Primary craniocervical dystonia (CCD) is generally attributed to functional abnormalities in the cortico-striato-pallido-thalamocortical loops, but cerebellar pathways have also been implicated in neuroimaging studies. Hence, our purpose was to perform a volumetric evaluation of the infratentorial structures in CCD. We compared 35 DYT1/DYT6 negative patients with CCD and 35 healthy controls. Cerebellar volume was evaluated using manual volumetry (DISPLAY software) and infratentorial volume by voxel based morphometry of gray matter (GM) segments derived from T1 weighted 3 T MRI using the SUIT tool (SPM8/Dartel). We used t-tests to compare infratentorial volumes between groups. Cerebellar volume was (1.14 ± 0.17) × 10(2) cm(3) for controls and (1.13 ± 0.14) × 10(2) cm(3) for patients; p = 0.74. VBM demonstrated GM increase in the left I-IV cerebellar lobules and GM decrease in the left lobules VI and Crus I and in the right lobules VI, Crus I and VIIIb. In a secondary analysis, VBM demonstrated GM increase also in the brainstem, mostly in the pons. While gray matter increase is observed in the anterior lobe of the cerebellum and in the brainstem, the atrophy is concentrated in the posterior lobe of the cerebellum, demonstrating a differential pattern of infratentorial involvement in CCD. This study shows subtle structural abnormalities of the cerebellum and brainstem in primary CCD.

Corticoteroid-induced glaucoma can result from either topical or systemic corticosteroid use. Compared with adults, the corticosteroid response in children is less well known. The case is reported of a child who developed glaucoma after receiving topical corticosteroids following a scleral re-inforcement procedure. The raised intraocular pressure was controlled after cessation of the corticosteroids and with the use of antiglaucoma therapy. As many forms of cortico-steroids are widely used, children on corticosteroids should have regular intraocular pressure measurements as part of their management.

Background: Thalamotomy has been reported to be successful in ameliorating the motor symptoms of tremor and/or rigidity in people with Parkinson's disease (PD), emphasising the bona fide contribution of this subcortical nucleus to the neural circuitry subserving motor function. Despite evidence of parallel yet segregated associative and motor cortico-subcortical-cortical circuits, comparatively few studies have investigated the effects of this procedure on cognitive functions. In particular, research pertaining to the impact of thalamotomy on linguistic processes is fundamentally lacking. Aims: The purpose of this research was to investigate the effects of thalamotomy in the language dominant and non-dominant hemispheres on linguistic functioning, relative to operative theoretical models of subcortical participation in language. This paper compares the linguistic profiles of two males with PD, aged 75 years (10 years of formal education) and 62 years (22 years of formal education), subsequent to unilateral thalamotomy procedures within the language dominant and non-dominant hemispheres, respectively. Methods & Procedures: Comprehensive linguistic profiles comprising general and high-level linguistic abilities in addition to on-line semantic processing skills were compiled up to 1 month prior to surgery and 3 months post-operatively, within perceived on'' periods (i.e., when optimally medicated). Pre- and post-operative language performances were compared within-subjects to a group of 16 non-surgical Parkinson's controls (NSPD) and a group of 16 non-neurologically impaired adults (NC). Outcomes & Results: The findings of this research suggest a laterality effect with regard to the contribution of the thalamus to high-level linguistic abilities and, potentially, the temporal processing of semantic information. This outcome supports the application of high-level linguistic assessments and measures of semantic processing proficiency to the clinical management of individuals with dominant thalamic lesions. Conclusions: The results reported lend support to contemporary theories of dominant thalamic participation in language, serving to further elucidate our current understanding of the role of subcortical structures in mediating linguistic processes, relevant to cortical hemispheric dominance.

Pesticide exposure during brain development could represent an important risk factor for the onset of neurodegenerative diseases. Previous studies investigated the effect of permethrin (PERM) administered at 34 mg/kg, a dose close to the no observable adverse effect level (NOAEL) from post natal day (PND) 6 to PND 21 in rats. Despite the PERM dose did not elicited overt signs of toxicity (i.e. normal body weight gain curve), it was able to induce striatal neurodegeneration (dopamine and Nurr1 reduction, and lipid peroxidation increase). The present study was designed to characterize the cognitive deficits in the current animal model. When during late adulthood PERM treated rats were tested for spatial working memory performances in a T-maze-rewarded alternation task they took longer to choose for the correct arm in comparison to age matched controls. No differences between groups were found in anxiety-like state, locomotor activity, feeding behavior and spatial orientation task. Our findings showing a selective effect of PERM treatment on the T-maze task point to an involvement of frontal cortico-striatal circuitry rather than to a role for the hippocampus. The predominant disturbances concern the dopamine (DA) depletion in the striatum and, the serotonin (5-HT) and noradrenaline (NE) unbalance together with a hypometabolic state in the medial prefrontal cortex area. In the hippocampus, an increase of NE and a decrease of DA were observed in PERM treated rats as compared to controls. The concentration of the most representative marker for pyrethroid exposure (3-phenoxybenzoic acid) measured in the urine of rodents 12 h after the last treatment was 41.50 µ/L and it was completely eliminated after 96 h.

RESUMO: Objectivos 1. Avaliar a morbilidade urinária e sexual secundária à braquiterapia prostática com implante de I125. 2. Avaliar a influência da hormonoterapia neoadjuvante e adjuvante na morbilidade urinária e sexual secundária à braquiterapia(I125). 3. Avaliar a influência da associação da radioterapia externa na morbilidade urinária e sexual secundária à braquiterapia(I125). 4. Avaliar a morbilidade urinária dos doentes com contra-indicação relativa (próstatas volumosas, IPSS elevado).Material e métodos De Setembro de 2000 a Dezembro de 2004 foram recrutados 204 doentes com o diagnóstico de carcinoma da próstata localizado (T1 e T2) ou localmente avançado (T3) e expectativa de vida superior a 10 anos. Foram submetidos a braquiterapia, com implante transperineal de Iodo 125 (I125) em monoterapia ou combinada com hormonoterapia e/ou radioterapia externa (tratamento trimodal). Definiram-se diversos sub-grupos de pacientes, consoante algumas características habitualmente referidas como factores de risco para a morbilidade do tratamento de braquiterapia prostática, com o objectivo de analisar a sua influência sobre a morbilidade urinária e sexual: Grupo 1: Braquiterapia em monoterapia (MONO) versus braquiterapia associada a radioterapia externa (BCOMB) Grupo 2: Próstatas volumosas (>50ml) versus próstatas não volumosas (<50ml)Grupo 3: Braquiterapia associada a hormonoterapia (HORM) versus braquiterapia sem hormonoterapia (NHORM)Grupo 4: IPSS elevado versus IPSS baixo Avaliou-se a evolução do IPSS, QoL, taxa de RTU-P e retenção urinária pós implante, e evolução do BSFI durante todo o período de seguimento. Resultados Grupo 1: Para o grupo MONO o IPSS iniciou-se com 7.1, sofreu agravamento para 16.1 e 15.9 ao primeiro e terceiro meses. Aos 12 meses, o IPSS desceu para 10.1 enquanto que, aos 18 e 24 meses, o IPSS foi de 7.3 e 5.8. O grupo BCOMB iniciou com IPSS de 9.4. Sofreu agravamento ligeiro e pouco acentuado até aos 6 meses (IPSS de 14). A evolução do IPSS foi, então, flutuante com IPSS de 5.9 aos 12 meses e 9.5 aos 18 meses. Aos 24 meses apresentava IPSS de 6.7. A taxa de retenção urinária (6.4% e 0%) e de RTU-P (2.0% e 0%) foram semelhantes nos grupos MONO e BCOMB, respectivamente (p=0.375 e p=1). A evolução da qualidade das erecções foi semelhante nos dois grupos excepto aos 6 meses em que MONO apresentou o valor 6 e BCOMB 3.7 (p=0.029). A percentagem de doentes potentes foi significativamente inferior nos primeiros 6 meses após a braquiterapia para o grupo BCOMB relativamente ao grupo MONO: 36%–74%; 33%–73%; 33%–75%. Após os 6 meses os grupos foram homogéneos. Grupo 2: O IPSS evoluiu nas próstatas <50ml e >50ml de 7–9 para 15-19 ao primeiro mês e 15-18 ao 3.º mês. Apenas ao primeiro mês é que as diferenças no IPSS foram significativas (p=0.061). Após o 3.º mês os dois grupos foram semelhantes: IPSS de 8 e 12 ao 12.º mês e 5.7 e 6 ao 24.º mês. As taxas de retenção urinária e de RTU-P foram semelhantes (p=0.054 e p=0.286) Grupo 3: A evolução do IPSS, taxas de retenção urinária e de RTU-P foram sobreponíveis em ambos os grupos. A evolução da líbido, erecções, percentagem de doentes potentes, ejaculação, incómodo e satisfação foi significativamente inferior no grupo HORM relativamente ao grupo N HORM apenas ao primeiro mês (valores de p<0.0001; <0.0001; < 0.0001; 0.009 e 0.002 respectivamente) Grupo 4: A evolução do IPSS nos doentes com IPSS elevado foi a seguinte: 22.17(0M); 19.5(1M); 20.5(3M); 15.3(6M); 15.7(12M); 11(18M); 8(24M) A evolução do IPSS nos doentes com IPSS baixo foi a seguinte: 5.9(0M); 15.3(1M); 14.9(3M); 12.2(6M); 8.9(12M); 7.2(18M), 5.5(24M) As taxas de RTU-P (2.8% e 0%) e retenção urinária (5.1% e 5.9%) foram semelhantes em ambos os grupos de doentes (p=1). Conclusões 1. A radioterapia intersticial da próstata com implante transperineal e ecoguiado de Iodo 125 é frequentemente acompanhada de morbilidade urinária transitória e de intensidade moderada. A Morbilidade consiste em sintomatologia do aparelho urinário baixo (“LUTS – lower urinary tract symptoms”) que, na maioria dos doentes, sofre um agravamento máximo do primeiro ao 3.º mês. Segue-se uma melhoria ligeira até ao 6.º mês que é mais acentuada daí em diante. Por volta do 12.º e 18.º mês, a maior parte dos doentes apresenta sintomatologia urinária muito semelhante à que apresentava antes do tratamento. Após o 18.º mês, os doentes mantêm uma melhoria da sintomatologia urinária para além da que apresentavam previamente ao implante. As taxas de retenção urinária e de ressecção transuretral prostática após o implante de braquiterapia são muito baixas, inferiores a 10%. 2. A associação da braquiterapia prostática com radioterapia externa adjuvante influencia a evolução da sintomatologia urinária: o aparecimento da sintomatologia urinária é mais lento, demorando 6 meses a atingir o seu valor máximo que, por sua vez, é de intensidade menos acentuada do que quando a braquiterapia é utilizada em monoterapia. 3. O volume prostático superior a 50 ml não influencia a morbilidade urinária. 4. A terapêutica hormonal, neoadjuvante e adjuvante, não influencia a sintomatologia urinária. 5. Os doentes com sintomatologia urinária prévia muito acentuada não sofrem agravamento da referida sintomatologia. Pelo contrário, apresentam uma melhoria de sintomas urinários desde o primeiro mês, e que se mantém ao longo dos 24 meses de seguimento, apresentando, no final deste período, sintomatologia urinária ligeira e muito inferior à que apresentavam antes do implante. As taxas de retenção urinária e RTU-P após a braquiterapia são semelhantes às que ocorrem nos doentes assintomáticos previamente ao implante. 6. A vida sexual está preservada, em mais de 70% dos casos, ao fim dos 24 meses de seguimento. No entanto, imediatamente após o primeiro mês de seguimento, ocorre uma diminuição ligeira da qualidade das erecções que se mantém, sem melhoria ou agravamento, durante todo o período de seguimento. A hormonoterapia afecta todos os parâmetros da vida sexual, embora de forma apenas temporária. Após a suspensão da terapêutica hormonal este grupo de doentes recupera a actividade sexual e apresenta-se idêntico ao grupo de doentes que não foram sujeitos a essa terapêutica.----------------ABSTRACT: Objectives 1. To assess urinary and sexual morbility after prostatic brachytherapy with the implant of I125 seeds. 2. To assess the influence of neoadjuvant and adjuvant hormone therapy in urinary and sexual morbility after prostatic brachytherapy with the implant of I125 seeds. 3. To assess the effects, on urinary and sexual morbility, of associating external radiotherapy after prostatic brachytherapy with the implant of I125 seeds. 4. To assess the urinary morbility in patients with relative contraindications (voluminous prostates, high IPSS). Material and Methods From September, 2000 to December, 2004 a total of 204 patients were recruited with a diagnosis of localized (T1 and T2) or locally advanced (T3) carcinoma of the prostate and a life expectancy in excess of 10 years. The patients underwent brachytherapy with transperineal seed implant of iodine (I125) as a monotherapy or in combination with hormone therapy and/or external radiotherapy (trimodal treatment). With the aim of evaluating the treatment’s influence on urinary and sexual morbility, a number of patient sub-groups were defined in accordance with certain characteristics normally mentioned as morbility risk factors for prostatic brachytherapy treatment: Group 1: Brachytherapy as monotherapy (MONO) versus brachytherapy in combination with external radiotherapy (BCOMB) Group 2: Voluminous prostates (>50ml) versus non- voluminous prostates (<50ml) Group 3: Brachytherapy in combination with hormone therapy (HORM) versus brachytherapy without hormone therapy (NHORM)Group 4: High IPSS versus a low IPSS. The evolution of the IPSS, QoL, TURP rate and post-implant urinary retention as well as the BSFI were assessed throughout the entire follow-up period. Results Group 1: For the MONO group the IPSS began at 7.1, and then rose to 16.1 and 15.9 in the first and third months, respectively. At month 12, the IPSS had dropped to 10.1 and at month 18 and 24 the IPSS was registered at 7.3 and 5.8, respectively. The BCOMB group started out with an IPSS of 9.4. It underwent a slight and little-significant rise until month 6 (IPSS at 14). The evolution of the IPSS then began to fluctuate from an IPSS of 5.9 at month 12 and 9.5 at month 18. At month 24 we registered an IPSS of 6.7. The urinary retention rate (6.4% and 0%) and TURP rate (2.0% e 0%) were similar to those of the MONO and BCOMB groups, respectively (p=0.375 and p=1). The evolution regarding the quality of erections was similar for the two groups except at 6 months when the MONO group displayed a value of 6 and the BCOMB group 3.7 (p=0.029). The percentage of sexually potent patients was significantly lower in the first six months after brachytherapy for the BCOMB group when compared with the MONO group: 36%–74%; 33%–73%; 33%–75%. After six months, the results became more consistent. Group 2: IPSS results evolved in <50ml and >50ml prostates from 7–9 to 15-19 in the first month and from 15-18 after the third month. It was only in the first month that the differences in the IPSS were significant (p=0.061). After the third month, the two groups displayed similar outcomes: IPSS 8 and 12 at month 12 and 5.7 and 6 at month 24. Urinary retention and TURP rates were similar (p=0.054 e p=0.286). Group 3: IPSS evolution and rates of urinary retention and TURP were identical in both groups. Figures regarding libido, erections, percentage of sexually potent patients, ejaculation, discomfort and sexual satisfaction were always significantly lower for the HORM group, when compared to the NHORM group in the first month only (values of p<0.0001; <0.0001; <0.0001; 0.009 e 0.002, respectively). Group 4: IPSS evolution in patients with a high IPSS was as follows: 22.17(0M); 19.5(1M); 20.5(3M); 15.3(6M); 15.7(12M); 11(18M); 8(24M) IPSS evolution in patients with a low IPSS was as follows: 5.9(0M); 15.3(1M); 14.9(3M); 12.2(6M); 8.9(12M); 7.2(18M), 5.5(24M)TURP rates (2.8% e 0%) and those for urinary retention (5.1% e 5.9%) were similar in both patient groups (p=1). Conclusions 1. Interstitial radiotherapy of the prostate with transperineal, ultrasound-guided implant of Iodine-125 seeds is often followed by transitory urinary morbility of moderate intensity. The morbility involves symptoms of the lower urinary tract which, in most cases are at their worst from the first to the third months. There is a slight improvement up to the sixth month, at which point improvement becomes more accentuated. Around months 12 or 18, most patients display urinary symptoms that are very similar to those noted before treatment. After month 18, patients’ urinary symptoms continue to improve past the point they displayed prior to the implant. Urinary retention rates and those for transurethral resection of the prostate are very low (below 10%), after brachytherapy seed implant. 2. The combination of prostatic brachytherapy and adjuvant external radiotherapy affects the evolution of urinary symptoms: the appearance of urinary symptoms is much slower, taking six months to peak, and is less intense than when brachytherapy is employed as the only means of treatment. 3. The fact that the prostate displays a volume greater than 50 ml does not influence urinary morbility. 4. Neoadjuvant and adjuvant hormone therapy do not influence urinary symptomology. 5. Patients with severe, preexisting symptoms of the urinary tract do not experience a worsening of those symptoms. On the contrary, they exhibit an improvement in urinary symptoms as of the first month. This improvement continues for the 24 months, after which patients display symptoms of the urinary tract that are slight and a noticeable improvement over the urinary complaints registered before the implant. Urinary retention and TURP rates subsequent to brachytherapy are similar to those registered for asymptomatic patients. 6. The patient’s sexual performance is maintained in more than 70% of the cases, as noted after 24 months of follow-up. However, immediately after the first follow-up month there is a lessening in the quality of erections that continues, without improving or worsening, for the whole follow-up period. Hormone therapy affects all the parameters of sexual performance, albeit temporarily. After suspending hormone therapy, this group recovered with regard to sexual performance, and showed itself to be identical to the group of patients that had not undergone hormone therapy.-------------------RESUMÉ:Objectives 1. Évaluer la morbilité urinaire et sexuelle après la realisation la curiethérapie de la prostate avec implant de I125. 2. Évaluer l’ influence de la thérapie hormonale néoadjuvante et adjuvante en ce qui concerne la morbilité urinaire et sexuelle après la réalisation de la curiethérapie (I125). 3. Évaluer l’influence de l’association de la radiothérapie externe dans la morbilité urinaire et sexuelle après la réalisation de la curiethérapie (I125). 4. Évaluer la morbilité urinaire des malades avec des contre indications relatives (prostates volumineuses, IPSS élevé). Matériel et méthodologie De Septembre 2000 à Décembre 2004, on a recruté 204 patients ayant pour diagnostique un carcinome de la prostate localisé (T1 et T2) ou localement avancé (T3) et dont l’expectative de vie était de plus de 10 ans. Ils ont été soumis au traitement de la curiethérapie avec l’implantation transpérinéal de l’iode 125 (I125) en monothérapie ou en traitement combiné avec une thérapie hormonale et/ou radiothérapie externe (traitement trimodale). Il y a eu plusieurs sous-catégories de patients, et cela dépend de quelques caractéristiques normalement considérées comme des facteurs à risque en ce qui concerne la morbilité du traitement de la curiethérapie de la prostate, et l’objective étant d’analyser son influence sur la morbilité urinaire et sexuelle. Groupe 1: Curiethérapie en traitement unique (MONO) par rapport à la curiethérapie associée au traitement externe (BCOMB). Groupe 2: Prostates volumineuses (>50ml) par rapport au prostates qui ne sont pas volumineuses (<50ml). Groupe 3: Curiethérapie associée au traitement hormonale (HORM) par rapport à la curiethérapie sans traitement hormonale (NHORM). Groupe 4: IPSS élevé par rapport au IPSS diminué. Nous avons évalué l’evolution du IPSS, Qualité de vie, le taux de RTU-P et la retention de l’urine après l’implant, BSFI pendant toute la période du traitement. Résultats Groupe 1: Pour le groupe MONO l’IPSS a commençé avec un taux de 7.1, et les patients ont souffert d’un empirement allant jusqu’à 16.1 et 15.9 pendant le premier et le troisième mois. 12 mois après l’IPSS diminua jusqu’à 10.1 À 18 mois le taux fût de 7.3 et à 24 mois il diminua encore jusqu’à atteindre 5.8. Le groupe BCOMB commença avec un taux d’IPSS de 9.4. Ils souffrirent un empirement légér et peu accentué jusqu’aux 6 premiers mois (IPSS de 14). L’évolution de l’IPSS était fluctuante allant de 5.9 à 12 mois et 9.5 à 18 mois. À 24 mois, l’IPSS était de 6.7. Le taux de retention de l’urine (6.4% et 0%) et de la RTU-P (2.0% et 0%) étaient simmilaires dans les groupes MONO et BCOMB respectivement (p=0.375 et p=1). L’ évolution de la qualité des érections fût semblable dans les 2 groupes excepté le groupe MONO qui présenta une valeure de 6 à 6 mois et le groupe BCOMB qui présenta une valeure de 3.7 (p=0.029). Le pourcentage des malades sexuellement puissants a été significativement inférieur pendant les 6 premiers mois depuis la curiethérapie pour le groupe BCOMB si on le compare au groupe MONO: 36%–74%; 33%–73%; 33%–75%. Après cette période, les groupes eûrent des résultats homogénes. Groupe 2: L’ IPSS a évolué dans les prostates <50ml et >50ml de 7–9 jusqu’à 15-19 pendant le premier mois et jusqu’à 15-18 au 3ème mois. C’est seulement pendant le premier mois que les différences de l’IPSS ont été significatives (p=0.061). Après le 3ème mois les deux groupes ont eu des résultats semblables: IPSS 8 et 12 à 12.º mois et 5.7 et 6 à 24.º mois. Le taux de retention de l’urine et de la RTU-P ont été simmilaires (p=0.054 e p=0.286). Groupe 3: L’evolution de l’ IPSS, les taux de retention de l’urine et de la RTU-P pourraient se surposer pour les deux groupes. L’évolution de la libido, des érections, le pourcentage des malades sexuellements puissants, l’incommodité et la satisfaction ont été toujours significativement infériures dans le groupe HORM par rapport au groupe NHORM dès le premier mois (valeurs de p <0.0001; <0.0001; < 0.0001; 0.009 et 0.002 respectivement) Groupe 4: L’évolution de l’ IPSS pour les malades ayant un IPSS élevé fût ainsi: 22.17(0M); 19.5(1M); 20.5(3M); 15.3(6M); 15.7(12M); 11(18M); 8(24M)L’évolution de l’ IPSS pour les malades ayant un IPSS diminué fût ainsi:5.9(0M); 15.3(1M); 14.9(3M); 12.2(6M); 8.9(12M); 7.2(18M), 5.5(24M) Le taux de RTU-P (2.8% e 0%) et de retention de l’urine (5.1% et 5.9%) ont été semblables dans les 2 groupes de malades (p=1). Conclusions 1. La radiothérapie interstitielle de la prostate avec implant transpérinéal de l’iode 125 est fréquamment acompagnée d’une morbilité urinaire transitoire et d’intensité modérée. La morbilité consiste d’une symptomatologie de l’apparéil urinaire inférieure qui, dans la plupart des malades, empire gravement du premier au troisième mois. La situation s’améliore légèrement jusqu’au 6ème mois, l’amélioration étant plus accentuée à partir de là. Autour du 12ème jusqu’au 18ème mois, la majorité des malades présente une symptomatologie urinaire qui se ressemble beaucoup à celle qu’ils avaient avant le traitement. Après le 18ème mois l’amélioration de la symptomatologie urinaire est constante par rapport à celle qu’ils présentaient avant l’implant. Le taux de retention de l’urine et de RTU-P après l’implant de la curiethérapie sont très basses, au dessus de 10%. 2. L’association de la curiethérapie de la prostate avec la radiothérapie externe adjuvante a une influence sur l’évolution de la symptomatologie urinaire: l’apparition des symptômes est plus lente, prenant jusqu’à 6 mois pour atteindre son niveau maximum, qui à son tour, a une intensité moins accentuée que lorsque la curiethérapie est utilisée en monothérapie. 3. Le volume de la prostate supérieure à 50 ml n’a pas d’influence sur la morbilité urinaire. 4. La thérapie hormonale, néoadjuvante et adjuvante, n’a pas d’influence sur la symptomatologie urinaire. 5. Les malades ayant une symptomatologie urinaire préalable et très accentuée ne souffrent pas d’empirement de leur situation. Au contraire, ils présentent une amélioration des symptômes urinaires à partir du premier mois et ceci se maintient tout au long des 24 mois que dure le traitement, ayant à la fin de cette période une symptomatologie urinaire légère et beaucoup plus basse que celle qu’ils présentaient avant l’implant. Le taux de retention de l’urine et de la RTU-P après la curiethérapie est simmilaire à celui que les malades qui n’ont pas de symptômes présentent avant l’implant. 6. La vie sexuelle est préservée dans plus du 70% des cas à la fin de la période de traitement (24 mois). Entretemps, immédiatement après le premier mois d’étude, on note une légère diminution de la qualité des érections qui se maintient sans amélioration ou empirement tout au long du traitement. Le traitement à base d’hormones affecte tous les paramètres de la vie sexuelle, mais de façon temporaire. Après la suspension de la thérapie hormonale ce groupe de malades récupère l’activité sexuelle et se présente de façon égale au groupe de malades qui n’ont pas été soumis à ce traitement.

Os autores fazem a análise retrospectiva dos 53 novos casos de Síndrome Nefrótico (S,N.) seguidos na Consulta de Nefrologia, de Novembro de 1988 a Marçoo de 1994, com o objectivo de avaliar alterações do padrão da doença em relação ao estudo feito anteriormente,em 1988. Foram estudados aspectos epidemiológicos, clínicos, terapêuticos e evolutivos. Identificaram-se 44 casos de S,N. primário (83%) - dos quais 61,4% comportaram-se como cortico-sensíveis, 25% como cortico-dependentes e 13,6% como cortico-resistentes. Com S.N. secundário a infecção (8 casos -15%), Lupus Eritematoso Sistémico (L.E.S.) e Amiloidose e 1 caso de S.N, congénito (2%). O elevado numero de cortico-dependemes atribui-se a um maior rigor no diagnóstico da recaida e/ou a alterações do padrão de vida das crianças. Verificou-se também um reduzido número de altas, relacionado com atitudes mais prudentes que se vêm adoptando em relação à evolução da doença.

Baseados num caso de associação de abcessos renal, perirrenal e de quisto renal infectado, ocorrendo após biopsia prostática, os autores fazem revisão bibliográfica dos abcessos renais, classificados segundo características anatómicas perinéfrico, cortical e cortico- -medular (nefrite bacteriana aguda focal/multifocal, pielonefrite enfisematosa e pielonefrite xantogranulomatosa). Os abcessos renais são mais frequentes em diabéticos, menos vezes em alcoólicos, desnutridos e imunodeprimidos, estando associados geralmente a pielonefrites recorrentes, refluxo vesicoureteral, litíase urinária, ou resultam de êmbolos sépticos hematogéneos originados em focos extra-renais de infecção. A Escherichia coli é responsável pela maioria dos casos, e menos vezes a Klebsiella Proteus, Enterobacter, Pseudomonas, e Estafilococo aureus. Descrevem-se a expressão clínica, complicações evolutivas e diagnóstico radiológico, enfatizando-se as infecções acompanhadas de formação de gás. Referem-se opções terapêuticas médico-cirúrgicas, cujas indicações dependem do diagnóstico precoce, complicações e factores predisponentes.

RESUMO: pela contracção involuntária de grupos musculares de extensão variável, originando movimentos involuntários e posturas anómalas, por vezes dolorosas. O tratamento convencional consiste em injecções localizadas de toxina botulínica, podendo, em casos refractários, estar indicado o tratamento por estimulação cerebral profunda. A neurobiologia da distonia focal primária permanece incompletamente compreendida. Os estudos de neuro-imagem estrutural e funcional revelam alterações subtis da anatomia e funcionamento do estriado e das vias cortico-basais, com destaque para o aumento do volume, da actividade metabólica e da neuroplasticidade do putamen e de áreas corticais motoras, pré-motoras e sensitivas. O conjunto destas alterações aponta para uma disrupção da regulação inibitória de programas motores automáticos sustentados pelo estriado e pelas vias ortico-subcorticais. Nos últimos anos tem crescido o interesse pelas manifestações psiquiátricas e cognitivas da distonia (estas últimas muito pouco estudadas). Tem despertado particular interesse a possível associação entre distonia focal primária e perturbação obsessivo-compulsiva (POC), cuja neurobiologia parece notavelmente sobreponível à da distonia primária. Com efeito, os estudos de neuro-imagem estrutural e funcional na POC revelam consistentemente aumento do volume e actividade do estriado e do córtex órbito-frontal, apontando mais uma vez para uma disfunção do controlo inibitório, no estriado, de programas comportamentais e cognitivos automáticos. Objectivos: 1. Explorar a prevalência e intensidade de psicopatologia em geral, e de psicopatologia obsessivo-compulsiva em particular, numa amostra de indivíduos com distonia focal primária; 2. Explorar a ocorrência, natureza e intensidade de alterações do funcionamento cognitivo numa amostra de indivíduos com distonia focal primária; 3. Investigar a associação entre a gravidade da distonia focal, a intensidade da psicopatologia, e a intensidade das alterações cognitivas. Metodologia: Estudo de tipo transversal, caso-controlo, observacional e descritivo, com objectivos puramente exploratórios. Casos: 45 indivíduos com distonia focal primária (15 casos de blefaroespasmo, 15 de cãibra do escrivão, 15 de distonia cervical espasmódica), recrutados através da Associação Portuguesa de Distonia. Critérios de inclusão: idade = 18; distonia focal primária pura (excluindo casos de distonia psicogénica possível ou provável de acordo com os critérios de Fahn e Williams); Metabolismo do cobre e Ressonância Magnética Nuclear sem alterações. Controlos doentes: 46 casos consecutivos recrutados a partir da consulta externa do Hospital Egas Moniz: 15 doentes com espasmo hemifacial, 14 com espondilartropatia cervical, 17 com síndrome do canal cárpico. Controlos saudáveis: 30 voluntários. Critérios de exclusão para todos os grupos: Mini-Mental State Examination patológico, tratamento actual com anti-colinérgicos, antipsicóticos, inibidores selectivos da recaptação da serotonina, antidepressivos tri- ou tetracíclicos. Avaliação: Avaliação neurológica: história e exame médico e neurológico completos. Cotação da gravidade da distonia com a Unified Dystonia Rating Scale. Avaliação psicopatológica: Symptom Check-List-90-Revised; entrevista psiquiátrica de 60 minutos incluindo a Mini-International Neuropsychiatric Interview (MINI), versão 4.4 (validada em Português), complementada com os módulos da MINI Plus versão 5.0.0 para depressão ao longo da vida e dependência/ abuso do álcool e outras substâncias ao longo da vida; Yale-Brown Obsessive-Compulsive Symptom Checklist e a Yale-Brown Obsessive-Compulsive Scale (Y-BOCS). Avaliação neuropsicológica: Wisconsin Card Sorting Test (WCST; flexibilidade cognitiva); Teste de Stroop (inibição de resposta); Block Assembly Test (capacidade visuo-construtiva); Teste de Retenção Visual de Benton (memória de trabalho visuo-espacial). Análise estatística:os dados foram analisados com a aplicação informática SPSS for Windows, versão 13. Para a comparação de proporções utilizaram-se o teste do Chi-quadrado e o teste de Fisher. Para a comparação de variáveis quantitativas entre dois grupos utilizou-se o teste t de Student ou o teste U de Mann-Whitney (teste de Wilcoxon no caso de amostras emparelhadas). Para comparações de médias entre três grupos recorreu-se à Análise de Variância a um factor (variáveis de intervalo e de rácio), ou ao teste de Kruskal-Wallis (variáveis ordinais). Para o estudo da associação entre variáveis foram utilizados os coeficientes de correlação de Pearson ou de Spearman, a análise de correlações canónicas, a análise de trajectórias e a regressão logística. Adoptou-se um Alpha de 0.05. Resultados: Os doentes com distonia focal primária apresentaram uma pontuação média na Y- -BOCS significativamente superior à dos dois grupos de controlo. Em 24.4% dos doentes com distonia a pontuação na Y-BOCS foi superior a 16. Estes doentes eram predominantemente mulheres, tinham uma maior duração média da doença e referiam predominantemente sintomas obsessivo-compulsivos (SOC) de contaminação e lavagem. Os dois grupos com doença crónica apresentaram pontuações médias superiores às dos indivíduos saudáveis nas escalas de ansiedade, somatização e psicopatologia geral. Os doentes com distonia tratados com toxina botulínica apresentaram pontuações inferiores às dos doentes não tratados nas escalas de ansiedade generalizada, fobia, somatização e depressão, mas não na Y-BOCS. Sessenta por cento dos doentes com distonia apresentavam pelo menos um diagnóstico psiquiátrico actual ou pregresso. O risco de apresentar um diagnóstico psiquiátrico actual era menor nos doentes tratados com toxina botulínica, aumentando com a gravidade da doença. A prevalência de POC foi 8,3% e a de depressão major 37,7%. No WCST e na Prova de Benton, os doentes com distonia focal primária demonstraram um desempenho inferior ao de ambos os grupos de controlo, cometendo sobretudo erros perseverativos. Os doentes com distonia e pontuação na Y-BOCS > 16 cometeram mais erros e respostas perseverativas no WCST do que os restantes doentes com distonia. As análises de correlações e de trajectórias revelaram que nos doentes com distonia a gravidade da distonia foi, juntamente com a idade e a escolaridade, o factor que mais interagiu com o desempenho cognitivo. Discussão: o nosso estudo é o primeiro a descrever, nos mesmos doentes com distonia focal primária, SOC significativos e alterações cognitivas. Os nossos resultados confirmam a hipótese de uma associação clínica específica entre distonia focal primária e psicopatologia obsessivo-compulsiva. Confirmam igualmente que a distonia focal primária está associada a um maior risco de desenvolver morbilidade psiquiátrica ansiosa e depressiva. O tratamento com toxina botulínica reduz este risco, mas não influencia os SOC. Entre os doentes com distonia, os que têm SOC significativos poderão diconstituir um grupo particular com maior duração da doença (mas não uma maior gravidade), predomínio do sexo feminino e predomínio de SOC de contaminação e limpeza. Em termos cognitivos, os indivíduos com distonia focal primária apresentam défices significativos de flexibilidade cognitiva (particularmente acentuados nos doentes com SOC significativos) e de memória de trabalho visuo-espacial. Estes últimos devem-se essencialmente a um défice executivo e não a uma incapacidade visuo-construtiva ou visuo-perceptiva. A disfunção cognitiva não é explicável pela psicopatologia depressiva nem pela incapacidade motora, já que os controlos com doença periférica crónica tiveram um desempenho superior ao dos doentes com distonia. No seu conjunto os nossos resultados sugerem que os SOC que ocorrem na distonia focal primária constituem uma das manifestações clínicas da neurobiologia desta doença do movimento. O predomínio de sintomas relacionados com higiene e o perfil disexecutivo de alterações cognitivas–perseveração e dificuldades executivas de memória de trabalho visuo-espacial – apontam para a via cortico-basal dorso-lateral e para as áreas corticais que lhe estão associadas como estando implicadas na tripla associação entre sintomas motores, obsessivo-compulsivos e cognitivos. Conclusões: A distonia focal primária é um síndrome neuropsiquiátrico complexo com importantes manifestações não motoras, nomeadamente compromisso cognitivo do tipo disexecutivo e sintomas obsessivo-compulsivos. Clinicamente estas manifestações representam necessidades de tratamento que vão muito para além da simples incapacidade motora, devendo ser activamente exploradas e tratadas.-------------- ABSTRACT: Introduction: primary focal dystonia is an idiopathic movement disorder that manifests as involuntary, sustained contraction of muscular groups, leading to abnormal and often painful postures of the affected body part. Treatment is symptomatic, usually with local intramuscular injections of botulinum toxin. The neurobiology of primary focal dystonia remains unclear. Structural and functional neuroimaging studies have revealed subtle changes in striatal and cortical-basal pathway anatomy and function. The most consistent findings involve increased volume and metabolic activity of the putamen and of motor, pre-motor and somato-sensitive cortical areas. As a whole, these changes have been interpreted as reflecting a failure of striatal inhibitory control over automatic motor programs sustained by cortical-basal pathways. The last years have witnessed an increasing interest for the possible non-motor – mainly psychiatric and cognitive – manifestations of primary focal dystonia. The possible association of primary focal dystonia with obsessive-compulsive disorder (OCD) has raised particular interest. The neurobiology of the two disorders has indeed remarkable similarities: structural and functional neuroimaging studies in OCD have revealed increased volume and metabolic activity of the striatum and orbital-frontal cortex, again pointing to a disruption of inhibitory control of automatic cognitive and behavioural programs by the striatum. Objectives: 1. To explore the prevalence and severity of psychopathology – with a special emphasis on obsessive-compulsive symptoms (OCS) – in a sample of patients with primary focal dystonia;2. To explore the nature and severity of possible cognitive dysfunction in a sample of patients with primary focal dystonia; 3. To explore the possible association between dystonia severity, psychiatric symptom severity, and cognitive performance, in a sample of patients with primary focal dystonia. Methods: cross-sectional, case-control, descriptive study. Cases: forty-five consecutive, primary pure focal dystonia patients recruited from the Portuguese Dystonia Association case register (fifteen patients with blepharospasm, 15 with cervical dystonia and 15 with writer’s cramp). Inclusion criteria were: age = 18; primary pure focal, late-onset dystonia (excluding possible or probable psychogenic dystonia according to the Fahn & Williams criteria); normal copper metabolism and Magnetic Resonance Imaging. Diseased controls: forty-six consecutive subjects from our hospital case register (15 patients with hemi-facial spasm; 14 with cervical spondilarthropathy and cervical spinal root compression; 17 with carpal tunnel syndrome). Healthy controls were 30 volunteers.Exclusion criteria for all groups: Mini-Mental State Examination score below the validated cut-off for the Portuguese population (<23 for education between 1 and 11 years; <28 for education >11 years); use of anti-cholinergics, neuroleptics, selective serotonin reuptake inhibitors, triciclic or tetraciclic antidepressants. Assessment: neurological assessment: complete medical and neurological history and physical examination; dystonia severity scoring with the Unified Dystonia Rating Scale. Psychiatric assessment:Symptom Check-List-90-Revised; 60 minute-long psychiatric interview, including Mini-International Neuropsychiatric Interview (MINI), version 4.4 (validated Portuguese version), extended with the sections for life-time major depressive disorder and life-time alcohol and substance abuse disorder from MINI-Plus version 5.0.0; Yale-Brown Obsessive-Compulsive Symptom Checklist and Yale-Brown Obsessive-Compulsive Scale (Y-BOCS). Cognitive assessment: Wisconsin Card Sorting Test (WCST; cognitive set-shifting ability); Stroop Test (response inhibition); Block Assembly Test(visual-constructive ability); Benton’s Visual Retention Test (visual-spatial working memory). Statistic analysis: Data were analyzed with SPSS for Windows version 13. Proportions were compared using Chi-Square test, or Fisher’s exact test when appropriate. Student’s t-test or Mann-Whitney’s U test (or Wilcoxon’s teste in the case of matched samples) were used for two-group comparisons. P-values were corrected for multiple comparisons. One-way ANOVA with Bonferroni post-hoc analysis (interval data), or the Kruskal-Wallis Test (ordinal data), were used for three-group comparisons. Associations were analysed with Pearson’s or Spearman’s correlation coefficients, canonical correlations, path analysis and logistic regression analysis. Alpha was set at 0.05. Results: Dystonia patients had higher Yale-Brown Obsessive-Compulsive Symptom scores than both control groups. 24.4% of primary dystonia patients had a Y-BOCS score > 16. These patients were predominantly women; they had longer disease duration, and showed a predominance of hygiene-related OCS. The two groups with chronic disease had higher anxiety, somatization and global psychopathology scores than healthy subjects. Primary dystonia patients undergoing treatment with botulinum toxin had lower anxiety, phobia, somatization and depression scores than their untreated counterparts, but similar Y-BOCS scores. Sixty percent of primary dystonia patients had at least one lifetime psychiatric diagnosis. The odds of having a currently active psychiatric diagnosis were lower in botulinum toxin treated patients, and increased with dystonia severity. The prevalence of OCD was 6.7%, and the lifetime prevalence of major depression was 37.7%. Primary dystonia patients had a lower performance than the two control groups in both the WCST and Benton’s Visual Retention Test, mainly due to an excess of perseveration errors. Primary dystonia patients with Y-BOCS score > 16 had much higher perseveration error and perseveration response scores than dystonia patients with Y-BOCS = 16. Correlation and path analysis showed that, in the primary dystonia group, dystonia severity, along with age and education, was the main factor influencing cognitive performance. Discussion: our study is the first description ever of concomitant significant OCS and cognitive impairment in primary dystonia patients. Our results confirm that primary dystonia is specifically associated with obsessive-compulsive psychopathology. They also confirm that primary focal dystonia patients are at a higher risk of developing anxious and depressive psychiatric morbidity. Treatment with botulinum toxin decreases this risk, but does not influence OCS. Primary focal dystonia patients with significant OCS may constitute a particular subgroup. They are predominantly women, with higher disease duration (but not severity) and a predominance of hygiene related OCS.In terms of cognitive performance, primary focal dystonia patients have significant deficits involving set-shifting ability and visual-spatial working memory. The latter result from an essentially executive deficit, rather than from a primary visual-constructive apraxia or perceptual deficit. Furthermore, cognitive flexibility difficulties were more prominent in the subset of primary dystonia patients with significant OCS. The cognitive dysfunction found in dystonia patients is not attributable to depressive psychopathology or motor disability, as their performance was significantly lower than that of similarly impaired diseased controls. Our results suggest that OCS in primary focal dystonia are a direct, primary manifestation of the motor disorder’s neurobiology. The predominance of hygiene-related symptoms and the disexecutive pattern of cognitive impairment – set-shifting and visual-spatial working memory deficits – suggest that the dorsal-lateral cortical-basal pathway may play a decisive role in the triple association of motor dysfunction, OCS and cognitive impairment. Conclusions: primary focal dystonia is a complex neuropsychiatric syndrome with significant non- -motor manifestations, namely cognitive executive deficits and obsessive-compulsive symptoms.Clinically, our results show that PFD patients may have needs for care that extend far beyond a merely motor disability and must be actively searched for and treated.

Learning novel actions and skills is a prevalent ability across multiple species and a critical feature for survival and competence in a constantly changing world. Novel actions are generated and learned through a process of trial and error, where an animal explores the environment around itself, generates multiple patterns of behavior and selects the ones that increase the likelihood of positive outcomes. Proper adaptation and execution of the selected behavior requires the coordination of several biomechanical features by the animal. Cortico-basal ganglia circuits and loops are critically involved in the acquisition, learning and consolidation of motor skills.(...)

The ligature of the left renal vein is an alternative whenever this vessel is injured. The purpose of this study was to evaluate the capacity of the affluents of the left renal vein, proximal to the ligature, to maintain tissue vitality and function of the left kidney. Fifteen mongrel male dogs were divided in 3 groups of 5 dogs: Group I (control) - a laparotomy was performed, and the abdominal structures were only identified; Group II - the left renal vein was tied, close to vena cava; Group III - the same procedure as for Group II and a right nephrectomy. Blood urea nitrogen and serum creatinine levels were measured before the procedure, and every 3 days during 4 weeks in the postoperative period. Renal arteriography and an excretory urogram were performed on the animals that survived 60 days. Thereafter, or immediately after precocious death, the kidneys were removed for histological examination. All the animals of Group III died before two months (mean = 10.5 +-3.2 days), while the animals of Group II survived during that period. There was a complete exclusion of the left kidney in all dogs that underwent renal vein ligature. In the animals of Group II, the renal cortico-medullary limits could not be identified. At microscopy, the aspect was suggestive of nephrosclerosis. In the animals of Group III, the left kidney was enlarged, and a great amount of intravascular and intrapelvic blood clots were observed. At microscopy, extensive areas of necrosis, inflammatory infiltration, and hemorrhage were identified. In conclusion, the tributaries of the renal vein were not sufficient to maintain the tissue vitality and function of the left kidney after ligature of its main vein.

There are only a few treatments available for Tourette syndrome (TS). These treatments frequently do notwork in patients with moderate to severe TS [1]. Neuroimaging studies show a correlation between tics severity and increased activation over motor pathways, along with reduced activation over the control areas of the cortico-striato-thalamo-cortical circuits [2]. Moreover, the temporal pattern of tic generation suggests that cortical activation especially in the SMA precedes subcortical activation [3]. Following this assumption, here we explored the brain effects of 10-daily sessions of cathodal transcranial Direct Current Stimulation (tDCS) delivered over the pre-SMA in a patient with refractory and severe TS and also assessed whether those changes were long lasting (up to 6 months).

Studies have been carried out on the method of Britton and Silvette modified by Reinecke and Kendall, for the evaluation of cortico-adrenal extracts, based on the deposition of glycogen in the liver of adrenaletomized rats. The test was performed in a total of 180 normal and adrenalectomized rats. The extracts tested were: a) an aqueous extract of cortico-adrenal cortex prepared by the Swingle and Pfiffner technique; b) the same extract added with ascorbic acid (Supracortin Labor); c) desoxycorticosterone acetate (Percortol Ciba and Syncortyl Roussell). Male rats were used, ranging from 40-200g, fed since the 18 th days old with a special diet, in which they were maintained until the day before the injection. Adrenalectomy was performed under urethane anesthesia. The fourth day after operation, food was removed and they were fasted for 24 hours. In the morning of the fifth day, injections of the material to be assayed were given at hourly and two hours intervals, during four to eight hours. One or two hours after the last injection, the animals were sacrified, the livers removed and dropped into a hot 30% solution of potassium hydroxide, and worked by Good, Kramer and Somogyi method. The glycogen was calculated as milligrams per lOOg of body and liver weight. The results obtained are shown in the tables I, II, and III. When several dosages of the same sample of extract were made (5 animals each dose), the amount of glycogen deposited in the liver per lOOg of body and liver weight, was found to be a positive function of the dose injected. The graph 2, shows these results. The synthetic compounds were ineffective. Our results are in agreement with those of Reinecke and Kendall and of Olson et al.

Résumé L'accident vasculaire cérébral sensoriel pur est un des syndromes lacunaires, dû à l'occlusion de petits vaisseaux cérébraux, souvent dans le cadre d'une lésion intéressant le noyau ventro-caudal du thalamus. Il produit un hémisyndrome sensitif pur, et parfois un syndrome douloureux se développe à distance de l'événement aigu. Afin d'étudier la récupération fonctionnelle dans le cortex somatosensoriel (SI) après une telle lésion dans le thalamus, un modèle de lésion excitotoxique a été développé dans le système somatosensoriel de la souris adulte, caractérisé par la présence de formations cytoarchitectoniques dans SI appelées "tonneaux". Chacun de ces tonneaux correspond à la représentation corticale d'une vibrisse du museau. L'activité métabolique a été mesurée dans SI à différents intervalles après la lésion, à l'aide de déoxyglucose marqué radioactivement. Dans les deux premiers jours suivant celle-ci, l'activité métabolique diminue de manière importante dans toutes les couches corticales, avec une atteinte plus marquée dans la couche IV, principale projection des axones thalamo-corticaux. Une récupération de l'activité métabolique se produit ensuite, d'autant plus marquée que le délai après la lésion est grand. Cette récupération s'observe dans toutes les couches coticales, les couches I et Vb récupérant plus rapidement que les couches II, III, IV, Va et VI. Cinq semaines après la lésion, l'absence des vibrisses correspondant à la partie déafférentée de SI diminue l'activité métabolique corticale de 32% et démontre l'activation par la périphérie de cette partie de l'écorce, malgré la perte des axones thalamo-corticaux provenant du noyau ventro-caudal. Des expériences de traçage rétrograde ont montré une augmentation des projections intracorticales sur la partie déafférentée de l'écorce, en particulier de longue distance, ainsi que des projections interhémisphériques, mais n'ont pas permis de mettre en évidence de nouvelle projection thalamique, indiquant une origine corticale à la récupération fonctionnelle observée. Abstract To study the degree and time course of the functional recovery in the somatosensory cortex (SI) after an excitotoxic lesion in the adult mouse thalamus, metabolic activity was determined in SI at various times points post lesion. Immediately after the lesion, metabolic activity in the thalamically deafferented part of SI was at its lowest value but increased progressively at subsequent time points. This was seen in all cortical layers, however, layers I and Vb recover more rapidly than layers II, III, IV, Va and VI. Removal of the mystacial whiskers corresponding to the deafferented area, 5 weeks after cortical recovery, produced a subsequent 32% drop in metabolic activity, demonstrating peripheral sensory activation of this part of the cortex. Tracing experiments revealed that the deafferented cortex did not receive a novel thalamic input, but cortico-cortical and contralateral barrel cortex projections to this area were reinforced. We conclude that the cortical functional recovery after a thalamic lesion is, at least partially, due to modified cortico-cortical and callosal projections to the deafferented cortical area.

Anatomical structures and mechanisms linking genes to neuropsychiatric disorders are not deciphered. Reciprocal copy number variants at the 16p11.2 BP4-BP5 locus offer a unique opportunity to study the intermediate phenotypes in carriers at high risk for autism spectrum disorder (ASD) or schizophrenia (SZ). We investigated the variation in brain anatomy in 16p11.2 deletion and duplication carriers. Beyond gene dosage effects on global brain metrics, we show that the number of genomic copies negatively correlated to the gray matter volume and white matter tissue properties in cortico-subcortical regions implicated in reward, language and social cognition. Despite the near absence of ASD or SZ diagnoses in our 16p11.2 cohort, the pattern of brain anatomy changes in carriers spatially overlaps with the well-established structural abnormalities in ASD and SZ. Using measures of peripheral mRNA levels, we confirm our genomic copy number findings. This combined molecular, neuroimaging and clinical approach, applied to larger datasets, will help interpret the relative contributions of genes to neuropsychiatric conditions by measuring their effect on local brain anatomy.Molecular Psychiatry advance online publication, 25 November 2014; doi:10.1038/mp.2014.145.
