FUNDAMENTO: A angina pré-infarto (API) pode ser um marcador de pré-condicionamento isquêmico. Foi demonstrada redução da área infartada, do remodelamento ventricular, da incidência de insuficiência cardíaca, choque cardiogênico ou morte, quando a API estava presente. Esses resultados foram mais evidentes em adultos, porém, não em idosos. OBJETIVO: Avaliar a relação entre API e a evolução clínica de pacientes idosos com infarto agudo do miocárdio (IAM). MÉTODOS: Estudo tipo série de casos com grupo de comparação. Foram incluídos 36 pacientes com diagnóstico de IAM com elevação do segmento ST. Os pacientes foram distribuídos em grupo A (21 pacientes com API) e grupo B (15 pacientes sem API). RESULTADOS: A idade média da população estudada foi 70,5 anos. A maioria (73%) dos pacientes era do sexo masculino. O índice de massa corpórea médio foi 25,3 kg/m2. A amostra era constituída por 77,8% de hipertensos, 27,8% de diabéticos e 32,4% de dislipidêmicos. Dor torácica tipo A foi relatada por 71,4% dos estudados. A maioria (72,2%) dos idosos foi classificada em Killip I. Os desfechos clínicos nos grupos A e B foram: angina pós-infarto 9,5% versus 20%, p=0,630; insuficiência cardíaca 23,8% versus 13,3%, p=0,674; revascularização de urgência 4,8% versus 6,7%, p=1; arritmia cardíaca 0% versus 6,7%, p=0,417. Não foi constatado nenhum caso de reinfarto, choque cardiogênico e morte até 30 dias em ambos os grupos. CONCLUSÃO: A presença da angina pré-infarto não se associou com uma melhor evolução clínica em idosos acometidos por IAM nesta série de casos.

FUNDAMENTO: A frequência cardíaca em repouso (cuja média está entre 60 e 80 bpm) é uma das mais simples variáveis cardiovasculares e tem sido considerada como um preditor de mortalidade cardiovascular e geral. OBJETIVO: Avaliar o valor preditivo da frequência cardíaca em repouso (FCR), antes do teste ergométrico (TE), na mortalidade cardiovascular (CV) e geral. MÉTODOS: Estudo de caso-controle, que utilizou informações contidas nos bancos de dados do laboratório de ergometria de um hospital especializado em cardiologia e os registros de óbitos da Secretaria da Saúde em uma cidade do sul do Brasil, de janeiro de 1995 a junho de 2007. Foram analisados 7.055 pacientes, sendo 1.645 (23,3%) do grupo caso (óbitos) e 5.410 (76,7%) do grupo controle (vivos). Foi calculado o ponto de corte da FCR para mortalidade, através da curva ROC e realizada a análise multivariada para as variáveis selecionadas. Os desfechos foram mortalidade CV e geral. RESULTADOS: A incidência de mortalidade CV foi de 674 casos (9,5%); a FCR > 78 bpm foi o ponto de corte. Após ajustado para as variáveis selecionadas, o odds ratio (OR) para FCR > 78 bpm foi de 3,5 (IC 95% = 2,9 - 4,2) para mortalidade CV e 3,6 (IC 95% = 3,2 - 4,0) para mortalidade geral. CONCLUSÃO: A FCR > 78 bpm é um preditor independente de mortalidade cardiovascular e geral.

And because the virus affects such a large and varying population – up to 170 million at last count – we think it is important to have a pocket reference especially devoted to hepatitis C. We look forward to your comments on the usefulness of our 2014 Short Guide to Hepatitis C, which is an expansion and update of the HCV chapters in Hepatology – A Clinical Textbook (2014), also published by Flying Publisher. As always, we invite qualified people everywhere to translate this book into other languages, and make them available widely. This web-based free-of- harge concept is made possible by unrestricted educational grants from the pharmaceutical industry and has allowed the material to reach countries usually not covered by print media. We are convinced that this new pocket guide concept, focusing here on hepatitis C, will become a valuable source of information for our readers.

FUNDAMENTO: Sabe-se que hábitos de vida inadequados favorecem a hipertensão, e os adventistas preconizam hábitos saudáveis. OBJETIVO: Avaliar a prevalência da hipertensão nos adventistas do sétimo dia na capital e no interior paulistas. MÉTODOS: Foram estudados 264 adventistas (41,17 ± 15,27 anos, 59,8% mulheres, com alto nível de religiosidade avaliada pela escala Duke-DUREL). A medida da pressão arterial foi realizada com aparelho automático validado. Nível de significância adotado foi p < 0,05. Resultados: A prevalência total de hipertensão foi 22,7%, (27,4% no interior e 15% na capital). Os adventistas da capital diferiram dos do interior (p < 0,05), respectivamente, quanto: escolaridade superior (62% vs 36,6%); ter vínculo empregatício (44%) vs autônomos (40,9%); renda familiar (8,39 ± 6,20 vs 4,59 ± 4,75 salários mínimos) e renda individual (4,54 ± 5,34 vs 6,35 ± 48 salários mínimos); casal responsável pela renda familiar (35% vs 39,6%); vegetarianismo (11% vs 3%); pressão arterial (115,38 ± 16,52/68,74 ± 8,94 vs 123,66 ± 19,62/74,88 ± 11,85 mmHg); etnia branca (65% vs 81,1%); casados (53% vs 68,9%); menor apoio social no domínio material (15,7 ± 5,41 vs 16,9 ± 4,32) e lembrar da última vez que mediu a pressão arterial (65% vs 48,8%). A análise multivariada associou hipertensão com: 1) vegetarianismo (OR 0,051, IC95% 0,004-0,681), 2) escolaridade (OR 5,317, IC95% 1,674-16,893), 3) lembrar quando mediu a pressão (OR 2,725, IC95% 1,275-5,821), 4) aposentado (OR 8,846, IC95% 1,406-55,668), 5) responsável pela renda familiar (OR 0,422, IC95% 0,189-0,942). CONCLUSÃO: A prevalência de hipertensão dos adventistas foi menor se comparada com estudos nacionais, sendo menor na capital em relação ao interior possivelmente por melhores condições socioeconômicas e hábitos de vida.

FUNDAMENTO: Poucos estudos analisaram os efeitos cardíacos do exercício físico prévio à oclusão coronariana. OBJETIVO: Avaliar, em ratas submetidas a exercício físico, as repercussões do infarto do miocárdio. MÉTODOS: Ratas foram submetidas à natação ou permaneceram sedentárias por oito semanas e foram randomizadas para oclusão coronariana ou cirurgia simulada, compondo quatro grupos: Sedentário (S), Exercício (E), Sedentário infarto (SIM) e Exercício infarto (EIM). Após seis semanas, foram analisados biometria, ecodopplercardiograma, hemodinâmica e mecânica miocárdica. RESULTADOS: Não foram observados cardioproteção nos animais EIM e diferença no tamanho do infarto (%VE) entre EIM (38,50 ± 4,60%) e SIM (36,58 ± 4,11%). Os teores de água pulmonar (%) de SIM (80 ± 0,59) e EIM (80 ± 0,57) foram maiores do que em S (78 ± 0,15) e E (78 ± 0,57). A pressão sistólica (mmHg) do ventrículo esquerdo (S: 130 ± 5; E: 118 ± 8; SIM: 91 ± 3; EIM: 98 ± 3) e a primeira derivada temporal (mmHg/s) positiva da pressão (S: 8.216 ± 385; E: 8.437 ± 572; SIM: 4.674 ± 455; EIM: 5.080 ± 412) de S e E foram maiores do que em SIM e EIM. As frações de encurtamento da área transversa (%) de SIM (27 ± 2) e EIM (25 ± 2) foram similares e menores do que E (65 ± 2) e S (69 ± 2). A relação E/A foi maior em SIM (5,14 ± 0,61) e EIM (4,73 ± 0,57) em relação a S (2,96 ± 0,24) e E (2,83 ± 0,21). Em estudos de músculos papilares isolados foi verificada depressão da capacidade contrátil semelhante em SIM e EIM, e não houve alteração da rigidez miocárdica. CONCLUSÃO: O treinamento prévio por natação não atenuou as repercussões cardíacas em virtude do infarto do miocárdio.

Delayed perfect monitoring in an infinitely repeated discounted game is modelled by letting the players form a connected and undirected network. Players observe their immediate neighbors' behavior only, but communicate over time the repeated game's history truthfully throughout the network. The Folk Theorem extends to this setup, although for a range of discount factors strictly below 1, the set of sequential equilibria and the corresponding payoff set may be reduced. A general class of games is analyzed without imposing restrictions on the dimensionality of the payoff space. This and the bilateral communication structure allow for limited results under strategic communication only. As a by-product this model produces a network result; namely, the level of cooperation in this setup depends on the network's diameter, and not on its clustering coefficient as in other models.

Numa pesquisa efetuada em gânglios linfáticos de 3.674 suínos, aparentemente normais, e abatidos no Matadouro Municipal de Salvador e no Frigorífico Frimisa, isolaram-se 503 amostras de Salmonellas, sendo 461 ou 91,65% provenientes de gânglios crurais e pré-crurais e 42 culturas restantes, ou sejam 8,35% obtidas de outros gânglios. o estudo das 503 amostras, representando 13,69% de isolamentos no total dos suínos examinados, mostrou a existência de 52 sorotipos diferentes com dominância do grupo sorológico somático E1 em 48,70% das salmonellas seguindo-se o grupo B com 27,43%, C1 com 6,16%, L com 4,97%, C2 com 4,77% e D1 com 3,38%. Em menor percentagem registraram-se os grupos G1, N1, J, E2, e3 e E4.

BACKGROUND: Data from prospective cohort studies regarding the association between subclinical hyperthyroidism and cardiovascular outcomes are conflicting.We aimed to assess the risks of total and coronary heart disease (CHD) mortality, CHD events, and atrial fibrillation (AF) associated with endogenous subclinical hyperthyroidism among all available large prospective cohorts. METHODS: Individual data on 52 674 participants were pooled from 10 cohorts. Coronary heart disease events were analyzed in 22 437 participants from 6 cohorts with available data, and incident AF was analyzed in 8711 participants from 5 cohorts. Euthyroidism was defined as thyrotropin level between 0.45 and 4.49 mIU/L and endogenous subclinical hyperthyroidism as thyrotropin level lower than 0.45 mIU/L with normal free thyroxine levels, after excluding those receiving thyroid-altering medications. RESULTS: Of 52 674 participants, 2188 (4.2%) had subclinical hyperthyroidism. During follow-up, 8527 participants died (including 1896 from CHD), 3653 of 22 437 had CHD events, and 785 of 8711 developed AF. In age- and sex-adjusted analyses, subclinical hyperthyroidism was associated with increased total mortality (hazard ratio[HR], 1.24, 95% CI, 1.06-1.46), CHD mortality (HR,1.29; 95% CI, 1.02-1.62), CHD events (HR, 1.21; 95%CI, 0.99-1.46), and AF (HR, 1.68; 95% CI, 1.16-2.43).Risks did not differ significantly by age, sex, or preexisting cardiovascular disease and were similar after further adjustment for cardiovascular risk factors, with attributable risk of 14.5% for total mortality to 41.5% forAF in those with subclinical hyperthyroidism. Risks for CHD mortality and AF (but not other outcomes) were higher for thyrotropin level lower than 0.10 mIU/L compared with thyrotropin level between 0.10 and 0.44 mIU/L(for both, P value for trend, .03). CONCLUSION: Endogenous subclinical hyperthyroidism is associated with increased risks of total, CHD mortality, and incident AF, with highest risks of CHD mortality and AF when thyrotropin level is lower than 0.10 mIU/L.

This paper considers the optimal degree of discretion in monetary policy when the central bank conducts policy based on its private information about the state of the economy and is unable to commit. Society seeks to maximize social welfare by imposing restrictions on the central bank's actions over time, and the central bank takes these restrictions and the New Keynesian Phillips curve as constraints. By solving a dynamic mechanism design problem we find that it is optimal to grant "constrained discretion" to the central bank by imposing both upper and lower bounds on permissible inflation, and that these bounds must be set in a history-dependent way. The optimal degree of discretion varies over time with the severity of the time-inconsistency problem, and, although no discretion is optimal when the time-inconsistency problem is very severe, our numerical experiment suggests that no-discretion is a transient phenomenon, and that some discretion is granted eventually.

Soil pseudomonads increase their competitiveness by producing toxic secondary metabolites, which inhibit competitors and repel predators. Toxin production is regulated by cell-cell signalling and efficiently protects the bacterial population. However, cell communication is unstable, and natural populations often contain signal blind mutants displaying an altered phenotype defective in exoproduct synthesis. Such mutants are weak competitors, and we hypothesized that their fitness depends on natural communities on the exoproducts of wild-type bacteria, especially defence toxins. We established mixed populations of wild-type and signal blind, non-toxic gacS-deficient mutants of Pseudomonas fluorescens CHA0 in batch and rhizosphere systems. Bacteria were grazed by representatives of the most important bacterial predators in soil, nematodes (Caenorhabditis elegans) and protozoa (Acanthamoeba castellanii). The gacS mutants showed a negative frequency-dependent fitness and could reach up to one-third of the population, suggesting that they rely on the exoproducts of the wild-type bacteria. Both predators preferentially consumed the mutant strain, but populations with a low mutant load were resistant to predation, allowing the mutant to remain competitive at low relative density. The results suggest that signal blind Pseudomonas increase their fitness by exploiting the toxins produced by wild-type bacteria, and that predation promotes the production of bacterial defence compounds by selectively eliminating non-toxic mutants. Therefore, predators not only regulate population dynamics of soil bacteria but also structure the genetic and phenotypic constitution of bacterial communities.

BACKGROUND: Contrast-enhanced ultrasonography (CEUS) is a novel imaging technique that is safe and applicable on the bedside. Recent developments seem to enable CEUS to quantify organ perfusion. We performed an exploratory study to determine the ability of CEUS to detect changes in renal perfusion and to correlate them with effective renal plasma flow. METHODS: CEUS with destruction-refilling sequences was studied in 10 healthy subjects, at baseline and during infusion of angiotensin II (AngII) at low (1 ng/kg/min) and high dose (3 ng/kg/min) and 1 h after oral captopril (50 mg). Perfusion index (PI) was obtained and compared with the effective renal plasma flow (ERPF) obtained by parallel para-aminohippurate (PAH) clearance. RESULTS: Median PI decreased from 188.6 (baseline) to 100.4 with low-dose AngII (-47%; P < 0.02) and to 66.1 with high-dose AngII (-65%; P < 0.01) but increased to 254.7 with captopril (+35%; P > 0.2). These changes parallelled those observed with ERPF, which changed from a median of 672.1 mL/min (baseline) to 572.3 (low-dose AngII, -15%, P < 0.05) and to 427.2 (high-dose AngII, -36%, P < 0.001) and finally 697.1 (captopril, +4%, P < 0.02). CONCLUSIONS: This study demonstrates that CEUS is able to detect changes in human renal cortical microcirculation as induced by AngII infusion and/or captopril administration. The changes in perfusion indices parallel those in ERPF as obtained by PAH clearance.

BACKGROUND: frailty is a concept used to describe older people at high risk of adverse outcomes, including falls, functional decline, hospital or nursing home admission and death. The associations between frailty and use of specific health and community services have not been investigated. METHODS: the cross-sectional relationship between frailty and use of several health and community services in the last 12 months was investigated in 1,674 community-dwelling men aged 70 or older in the Concord Health and Ageing in Men study, a population-based study conducted in Sydney, Australia. Frailty was assessed using a modified version of the Cardiovascular Health Study criteria. RESULTS: overall, 158 (9.4%) subjects were frail, 679 (40.6%) were intermediate (pre-frail) and 837 (50.0%) were robust. Frailty was associated with use of health and community services in the last 12 months, including consulting a doctor, visiting or being visited by a nurse or a physiotherapist, using help with meals or household duties and spending at least one night in a hospital or nursing home. Frail men without disability in activities of daily living were twice more likely to have seen a doctor in the previous 2 weeks than robust men (adjusted odds ratio 2.04, 95% confidence interval 1.21-3.44), independent of age, comorbidity and socio-economic status. CONCLUSION: frailty is strongly associated with use of health and community services in community-dwelling older men. The high level of use of medical services suggests that doctors and nurses could play a key role in implementation of preventive interventions.
