El presente trabajo consiste en la selección, análisis y traducción de 8 documentos con especialidad médica publicados por la institución sanitaria NorthShore University Health System. La elección de esta temática en la elaboración de este proyecto está vinculada al hecho de que en la actualidad trabajo como intérprete con especialidad en medicina para esta organización. Durante el año y medio que llevo en este organismo, he podido observar que existe un gran número de artículos, formularios, folletos informativos, consentimientos quirúrgicos y un largo etcétera que no están traducidos al castellano, cuestión que dificulta enormemente la comunicación, entendimiento y funcionamiento de las relaciones entre los pacientes, familiares y el equipo médico. El NorthShore University Health System tiene un Departamento de Interpretación con 12 intérpretes de castellano en plantilla, 3 intérpretes de ruso, 2 intérpretes de polaco, un intérprete de coreano y un intérprete de árabe. Lamentablemente, el NorthShore no posee un Departamento de Traducción para la traducción de los documentos destinados a los pacientes. El Departamento de Interpretación, bajo la dirección de la supervisora Erika Erdbeer, contrata a una agencia de traducción certificada por la American Translators Association para la traducción de estos documentos. Este proceso supone un alto costo para el Departamento de Interpretación en particular, y para la institución sanitaria en general. De igual modo, el volumen de documentos en necesidad de ser traducidos es sustancialmente más elevado que los recursos económicos disponibles para la traducción de los mismos, y esto supone que no haya presupuesto para traducir muchos de los textos esenciales. En términos generales, esta situación va en detrimento de los derechos e intereses de todos aquellos pacientes que tienen un conocimiento limitado del inglés.A su vez, la labor de los intérpretes se hace muy dificultosa debido al hecho de que en numerosas ocasiones tenemos que realizar traducciones a la vista (sight translate) de una variedad de permisos, formularios, documentos legales, hojas de consentimiento y un largo etcétera que deberían estar traducidos al castellano, y que dada la amplia carga de trabajo que tenemos, no podemos emplear el tiempo necesario para ofrecer al paciente una traducción oral de calidad. Por consiguiente, en un intento por mejorar esta situación, y con el propósito de ofrecer a la comunidad hispanoparlante que acude a esta institución unos servicios de calidad, hemos querido diseñar un proyecto basado en la selección y traducción de los documentos médicos más utilizados por los pacientes hispanoparlantes. Para el proceso de traducción hemos utilizado la herramienta de Traducción Asistida SDL Trados Studio 2009, con la intención de crear una memoria de traducción que pueda ser utilizada en futuros proyectos. Las traducciones realizadas en este trabajo serán publicadas y distribuidas en los pertinentes departamentos del NorthShore. Como paso previo a la publicación de estos documentos, Erika Erdbeer, supervisora del Departamento de Interpretación, enviará las traducciones a la agencia de traducción MetaPhrasis para ser corregidas y revisadas. Por motivos de responsabilidad legal, el NorthShore University Health System tiene la obligación de contratar traductores certificados por la American Translators Association. En la actualidad la autora de este proyecto se encuentra en el proceso de sacar dicha certificación, por lo que será necesario enviar las traducciones para que sean corregidas y revisadas a un traductor/a certificado. Tras la revisión y corrección de los documentos el NorthShore University Health System procederá a la publicación y distribución de los mismos tan pronto lo estime conveniente.

S'analitzen de manera comparativa les directrius de biblioteques de presons de l'IFLA, la Library Association (LA) i l'American Library Association (ALA). Després de fer un breu repàs del procés d'elaboració de cadascuna d'aquestes directrius, es comparen les tres propostes dividides en nou apartats: administració, pressupost, personal, localització i condicionament, serveis, col·lecció, catàleg, accessibilitat i promoció. A partir d'aquesta anàlisi, es fa una recomanació sobre quina directriu podria ser el punt de partida per desenvolupar un text similar que es pogués aplicar a les biblioteques de presons de Catalunya.

The recent release of the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) by the American Psychiatric Association has led to much debate. For this forum article, we asked BMC Medicine Editorial Board members who are experts in the field of psychiatry to discuss their personal views on how the changes in DSM-5 might affect clinical practice in their specific areas of psychiatric medicine. This article discusses the influence the DSM-5 may have on the diagnosis and treatment of autism, trauma-related and stressor-related disorders, obsessive-compulsive and related disorders, mood disorders (including major depression and bipolar disorders), and schizophrenia spectrum disorders.

BACKGROUND: Chest pain can be caused by various conditions, with life-threatening cardiac disease being of greatest concern. Prediction scores to rule out coronary artery disease have been developed for use in emergency settings. We developed and validated a simple prediction rule for use in primary care. METHODS: We conducted a cross-sectional diagnostic study in 74 primary care practices in Germany. Primary care physicians recruited all consecutive patients who presented with chest pain (n = 1249) and recorded symptoms and findings for each patient (derivation cohort). An independent expert panel reviewed follow-up data obtained at six weeks and six months on symptoms, investigations, hospital admissions and medications to determine the presence or absence of coronary artery disease. Adjusted odds ratios of relevant variables were used to develop a prediction rule. We calculated measures of diagnostic accuracy for different cut-off values for the prediction scores using data derived from another prospective primary care study (validation cohort). RESULTS: The prediction rule contained five determinants (age/sex, known vascular disease, patient assumes pain is of cardiac origin, pain is worse during exercise, and pain is not reproducible by palpation), with the score ranging from 0 to 5 points. The area under the curve (receiver operating characteristic curve) was 0.87 (95% confidence interval [CI] 0.83-0.91) for the derivation cohort and 0.90 (95% CI 0.87-0.93) for the validation cohort. The best overall discrimination was with a cut-off value of 3 (positive result 3-5 points; negative result <or= 2 points), which had a sensitivity of 87.1% (95% CI 79.9%-94.2%) and a specificity of 80.8% (77.6%-83.9%). INTERPRETATION: The prediction rule for coronary artery disease in primary care proved to be robust in the validation cohort. It can help to rule out coronary artery disease in patients presenting with chest pain in primary care.

Objective To determine the clinical and electrophysiological characteristics of patients with paroxysmal palpitations and neck pounding during sinus rhythm. Methods Clinical, electrocardiographic, and electrophysiological characteristics of six patients with paroxysmal palpitations and neck pounding during sinus rhythm were studied in basal conditions and when symptomatic. Response to treatment was observed. Results Baseline ECGs were normal (four patients) or had first degree atrioventricular block with intermittent PR shortening. During symptoms, narrow QRS rhythms were seen without visible P waves (three patients) or with P waves partially hidden in the QRS complex (three patients). Dual atrioventricular nodal pathways were found in all five patients who had electrophysiological studies. In these patients the slow pathway conduction time was long enough (mean (SD), 425 (121)¿ms) for ventricular activation after slow pathway conduction during sinus rhythm to coincide with the next atrial depolarisation, causing neck pounding during exercise (four patients) or at rest (two patients). Tachycardia was not induced in any patient. Medical treatment aggravated symptoms in three patients. A pacemaker was successfully used in two. Conclusions Neck pounding during sinus rhythm is a clinical manifestation of dual atrioventricular nodal pathways. Medical treatment may aggravate symptoms but a pacemaker may offer definitive relief.

One hundred and eighty nine patients with primary biliary cirrhosis were entered into a double blind, placebo controlled randomised trial starting in January 1978 to assess the therapeutic value of d-penicillamine 1200 mg daily. Eighteen of the 98 patients receiving d-penicillamine and 22 of the 91 placebo treated patients died during the study. Thirty six per cent of those on d-penicillamine and 8% of those on placebo were withdrawn from the study. No difference in overall survival was noted between the two groups of patients whether the results were analysed for the entire period of observation or only during the period in which the patients were receiving therapy. The mortality rate of those receiving d-penicillamine in histological stage I to II, however, was one third of that of the placebo group although this difference did not reach statistical significance. The use of ecological momentary assessment (EMA) for studying parenting has been rare. We examined the psychometric properties and structural validity of an EMA Parenting Scale based on 32 mothers' reports of their parenting over a period of 10 consecutive days, and explored the acceptance of the scale and compliance with the procedure. The results suggested that the EMA Parenting Scale was well accepted for the assessment of daily parenting, and that it consistently captured the overreactive and lax dimensions of parenting across different episodes of child misbehavior. Moreover, multilevel analyses suggested that the scale was sensitive to change across different parenting episodes, and that it reliably assessed the dimensions at both the personal and situational levels. As population ages, a growing number of older patients present the constellation of diabetes and dementia. Numerous recent studies highlight that diabetes may increase the risk for Alzheimer and vascular dementia. Among patients with previous severe hypoglycemia, that risk may even double. Inversely demented patients have about three times higher risk of hypoglycemia. Given that spiral link between hypoglycemia and dementia, the latter should be considered as a possible complication of diabetes and consistently be screened for among older diabetic patients. Furthermore, the American Diabetes Association and American Geriatric Society consensus recommends a more flexible glycemic treatment goal of AIC among demented patients, with a target range between 8 and 9%.

Los antibióticos pueden administrarse de forma terapéutica -en una infección ya establecida-o de forma profiláctica. La necesidad de profilaxis antibiótica en cirugía bucal es un tema controvertido, así como su correcta administración. Debe realizarse únicamente en aquellos procedimientos que supongan un elevado riesgo de infección (extracciones traumáticas del tercer molar o qúe haya sufrido repetidos procesos de pericoronaritis) y en determinados casos de pacientes médicamente comprometidos. El antibiótico de elección es la penicilina, aunque en determinados casos puede usarse algún medicamento alternativo como el metronidazol. En pacientes sanos debe administrarse una dosis preoperatoria doble a la terapéutica y una o dos dosis postoperatorias. En pacientes médicamente comprometidos con riesgo a sufrir una endocarditis bacteriana se seguirán las recomendaciones de la British Society for Antimicrobial Chemoterapy (BSAC) o de la American Heart Association (AHA).

Background Impaired glucose regulation (IGR) is associated with detrimental cardiovascular outcomes such as cardiovascular disease risk factors (CVD risk factors) or intima-media thickness (IMT). Our aim was to examine whether these associations are mediated by body mass index (BMI), waist circumference (waist) or fasting serum insulin (insulin) in a population in the African region. Methods Major CVD risk factors (systolic blood pressure, smoking, LDL-cholesterol, HDL-cholesterol,) were measured in a random sample of adults aged 25-64 in the Seychelles (n=1255, participation rate: 80.2%). According to the criteria of the American Diabetes Association, IGR was divided in four ordered categories: 1) normal fasting glucose (NFG), 2) impaired fasting glucose (IFG) and normal glucose tolerance (IFG/NGT), 3) IFG and impaired glucose tolerance (IFG/IGT), and 4) diabetes mellitus (DM). Carotid and femoral IMT was assessed by ultrasound (n=496). Results Age-adjusted levels of the major CVD risk factors worsened gradually across IGR categories (NFG < IFG/NGT < IFG/IGT < DM), particularly HDL-cholesterol and blood pressure (p for trend <0.001). These relationships were marginally attenuated upon further adjustment for waist, BMI or insulin (whether considered alone or combined) and most of these relationships remained significant. With regards to IMT, the association was null with IFG/NGT, weak with IFG/IGT and stronger with DM (all more markedly at femoral than carotid levels). The associations between IMT and IFG/IGT or DM (adjusted by age and major CVD risk factors) decreased only marginally upon further adjustment for BMI, waist or insulin. Further adjustment for family history of diabetes did not alter the results. Conclusions We found graded relationships between IGR categories and both major CVD risk factors and carotid/femoral IMT. These relationships were only partly accounted for by BMI, waist and insulin. This suggests that increased CVD-risk associated with IGR is also mediated by factors other than the considered markers of adiposity and insulin resistance. The results also imply that IGR and associated major CVD risk factors should be systematically screened and appropriately managed.

The gap junction protein connexin37 (Cx37) plays an important role in cell-cell communication in the vasculature. Cx37 is expressed in endothelial cells, platelets and megakaryocytes. We have recently shown that Cx37 limits thrombus propensity by permitting intercellular signaling between aggregating platelets. Here, we have performed high throughput phage display to identify potential binding partners for the regulatory intracellular C-terminus of Cx37 (Cx37CT). We retrieved 2 consensus binding motifs for Cx37CT: WHK...[K,R]XP... and FH-K...[K,R]XXP.... Sequence alignment against the NCBI protein database indicated 66% homology of one the selected peptides with FVIII B-domain. We performed cross-linking reactions using BS3 and confirmed that an 11-mer peptide of the FVIII B-domain sequence linked to recombinant Cx37CT. In vitro binding of this peptide to Cx37CT was also confirmed by surface plasmon resonance. The dissociation constant of FVIII B-domain peptides to Cx37CT was ~20 uM. Other peptide sequences, designed upstream or downstream of the FVIII B-domain sequence, showed very low or no affinity for Cx37CT. Finally, in vivo studies revealed that thrombin generation in platelet-poor plasma from Cx37-/- mice (endogenous thrombin potential: 634±11 nM min, mean±SEM) was increased compared to Cx37+/+ mice (427±12, P<0.001). Moreover, partial activated thromboplastin time (aPTT) was shorter in Cx37-/- (39.7±1.5 s) than in Cx37+/+ mice (45.9±1.8, P=0.03), whereas prothrombin time was comparable. The shorter aPTT in Cx37-/- mice correlated with higher circulating FVIII activity (46.0±0.7 vs. 53.5±2.7 s for Cx37+/+, P=0.03). Overall, our data show for the first time a functional interaction between FVIII and Cx37. This interaction may be relevant for the control of FVIII secretion and, thereby, in the regulation of levels of FVIII circulating in blood. In addition, these results may open new perspectives to improve the efficiency of recombinant FVIII manufacturing.

Background: C-reactive protein (CRP) is associated with risk of coronary heart disease (CHD). Whether CRP is causally associated with CHD or merely a marker of underlying atherosclerosis is uncertain. Methods: We used a Mendelian randomisation design to investigate the causal relationship of CRP with CHD. We identified three genetic variants in the CRP locus (rs7553007, rs1130864 and rs1205) which influence CRP levels. We tested the three SNPs for association with CHD amongst 28,112 CHD cases and 100,823 controls. We then compared the observed relationship between the SNPs and CHD, with that predicted from the association of SNPs with CRP levels, and of CRP levels with CHD. Results: SNPs in the CRP locus were not associated with CHD: rs7553007, OR 0.98 (95% CI, 0.94-1.01); rs1130864, OR 1.00 (95% CI, 0.86-1.15); rs1205, OR 1.03 (95% CI, 0.99-1.07); combined OR for all three SNPs, 1.00 (95% CI, 0.97-1.02), per 20% lower CRP (figure). In contrast, the predicted OR for CHD from a 20% lower CRP level is 0.94 (95% CI, 0.94- 0.95), based on meta-analysis of observational studies. Conclusions: Though CRP variants are associated with CRP levels, and CRP levels with risk of CHD, we observed that CRP variants are not associated with CHD risk. Our Mendelian randomisation experiment strongly argues against a causal association of CRP with CHD.
