Adiponectin, which plays a pivotal role in metabolic liver diseases, is reduced in concentration in patients with NASH (non-alcoholic steatohepatitis). The aim of the present study was to determine adiponectin concentrations in patients with different forms and stages of chronic liver diseases. Serum adiponectin concentrations were measured in 232 fasting patients with chronic liver disease: 64 with NAFLD (non-alcoholic fatty liver disease), 123 with other chronic liver disease (e.g. viral hepatitis, n=71; autoimmune disease, n=18; alcohol-induced liver disease, n=3; or elevated liver enzymes of unknown origin, n=31) and 45 with cirrhosis. Circulating adiponectin levels were significantly lower in patients with NAFLD in comparison with patients with other chronic liver disease (4.8+/-3.5 compared with 10.4+/-6.3 microg/ml respectively; P<0.0001). Circulating adiponectin levels were significantly higher in patients with cirrhosis in comparison with patients without cirrhosis (18.6+/-14.5 compared with 8.4+/-6.1 microg/ml respectively; P<0.0001). Adiponectin concentrations correlated negatively with body weight (P<0.001), serum triacylglycerols (triglycerides) (P<0.001) and, in women, with BMI (body mass index) (P<0.001). Adiponectin concentrations correlated positively with serum bile acids (P<0.001), serum hyaluronic acid (P<0.001) and elastography values (P<0.001). Adiponectin levels were decreased in patients with NAFLD. In conclusion, adiponectin levels correlate positively with surrogate markers of hepatic fibrosis (transient elastography, fasting serum bile acids and hyaluronate) and are significantly elevated in cases of cirrhosis.

Este estudo teve como objetivo conhecer a percepção dos enfermeiros acerca do uso do protocolo de suas atribuições na assistência pré-natal, identificando as ações de saúde desenvolvidas por esses profissionais, assim como os pontos facilitadores e dificultadores no uso do referido protocolo. Trata-se de um estudo qualitativo, desenvolvido junto aos enfermeiros da Estratégia de Saúde da Família do município de Divinópolis, Minas Gerais. Para o levantamento dos dados, foram realizadas entrevistas com cinco enfermeiros. Os dados foram analisados pelo conteúdo, na modalidade temática. Os resultados demonstraram a necessidade de investimentos na formação de pessoal qualificado para o atendimento à mulher no ciclo gravídico-puerperal, assim como a criação e a incorporação de protocolos que promovam uma melhor interação do trabalho médico e de enfermagem.

Estudo transversal com 301 gestantes atendidas, em 2009, em uma maternidade filantrópica da cidade de São Paulo (Pré-Natal do Amparo Maternal - PN-AM), com os objetivos de avaliar o pré-natal, segundo a idade gestacional de início, o número de consultas realizadas e a continuidade do atendimento e relacionar a adequação com as variáveis sociodemográficas, obstétricas e locais de início do pré-natal. O critério de análise utilizado foi o início até 120 dias da gestação e a realização de, no mínimo, seis consultas. A relação entre as variáveis foi analisada pelo Teste Qui-Quadrado. Os resultados mostraram que 41,5% das gestantes iniciaram o pré-natal em outro serviço de saúde e transferiram-se espontaneamente para o PN-AM; 74,1% iniciaram precocemente e 80,4% realizaram, pelo menos, seis consultas; 63,1% atenderam aos dois critérios simultaneamente. O pré-natal adequado apresentou diferença estatística significante para idade materna, parceiro fixo, trabalho, local de residência, acompanhante à consulta e local de início do pré-natal.

Sleep disorders, especially insomnia, daytime sleepiness, sleep apnea syndrome and restless legs syndrome are very frequently encountered in patients with chronic renal failure whether or not they undergo renal replacement therapy. The causes of sleep disorders are multifactorial and not only linked to the renal disease itself, but also to its treatment and its associated psychosocial factors. This article discusses the prevalence and physiopathology of the most frequently encountered sleep disorders in chronic renal failure patients, and highlights the actually available therapeutic options.

Metabolic acidosis is a prevalent complication in moderate and late stages of chronic kidney disease (CKD). It is established that the correction of metabolic acidosis may improve metabolic bone disorders and protein degradation in the skeletal muscle, two characteristic complications of patients with advanced CKD. In the last 18 months, three randomized controlled trials have drawn the attention on a novel indication to correct metabolic acidosis in these patients, i.e., halting CKD progression. These data show that sodium bicarbonate, a cheap and easily manageable treatment, may delay the progression of CKD and the need of a renal replacement therapy such as dialysis or kidney transplantation.

Genetic background, prenatal and post-natal early-life conditions influence the development of interconnected physiological systems and thereby shape the phenotype. Certain combinations of genotypes and pre- and post-natal conditions may provide higher fitness in a specific environmental context. Here, we investigated how grey partridges Perdix perdix of two strains (wild and domesticated) cope physiologically with pre- and post-natal predictable vs. unpredictable food supply. Food unpredictability occurs frequently in wild environments and requires physiological and behavioural adjustments. Well-orchestrated and efficient physiological systems are presumably more vital in a wild environment as compared to captivity. We thus predicted that wild-strain grey partridges have a stronger immunity, glucocorticoid (GC) stress response and oxidative stress resistance (OSR) than domesticated birds, which have undergone adaptations to captivity. We also predicted that wild-strain birds react more strongly to environmental stimuli and, when faced with harsh prenatal conditions, are better able to prepare their offspring for similarly poor post-natal conditions than birds of domesticated origin. We found that wild-strain offspring were physiologically better prepared for stressful situations as compared to the domesticated strain. They had a high GC stress response and a high OSR when kept under predictable food supply. Wild-strain parents reacted to prenatal unpredictable food supply by lowering their offspring's GC stress response, which potentially lowered GC-induced oxidative pressure. No such pattern was evident in the domesticated birds. Irrespective of strain and prenatal feeding scheme, post-natal unpredictable food supply boosted immune indices, and GC stress response was negatively related to antibody response in females and to mitochondrial superoxide production. Wild-strain grey partridge showed fitness-relevant physiological advantages and appeared to prepare their offspring for the prospective environment. Negative relationships between GC stress response, immunity and oxidative indices imply a pivotal role of an organism's oxidative balance and support the importance of considering multiple physiological systems simultaneously.

BACKGROUND AND STUDY AIMS: This was an observational, non-interventional, multicenter, phase IV study, in patients with genotype 1/4/5/6 chronic hepatitis C (CHC). The primary objectives were to evaluate SVR in patients with no or minimal fibrosis (METAVIR F0-F1) versus well established fibrosis (F2-F4), and to estimate response on Weeks 12, 24 and 48 on treatment in previously untreated patients with genotypes 1/4/5/6 CHC. PATIENTS AND METHODS: 538 patients treated with pegylated interferon alfa 2b 1.5 mcg/kg in combination with ribavirin 800-1200 mg/day were enrolled in 55 sites in Belgium and Luxembourg, 505 being considered for the analysis. 40% of the patients were female and 60% male, the average age was 47.5 years, 10.5% were 65 or older. RESULTS: SVR was observed in 35% of the patients, EVR in 68%, of which pEVR in 33% and cEVR in 35%. SVR was observed in 43% of the low fibrosis group (F0, F1) and 30% of the high fibrosis group (F2, F3, F4) (p = 0.005). SVR rates were 34% for genotype 1, 37% for genotype 4, and 47% for genotype 5 (NS). Multivariate analysis showed that EVR and baseline METAVIR score are independent prognostic factors for SVR. CONCLUSIONS: This trial confirms that fibrosis stage and early viral response are the most important key-factors to predict sustained response, suggesting that the earlier patients are treated, the better the outcome. Non-invasive techniques enable us to closely monitor progression of fibrosis, allowing a better selection of patients for antiviral treatment in the DAA-era.

In the last 15 years, the therapeutical options for the treatment of chronic inflammatory diseases in rheumatology have increased a lot. Nevertheless, some patients do not respond or respond partially to the current therapies--including to the biologics therapy. Tofacitinib (Xeljanz) is now on the Swiss market. It inhibits the JAK pathway. Tofacitinib--as monotherapy or with methotrexate--improves the control of rheumatoid arthritis (RA). In a comparative study, tofacitinib was as effective as adalimumab. Further, tofacitinib reduced structural damages in RA and is considered as an alternative, in case of non-response, to anti-TNF and probably to other biologics therapy. The side effects are upper respiratory tract and opportunist infections and tuberculosis. Blood count, lipids, kidney function, liver tests, CK and blood pressure have to be monitored.

In this study, the influence of paternal involvement in caregiving on infant sociability was assessed using a strange situation paradigm adapted from the work of Ainsworth, Blehar, Waters, and Wall (1978). Thirty-seven families participated with their firstborn child (aged 12-14 months). According to the questionnaire, 20 nontraditional fathers were very involved in caretaking, and 17 fathers were less or not at all involved. Infant sociability was assessed using three of Ainsworth et al.'s rating scales: proximity or contact-seeking, avoidance, and distance interaction. Results indicated that infants of nontraditional fathers were globally more sociable with all their partners (father, mother, and stranger) than infants of traditional fathers. Furthermore, results suggested that it was not only the father-infant relationship or infant development which were affected by the amount of paternal involvement in daily caretaking but the family system as a whole. Indeed, infants from nontraditional families appeared to interact equally with their fathers and mothers in direct interaction. In addition, these infants interacted at a distance with a stranger as much in the presence of their fathers as in the presence of their mothers, thereby suggesting that both parents represented an equally secure base.

Elevated serum ferritin levels may reflect a systemic inflammatory state as well as increased iron storage, both of which may contribute to an unfavorable outcome of chronic hepatitis C (CHC). We therefore performed a comprehensive analysis of the role of serum ferritin and its genetic determinants in the pathogenesis and treatment of CHC. To this end, serum ferritin levels at baseline of therapy with pegylated interferon-alpha and ribavirin or before biopsy were correlated with clinical and histological features of chronic hepatitis C virus (HCV) infection, including necroinflammatory activity (N = 970), fibrosis (N = 980), steatosis (N = 886), and response to treatment (N = 876). The association between high serum ferritin levels (> median) and the endpoints was assessed by logistic regression. Moreover, a candidate gene as well as a genome-wide association study of serum ferritin were performed. We found that serum ferritin ≥ the sex-specific median was one of the strongest pretreatment predictors of treatment failure (univariate P < 0.0001, odds ratio [OR] = 0.45, 95% confidence interval [CI] = 0.34-0.60). This association remained highly significant in a multivariate analysis (P = 0.0002, OR = 0.35, 95% CI = 0.20-0.61), with an OR comparable to that of interleukin (IL)28B genotype. When patients with the unfavorable IL28B genotypes were stratified according to high versus low ferritin levels, SVR rates differed by > 30% in both HCV genotype 1- and genotype 3-infected patients (P < 0.001). Serum ferritin levels were also independently associated with severe liver fibrosis (P < 0.0001, OR = 2.67, 95% CI = 1.68-4.25) and steatosis (P = 0.002, OR = 2.29, 95% CI = 1.35-3.91), but not with necroinflammatory activity (P = 0.3). Genetic variations had only a limited impact on serum ferritin levels. Conclusion: In patients with CHC, elevated serum ferritin levels are independently associated with advanced liver fibrosis, hepatic steatosis, and poor response to interferon-alpha-based therapy.

Este estudo qualitativo e exploratório-descritivo objetivou conhecer a percepção de puerpéras sobre atendimento em serviço de pré-natal. Os dados coletados mediante entrevista semiestruturada foram submetidos à análise temática, tendo por sujeitos oito puérperas. O acesso, agendamento das consultas e atendimento prioritário em caso de intercorrências foram destacados como qualificadores da atenção pré-natal. O estudo apontou fragilidades no que tange à integralidade, humanização, acolhimento, ausência de vínculo, uso indiscriminado de tecnologias e intervenções desnecessárias, as quais podem produzir e/ou potencializar situações de vulnerabilidade. Os resultados sinalizam a necessidade de reorganização da atenção no pré-natal e nascimento, sob a lógica da longitudinalidade do cuidado, tanto nos serviços públicos como privados e de pactuação de ações intersetoriais nos modos de promoção da saúde das mulheres e de fomento à formulação de políticas públicas mais equânimes e positivas na perspectiva da integralidade da atenção.

Background: Chronic post-lobectomy empyema is rare but may require space obliteration for infection control. We report our experience by using a tailored thoracomyoplasty for this specific indication with respect to infection control and functional outcome. Patients and Methods: We retrospectively analysed 17 patients (11 men, 6 women) with chronic post-lobectomy empyema and treated by thoracomyoplasty in our institution between 2000 and 2011. All patients underwent an initial treatment attempt by use of chest tube drainage and antibiotics except those with suspicion of pleural aspergillosis (n=6). In 5 patients, bronchus stump insufficiency was identified at preoperative bronchoscopy. A tailored thoracoplasty was combined with a serratus anterior - rhomboid myoplasty which also served to close a broncho-pleural fistula, if present. The first rib was resected in 11/17 patients. Results: The 90-day mortality was 11.7%. Thoracomyoplasty was successful in all surviving patients with respect to infection control, space obliteration and definitive closure of broncho-pleural fistula, irrespective of the type of infection, the presence of a broncho-pleural fistula and whether a 1st rib resection was performed . Post-lobectomy pulmonary function testing before and after thoracoplasty revealed a mean predicted FEV1 of 63.0±8.5% and 51.5±4.2% (p=0.01), and a mean predicted DLCO of 59.8±11.6% and 54.5±12.5%, respectively. Postoperative shoulder girdle dysfunction and scoliosis were prevented in patients willing to undergo intense physiotherapy. Conclusions: Tailored thoracomyoplasty represents a valid option for patients with chronic post-lobectomy empyema without requiring a preceding open window thoracostomy. Space obliteration and infection control was equally obtained with and without first rib resection.

Esta pesquisa objetivou analisar o processo de produção de dados para o SISPRENATAL em Unidades Básicas de Saúde de Cuiabá, MT, Brasil. Um estudo qualitativo, exploratório e descritivo foi desenvolvido em oito unidades da Coordenadoria da Atenção Básica por meio de entrevistas semiestruturadas com profissionais que trabalham com o SISPRENATAL (enfermeiros, médicos, gestores e digitador) e análise documental comparativa entre os dados do sistema e os prontuários. A análise dos dados revelou inexistência de definição da participação da equipe na produção dos dados e modos diversos de preenchimento das fichas do sistema. O conhecimento dos profissionais sobre muitos aspectos das fichas foi divergente, o preenchimento das fichas foi feito de modo inadequado e foram verificadas falhas no sistema informatizado. Medidas como a capacitação dos profissionais, a revisão do sistema e de suas fichas são indispensáveis para a produção de informações fidedignas sobre a assistência pré-natal no município.
