OBJECTIVE: To assess the effect of a health promotion program on cardiometabolic risk profile in Japanese-Brazilians. METHODS: A total of 466 subjects from a study on diabetes prevalence conducted in the city of Bauru, southeastern Brazil, in 2000 completed a 1-year intervention program (2005-2006) based on healthy diet counseling and physical activity. Changes in blood pressure and metabolic parameters in the 2005-2006 period were compared with annual changes in these same variables in the 2000-2005 period. RESULTS: During the intervention, there were greater annual reductions in mean (SD) waist circumference [-0.5(3.8) vs. 1.2(1.2) cm per year, p<0.001], systolic blood pressure [-4.6(17.9) vs. 1.8(4.3) mmHg per year, p<0.001], 2-hour plasma glucose [-1.2(2.1) vs. -0.2(0.6) mmol/L per year, p<0.001], LDL-cholesterol [-0.3(0.9) vs. -0.1(0.2) mmol/L per year, p<0.001] and Framingham coronary heart disease risk score [-0.25(3.03) vs. 0.11(0.66) per year, p=0.02] but not in triglycerides [0.2(1.6) vs. 0.1(0.42) mmol/L per year, p<0.001], and fasting insulin level [1.2(5.8) vs. -0.7(2.2) IU/mL per year, p<0.001] compared with the pre-intervention period. Significant reductions in the prevalence of impaired fasting glucose/impaired glucose tolerance and diabetes were seen during the intervention (from 58.4% to 35.4%, p<0.001; and from 30.1% to 21.7%, p= 0.004, respectively). CONCLUSIONS: A one-year community-based health promotion program brings cardiometabolic benefits in a high-risk population of Japanese-Brazilians.

Background. The increasing prevalence of overweight among youth in the United States, and the parallel rise in related medical comorbidities has led to a growing need for efficient weight-management interventions. Purpose. The aim of this study was to evaluate the effects of the Choosing Health and Sensible Exercise (C.H.A.S.E.) childhood obesity prevention program on Body Mass Index (BMI), physical activity and dietary behaviors. Methods. This study utilized de-identified data collected during the fall 2006 session of the C.H.A.S.E. program. A total of 65 students at Woodview Elementary School and Deepwater Elementary School participated in this intervention. The C.H.A.S.E. program is a 10-week obesity prevention program that focuses on nutrition and physical activity education. Collection of height and weight data, and a health behavior survey was conducted during the first and last week of the intervention. Paired t-tests were used to determine statistically significant differences between pre- and post-intervention measurements. One-way analysis of variance was used to adjust for potential confounders, such as gender, age, BMI category ("normal weight", "at risk overweight", or "overweight"), and self-reported weight loss goals. Data were analyzed using STATA, v. 9.2. Results. A significant decrease in mean BMI (p< 0.05) was found after the 10-week intervention. While the results were statistically significant for the group as a whole, changes in BMI were not significant when stratified by age, sex, or ethnicity. The mean overall scores for the behavior survey did not change significantly pre- and post-intervention; however, significant differences were found in the dietary intention scale, indicating that students were more likely to intend to make healthier food choices (p<0.05). No statistically significant decreases in BMI were found when stratified for baseline BMI-for-age percentiles or baseline weight loss efforts (p>0.05). Conclusion. The results of this evaluation provide information that will be useful in planning and implementing an effective childhood obesity intervention in the future. Changes in the self-reported dietary intentions and BMI show that the C.H.A.S.E. program is capable of modifying food choice selection and decreasing BMI. Results from the behavior questionnaire indicate that students in the intervention program were making changes in a positive direction. Future implementation of the C.H.A.S.E. program, as well as other childhood obesity interventions, may want to consider incorporating additional strategies to increase knowledge and other behavioral constructs associated with decreased BMI. In addition, obesity prevention programs may want to increase parental involvement and increase the dose or intensity of the intervention. ^

The prevalence of obesity has reached epidemic proportions in the United States. Twenty-five percent of school aged students are overweight. Schools have the opportunity to help slow this epidemic. School cafeterias in the United States feed millions of students every day through the National School Lunch Program.^ Point-of-sale machines are used in most school cafeterias to help streamline the process of purchasing school lunches. The point-of-sale software allows school personnel to place special notes on student's accounts to provide alerts about parental requests. This study investigated what the alerts are used for, who uses the alerts, and if there are any patterns by demographic characteristics. ^ Counts and percentages were used to determine what the alerts were used for and who used them. This study found that students who were white non-Hispanic, paid status, or in elementary school were most likely to have alerts placed on their accounts. Also, the majority of point-of-sale alerts were used as allowances (i.e., allowed to purchase snacks from the balance on the school lunch account), rather than restrictions (i.e., restricted from purchasing high calorie foods or specific food items). Using chi-square analysis, a total of 688 alerts were analyzed. There were significant differences in alert frequencies for intent category by grade level (p=0.000), snack access (p=0.000), and gender (p=0.002). Therefore, the results are significant, and one can conclude there is a significant relationship between gender, grade level, and snack access, and the presence of an alert on the school lunch account.^ Also, school administrators may want to take into consideration possible changes to their program, such as requiring more time to run the software. The results of this study can assist school administrators to better understand that a point-of-sale alert program may help their school lunch programs run more efficiently, while also providing parental influence on students’ food choices at the point-of-sale.^ School food service authorities should consider implementing a structured point-of-sale alert policy to encourage parental input on their children's food choices. When implementing the point-of-sale policy, schools should publicize this policy online, through school lunch menus, and parent communications increase participation throughout the school district.^

Purpose. The purpose of this study was to investigate the impact of a motivational weight management DVD on knowledge of obesity related diseases, readiness, motivation, and self-efficacy to lose weight, connectedness to their care provider, and patients return to clinic. Design. A randomized control trial was conducted in which 40 overweight/obese adolescents and their parents/caregivers were randomly assigned to standard care alone or standard care plus DVD. Subjects completed a set of pre- and post-questionnaire measures. A group of 22 patients was also formed as a historical control group in order to account for the potential effect of extra attention given to subjects prospectively enrolled. Methods. The adolescents and their parent/caregiver were placed into a patient room. Consent was obtained and a set of written pre-questionnaires were given to both the parent and the adolescent. Standard care was provided to all patients by the Registered Dietitian and physician; the DVD was shown in addition to standard care among the intervention group. A set of post-questionnaires were given and compensation was provided. Analysis. Groups were compared to determine equivalence at baseline. Analysis of covariance was used to evaluate changes over time, while controlling for pre-test scores and race/ethnicity. Results. Parents who viewed the DVD experienced greater changes in correct knowledge as compared to parents who did not view the DVD. Conclusion. Our study found only one substantial benefit of the DVD beyond standard clinical practices. This is an important area for change as it increased awareness of obesity as a serious disease and has future clinical implications.^

Transitional homes present a window of opportunity to address the nutrition-related chronic diseases of previously incarcerated women. However, few transitional facilities offer nutrition education programs. This study assesses the nutritional status of 9 previously incarcerated women living at a transitional home in Houston, Texas and makes recommendations for effective nutrition education programs. Data was collected through individual interviews, questionnaires and a 24-hour dietary recall. Participants differed significantly from national nutrition recommendations when comparing BMI values and fruit, vegetable and fiber intake. Qualitative interview themes concerned key barriers to healthful dietary intake such as inadequate food storage and inconvenient cooking environment. Nutrition education programs at transitional homes should focus on healthy meals and snacks that can be quickly prepared and easily stored in small spaces. ^

Supermarket nutrient movement, a community food consumption measure, aggregated 1,023 high-fat foods, representing 100% of visible fats and approximately 44% of hidden fats in the food supply (FAO, 1980). Fatty acid and cholesterol content of foods shipped from the warehouse to 47 supermarkets located in the Houston area were calculated over a 6 month period. These stores were located in census tracts with over 50% of a given ethnicity: Hispanic, black non-Hispanic, or white non-Hispanic. Categorizing the supermarket census tracts by predominant ethnicity, significant differences were found by ANOVA in the proportion of specific fatty acids and cholesterol content of the foods examined. Using ecological regression, ethnicity, income, and median age predicted supermarket lipid movements while residential stability did not. No associations were found between lipid movements and cardiovascular disease mortality, making further validation necessary for epidemiological application of this method. However, it has been shown to be a non-reactive and cost-effective method appropriate for tracking target foods in populations of groups, and for assessing the impact of mass media nutrition education, legislation, and fortification on community food and nutrient purchase patterns. ^

Childhood obesity is a significant public health problem. Over 15 percent of children in the United States are obese, and about 25 percent of children in Texas are overweight (CDC NHANES). Furthermore, about 30 percent of elementary school aged children in Harris County, Texas are overweight or obese (Children at Risk Institute 2010). In addition to actions such as increasing physical activity, decreasing television watching and video game time, decreasing snacking on low nutrient calorie dense foods and sugar sweetened beverages, children need to consume more fruits and vegetables. According to the National Health and Nutrition Examination Survey (NHANES) from 2002, about 26 percent of U.S. children are meeting the recommendations for daily fruit intake and about 16 percent are meeting the recommendations for daily vegetable intake (CDC NHANES). In 2004, the average total intake of vegetables was 0.9 cups per day and 1.1 cups of fruit per day by children ages four to nine years old in the U.S. (CDC NHANES). Not only do children need effective nutrition education to learn about fruits and vegetables, they also need access and repeated exposure to fruits and vegetables (Anderson 2009, Briefel 2009). Nutrition education interventions that provide a structured, hands-on curriculum such as school gardens have produced significant changes in child fruit and vegetable intake (Blair 2009, McAleese 2007). To prevent childhood obesity from continuing into adolescence and adulthood, effective nutrition education interventions need to be implemented immediately and for the long-term. However, research has shown short-term nutrition education interventions such as summer camps to be effective for significant changes in child fruit and vegetable intake, preferences, and knowledge (Heim 2009). ^ A four week summer camp based on cooking and gardening was implemented at 6 Multi-Service centers in a large, urban city. The participants included children ranging in age from 7 to 14 years old (n=64). The purpose of the camp was to introduce children to their food from the seed to the plate through the utilization of gardening and culinary exercises. The summer camp activities were aimed at increasing the children's exposure, willingness to try, preferences, knowledge, and intake of fruits and vegetables. A survey was given on the first day of camp and again on the last day of camp that measured the pre- and post differences in knowledge, intake, willingness to try, and preferences of fruits and vegetables. The present study examined the short-term effectiveness of a cooking and garden-based nutrition education program on the knowledge, willingness, preferences, and intake among children aged 8 to 13 years old (n=40). The final sample of participants (n=40) was controlled for those who completed pre- and post-test surveys and who were in or above the third grade level. Results showed a statistically significant increase in the reported intake of vegetables and preferences for vegetables, specifically green beans, and fruits. There was also a significant increase in preferences for fruits among boys and participants ages 11 to 13 years. The results showed a change in the expected direction of willingness to try, preferences for vegetables, and intake of fruit, however these were not statistically significant. Interestingly, the results also showed a decrease in the intake of low nutrient calorie dense foods such as sweets and candy.^

En el presente trabajo se ha analizado la situación actual de los hábitos de alimentación en los snack de media mañana en la población ESO y Bachillerato y los motivos de esta situación. También se ha estudiado la labor de los centros educativos y de los profesores respecto a la educación alimenticia de los estudiantes. Se ha realizado un estudio en dos centros escolares de la Comunidad de Madrid a los alumnos de ESO y Bachillerato, el Colegio Castilla de Torrejón de la Calzada y el IES Manuel Elkin Patarroyo, de las tendencias nutricionales durante los recreos. Se han obtenido hábitos y conductas alimenticias inadecuadas, por lo que se han propuesto varias aplicaciones educativas para emplear dentro del aula. Los centros educativos necesitan fomentar políticas acerca de educación nutricional que forme a los adolescentes en futuros adultos con hábitos de vida saludables. ABSTRACT The study analyzes the current situation of the eating habits in the mid-morning snack in adolescents at school population and the reasons for this situation. Regarding the type that eating students have, the work of schools regarding nutritional education is also studied. The study was conducted in two schools in the Comunidad de Madrid for ESO and Bachillerato students, (Colegio Castilla from Torrejón de la Calzada and IES Manuel Elkin Patarroyo from Parla), about nutritional trends during playground. The results showed inadequate eating habits and behaviors so it has proposed various educational proposals for use in the classroom. The schools need to promote policies on nutrition education that teach teenagers in future adults with healthy living habits

Introdução: A adolescência é uma fase importante para formação e consolidação de hábitos alimentares adequados à promoção da saúde. Entretanto, dados estatísticos recentes indicam enorme comprometimento no padrão alimentar de indivíduos nesse período da vida. Objetivo: Avaliar a qualidade da dieta e identificar seus fatores associados em adolescentes de três regiões administrativas do município de Vitória. Métodos: Estudo transversal de base populacional com 396 adolescentes de ambos os sexos, de 8 a 17 anos, de escolas públicas e privadas de três regiões administrativas de Vitória-ES. Foram coletados dados antropométricos, socioeconômicos, demográficos e alimentares. Para a entrevista alimentar utilizou-se o método de Recordatório de 24h. A qualidade da dieta foi avaliada pelo Índice de Qualidade da Dieta adaptado (IQDa). Análises de regressão logística foram utilizadas para verificar a associação das variáveis independentes sobre a qualidade da dieta dos adolescentes. Resultados: A média do IQDa foi de 35,6 pontos. Os componentes verduras e legumes, leite e derivados e frutas apresentaram os mais baixos valores médios de pontos; e carnes e ovos apresentaram a pontuação média mais alta do IQDa. Na análise de regressão observou-se que quanto maior a idade, menor a pontuação do IQDa e que os adolescentes do sexo masculino apresentaram IQDa maior que as do sexo feminino. Conclusão: A qualidade da dieta está associada ao sexo e a idade e necessita de melhorias. Conhecendo esses fatores associados é possível analisar e identificar as práticas alimentares não saudáveis e abordá-las em programas de educação nutricional, proporcionando melhores condições de saúde a essa população.

Introdução: O Direito Humano a Alimentação Adequada (DHAA), na perspectiva da Segurança Alimentar e Nutricional (SAN), destacou-se devido à compreensão dos determinantes para a Promoção da Saúde (PS). A Educação Alimentar e Nutricional (EAN) é uma ferramenta capaz de promover a reflexão dos cidadãos sobre como realizar esse direito. No Brasil, o quadro de insegurança alimentar entre crianças e adolescentes torna os profissionais de saúde da Atenção Primária à Saúde (APS) atores promissores para a reversão desse quadro, já que esses trabalham com os principais influenciadores desse público: a família. Objetivo: Analisar a atuação de profissionais de saúde não nutricionistas coordenadores de grupos educativos com conteúdo de alimentação e nutrição, desenvolvidos na APS do município de São Paulo. Métodos: Estudo qualitativo, com aplicação de entrevistas semiestruturadas e análise por meio do Discurso do Sujeito Coletivo. Foram levantados os dados a respeito da formação desses profissionais e identificadas suas percepções sobre seus papéis nos grupos que coordenam e a importância atribuída a eles. Resultados: A profissão dos 21 entrevistados reflete a atual configuração da Estratégia Saúde da Família. Há predominância de profissionais do sexo feminino com pós-graduação em temas de saúde coletiva. Foram identificadas 13 Ideias Centrais dividas em 2 Eixos Temáticos. Levantaram-se percepções contrárias e outras a favor aos referenciais teóricos trabalhados. Como favoráveis, identificou-se a valorização dos grupos como espaços de participação, troca de experiências e criação de vínculo entre seus membros, sendo o coordenador do grupo responsável pela condução desses. A importância na atuação interprofissional para o atendimento integral à saúde e atualização entre os profissionais também foi destacada. Já as desfavoráveis trouxeram a desvalorização das atividades em grupo, ou a atribuição de sua importância como forma de acesso a serviços, medicamentos ou informação, a identificação dos coordenadores como responsáveis por mudanças de comportamentos nos participantes, modelos a serem seguidos, e sendo considerados detentores do conhecimento, o que parece sobrecarregá-los, desmotivá-los e frustrá-los. Assim, alguns buscam seu reconhecimento trazendo atividades que agradam os usuários, independentemente da constatação das necessidades do território. Conclusões: A percepção dos profissionais parece refletir a forma em que atuam, evidenciando um momento heterogêneo sobre as formas de se abordar os aspectos relacionados à alimentação, além do despreparo para a coordenação de grupos. Dessa forma, recomenda-se a aproximação entre os campos da saúde e da educação, visando práticas mais significativas e libertadoras, bem como a reflexão sobre a formação desses profissionais, já que suas atuações parecem refletir a educação na qual foram moldados. Os princípios da PS, do DHAA, da SAN, da EAN e das características essenciais a um coordenador de grupos, devem ser trabalhados com esses atores, e, para tanto, como produto dessa pesquisa, sugeriu-se um curso de atualização.

El trabajo aborda los antecedentes históricos de las políticas de alimentación escolar en la España del siglo XX, a través del análisis del corpus normativo y de los textos y materiales que generaron organismos nacionales e internacionales que participaron en su implementación. La investigación muestra el impacto que tuvo el contexto internacional y en particular los acuerdos que se alcanzaron durante el primer franquismo con los Estados Unidos y con la FAO y UNICEF, en el desarrollo de dichas políticas. El carácter filantrópico de las primeras cantinas fue sustituido por una creciente implicación del ámbito público, al incorporar la alimentación de los escolares como objetivo de las políticas sanitarias y educativas de la Segunda República. La Ley de Educación Primaria de 1945 permitió la gradual institucionalización de la alimentación escolar. La creación en 1954 del Servicio Escolar de Alimentación y Nutrición y la necesidad de coordinar la ayuda norteamericana facilitaron la implementación de programas como el de Productos Lácteos pro Bienestar Infantil y Social o el de Educación en Alimentación y Nutrición, lo cual permitió implantar el complemento alimenticio en escolares y llevar a cabo un conjunto de actividades educativas coordinadas encaminadas a mejorar su alimentación.

El objetivo de este trabajo es analizar desde una perspectiva de género, a través del ejemplo del Programa de Educación en Alimentación y Nutrición (EDALNU) que se desarrolló en España en las décadas finales del siglo XX, los mensajes y estrategias de educación alimentaria nutricional que iban dirigidos a mejorar la capacitación de las amas de casa como garantes del bienestar familiar y como expertas en la gestión de los diversos aspectos del hogar. Su acción específica sobre las amas de casa y los argumentos sobre los que se sostenía dicha intervención afianzaron el modelo social de género en el que los hombres eran los únicos responsables de los recursos económicos del hogar y las mujeres las proveedoras de cuidados familiares y en concreto los relacionados con la alimentación y la nutrición.

A lo largo del siglo XX, la población española fue consolidando su proceso de transición nutricional y alimentaria. En la etapa pre-transicional, se produjo un renovado interés por las deficiencias que mostraba la alimentación de los niños y se apostó por la educación alimentaria-nutricional para superarlas. El objetivo del trabajo es analizar el estado nutricional, a través de la talla como parámetro antropométrico, que mostraba la población escolar del ámbito rural español, entre las décadas de 1950 y 1970, su evolución y las diferencias que existían entre regiones. Los resultados ponen de manifiesto que al inicio de la década de 1960 se apreciaban dos patrones: un primer grupo donde la estatura de los niños de la costa cantábrica, zona de Levante, Cataluña y Baleares, mostraba niveles similares a los de niños bien alimentados; y un segundo grupo donde las tallas eran inferiores, en el que se encontraban las regiones de Andalucía, Extremadura y Galicia. Entre 1954 y 1977, las tallas de los niños bien alimentados mostraron un incremento que fue especialmente intenso entre 1954 y la primera mitad de la década de 1960. En el caso de los niños y niñas que acudían a las escuelas nacionales del ámbito rural, también se produjo un significativo incremento en la talla, además de corregirse las diferencias regionales que existían al principio de los años sesenta. En todos aquellos avances habrían influido tanto el complemento alimenticio, como las actividades de educación alimentación y nutrición que se llevaron a cabo en el marco del Programa EDALNU.

Introducción: La obesidad en la población infantil y juvenil en España se sitúa actualmente en el 13,9%, y el sobrepeso se encuentra en el 26,3%. Esto hace que sea fundamental la adopción en los primeros años de vida de unos hábitos alimentarios correctos, donde la educación alimentaria y nutricional, desarrollada dentro de los programas de educación para la salud en el medio escolar, juega un papel fundamental. El objetivo del presente trabajo fue analizar, mediante revisión bibliométrica, la producción científica existente sobre programas o intervenciones de educación alimentaria y nutricional desarrollados en el medio escolar en España que muestran influencia en la salud y/o en los hábitos alimentarios de los escolares. Material y Métodos: Estudio descriptivo transversal de los resultados obtenidos en la búsqueda bibliográfica de las bases de datos Medline, Cochrane Library Plus en Español, Cuiden, Excelencia clínica, IBECS, Scielo, CSIC (ICYT, ISOC e IME), Lilacs, Cuidatge y Teseo. Se estudiaron indicadores bibliométricos: bases de datos, revistas, documentos publicados, idiomas, autoría, Índice de colaboración y grado de obsolescencia (Semiperíodo de Burton y Kebler e Índice de Price), entre otros. Resultados: La búsqueda proporcionó un total de 148 registros. La tasa final de artículos pertinentes fue de 49 (33,11%). La base de datos que más registros pertinentes proporcionó fue Medline con 24 (48,98%). 42 de los registros seleccionados (85,71%) pertenecían a artículos originales. La revista Nutrición Hospitalaria fue la que presentó mayor número de publicaciones, con 11 (22,45%). El Semiperíodo de Burton y Kebler fue de 6 años y el Índice de Price fue de 42,86%. Conclusiones: Existe una gran producción de artículos científicos relacionados con programas de educación alimentaria y nutricional en el medio escolar en España, lo cual es un indicador de la importancia de la adquisición de hábitos alimentarios saludables en la población infantil y juvenil y del interés que despiertan.

Introducción: Analizar la calidad de las páginas web de los servicios de catering en el ámbito escolar y su contenido en educación alimentaria, y tener una primera experiencia con la herramienta de evaluación EDALCAT. Material y métodos: Estudio descriptivo transversal. La población de estudio son páginas web de empresas de catering encargadas de la gestión de los comedores escolares. La muestra se obtuvo utilizando el buscador Google y un Ranking de las principales empresas de catering por facturación, escogiendo aquellas que tenían página web. Para la prueba piloto se seleccionaron diez páginas web según proximidad geográfica a la ciudad de Alicante y nivel de facturación. Para la evaluación de los sitios web se diseñó un cuestionario (EDALCAT), compuesto de un primer bloque de predictores de calidad con 19 variables de fiabilidad, diseño y navegación; y de un segundo bloque de contenidos específicos de educación alimentaria con 19 variables de contenido y actividades educativas. Resultados: Se han obtenido resultados positivos en 31 de las 38 variables del cuestionario, excepto en los ítems: “Buscador”, “Idioma” (40%) y “Ayuda” (10%) del bloque predictores de calidad y en los ítems: “Talleres”, “Recetario”, “Web alimentación-nutrición” (40%) y “Ejemplos” (30%) del bloque de contenidos específicos de educación alimentaria. Todas las páginas web evaluadas superan valores del 50% de cumplimiento de criterios de calidad y de contenidos mínimos en educación alimentaria, y sólo una de ellas, incumple el nivel de actividad mínimo establecido. Conclusiones: Los predictores de calidad y los contenidos específicos en educación alimentaria dieron buenos resultados en todas las páginas web evaluadas. La mayoría de ellas obtuvieron una alta puntuación en su valoración, y en su análisis individual por bloques. Tras el estudio piloto el cuestionario se ha modificado y se obtiene el EDALCAT definitivo. En líneas generales EDALCAT parece ser adecuado para evaluar la calidad de las páginas web de servicios de catering y su contenido en educación alimentaria, sin embargo el presente estudio no puede considerarse como validación del mismo.
