW ratio, cardiomyocyte CSA, and SERCA2a, PLB and RyR gene expressions. The RT group showed a significant (p < 0.05) increase in collagen fraction compared to the other groups. Additionally, the trained groups (RT and RTGH) had greater CK-MB levels compared to the untrained groups (CT and GH). Conclusion: GH may attenuate the negative effects of RT on cardiac remodeling by counteracting the increased collagen synthesis, without affecting the gene expression that regulates cardiac Ca2+ transport.

Abstract Background: A significant variation in pulmonary embolism (PE) mortality trends have been documented around the world. We investigated the trends in mortality rate from PE in Brazil over a period of 21 years and its regional and gender differences. Methods: Using a nationwide database of death certificate information we searched for all cases with PE as the underlying cause of death between 1989 and 2010. Population data were obtained from the Brazilian Institute of Geography and Statistics (IBGE). We calculated age-, gender- and region-specific mortality rates for each year, using the 2000 Brazilian population for direct standardization. Results: Over 21 years the age-standardized mortality rate (ASMR) fell 31% from 3.04/100,000 to 2.09/100,000. In every year between 1989 and 2010, the ASMR was higher in women than in men, but both showed a significant declining trend, from 3.10/100,000 to 2.36/100,000 and from 2.94/100,000 to 1.80/100,000, respectively. Although all country regions showed a decline in their ASMR, the largest fall in death rates was concentrated in the highest income regions of the South and Southeast Brazil. The North and Northeast regions, the lowest income areas, showed a less marked fall in death rates and no distinct change in the PE mortality rate in women. Conclusions: Our study showed a reduction in the PE mortality rate over two decades in Brazil. However, significant variation in this trend was observed amongst the five country regions and between genders, pointing to possible disparities in health care access and quality in these groups.

A flutuação de proteína total sérica foi avaliada em 37 bezerros, machos e fêmeas, da raça Holandesa Preta e Branca, apresentando grupos genéticos 15/16 Holandês: Guzerá, puro por cruza e puro de origem. O período experimental abrangeu os primeiros 70 dias de vida do bezerro. Os tratamentos foram os seguintes: A1 - leite integral, colostro e sucedâneo (1 :1 :2) , com desmama aos 30 dias de idade; A2 leite integral, colostro e sucedâneo (1:1:2), com desmama aos 45 dias de idade; A3 - leite integral, colostro e sucedâneo (1:1:2), com desmama aos 60 dias de idade; B - leite integral, com desmama aos 45 dias de idade; C -sucedâneo, com desmama aos 45 dias de idade. Em todos os tratamentos os bezerros receberam colostro até 72 horas após o nascimento. O sucedâneo empregado foi leite em pó comercial, cuja fonte protéica é subproduto do leite. O concentrado utilizado foi uma ração comercial para desmama precoce. A coleta de sangue dos bezerros obedeceu ao seguinte esquema: imediatamente pós-natal (antes de qualquer ingestão de colostro) e 1, 2, 5, 10, 15, 20, 30, 40, 50, 60 e 70 dias de vida. Para análise da proteína foi utilizado o método de biureto. Para interpretação dos resultados, os dados foram submetidos às seguintes análises estatísticas: análise da variância, teste Tukey, correlação e regressão linear e análise da variância sob modelo fatorial, envolvendo os efeitos de tratamentos, coletas e interações. A concentração média de proteína total sérica apresentou o valor mais baixo por ocasião do nascimento (5,02 ± 0,51 g/100 ml) e o mais elevado (8,09 ± 0,51 g/100 ml) 24 horas pós-parto. No período de 1 a 30 dias, o tratamento A (A1 + A2 + A3) apresentou uma concentração de proteína total sérica significativamente mais elevada do que o tratamento C, não diferindo do tratamento B. Foi verificada uma redução linear e quadrática do teor de proteína total sérica nos primeiros 30 dias, seguida de elevação linear no período de 50 a 70 dias.

Com o objetivo de avaliar a interação micorrízica X Al tóxico no substrato efetuou-se um experimento fatorial 2x2x2, em casa de vegetação, no Departamento de Genética da ESALQ/USP, em Piracicaba. Foram empregadas duas cultivares de Leucaena leucocephala, uma Al - tolerante (Planta 49 - Estrada do Bongue) e outra Al-intolerante (NO749), cultivadas em areia lavada e esterilizada, com adição de solução nutritiva. A micorriza foi estabelecida com a inoculação de Glomus teptotichum Shenck e Smith no substrato, mantendo-se como testemunha a ausência do fungo. Dois níveis de Al foram avaliados, 0 e 9 ppm, adicionados à solução nutritiva. Aos 65 dias após o transplante das plântulas procedeu-se à colheita e foram determinados o peso da metária seca da parte aérea, altura de planta, porcentagem de colonização radicular, teor e quantidade acumulada de N, P, K, Ca, Mg e Al na parte aérea. Observou-se um acentuado efeito promotor da micorriza em Leucaena, o que geroulhe incrementos tanto no desenvolvimento da planta como na absorção de nutrientes. A cultivar intolerante ao Al tóxico mostrou-se mais dependente da micorriza e inclusive mais beneficiada,apresentando os maiores acréscimos na produção de matéria seca e acúmulo de nutrientes na parte aérea. Nas condições do experimento não houve acentuada diferença entre as doses de Al empregadas e, apesar do maior acúmulo do elemento em plantas micorrizadas, não foi observado interferência no desenvolvimento das cultivares.

Runibia Stål, 1861 is redescribed, as well as the species R. dallasi Rider, 1998, R. decorata (Dallas, 1851), R. discoidea (Fabricius, 1787), R. euopta (Walker, 1867) and R. perspicua (Fabricius, 1798). A new species, R. caribeana, is described from Virgin Islands. Strachia alligata Walker, 1867, R. decorata var. alligata, and R. picturata Breddin, 1904 were considered junior synonyms of R. decorata. Lectotypes of R. dallasi, R. euopta and R. alligata were designated. Male and female genitalia for all species are described, except the phallus and ectodermal genital duct for R. dallasi. A key and a geographical distribution map are also provided.

Morphometric characters of two populations of Difflugia corona Wallich, 1864 collected in two climatic stations (Spring, Autumn) in lentic environments of Chaco Province, Argentina, are studied. In the first climatic station it was registered specimens of bigger size; in the second, the size of the individuals was very below the minimum values registered. It is suggested a possible relationship between the size of the organisms and the availability of the inhabitable vegetable substratum.

Os ovos e os cinco estádios ninfais de Phloea subquadrata Spinola, 1837 são descritos e ilustrados. Três novas plantas hospedeiras são registradas, e dados sobre o desenvolvimento e a biologia do inseto são apresentados e discutidos.

The liver is an organ notable for its sensitivity to a great variety of environmental factors. It is composed of a parenchyma divided into irregular lobules by the exocrine pancreas or hepatopancreas; the pancreatic cells are arranged around a branch of the portal vein. The hepatocytes are radially arranged in cords around a central sinusoid. The liver histomorphology and the organization of exocrine pancreatic tissue of O. jenynsii (Günther, 1864) is similar to the acinar morphology of many teleosts, including freshwater and marine species. The aim of the present work was to carry out the histological analysis of the liver of Oligosarcus jenynsii, one of the most common species inhabiting Los Padres Lake (Buenos Aires Province, Argentina).
