This article discusses a community music project in rural East Timor. Australian musician Gillian Howell lived for three months in the isolated town of Lospalos as an Asialink artist-in-residence, where she worked with local community members and visiting Australian musicians to share music and ideas, and to communicate across cultures. Three activities are described in detail: a songwriting project, a large-scale community music event and a series of informal jam sessions, particularly with respect to the context, teaching and learning models used. An evaluation of the impact of the project on participants, other community members and visiting musicians, indicated that stakeholders valued the project highly for a range of different reasons. These included fun and enjoyment, maintenance of cultural heritage, creative expression, English language learning and cross-cultural exchange. Learnings and recommendations for future similar cross-cultural collaborations include the value of integrating local music traditions with new participatory arts approaches.

Context: Serum 25-hydroxyvitamin D [25(OH)D] concentration has been inversely associated with the prevalence of metabolic syndrome (MetS), but the relationship between 25(OH)D and incident MetS remains unclear.

Objective: We evaluated the prospective association between 25(OH)D, MetS, and its components in a large population-based cohort of adults aged 25 yr or older.

Design: We used baseline (1999–2000) and 5-yr follow-up data of the Australian Diabetes, Obesity, and Lifestyle Study (AusDiab).

Participants: Of the 11,247 adults evaluated at baseline, 6,537 returned for follow-up. We studied those without MetS at baseline and with complete data (n = 4164; mean age 50 yr; 58% women; 92% Europids).

Outcome Measures: We report the associations between baseline 25(OH)D and 5-yr MetS incidence and its components, adjusted for age, sex, ethnicity, season, latitude, smoking, family history of type 2 diabetes, physical activity, education, kidney function, waist circumference (WC), and baseline MetS components.

Results: A total of 528 incident cases (12.7%) of MetS developed over 5 yr. Compared with those in the highest quintile of 25(OH)D (≥34 ng/ml), MetS risk was significantly higher in people with 25(OH)D in the first (<18 ng/ml) and second (18–23 ng/ml) quintiles; odds ratio (95% confidence interval) = 1.41 (1.02–1.95) and 1.74 (1.28–2.37), respectively. Serum 25(OH)D was inversely associated with 5-yr WC (P < 0.001), triglycerides (P < 0.01), fasting glucose (P < 0.01), and homeostasis model assessment for insulin resistance (P < 0.001) but not with 2-h plasma glucose (P = 0.29), high-density lipoprotein cholesterol (P = 0.70), or blood pressure (P = 0.46).

Conclusions: In Australian adults, lower 25(OH)D concentrations were associated with increased MetS risk and higher WC, serum triglyceride, fasting glucose, and insulin resistance at 5 yr. Vitamin D supplementation studies are required to establish whether the link between vitamin D deficiency and MetS is causal.

Background: Lower socioeconomic status (SES) is strongly associated with a higher prevalence of major cardiovascular risk factors, but few studies have examined changes in these risk factors over time according to SES. We aimed to determine whether SES is a predictor of the change in cardiovascular risk factor levels in a contemporary Australian adult cohort

Methods: Participants in the population-based AusDiab study aged 25+ years who attended both baseline and 5-year follow-up examinations (n=5 954) were categorised according to their level of education at baseline. Cardiovascular risk factor data at both time points were ascertained through questionnaire and physical measurement. Analysis was stratified by gender.

Results: The mean levels of systolic blood pressure, total cholesterol and the prevalence of smoking decreased between the two time points across all educational categories. Increases were also seen in mean BMI and the prevalence of diabetes. For blood pressure, the smallest decrease was seen among men with lower education (age-adjusted difference from higher education 2.8 mmHg, 95% CI 1.0 to 4.6). For total cholesterol, the decrease was greatest among women with lower education (age-adjusted difference from higher education 0.11 mmol/l, 95% CI 0.19 to 0.02). Among those "not at risk" at baseline for each risk factor, women with lower education were more likely than those with higher education to progress to being "at risk" for BMI (age-adjusted odds ratio 1.60, 95% CI 1.09 to 2.35).

Conclusion: Educational gradients narrowed for total cholesterol in women, but widened for systolic blood pressure in men and remained static for other risk factors. Lower education was also associated with an earlier onset of overweight or obesity in women. Given current socioeconomic gradients in risk factors levels, these findings suggest that social inequalities in CVD will persist and may even widen in the future.

Objective To investigate the longitudinal changes in children's recess and lunchtime physical activity levels and in the contribution of recess and lunchtime to daily physical activity levels over 5 years among 5–6- and 10–12-year olds.

Methods
Data were drawn from two longitudinal studies that were conducted in metropolitan Melbourne, Australia. Boys and girls (n=2782) aged 5–6 years and 10–12 years participated in baseline (T0) measures. Physical activity (n=2490) was measured every 60 s for eight consecutive days using hip-mounted accelerometry. Subsequent measurements were taken at 3-year (T1; n=773) and 5-year (T2; n=634) follow-up. Physical activity intensities were derived using age-adjusted cut-points. Sedentary time was defined as 100 counts/min. Longitudinal data were analysed using three-level (time, child, school) multilevel analyses, stratified by sex and cohort, and adjusted for potential confounding variables.

Results Significant decreases in recess and lunchtime moderate and vigorous physical activity were observed (p<0.001), with larger decreases occurring in the older cohort. Associated increases were observed in sedentary time over time (p<0.01). Although the contribution of recess to daily moderate intensity physical activity increased in the younger cohort over time (p<0.001), significant decreases were observed in the older cohort (p<0.001).

Conclusion Physical activity levels during recess and lunchtime decreased in both cohorts over time. Decreases in the contribution of recess and lunchtime to older children's daily physical activity were also observed. Interventions are needed in both primary and secondary schools to promote physical activity levels during recess and lunchtime, particularly during the early years of secondary school.

This paper discusses results from a design research in line with Realistic Mathematics Education (RME). Daily cycles of design, classroom experiments, and retrospective analysis are enacted in five days of working about division by fractions. Data consists of episodes of video classroom discussions, and samples of students’ work. The focus of discussion and analysis centres on the role of contexts and the role of teachers’ probing questions to elicit students’ thinking. Our findings suggest that contexts that are meaningful for and understandable by students bring out rich mathematical thinking and discussion amongst students. Meaningful contexts combined with teacher’s probing questions - highlighting big mathematical ideas - allow students to attain various approaches at different levels of formal mathematics.

Information Systems is a practice-based discipline. It experiences periodic debates about the rigour and relevance of its research. The tensions between pretensions to be a ‘real’ science (rigour) and the need to contribute to practice (relevance) are intensified at a time of low student enrolment, lack of a clear identity, and uncertainties about the viability of our discipline. This essay argues that decomposing phenomena into narrow topics of research to achieve rigour is damaging to our discipline if we fail to then ‘recompose’ or integrate these back into understanding, lessons and guidelines for application to real-world practices. This argument is illustrated through recent work on the motors that drive changes in technology appropriation. It highlights the importance of plurality of theories and methods in understanding complex real-world phenomena in order to achieve both rigour and relevance.

Background : The development of e-mental health interventions to treat or prevent mental illness and to enhance wellbeing has risen rapidly over the past decade. This development assists the public in sidestepping some of the obstacles that are often encountered when trying to access traditional face-to-face mental health care services. Objective : The objective of our study was to investigate the posttreatment effectiveness of five fully automated self-help cognitive behavior e-therapy programs for generalized anxiety disorder (GAD), panic disorder with or without agoraphobia (PD/A), obsessive–compulsive disorder (OCD), posttraumatic stress disorder (PTSD), and social anxiety disorder (SAD) offered to the international public via Anxiety Online, an open-access full-service virtual psychology clinic for anxiety disorders. Methods : We used a naturalistic participant choice, quasi-experimental design to evaluate each of the five Anxiety Online fully automated self-help e-therapy programs. Participants were required to have at least subclinical levels of one of the anxiety disorders to be offered the associated disorder-specific fully automated self-help e-therapy program. These programs are offered free of charge via Anxiety Online. Results : A total of 225 people self-selected one of the five e-therapy programs (GAD, n = 88; SAD, n = 50; PD/A, n = 40; PTSD, n = 30; OCD, n = 17) and completed their 12-week posttreatment assessment. Significant improvements were found on 21/25 measures across the five fully automated self-help programs. At postassessment we observed significant reductions on all five anxiety disorder clinical disorder severity ratings (Cohen d range 0.72–1.22), increased confidence in managing one’s own mental health care (Cohen d range 0.70–1.17), and decreases in the total number of clinical diagnoses (except for the PD/A program, where a positive trend was found) (Cohen d range 0.45–1.08). In addition, we found significant improvements in quality of life for the GAD, OCD, PTSD, and SAD e-therapy programs (Cohen d range 0.11–0.96) and significant reductions relating to general psychological distress levels for the GAD, PD/A, and PTSD e-therapy programs (Cohen d range 0.23–1.16). Overall, treatment satisfaction was good across all five e-therapy programs, and posttreatment assessment completers reported using their e-therapy program an average of 395.60 (SD 272.2) minutes over the 12-week treatment period. Conclusions : Overall, all five fully automated self-help e-therapy programs appear to be delivering promising high-quality outcomes; however, the results require replication.

International donors are substantially scaling-up aid programmes. At the same time, there are widespread reservations over how much aid recipient countries can use effectively. Such concerns are supported by the aid effectiveness literature which finds that there are limits to the amounts of aid recipients can efficiently absorb. This article demonstrates that a ‘big push’ in foreign aid will not lead to diminishing returns as long as donors get the inter-country allocation of aid right. This is true even if donors provide aid at levels equal to the well-known target of 0.7 per cent of their gross national income
