Purpose: To verify the influence of cavity access diameter on demineralized dentin removal in the ART approach. Methods: 40 non-carious human premolars were randomly divided into four groups. The occlusal surface was ground flat and the teeth were sectioned mesio-distally. The hemi-sections were reassembled and occlusal access preparations were carried out using ball-shaped diamonds. The resulting size of the occlusal opening was 1.0 mm, 1.4 mm, 1.6 mm and 1.8 mm for Groups A, B, C, and D, respectively. Standardized artificial carious lesions were created and demineralized dentin was excavated. After excavation, the cavities were analyzed using: (a) the tactile method, (b) caries-detection dye to stain demineralized dentin, as proposed by Smales & Fang, and (c) Demineralized Tissue Removal index, as proposed in this study. Statistical analysis was performed using Fisher, Spearman correlation coefficient, kappa, Kruskal-Wallis and Miller tests (P < 0.05). Results: The three methods of evaluation showed no significant difference between Groups A vs. B, and C vs. D, while statistically significant differences were observed between Groups A vs. C, A vs. D, B vs. C and B vs. D. Based on the results of this study, the size of occlusal access significantly affected the efficacy of demineralized tissue removal.

Purpose: The aims of this randomized controlled trial were to determine whether there were differences in the disease-free survival (DFS) and toxicity between conventional radiotherapy (CRT) and a continuous 3 week accelerated radiotherapy regimen (ART) in stage III and IV squamous cell carcinoma of the oral cavity, oropharynx, larynx and hypopharynx. Patients and methods: Patients from 14 centres throughout Australia and New Zealand were randomly assigned to either CRT, using a single 2 Gy/day to a dose of 70 Gy in 35 fractions in 49 days or to ART, using 1.8 Gy twice a day to a dose of 59.4 Gy in 33 fractions in 24 days. Treatment allocation was stratified for site and stage. The accrual began in 1991 and the trial was closed in 1998 when the target of 350 patients was reached. Results: The median potential follow-up time was 53 months (range, 14-101). The DFS at 5 years was 41% (95% CI, 33-50%) for ART and 35% (95% CI, 27-43%) for CRT (P = 0.323) and the hazard ratio was 0.87 in favour of ART (95% CI, 0.66-1.15). The 5-year disease-specific survival rates were 40% for CRT and 46% for ART (P = 0.398) and the loco-regional control was 47% for CRT vs. 52% for ART (P = 0.300). The respective hazard ratios were 0.88 (95% CI, 0.65-1.2) and 0.85 (0.62-1.16), favouring the accelerated arm. In the ART arm, confluent mucositis was more severe (94 vs. 71%; P < 0.001) and peaked about 3 weeks earlier than in the CRT arm, but healing appeared complete in all cases. There were statistically significant reductions in the probability of grade 2 or greater late soft tissue effects over time in the ART arm (P < 0.05), except for the mucous membrane where late effects were similar in both arms. Conclusions: Differences in DFS, disease-specific survival and loco-regional control have not been demonstrated. ART resulted in more acute mucosal toxicity, but this did not result in greater prolongation of the treatment time compared with the CRT arm. There were less late effects in the ART arm, with the exception of late mucosal effects. This trial has confirmed that tumour cell repopulation occurs during conventionally fractionated radiotherapy for head and neck cancer. However, it has also provided additional evidence that overall improvements in the therapeutic ratio using accelerated fractionation strategies are seriously constrained by the need to limit total doses to levels that do not exceed acute mucosal tolerance. The accelerated schedule tested has been shown in this trial to be an acceptable alternative to conventionally fractionated irradiation to 70 Gy. (C) 2001 Elsevier Science Ireland Ltd. In situ hybridization to mRNA in embryo sections or wholemount embryos is one of the most powerful analytical tools available to the molecular developmental biologist. For many workers, this procedure provides the first insights into the function of newly isolated genes, allowing the formulation of hypotheses and setting the course for further research. This paper presents a personal historical perspective of the development of in situ hybridization, looks at the theory and practice of the technique, summarizes the current state of the art, and speculates on possible directions for the future as a tool in functional genomics.

The relationship between evidence-based medicine (EBM) and clinical judgement is the subject of conceptual and practical dispute. For example, EBM and clinical guidelines are seen to increasingly dominate medical decision-making at the expense of other, human elements, and to threaten the art of medicine. Clinical wisdom always remains open to question. We want to know why particular beliefs are held, and the epistemological status of claims based in wisdom or experience. The paper critically appraises a number of claims and distinctions, and attempts to clarify the connections between EBM, clinical experience and judgement, and the objective and evaluative categories of medicine. I conclude that to demystify clinical wisdom is not to devalue it. EBM ought not be conceived as needing to be limited or balanced by clinical wisdom, since if its language is translatable into terms comprehensible and applicable to individuals, it helps constitute clinical wisdom. Failure to appreciate this constitutive relation will help perpetuate medical paternalism and delay the adoption of properly evidence-based practice, which would be both unethical and unwise.

O advento da economia da informa????o indica claramente a import??ncia do uso intensivo de m??o-de-obra qualificada no processo de ajuste microecon??mico pelo qual t??m passado as empresas, para fazer frente ?? globaliza????o dos mercados e ?? reestrutura????o produtiva. No plano macro, o controle da infla????o relativizou enormemente o peso (e o valor de mercado) dos executivos financeiros em favor de profissionais das ??reas de produ????o, marketing e recursos humanos. Mesmo promissor nos anos 70 e 80, o campo profissional relacionado com a negocia????o dos interesses entre capital e trabalho est?? encolhendo com o decl??nio do fordismo e o advento da acumula????o flex??vel. Mas estas tend??ncias produziram impactos e continuam repercutindo principalmente no setor privado. Enquanto isso, come??am a se espalhar em um processo de lenta difus??o no ??mbito do setor p??blico

O controle de ag??ncias de defesa da concorr??ncia sobre a estrutura dos mercados constitui pr??tica comum em pa??ses maduros e vem ganhando import??ncia crescente em economias emergentes. Tal atividade envolve exame de atos de concentra????o econ??mica tais como fus??es, aquisi????es, joint ventures ou incorpora????es. No Brasil este tipo de controle ?? recente, tendo sido institu??do pela Lei n?? 8.884, de junho de 1994. Desde ent??o, o Conselho Administrativo de Defesa da Concorr??ncia - CADE julgou 29 (vinte e nove) atos de concentra????o, em contraste com a experi??ncia secular de pa??ses como EUA, Canad?? e Austr??lia e de v??rias d??cadas nas na????es europ??ias depois da Segunda Guerra. A exemplo de v??rias outras ??reas de pol??tica p??blica, tornou-se imperativo agilizar os ??rg??os de defesa da concorr??ncia. Isto decorre, entre outros fatores, do intenso processo de reestrutura????o produtiva em curso no pa??s associados ?? desestatiza????o e abertura da economia. Diante das circunst??ncias descritas, ?? preciso assegurar agilidade, transpar??ncia, excel??ncia t??cnica no processo decis??rio e estabilidade de regras, todos ingredientes indispens??veis para gerar seguran??a jur??dica. Esta ??ltima, por seu turno, diminui o risco do investimento, estimulando invers??es, produ????o e emprego. Nesse sentido, o CADE promoveu mudan??as importantes no procedimento de an??lise de atos de concentra????o em colabora????o com a Secretaria de Direito Econ??mico do Minist??rio da Justi??a (SDE) e a Secretaria de Acompanhamento Econ??mico do Minist??rio da Fazenda (SEAE) mediante a edi????o da Resolu????o 5, de 28 de agosto de 1996

Um novo paradigma emerge, em que o conhecimento e a inova????o s??o as principais fontes de riqueza e tamb??m de diferen??as entre as na????es, as empresas e as pessoas. Era preciso definir a via europ??ia de transi????o para essa economia baseada na inova????o e no conhecimento, com marcas distintas que v??o da salvaguarda da coes??o social e da diversidade cultural ??s pr??prias escolhas tecnol??gicas. Criar uma nova plataforma competitiva ?? fundamental para sustentar o modelo social europeu, que tamb??m dever?? ser renovado.
