Mechanically ventilated patients in hospitals are subjected to an increased risk of acquiring nosocomial pneumonia that sometimes has a lethal outcome. One way to minimize the risk could be to make the surfaces on endotracheal tubes antibacterial. In this study, bacterial growth was inhibited or completely prevented by silver ions wet chemically and deposited onto the tube surface. Through the wet chemical treatment developed here, a surface precipitate was formed containing silver chloride and a silver stearate salt. The identity and morphology of the surface precipitate was studied using x-ray photoelectron spectroscopy, Fourier transform infrared spectroscopy, scanning electron microscopy, and x-ray powder diffraction. Leaching of silver ions into solution was examined, and bacterial growth on the treated surfaces was assayed using Pseudomonas aeruginosa wild type (PAO1) bacteria. Furthermore, the minimum inhibitory concentration of silver ions was determined in liquid- and solid-rich growth medium as 23 and 18 microM, respectively, for P. aeruginosa.

The degradation of fatty acids having cis- or trans-unsaturated bond at an even carbon was analyzed in Saccharomyces cerevisiae by monitoring polyhydroxyalkanoate production in the peroxisome. Polyhydroxyalkanaote is synthesized by the polymerization of the beta-oxidation intermediates 3-hydroxy-acyl-CoAs via a bacterial polyhydroxyalkanoate synthase targeted to the peroxisome. The synthesis of polyhydroxyalkanoate in cells grown in media containing 10-cis-heptadecenoic acid was dependent on the presence of 2,4-dienoyl-CoA reductase activity as well as on Delta3,Delta2-enoyl-CoA isomerase activity. The synthesis of polyhydroxyalkanoate from 10-trans-heptadecenoic acid in mutants devoid of 2,4-dienoyl-CoA reductase revealed degradation of the trans fatty acid directly via the enoyl-CoA hydratase II activity of the multifunctional enzyme (MFE), although the level of polyhydroxyalkanoate was 10-25% to that of wild type cells. Polyhydroxyalkanoate produced from 10-trans-heptadecenoic acid in wild type cells showed substantial carbon flux through both a reductase-dependent and a direct MFE-dependent pathway. Flux through beta-oxidation was more severely reduced in mutants devoid of Delta3,Delta2-enoyl-CoA isomerase compared to mutants devoid of 2,4-dienoyl-CoA reductase. It is concluded that the intermediate 2-trans,4-trans-dienoyl-CoA is metabolized in vivo in yeast by both the enoyl-CoA hydratase II activity of the multifunctional protein and the 2,4-dienoyl-CoA reductase, and that the synthesis of the intermediate 3-trans-enoyl-CoA in the absence of the Delta3,Delta2-enoyl-CoA isomerase leads to the blockage of the direct MFE-dependent pathway in vivo.

A survey was undertaken among Swiss occupational health and safety specialists in 2004 to identify uses, difficulties, and possible developments of exposure models. Occupational hygienists (121), occupational physicians (169), and safety specialists (95) were surveyed with an in depth questionnaire. Results obtained indicate that models are not used very much in practice in Switzerland and are reserved to research groups focusing on specific topics. However, various determinants of exposure are often considered important by professionals (emission rate, work activity), and in some cases recorded and used (room parameters, operator activity). These parameters cannot be directly included in present models. Nevertheless, more than half of the occupational hygienists think that it is important to develop quantitative exposure models. Looking at research institutions, there is, however, a big interest in the use of models to solve problems which are difficult to address with direct measurements; i. e. retrospective exposure assessment for specific clinical cases and prospective evaluation for new situations or estimation of the effect of selected parameters. In a recent study about cases of acutepulmonary toxicity following water proofing spray exposure, exposure models have been used to reconstruct exposure of a group of patients. Finally, in the context of exposure prediction, it is also important to report about a measurement database existing in Switzerland since 1991. [Authors]

Contexte Le plasmocytome isolé osseux est une tumeur maligne rare des cellules plasmocytaires. Les données issues de la littérature ne permettent pas de se déterminer sur la dose radiothérapeutique optimale. Dans cette perspective nous avons conduit une vaste étude rétrospective dans le but d'évaluer l'évolution, les facteurs pronostiques aunsi que la dose radiothérapeutique optimale chez les patients présentant un plasmocytome isolé. Méthodes Nous avons réunis les données de 206 patients présentant un plasmocytome isolé osseux sans évidence de myélome multiple. Chaque cas a été documenté histopathologiquement. La majorité des patients (n=169) ont été traités par radiothérapie seule, 32 par une combinaison radiothérapie-chimiothérapie, et 5 par chirurgie. La durée de suivi médiane fut de 54 mois (7-245) Résultats A 5 ans, la survie globale est de 70%, la survie sans maladie de 46% et le contrôle local de 88%. La durée médiane de développement vers une myélome multiple est de 21 mois (2-135) avec une probabilité à 5 ans de 51 %. Les analyses multivariées indiquent l'âge (<60 ans) et la taille de la tumeur (<5cm) comme facteur favorables pour survie. L'âge (<60ans) se dégage comme facteur favorable pour la survie sans maladie. La localisation de la tumeur (vertébrale vs autre) indique la probabilité de contrôle local. L'âge plus avancé (>60 ans) est le seul prédicteur de myélome multiple. Aucune relation dose-réponse n'est mise en évidence pour les doses supérieures à 30 Gy, même pour lés tumeurs les plus étendues. Conclusions Les patients les plus jeunes, principalement ceux présentant une localisation vertébrale, présentent la meilleure évolution sous traitement radiothérapeutique modéré. La progression vers le myélome multiple reste le problème thérapeutique principal. Les futures investigations devraient se focaliser sur les chimiothérapies adjuvantes ainsi que sur les nouveaux agents thérapeutiques.

Purpose: To identify patterns of initially pain freedom response in patients with classical trigeminal neuralgia (CTN) with Gamma Knife surgery (GKS) and to compare their associated hypoesthesia and recurrence rates. Methods: In this study we analysed only 497 patients treated between July 1992 and November 2010, with a follow-up longer than 1 year, after excluding megadolichobasilar artery and multiple sclerosis related trigeminal neuralgia, as well as the second GKS treatments so as to have only cases with CTN and single radiosurgical treatment. GKS using a Gamma Knife (model B or C or Perfexion) was performed, based on both MRI and computer tomography (CT) targeting. A single 4-mm isocenter was positioned in the cisternal portion of the trigeminal nerve at a median distance of 7.8 mm (range 4.5-14) anteriorly to the emergence of the nerve. A median maximum dose of 85 Gy (range 70-90) was delivered. After empiric methods but also by using a chart with clear cut-off periods of pain free distribution, we were able to divide the initially pain free patients into 3 separate groups: within the first 48 hours, after 48 hours till 30 days and after 30 days, respectively. Results: The median follow- up period was 43.75 months (range 12-174.41). Four hundred and fifty-four patients (91.75%) were initially pain free in a median time of 10 days (range 1-459): 169 (37.2%) became pain free within the first 48 hours (pf<=48 h), compared to 194 (42.8%) between the 3-rd day and the day 30 (pf (>48 h, <=30 d)), inclusively and 91 (20%) patients after 30 days (pf>30d). Differences in terms of postoperative hypoesthesia were found with a p value of 0.014 as follows: the group pf<=48 h had 18 (13.7%) compared to pf (>48 h, <=30 d) with 30 (19%) and pf>30d with 22 (30.5%) patients developing a postoperative GKS hypoesthesia. One hundred and fifty seven (34.4%) patients initially pain free experienced a recurrence with a median delay of 24 months (range 0.62-150.06). There were no statistically significant differences between the three groups concerning recurrence (p value of 0.515). Conclusions: An important number of patients (169 cases, 37.2%) became pain free in the first 48 hours. Hypoesthesia rate was higher within the group becoming pain free after 30 days with a statistically significant difference between the three populations (p= 0.014). Further analysis will eventually help to elucidate the differences observed among groups.

Rapport de SynthèseIntroductionLa recherche des apnées du sommeil est recommandée dans la prise en charge préopératoire des patients obèses chez qui une chirurgie bariatrique est envisagée. Toutefois le type d'examen nécessaire pour la détection des apnées dans cette population reste encore discuté. L'objectif de cette étude était de comparer la sensibilité de l'oxymétrie par rapport à la polygraphie lors du screening préopératoire de l'apnée obstructive du sommeil.MéthodeNous avons analysé rétrospectivement les données d'enregistrement de la polygraphie (moniteur portable de type III) et de l'oxymétrie de 68 patients consécutifs adressés au Centre du sommeil dans le cadre de leur bilan avant chirurgie bariatrique.Nous avons comparé la sensibilité de l'index de désaturation 3% ou 4% (à partir de l'oxymétrie seule) avec l'index d'apnée-hypopnée (à partir de la polygraphie) pour diagnostiquer l'apnée obstructive du sommeil. Les patients ont été réparti en 3 groupes selon la sévérité de leur atteinte: normale (< 10 événements/heure), faible à moyenne (10-30 événements/heure), sévère (>30 événements/heure).RésultatsSi l'on considère l'index d'apnée-hypopnée (polygraphie), la prévalence de l'apnée obstructive du sommeil avec un index d'apnée-hypopnée supérieur à 10 événements par heure était de 57,4% : 16,2% des patients étaient classés comme sévèrement atteints, 41,2% comme faiblement à moyennement atteints et 42.6% comme normaux.Si l'on considère l'index de désaturation 3%, 22,1% des patients étaient classés commes sévères , 47,1% comme faiblement à moyennement atteints et 30,9% comme normaux.Avec un index de désaturation de 4%, 17,6% étaient classés comme sévères, 32,4 % comme faiblement à moyennement atteints et 50% comme normaux.En comparant l'index de désaturation 3% à l'index d'apnée-hypopnée (>10 événements/heure), nous avons obtenu une valeur prédictive négative de 95% pour exclure une apnée obstructive du sommeil et une sensibilité de 100% dans la détection des cas sévères d'apnées obstructives du sommeil (index apnée-hypopnée >30 événements/heure). Le coefficient de concordance entre l'index d'apnée-hypopnée et l'index de désaturation 3% était de 0,759 alors qu'il était de 0,856 entre l'index d'apnée-hypopnée et l'index de désaturation de 4%.

The authors give the first description of evangelicalism in Switzerland using representative data. It is shown that evangelicalism can be conceived of as a "milieu" which is characterized by certain structural and cultural traits, boundaries and high internal communication. The relative success of the evangelical milieu compared to other religious milieus is explained by its remarkable ability to retain its own numerous offspring, while on the other hand providing a "religious product" that is also attractive to people without an evangelical familial background. Les auteurs utilisent des données représentatives afin de faire, pour la première fois, une description de l'évangélisme en Suisse. Ils montrent qu'on peut parler de l'évangélisme comme d'un "milieu'' qui se caractérise par certains attributs structurels et culturels, des frontières et une communication interne élevée. Le succès relatif du milieu évangélique comparé à d'autres milieux s'explique par sa capacité remarquable à retenir les enfants de ses membres dans le mouvement tout en offrant un "produit religieux'' attractif aux personnes sans arrière-fond évangélique.

Purpose: To evaluate the current management, and adherence to recommendations, of patients on oral anticoagulation (OAC) undergoing coronary stent implantation (PCI-S). Methods: By means of a contact person who had been previously identified in 8 European countries, a questionnaire was electronically forwarded between April and July 2010 to the national institutions where PCI-S is performed. Results: A total of 202 questionnaires (median response rate: 50%, range 33-78%) was received. The prevalence of OAC patients among those undergoing PCI-S is mostly reported 5-10% (97%). The peri-procedural pharmacological management mostly encompasses: preprocedural OAC interruption and bridging with low-molecular-weight heparin (59%), intraprocedural administration of an unfractionated heparin bolus (81%), and use of glycoprotein IIb/IIIa inhibitors on an individual basis (79%). The radial approach is reported as the preferred option (58%), as well as the implantation of bare metal stents (76%). Triple therapy (warfarin, aspirin, clopidogrel) is the most frequently prescribed (80%), generally for 1 month after bare metal stent (77%) and for at least 12 months after drug-eluting stent (60%). Throughout triple therapy, the International Normalized Ratio is mostly targeted to the lower end of the therapeutic range (77%), and gastric protection is routinely prescribed (69%), mostly by giving proton-pump inhibitors (70%). Conclusions: Among the 202 interventional cardiologists from the 8 European countries interviewed, the management of patients on OAC undergoing PCI-S appears variable and only partially adherent to currently available recommendations. Background To examine the association of education with body mass index (BMI) and waist circumference (WC) in the European Prospective Investigation into Cancer and Nutrition (EPIC). Method This study included 141,230 male and 336,637 female EPIC-participants, who were recruited between 1992 and 2000. Education, which was assessed by questionnaire, was classified into four categories; BMI and WC, measured by trained personnel in most participating centers, were modeled as continuous dependent variables. Associations were estimated using multilevel mixed effects linear regression models. Results Compared with the lowest education level, BMI and WC were significantly lower for all three higher education categories, which was consistent for all countries. Women with university degree had a 2.1 kg/m2 lower BMI compared with women with lowest education level. For men, a statistically significant, but less pronounced difference was observed (1.3 kg/m2). The association between WC and education level was also of greater magnitude for women: compared with the lowest education level, average WC of women was lower by 5.2 cm for women in the highest category. For men the difference was 2.9 cm. Conclusion In this European cohort, there is an inverse association between higher BMI as well as higher WC and lower education level. Public Health Programs that aim to reduce overweight and obesity should primarily focus on the lower educated population.
