Community acquired methicillin Staphylococcus aureus (CA-MRSA) was first reported in 1981, infecting people without risk factors. These strains harbor the Staphylococcal Cassette Chromosome mec (SCCmec) type IV, which contains the mecA gene codifying for methicillin resistance. CA-MRSA strains usually carry PVL (Panton-Valentine Leukocidin), a virulence factor responsible for tissue invasion, causing systemic infections and leading to serious complications. The aims of this work were to detect the mecA gene, SCCmec characterization and to detect the PVL gene of the S. aureus strains isolated from patients diagnosed with skin and soft tissue infections attending the Dermatology Service, of the Botucatu Medical School, Brazil. Among 127 collected samples, 66 (51.9%) were S. aureus and, from these, 7 (10.6%) harbored mecA gene, 3 (42%) with cassette type IV, and none of these samples carried the PVL gene, but it was detected in 10 (15.1%) samples of S. aureus methicillin sensitive (MSSA). Our study suggests the S. aureus strains that harbor PVL gene and MRSA are present in the community as important pathogens.

The concern with infant mortality has been a priority in public policies, especially for Brazil to achieve the Millennium Development Goal number four: reducing child mortality by 75% by the year 2015. It is known that prematurity has an intimate relationship with mortality of children under one year and therefore it is necessary an effective intervention in risk factors linked to premature births. To evaluate the profile of mothers and newborn babies living in Botucatu-SP, in the period 2001 to 2009, focusing on prematurity. A quantitative study, retrospective, descriptive, epidemiological, from the Information System Newborn Alive. The prevalence of preterm births was 15.1%, and low birth weight 14.7%. Among infants, the Apgar score below 7 at 1 and at 5 minutes was 13.1% and 2.4% respectively. Considering the total of pregnant women studied, 20.3% were adolescents and 10.3% were aged 35 years or more and most (63.2%) had eight or more years of school approval. The profile of mothers of premature infants shows that 23.6% were teenagers, 14.1% had at least 35 years and 60.4% had at least primary education. With regard to premature deliveries, caesarean sections and 58.4% were between the total newborns, this type of delivery was achieved in 46.6% of cases. Prematurity was associated with an Apgar score below seven in the first and fifth minutes of life, presence of malformation and multiple pregnancy. Among maternal variables, prematurity was associated with extremes of maternal age, maternal education, history of stillbirth, primiparas, white and presence of the mother's partner. With regard to delivery was associated with cesarean section and outside the hospital. This study assessed maternal characteristics associated with prematurity and point out how relevant the pregnancies at the extremes of age and those that occur in women with low education level, indicating the interference... The arterial hypertension is a chronic disease, which can be controlled by changing the way of life, as well as by drug treatment, which demand specific Health Care sequence. The lack of adherence to sequence/treatment is one of the main obstacles the disease control. Characterize and analyze the profile of Health Care usage by a 192 patient cohort diagnosed with arterial hypertension in 1995, between the period of 2001 – 2005 and 2006 – 2010. It is a longitudinal study, retrospective and descriptive developed on School Health Center(SHC) which belongs to School of Medicine Botucatu –UNESP, in continuity of the previous research which has analyzed the sequence of the referred sample between the period of 1995 – 1999. The database was obtained from the patients records by using structured adapted forms appointed in the previous study phase. In the case there were transfers to other Health Care facilities, the database was obtained by the records either, while the patients attended the CSE. The database was analyzed by means of descriptive statistics. Predominated the patients in the age from 50 – 69 (47,9%), whites (93,2%), female (56,7%) with low level of education (72,7%). In the period of 2001 - 2005, 76 (39,5%) of the patients remained under sequence, and that 44 (22,9%) belonged to adherence group (GAD), 17 belonged to abandonment/adherent group (GAB/GAD) and 15 to the abandonment group (GAB), groups which were already identified by the study which has analyzed the period of 1995 – 1999. At the end of the third period of the sample sequence (2006 – 2010), 60 (31,2%) of the patients kept under medical sequence. The cohort’s mortality rate in the period reached 15,1% and 21,9% were transferred to other Municipal Health Care facilities. Throughout the analysis of the collections JJ, CEVAP, IBSP, ZUEC and UEL, a study on the ophiofauna of Botucatu was made. We present a preliminary list of the species of snakes here found, with small comments on their natural history. The number of specimens studied was 943. We registered, for Botucatu, 51 species of snakes, distributed in 31 genera and 6 families. The families here found were Anomalepididae (1 spp.), Boidae (1 spp.), Colubridae (7 spp.), Dipsadidae (32 spp.), Elapidae (3 spp.) and Viperidae (7 spp.). The analysis of the relative abundance shows that Dipsadidae was the most abundant family, with n=425 (44,83%), followed by Viperidae, with n=388 (40,93%), Boidae, n=62 (6,54%), Colubridae, n=57 (6,01%), Elapidae, n=15 (1,6%) and, at last, Anomalepididae, with n=1 (0,1%). The five more representative species were Crotalus durissus (n=135, 14,31%), Oxyrhopus guibei (n=123, 12,8%) Bothropoides jararaca (N=121, 12,6%), Bothropoides neuwiedi (N=95, 9,88%) and Sibynomorphus mikani (N=65, 6,76%). A higher number of individuals collected was registered for the months of january to april that, together, sum up to almost 50% of the total. The months of june to september registered a lower number of individuals. The spacial distribution analysis shows that a higher number of snakes was found on the country areas of Botucatu (n=270, 41,54%), specially on pastures. Due to the lack of studies of Botucatu’s ophiofauna, this list is probably underestimated. It is mandatory that future studies approaching this group and its ecological components on this region are made, using appropriate sampling methodologies, in order to form an accurate list of the species of snakes of Botucatu

Objectives: The aim of this study is to report on the treatment of mandibular Class II furcation defects with enamel matrix protein derivative (EMD) combined with a beta TCP/HA (beta-tricalcium phosphate/hydroxyapatite) alloplastic material. Method and Materials: Thirteen patients were selected. All patients were nonsmokers, systemically healthy, and diagnosed with chronic periodontitis; had not taken medications known to interfere with periodontal tissue health and healing; presented one Class II mandibular furcation defect with horizontal probing equal to or greater than 4 mm at buccal site. The clinical parameters evaluated were probing depth (PD), relative gingival margin position (RGMP), relative vertical clinical attachment level (RVCAL), and relative horizontal clinical attachment level (RHCAL). A paired Student t test was used to detect differences between the baseline and 6-month measurements, with the level of significance of .05. Results: After 6 months, the treatment produced a statistically significant reduction in PD and a significant gain in RVCAL and RHCAL, but no observable change in RGMP. RVCAL ranged from 13.77 (+/- 1.31) at baseline to 12.15 (+/- 1.29) after 6 months, with a mean change of -1.62 +/- 1.00 mm (P<.05). RHCAL ranged from 5.54 (+/- 0.75) to 2.92 (+/- 0.92), with a mean change of -2.62 +/- 0.63 mm (P<.05). After 6 months, 76.92% of the patients improved their diagnosis to Class I furcation defects while 23.08% remained as Class II. Conclusion: The present study has shown that positive clinical results may be expected from the combined treatment of Class II furcation defects with EMD and beta TCP/HA, especially considering the gain of horizontal attachment level. Despite this result, controlled clinical studies are needed to confirm our outcomes.

Background: Depression and anxiety are psychiatric disorders that are related to tobacco use and associated with dependence, the process of cessation, lapses and relapses after quitting smoking. Objective: To analyze the association of nicotine dependence with the level of anxiety and depression in patients who are in the process of smoking cessation. Methods: Cross-sectional study conducted with patients who sought the smoking cessation program in Cuiabá/MT. All the smokers enrolled from May to August 2012, participated in this study. Four instruments were applied: Socio-demographic Questionnaire, Fagerstrom test, Beck Anxiety Inventory and Beck Depression Inventory. Following bivariate analysis, using the crude prevalence ratio, with level of significance lower than 5% (p < 0.05), having as variable outcome the nicotine dependence >= 5 (Fagerstrom). The associations with p < 0.20 were selected for robust Multiple Poisson Regression (RPa). Results: Associations of Fagerstrom >= 5 with the male sex (RP = 1.15 CI 95% 1.03-1.28); number of cigarettes/day (RP = 1,33; CI 95% 1.19-1.48); and moderate/severe level of depression (RP = 1.15; CI 95% 1.04-1.28) were found. In the final model (RPa), the following variables remained associated: male gender (RPa 1.12; CI 95% 1.01-1.24), number of cigarettes/day (RPa 1.28; CI 95% 1.15-1.43) and high level of depression (RPa 1.12; CI 95% 1.01-1.23). Discussion: High level of nicotine dependence was associated significantly with the level of depression, emphasizing the association between smoking and psychiatric comorbidities.

BACKGROUND AND OBJECTIVES: Vaginal birth delivery may result in acute and persistent perineal pain postpartum. This study evaluated the association between catastrophizing, a phenomenon of poor psychological adjustment to pain leading the individual to magnify the painful experience making it more intense, and the incidence and severity of perineal pain and its relationship to perineal trauma. METHOD: Cohort study conducted with pregnant women in labor. We used the pain catastrophizing scale during hospitalization and assessed the degree of perineal lesion and pain severity in the first 24 hours and after 8 weeks of delivery using a numerical pain scale. RESULTS: We evaluated 55 women, with acute pain reported by 69.1%, moderate/severe pain by 36.3%, and persistent pain by 14.5%. Catastrophizing mean score was 2.15 ± 1.24. Catastrophizing patients showed a 2.90 relative risk (RR) for perineal pain (95% CI: 1.08-7.75) and RR: 1.31 for developing persistent perineal pain (95% CI: 1.05-1.64). They also showed a RR: 2.2 for developing acute and severe perineal pain (95% CI: 1.11-4.33). CONCLUSIONS: The incidence of acute and persistent perineal pain after vaginal delivery is high. Catastrophizing pregnant women are at increased risk for developing acute and persistent perineal pain, as well as severe pain. Perineal trauma increased the risk of persistent perineal pain.
