OBJECTIVE: To evaluate the prognostic value of postoperative concentration of carcinoembryonic antigen (CEA) and extent of surgical margins after resection of liver metastases from colorectal cancer. DESIGN: Retrospective study. SETTING: Teaching hospital, Switzerland. SUBJECTS: 49 patients with hepatic metastases after primary colorectal cancer. INTERVENTIONS: Resection of hepatic metastases MAIN OUTCOME MEASURES: Assessment of prognostic value of variables by univariate and multivariate analysis. RESULTS: Median survival was 24 months (range 5-86 months). Resection margins were clear (> 1-cm) in 10, close (< 1-cm) in 25 and invaded in 9 patients. On univariate analysis, a postoperative concentration of CEA of <4ng/ml was correlated with prolonged survival (p < 0.001), but the width of the resection margin was not of prognostic importance. There was no correlation between width of resection margins and postoperative concentration of CEA (p = 0.5). On multivariate analysis, postoperative concentrations of CEA of 4 ng/ml or more were associated with increased risk of death (relative risk 7.3; 95% confidence interval (CI) 2.8-18.7, p < 0.001). CONCLUSION: Postoperative CEA offers better prognostic discrimination than the width of resection margins after resection of liver metastases from colorectal tumours. Some patients with invaded resection margins did survive for 3 years, but no patient did whose CEA concentration was 4 ng/ml or more. The definition of a potentially curative hepatic resection should include a postoperative CEA concentration of <4 ng/ml (within the reference range).

Replacement of the hyperimmune anti-Rhesus (Rh) D immunoglobulin, currently used to prevent haemolytic disease of the newborn, by fully recombinant human anti-RhD antibodies would solve the current logistic problems associated with supply and demand. The combination of phage display repertoire cloning with precise selection procedures enables isolation of specific genes that can then be inserted into mammalian expression systems allowing production of large quantities of recombinant human proteins. With the aim of selecting high-affinity anti-RhD antibodies, two human Fab libraries were constructed from a hyperimmune donor. Use of a new phage panning procedure involving bromelin-treated red blood cells enabled the isolation of two high-affinity Fab-expressing phage clones. LD-6-3 and LD-6-33, specific for RhD. These showed a novel reaction pattern by recognizing the D variants D(III), D(IVa), D(IVb), D(Va), D(VI) types I and II. D(VII), Rh33 and DFR. Full-length immunoglobulin molecules were constructed by cloning the variable regions into expression vectors containing genomic DNA encoding the immunoglobulin constant regions. We describe the first, stable, suspension growth-adapted Chinese hamster ovary (CHO) cell line producing a high affinity recombinant human IgG1 anti-RhD antibody adapted to pilot-scale production. Evaluation of the Fc region of this recombinant antibody by either chemiluminescence or antibody-dependent cell cytotoxicity (ADCC) assays demonstrated macrophage activation and lysis of red blood cells by human lymphocytes. A consistent source of recombinant human anti-RhD immunoglobulin produced by CHO cells is expected to meet the stringent safety and regulatory requirements for prophylactic application.

While interleukin (IL)-1β plays an important role in combating the invading pathogen as part of the innate immune response, its dysregulation is responsible for a number of autoinflammatory disorders. Large IL-1β activating platforms, known as inflammasomes, can assemble in response to the detection of endogenous host and pathogen-associated danger molecules. Formation of these protein complexes results in the autocatalysis and activation of caspase-1, which processes precursor IL-1β into its secreted biologically active form. Inflammasome and IL-1β activity is required to efficiently control viral, bacterial and fungal pathogen infections. Conversely, excess IL-1β activity contributes to human disease, and its inhibition has proved therapeutically beneficial in the treatment of a spectrum of serious, yet relatively rare, heritable inflammasomopathies. Recently, inflammasome function has been implicated in more common human conditions, such as gout, type II diabetes and cancer. This raises the possibility that anti-IL-1 therapeutics may have broader applications than anticipated previously, and may be utilized across diverse disease states that are linked insidiously through unwanted or heightened inflammasome activity.

During thymus development, the TCR beta locus rearranges before the TCR alpha locus. Pairing of productively rearranged TCR beta-chains with an invariant pT alpha chain leads to the formation of a pre-TCR and subsequent expansion of immature pre-T cells. Essentially nothing is known about the TCR V beta repertoire in pre-T cells before or after the expression of a pre-TCR. Using intracellular staining, we show here that the TCR V beta repertoire is significantly biased at the earliest developmental stage in which VDJ beta rearrangement has occurred. Moreover (and in contrast to the V(H) repertoire in immature B cells), V beta repertoire biases in immature T cells do not reflect proximity of V beta gene segments to the DJ beta cluster, nor do they depend upon preferential V beta pairing with the pT alpha chain. We conclude that V gene repertoires in developing T and B cells are controlled by partially distinct mechanisms.

CD4(+) alpha beta T cells from either normal C57BL/6 (B6) or MHC-II-deficient (A alpha(-/-) or A beta(-/-)) B6 donor mice engrafted into congenic immunodeficient RAG1(-/-) B6 hosts induced an aggressive inflammatory bowel disease (IBD). Furthermore, CD4(+) T cells from CD1d(-/-) knockout (KO) B6 donor mice but not those from MHC-I(-/-) (homozygous transgenic mice deficient for beta(2)-microglobulin) KO B6 mice induced a colitis in RAG(-/-) hosts. Abundant numbers of in vivo activated (CD69(high)CD44(high)CD28(high)) NK1(+) and NK1(-) CD4(+) T cells were isolated from the inflamed colonic lamina propria (cLP) of transplanted mice with IBD that produced large amounts of TNF-alpha and IFN-gamma but low amounts of IL-4 and IL-10. IBD-associated cLP Th1 CD4(+) T cell populations were polyclonal and MHC-II-restricted when derived from normal B6 donor mice, but oligoclonal and apparently MHC-I-restricted when derived from MHC-II-deficient (A alpha(-/-) or A beta(-/-)) B6 donor mice. cLP CD4(+) T cell populations from homozygous transgenic mice deficient for beta(2)-microglobulin KO B6 donor mice engrafted into RAG(-/-) hosts were Th2 and MHC-II restricted. These data indicate that MHC-II-dependent as well as MHC-II-independent CD4(+) T cells can induce a severe and lethal IBD in congenic, immunodeficient hosts, but that the former need the latter to express its IBD-inducing potential.

It has been suggested that primate mating and social behaviours may be influenced by variation in promoter region repetitive DNA of the vasopressin receptor 1a gene (avpr1a). We show that male mating behaviour does not covary in a simple way with promoter repetitive DNA in 12 Old World primates. We found that one microsatellite (-553 bp upstream) was present in all species, irrespective of their behaviour. By contrast, two microsatellites (-3956 and -3625 bp upstream) were present only in some species, yet this variation did not correlate with behaviour. These findings agree with a recent comparative analysis of voles and show that the variation in repetitive DNA in the avpr1a promoter region does not generally explain variation in male mating behaviour. Phylogenetic analysis revealed a GAGTA motif that has been independently deleted three times and involved in another larger deletion. Importantly, the presence/absence of this GAGTA motif leads to changes in predicted transcription factor-binding sites. Given the repeated loss of this motif, we speculate that it might be of functional relevance. We suggest that such non-repetitive variation, either in indels or in sequence variation, are likely to be important in explaining interspecific variation in avpr1a expression.

Hypoglycemia is a potentially serious complication of insulin therapy. Some insulin-dependent diabetic patients can benefit from continuous subcutaneous insulin infusion therapy (an "insulin pump"), which in most case improves glycemia control and decreases the occurrence of hypoglycemic episodes. However, such events may occur, particularly during initial treatment phases or pregnancy. Severe hypoglycemia is mainly managed by stopping the insulin pump and insuring an adequate carbohydrate intake. Patients with insulin pumps and their entourage should receive specific instruction in the adjustment of pump flow in the presence of dysglycemia-inducing circumstances (illness, physical exertion), as well as in anticipation of high-risk situations, such as motor-vehicle driving.

Distintos trabajos han analizado la relevancia del desajuste educativo y de sus consecuencias sobre los trabajadores que la padecen. Dicho análisis es especialmente importante en el caso de España, ya que presenta uno de los porcentajes de sobreeducación más elevado de los países de la OCDE. Un aspecto que, sin embargo, no ha sido estudiado hasta el momento y que tiene un claro interés en el contexto de la economía de la educación es el posible efecto intergeneracional del desajuste educativo. El objetivo del trabajo consiste en analizar si el desajuste educativo de los padres genera algún efecto desincentivador sobre la educación de sus hijos. En concreto, se analiza si el desajuste educativo de los padres afecta a los resultados educativos de los hijos. A partir de los microdatos de la encuesta PISA para España referidos al año 2009. Dicha encuesta facilita información detallada sobre la formación de los alumnos de 15 años en las materias de matemáticas, ciencia y lengua, sus características personales y la de su entorno escolar y familiar lo que la hace idónea para llevar a cabo el estudio planteado. Los resultados obtenidos muestran que los estudiantes con progenitores sobreeducados tienen una penalización en su rendimiento académico en las tres materias analizadas, siendo ésta más intensa para los estudiantes con peores resultados educativos.

El proyecto de investigación La Teoría de la Organización aplicada a la acción tutorial del Prácticum, tiene como principal objetivo describir la importancia que la acción tutorial y el modelo de organización adquieren sobre la formación académica, personal y profesional de los estudiantes de Magisterio de la Facultad de Educación de la Universitat Internacional de Catalunya, todo ello considerando el nuevo Espacio Europeo de Educación Superior (EEES). Nuestra hipótesis de trabajo supone que la relación tutorial organizada y definida en términos institucionales, se adapta mejor al Prácticum de los nuevos estudios de grado de Magisterio, puesto que supone considerar la función tutorial de manera interinstitucional y requiere del trabajo en equipo de distintos profesionales tanto de manera presencial como en red. La presentación del proyecto hace patente la necesidad de profundizar en el estado de la cuestión y los antecedentes que constituirán los diferentes marcos teóricos. Por ello, aparte de recoger la experiencia vivida desde la coordinación y la tutorización del Prácticum, se analizan en primer lugar la teoría de la organización, en segundo lugar los referentes legislativos que preceden los cambios en el sistema educativo universitario y especialmente los vinculantes al Prácticum de los estudios de Grado de Magisterio, en tercer lugar se describe la importancia de la asignatura del prácticum y su tutorización en el proceso de formación de los futuros docentes y finalmente se hace referencia a los modelos de relación existentes entre las universidades y los centros educativos, para considerar nuevas estrategias que optimicen el proceso de formación que supone el Prácticum. El aprendizaje servicio APS, se presenta al final como modelo y estrategia de colaboración, y como metodología de enseñanza y aprendizaje que se adapta al modelo organizativo propuesto integrando el servicio a la comunidad con el aprendizaje.

The systemic treatment of kidney cancers is promising regarding the first results of the inhibiting molecules of the angiogenesis. Projections in research are encouraging for more specific and sensitive markers of the prostate cancer. For this last the intermittent hormonotherapy improves the quality of life of the patients. The overweight control in infertility allows greater chances of giving birth. The morbidity of the kidney percutaneous surgery is decreased by the use of smaller tools. Reduction rate of reobstruction thanks to new manufactoring stents. The botulinic toxin for the hyperactive bladder refunded by the health insurances.

Trabajo de investigación realizado en desarrollo de los estudios del Doctorado en Educación y Sociedad. Identifica tres etapas de la formación ambiental superior en el mundo y en Colombia. Demuestra que la formación ambiental superior a nivel internacional surge a finales de la década de los cuarenta y pocos años después en este país. Plantea que en el ámbito mundial surge como recomendaciones de agencias y reuniones intergubernamentales y a nivel interno como programas de formación profesional universitaria. Reconstruye un inventario completo de los primeros programas de este tipo en Colombia que se debería levantar también en otros países

Eladeb est un instrument d'auto-évaluation des difficultés psychosociales et des besoins d'aide de patients psychiatriques. Basé sur une procédure de tri de cartes figuratives, l'outil se démarque des questionnaires et entretiens structurés présents dans l'évaluation psychométrique clinique, en proposant une activité ludique qui laisse aussi un large espace de dialogue pour l'expression qualitative des difficultés et des besoins. Les expériences conduites sur le terrain montrent qu'il est bien accepté par les patients et les professionnels. Après une brève présentation de l'instrument, cet article expose une vignette clinique ainsi que le témoignage de son utilisation dans le cadre d'une fonda~ion spécialisée dans le suivi de patients psychiatriques.
