La cardiomiopatía chagásica es la más importante y severa manifestación de la enfermedad crónica, los pacientes pueden cursar con falla cardiaca, arritmias, bloqueos cardiacos, tromboembolismo y muerte súbita. El diagnóstico es tardío, debido a que se confunden con cardiopatías de otra etiología y el manejo se realiza con base en guías y protocolos dirigidos hacia el tratamiento de falla cardiaca de origen no chagásico. Métodos: Se realizó una revisión sistemática y tuvo como objetivo responder a las siguientes Preguntas clínicas: PREGUNTA 1. ¿El manejo actual para la cardiomiopatía chagásica (betabloqueadores, IECA, ARA II, Diuréticos, Inhibidores de la fosfodiesterasa, Estatinas, antiagragantes plaquetarios) que es extrapolado del manejo de falla cardiaca de origen no chagásico tiene impacto en la calidad de vida, sobrevida, seguridad, estancia hospitalaria y disminución del número de hospitalizaciones, mejoría de síntomas, de los pacientes adultos con cardiopatía chagásica?. PREGUNTA 2. ¿En pacientes con cardiomiopatía chagásica el uso de fármacos tripanocidas mejora la sobrevida, calidad de vida, estancia hospitalaria, disminución del número de hospitalizaciones, y resolución de síntomas? PREGUNTA 3. ¿En pacientes con cardiomiopatía chagásica el uso de cardiodesfibriladores mejora la sobrevida, calidad de vida, estancia hospitalaria, disminución del número de hospitalizaciones, y resolución de síntomas? PREGUNTA 4. ¿En pacientes con cardiomiopatía chagásica el uso de marcapasos mejora la sobrevida, calidad de vida, estancia hospitalaria, disminución del número de hospitalizaciones, y resolución de síntomas? PREGUNTA 5. ¿En pacientes con cardiomiopatía chagásica el uso de trasplante de corazón mejora la sobrevida, calidad de vida, estancia hospitalaria, disminución del número de hospitalizaciones, y resolución de síntomas? Se realizaron búsquedas en: MEDLINE, Colaboración Cochrane, Trip database, y otras importantes bases de datos desde 1996 hasta 2010, limitando la búsqueda. Los estudios se seleccionaron de acuerdo a criterios de pertinencia PICO y se evaluó la calidad, usando la metodología recomendada en Scottish Intercollegiate Guidelines Network. Resultados: Se encontraron 21 estudios, que incluyen revisiones sistemáticas, ensayos clínicos controlados y aleatorizados, ensayos clínicos, cohortes y, casos y controles. Estos estudios cumplieron con los criterios de inclusión. Discusión: En esta revisión sistemática se presenta un consolidado de la evidencia disponible acerca de la eficacia de las siguientes intervenciones: Betabloqueadores, IECAS, PDE, Digoxina, nitroderivados, cardiodesfibriladores, marcapasos y trasplante de corazón, en pacientes con cardiopatía chagásica; los estudios encontrados en su mayoría son de baja evidencia.

Resumen Introducción: el tratamiento de cicatrices de acné y post-quirúrgicas con láseres fraccionales abre una frontera en el manejo de una patología con una alta incidencia alrededor del mundo y sobre la cual no existe un tratamiento gold-standar. Objetivo: determinar la efectividad de los láseres fraccionales en el manejo de cicatrices post acné y postquirúrgicas y al determinar los eventos adversos más comunes asociados al uso de esta tecnología. Materiales y métodos: Se realizó una búsqueda sistemática (Cochrane, Pub Med, Science Direct, Ovid, Hinari, Interscience y Lilacs,) de ensayos clínicos controlados publicados hasta diciembre de 2009. Se seleccionaron cuatro ensayos clínicos controlados aleatorizados con base en su calidad metodológica y se extrajeron los siguientes datos: tamaño de la muestra, parámetros, tipo de cicatrices, resultados, eventos adversos y grado de satisfacción. Igualmente se estudiaron una amplia variedad de revisiones en cuanto a la fisiopatología, morfología y severidad de las cicatrices y los métodos actuales de tratamiento. Resultados: la tecnología fraccional es efectiva en el tratamiento de cicatrices de acné y post-quirúrgicas al lograr mejoría en textura, color y aspecto general, que osciló entre el 50 y 70% evidenciada en los estudios incluidos en la revisión (24, 25, 26, 27). La satisfacción de los pacientes superó el 60%. Se observó una baja incidencia de efectos adversos de carácter leve y transitorio, corta incapacidad y la principal de ellas fue la hiperpigmentación postinflamatoria. Conclusión: la fototermólisis fraccional es efectiva y segura en el tratamiento de cicatrices de acné y post-quirúrgicas y presenta ventajas frente a otros sistemas (26, 27). Los métodos que combinan diferentes láseres aportan un mayor beneficio. A artrite reumatoide é uma patologia de natureza autoimmune que apresenta diversas intervenções farmacológicas e não-farmacológicas para seu tratamento e controle, ncluindo o rituximabe. O objetivo deste trabalho consistiu em avaliar a eficácia, segurança e tolerabilidade do rituximabe no tratamento da artrite reumatoide. Foram realizadas uma revisão sistemática (até junho de 2011) e meta-análises incluindo estudos clínicos controlados randomizados que comparassem o rituximabe com o placebo, ambos com concomitante metotrexato. Somente estudos com qualidade média ou alta foram ncluídos. A eficácia foi avaliada baseando-se nas mudanças dos ACR20, 50 e 70; a segurança foi avaliada baseando-se nos eventos adversos sérios; a tolerabilidade foi avaliada pelos abandonos do tratamento devido a eventos adversos. Todos os parâmetros foram avaliados após 24 semanas de tratamento. Quatro estudos atingiram os critérios de inclusão, incluindo 1280 pacientes. O grupo do rituximabe apresentou maior eficácia para os parâmetros avaliados (ACR20, ACR50 e ACR70 – Risco Relativo (RR) de 2,24 [1,86; 2,71], 3,29 [2,31; 4,68] e 3,90 [1,88; 8,09], respectivamente). Para os eventos adversos sérios, não foi detectada diferença significativa entre os grupos (RR = 0,83 [0,54; 1,26]. Para os abandonos devido a eventos adversos, também não foi detectada diferença estatisticamente significativa entre os grupos (RR = 1,49 [0,46; 4,84]. Em conclusão, o rituximabe apresentou maior eficácia, quando comparado ao placebo, sem diferenças significativas entre os grupos em termos de segurança e tolerabilidade.

Cosmetic Dermatology is a growing subspecialty. High-quality basic science studies have been published; however, few double-blind, randomized controlled clinical trials, which are the major instrument for evidence-based medicine, have been conducted in this area. Clinical research is essential for the discovery of new knowledge, improvement of scientific basis, resolution of challenges, and good clinical practice. Some basic principles for a successful researcher include interest, availability, persistence, and honesty. It is essential to learn how to write a protocol research and to know the international and national regulatory rules. A complete clinical trial protocol should include question, background, objectives, methodology (design, variable description, sample size, randomization, inclusion and exclusion criteria, intervention, efficacy and safety measures, and statistical analysis), consent form, clinical research form, and references. Institutional ethical review board approval and financial support disclosure are necessary. Publication of positive or negative results should be an authors' commitment. © 2013 by Anais Brasileiros de Dermatologia.

This is an integrative literature review with the aim of summarizing the prevention measures and treatment of thrombotic obstruction of long-term semi-implanted central venous catheters, in patients undergoing hematopoietic stem cell transplantation. The sample consisted of seven studies, being two randomized controlled clinical trials, three cohort studies and two case series. Regarding the prevention measures, one single study demonstrated effectiveness, which was a cohort study on the oral use of warfarin. In relation to the treatment measures, three studies evidenced effectiveness, one highlighted the efficacy of streptokinase or urokinase, one demonstrated the benefit of using low-molecular-weight heparin and the other treated the obstruction with heparin or urokinase. Catheter patency research shows a restricted evolution that does not follow the evolution of transplantations, mainly regarding nursing care.

Blockade of cytokines, particularly of tumour necrosis factor alpha (TNF-alpha), in immuno-inflammatory diseases, has led to the greatest advances in medicine of recent years. We did a thorough review of the literature with a focus on inflammation models in rodents on modified gene expression or bioactivity for IL-1, IL-6, and TNF-alpha, and we summarized the results of randomized controlled clinical trials in human disease. What we have learned herewith is that important information can be achieved by the use of animal models in complex, immune-mediated diseases. However, a clear ranking for putative therapeutic targets appears difficult to obtain from an experimental approach alone. This is primarily due to the fact that none of the disease models has proven to cover more than one crucial pathogenetic aspect of the complex cascade of events leading to characteristic clinical disease signs and symptoms. This supports the notion that the addressed human immune-mediated diseases are polygenic and the summation of genetic, perhaps epigenetic, and environmental factors. Nevertheless, it has become apparent, so far, that TNF-alpha is of crucial importance in the development of antigen-dependent and antigen-independent models of inflammation, and that these results correlate well with clinical success. With some delay, clinical trials in conditions having some relationship with rheumatoid arthritis (RA) indicate new opportunities for blocking IL-1 or IL-6 therapeutically. It appears, therefore, that a translational approach with critical, mutual reflection of simultaneously performed experiments and clinical trials is important for rapid identification of new targets and development of novel treatment options in complex, immune-mediated, inflammatory diseases.

PURPOSE: A systematic review was performed to find evidence for prophylactic administration of antibiotics in relation to treatment of maxillofacial fractures. METHODS: Four studies were retrieved that fulfilled most of the requirements of being randomized controlled clinical trials. RESULTS: An analysis of these studies showed a 3-fold decrease in the infection rate of mandibular fractures in the antibiotic treated groups compared with the control groups. A variety of antibiotics had been used with an apparently uniform effect. A "1-shot" regimen or a 1-day treatment course had a similar or perhaps even better effect than 7 days of treatment. No infections were related to condylar, maxillary, or zygoma fractures. CONCLUSION: A 1-shot or 1-day administration of prophylactic antibiotics seem to be the best documented to reduce infections in the management of mandibular fractures not involving the condylar region.

Stent thrombosis (ST) after percutaneous coronary intervention has been the focus of intense interest because of its attendant morbidity and mortality. There is controversy about several facets of the problem. These include the frequency of ST with drug-eluting stents (DES) versus bare-metal stents (BMS), the timing of the event, clinical consequences, risk factors, adjunctive therapy, and new preventive approaches. Information has accrued rapidly from several sources, including randomized controlled clinical trials of DES versus BMS in carefully selected subsets of patients and registry experiences in larger patient groups, which provide a more universal real-world picture. The results from these different data sets are not completely concordant. However, several general conclusions can be made: 1) ST is an infrequent but very severe complication of both BMS and DES; 2) at the present time, during 4 years of follow-up from randomized controlled trials that compared DES and BMS, there is no apparent difference in overall ST frequency, although the time course for occurrence appears to differ, with a relative numeric excess of ST late after DES implant; 3) despite this relative imbalance, no differences in the end points of death or death and infarction between DES and BMS are observed; 4) longer-term follow-up of these patients as well as larger angiographic and clinical subsets of patients who receive this technology outside of randomized trials are required to fully study this issue; and 5) advances in stent platforms for drug elution as well as adjunctive pharmacologic therapy are being evaluated to enhance long-term safety.

OBJECTIVES: To review the evidence for the association between diabetes and periodontal and peri-implant conditions and the impact of periodontal therapy in subjects with diabetes. MATERIAL AND METHODS: A search of MEDLINE-PubMed was performed up to and including December 2007. The search was limited to clinical studies published in English. Publications on animal studies were excluded. The selection criteria included all levels of available evidence. RESULTS: Evidence on the association between diabetes and periodontitis supports the concept of increased severity but not extent of periodontitis in subjects with poorly controlled diabetes. Subjects with controlled diabetes do not show an increase in extent and severity of periodontitis. Periodontitis is associated with poor glycaemic control and diabetes-related complications. It is inconclusive that periodontal therapy with or without the use of antibiotics results in improvements of glycaemic control and of markers of systemic inflammation. Evidence is lacking to indicate that implant therapy in subjects with diabetes yields long-term outcomes comparable with those of non-diabetic subjects. CONCLUSIONS: Poorly controlled diabetes may be considered a risk factor for increased severity of periodontitis. The effects of periodontal therapy on glycaemic control and systemic inflammation is not proven beyond doubt and need to be confirmed in large-scale randomized-controlled clinical trials.

OBJECTIVES: The objective of this systematic review was to assess the 5-year survival rates and incidences of complications associated with ceramic abutments and to compare them with those of metal abutments. METHODS: An electronic Medline search complemented by manual searching was conducted to identify randomized-controlled clinical trials, and prospective and retrospective studies providing information on ceramic and metal abutments with a mean follow-up time of at least 3 years. Patients had to have been examined clinically at the follow-up visit. Assessment of the identified studies and data abstraction was performed independently by three reviewers. Failure rates were analyzed using standard and random-effects Poisson regression models to obtain summary estimates of 5-year survival proportions. RESULTS: Twenty-nine clinical and 22 laboratory studies were selected from an initial yield of 7136 titles and data were extracted. The estimated 5-year survival rate of ceramic abutments was 99.1% [95% confidence interval (CI): 93.8-99.9%] and 97.4% (95% CI: 96-98.3%) for metal abutments. The estimated cumulative incidence of technical complications after 5 years was 6.9% (95% CI: 3.5-13.4%) for ceramic abutments and 15.9% (95% CI: 11.6-21.5%) for metal abutments. Abutment screw loosening was the most frequent technical problem, occurring at an estimated cumulative incidence after 5 years of 5.1% (95% CI: 3.3-7.7%). All-ceramic crowns supported by ceramic abutments exhibited similar annual fracture rates as metal-ceramic crowns supported by metal abutments. The cumulative incidence of biological complications after 5 years was estimated at 5.2% (95% CI: 0.4-52%) for ceramic and 7.7% (95% CI: 4.7-12.5%) for metal abutments. Esthetic complications tended to be more frequent at metal abutments. A meta-analysis of the laboratory data was impossible due to the non-standardized test methods of the studies included. CONCLUSION: The 5-year survival rates estimated from annual failure rates appeared to be similar for ceramic and metal abutments. The information included in this review did not provide evidence for differences of the technical and biological outcomes of ceramic and metal abutments. However, the information for ceramic abutments was limited in the number of studies and abutments analyzed as well as the accrued follow-up time. Standardized methods for the analysis of abutment strength are needed.

PURPOSE: To evaluate early and immediate loading of implants in the posterior maxilla and to investigate whether there is a difference in success rates, survival rates, and peri-implant parameters, including marginal bone level changes. MATERIALS AND METHODS: A comprehensive systematic review of the literature was conducted. The selection of publications reporting on human clinical studies was based on predetermined inclusion criteria and was agreed upon by two reviewers. RESULTS: Twelve papers were identified on early loading (two randomized controlled clinical trials [RCTs] and 10 prospective case series studies). Six papers were found on immediate loading (one RCT, four prospective case series, and one retrospective study). CONCLUSIONS: Under certain circumstances it is possible to successfully load dental implants in the posterior maxilla early or immediately after their placement in selected patients. The success rate appears to be technique sensitive, although no study has directly assessed this. A high degree of primary implant stability (high value of insertion torque) and implant surface characteristics play an important role. It is not possible to draw evidence-based conclusions concerning contraindications, threshold values for implant stability, bone quality and quantity needed, or impact of occlusal loading forces. As for the impact of the surgical technique on implant outcome in different bone densities, no studies prove significant superior results with one technique over another. Well-designed RCTs with a large number of patients are necessary to make early/immediate loading protocols in posterior maxilla evidence based, but ethical and practical considerations may limit the real possibility of such studies in the near future.

Cardiovascular disease (CVD) is the single greatest cause of death in the United States, accounting for nearly 2400 deaths each day. It is estimated that 79.4 million American adults have some form of the disease, and CVD mortality rates are greater than those of cancer, chronic lower respiratory diseases, accidents and diabetes mellitus combined. Psychosocial stress is a nontraditional risk factor for CVD, and can contribute to the clustering of traditional risk factors as well as to vascular manifestations of the disease. The Transcendental Meditation (TM) technique has been researched as a cost effective intervention aimed at decreasing psychosocial stress. This literature review attempts to analyze randomized controlled clinical trials of TM on cardiovascular disease outcomes. Eleven studies met inclusion criteria and are described below, with statistically significant positive outcomes observed in each study. Studies are grouped by primary outcome reported in the categories of cardiovascular function, blood pressure, and exercise tolerance. The TM intervention significantly decreased insulin resistance, heart rate variability, and carotid intima media thickness and improved exercise tolerance compared to control groups. Seven studies also reported significant decreases in blood pressure among hypertensive and normotensive subjects. Five studies focused solely on African American subjects, a population that has disproportionately higher rates of CVD and hypertension, and found significant improvements in CVD outcomes. Further research is recommended to establish the efficacy of TM on CVD outcomes. Future trials should include larger sample populations, wider ethnic distributions of subjects, and longer follow-up to ascertain the impact of this particular stress reduction technique on cardiovascular disease.^

Purpose: This study involved an extensive search for randomized controlled clinical trials comparing bilateral balanced and canine-guided dentures, and questioned whether a bilateral balanced occlusion is imperative for successful denture treatment. Materials and Methods: Studies were identified by searching electronic databases (PubMed/MEDLINE, ISI Web of Science, LILACS, and BBD). The keywords “denture” and “occlusion” were used. The minimum inclusion requirements were (1) randomized controlled trials with patients of any age wearing both maxillary and mandibular conventional complete dentures (CDs), (2) comparison between bilateral balanced and canine-guided dentures, and (3) assessment of masticatory function and/or patients’ satisfaction. Results: The search resulted in the identification of 5166 articles. Subsequently, 5156 articles were excluded on the basis of title and abstract. By the end of the search phase, seven randomized controlled trials were considered eligible. Conclusions: Current scientific evidence suggests that bilateral balanced occlusion is not imperative for successful treatment with conventional CDs in average patients. More studies are necessary to identify if specific clinical conditions may benefit from a balanced occlusion.

STATEMENT OF PROBLEM: A number of methods have been described for the fabrication of complete dentures. There are 2 common ways to make conventional complete dentures: a traditional method and a simplified method. PURPOSE: The purpose of this study was to conduct a systematic review to compare the efficiency of simplified and traditional methods for the fabrication of complete dentures. MATERIAL AND METHODS: The review was conducted by 3 independent reviewers and included articles published up to December 2013. Three electronic databases were searched: MEDLINE-PubMed, The Cochrane Library, and ISI Web of Science. A manual search also was performed to identify clinical trials of simplified versus traditional fabrication of complete dentures. RESULTS: Six articles were classified as randomized controlled clinical trials and were included in this review. The majority of the selected articles analyzed general satisfaction, denture stability, chewing ability and function, comfort, hygiene, esthetics, speech function, quality of life, cost, and fabrication time. CONCLUSIONS: Although the studies reviewed demonstrate some advantages of simplified over traditional prostheses, such as lower cost and clinical time, good chewing efficiency, and a positive effect on the quality of life, the reports related the use of different simplified methods for the fabrication of complete dentures. Additional randomized controlled trials that used similar simplified techniques for the fabrication of complete dentures should be performed with larger sample sizes and longer follow-up periods.
