The objective of the present study was to determine the possible prognostic factors which may explain the difference in the survival of patients with cystic fibrosis (CF) with and without meconium ileus. Over a period of 20 years, 127 patients with CF, whose diagnosis was confirmed by typical clinical characteristics and altered sweat chloride levels, were studied retrospectively. The patients were divided into two groups: group 1 consisted of patients who presented CF and meconium ileus (N = 9), and group 2 consisted of patients with CF without meconium ileus (N = 118). The characteristics studied were based on data obtained upon admission of the patients using a specific protocol. Demographic, clinical, nutritional and laboratory data were obtained. The genotype was determined in 106 patients by PCR. Survival was analyzed using the Kaplan-Meier method. The median follow-up period was 44 months. A statistically significant difference was observed between the groups studied regarding the following variables: age at diagnosis and weight and height z scores. The presence of meconium ileus was associated with an earlier diagnosis; these patients had greater deficits in height and weight at the time of diagnosis and at the end of the study. The estimated probability of survival for patients with CF without meconium ileus was 62 ± 14% and for those with meconium ileus 32 ± 18%. Patients with CF and meconium ileus presented a poor nutritional status at diagnosis and a lower survival rate compared to the general CF population.

Hydration is recommended in order to decrease the overload on the cardiovascular system when healthy individuals exercise, mainly in the heat. To date, no criteria have been established for hydration for hypertensive (HY) individuals during exercise in a hot environment. Eight male HY volunteers without another medical problem and 8 normal (NO) subjects (46 ± 3 and 48 ± 1 years; 78.8 ± 2.5 and 79.5 ± 2.8 kg; 171 ± 2 and 167 ± 1 cm; body mass index = 26.8 ± 0.7 and 28.5 ± 0.6 kg/m²; resting systolic (SBP) = 142.5 and 112.5 mmHg and diastolic blood pressure (DBP) = 97.5 and 78.1 mmHg, respectively) exercised for 60 min on a cycle ergometer (40% of VO2peak) with (500 ml 2 h before and 115 ml every 15 min throughout exercise) or without water ingestion, in a hot humid environment (30ºC and 85% humidity). Rectal (Tre) and skin (Tsk) temperatures, heart rate (HR), SBP, DBP, double product (DP), urinary volume (Vu), urine specific gravity (Gu), plasma osmolality (Posm), sweat rate (S R), and hydration level were measured. Data were analyzed using ANOVA in a split plot design, followed by the Newman-Keuls test. There were no differences in Vu, Posm, Gu and S R responses between HY and NO during heat exercise with or without water ingestion but there was a gradual increase in HR (59 and 51%), SBP (18 and 28%), DP (80 and 95%), Tre (1.4 and 1.3%), and Tsk (6 and 3%) in HY and NO, respectively. HY had higher HR (10%), SBP (21%), DBP (20%), DP (34%), and Tsk (1%) than NO during both experimental situations. The exercise-related differences in SBP, DP and Tsk between HY and NO were increased by water ingestion (P < 0.05). The results showed that cardiac work and Tsk during exercise were higher in HY than in NO and the difference between the two groups increased even further with water ingestion. It was concluded that hydration protocol recommended for NO during exercise could induce an abnormal cardiac and thermoregulatory responses for HY individuals without drug therapy.

The present study evaluated whether the luteal phase elevation of body temperature would be offset during exercise by increased sweating, when women are normally hydrated. Eleven women performed 60 min of cycling exercise at 60% of their maximal work load at 32ºC and 80% relative air humidity. Each subject participated in two identical experimental sessions: one during the follicular phase (between days 5 and 8) and the other during the luteal phase (between days 22 and 25). Women with serum progesterone >3 ng/mL, in the luteal phase were classified as group 1 (N = 4), whereas the others were classified as group 2 (N = 7). Post-exercise urine volume (213 ± 80 vs 309 ± 113 mL) and specific urine gravity (1.008 ± 0.003 vs 1.006 ± 0.002) changed (P < 0.05) during the luteal phase compared to the follicular phase in group 1. No menstrual cycle dependence was observed for these parameters in group 2. Sweat rate was higher (P < 0.05) in the luteal (3.10 ± 0.81 g m-2 min-1) than in the follicular phase (2.80 ± 0.64 g m-2 min-1) only in group 1. During exercise, no differences related to menstrual cycle phases were seen in rectal temperature, heart rate, rate of perceived exertion, mean skin temperature, and pre- and post-exercise body weight. Women exercising in a warm and humid environment with water intake seem to be able to adapt to the luteal phase increase of basal body temperature through reduced urinary volume and increased sweating rate.

The aim of the present study was to determine the effect of volume and composition of fluid replacement on the physical performance of male football referees. Ten referees were evaluated during three official matches. In one match the participants were asked to consume mineral water ad libitum, and in the others they consumed a pre-determined volume of mineral water or a carbohydrate electrolyte solution (6.4% carbohydrate and 22 mM Na+) equivalent to 1% of their baseline body mass (half before the match and half during the interval). Total water loss, sweat rate and match physiological performance were measured. When rehydrated ad libitum (pre-match and at half time) participants lost 1.97 ± 0.18% of their pre-match body mass (2.14 ± 0.19 L). This parameter was significantly reduced when they consumed a pre-determined volume of fluid. Sweat rate was significantly reduced when the referees ingested a pre-determined volume of a carbohydrate electrolyte solution, 0.72 ± 0.12 vs 1.16 ± 0.11 L/h ad libitum. The high percentage (74.1%) of movements at low speed (walking, jogging) observed when they ingested fluid ad libitum was significantly reduced to 71% with mineral water and to 69.9% with carbohydrate solution. An increase in percent movement expended in backward running was observed when they consumed a pre-determined volume of carbohydrate solution, 7.7 ± 0.5 vs 5.5 ± 0.5% ad libitum. The improved hydration status achieved with the carbohydrate electrolyte solution reduced the length of time spent in activities at low-speed movements and increased the time spent in activities demanding high-energy expenditure.

Gene therapy is an alternative treatment for genetic lung disease, especially monogenic disorders such as cystic fibrosis. Cystic fibrosis is a severe autosomal recessive disease affecting one in 2500 live births in the white population, caused by mutation of the cystic fibrosis transmembrane conductance regulator (CFTR). The disease is classically characterized by pancreatic enzyme insufficiency, an increased concentration of chloride in sweat, and varying severity of chronic obstructive lung disease. Currently, the greatest challenge for gene therapy is finding an ideal vector to deliver the transgene (CFTR) to the affected organ (lung). Adeno-associated virus is the most promising viral vector system for the treatment of respiratory disease because it has natural tropism for airway epithelial cells and does not cause any human disease. This review focuses on the basic properties of adeno-associated virus and its use as a vector for cystic fibrosis gene therapy.

Individuals with systemic arterial hypertension have a higher risk of heat-related complications. Thus, the aim of this study was to examine the thermoregulatory responses of hypertensive subjects during recovery from moderate-intensity exercise performed in the heat. A total of eight essential hypertensive (H) and eight normotensive (N) male subjects (age=46.5±1.3 and 45.6±1.4 years, body mass index=25.8±0.8 and 25.6±0.6 kg/m2, mean arterial pressure=98.0±2.8 and 86.0±2.3 mmHg, respectively) rested for 30 min, performed 1 h of treadmill exercise at 50% of maximal oxygen consumption, and rested for 1 h after exercise in an environmental chamber at 38°C and 60% relative humidity. Skin and core temperatures were measured to calculate heat exchange parameters. Mean arterial pressure was higher in the hypertensive than in the normotensive subjects throughout the experiment (P<0.05, unpaired t-test). The hypertensive subjects stored less heat (H=-24.23±3.99 W·m−2vs N=-13.63±2.24 W·m−2, P=0.03, unpaired t-test), experienced greater variations in body temperature (H=-0.62±0.05°C vsN=-0.35±0.12°C, P=0.03, unpaired t-test), and had more evaporated sweat (H=-106.1±4.59 W·m−2vs N=-91.15±3.24 W·m−2, P=0.01, unpaired t-test) than the normotensive subjects during the period of recovery from exercise. In conclusion, essential hypertensive subjects showed greater sweat evaporation and increased heat dissipation and body cooling relative to normotensive subjects during recovery from moderate-intensity exercise performed in hot conditions.

Basal body temperature (BBT) and thermoeffector thresholds increase following ovulation in many women. This study investigated if solely central thermoregulatory alterations are responsible. Seven females in a non-contraceptive group (NCG) were compared with 5 monophasic contraceptive users (HCG) on separate accounts: pre-ovulation (Trial I; d 2-5) and post-ovulation (Trial 2; 4-8 d post-positive ovulation) for NCG, and active phase for HCG (d 2-5, d 18-21). During immersion in 28°C water to the axilla, participants exercised for 20-30 min on an underwater ergometer. After steadily sweating, immersion continued until metabolism increased two-fold due to shivering. Rectal (Tre) BBT was not different between trials for neither NCG (1: 37.34±0.16°C; 2: 37.35±0.27°C) nor HCG. At exercise termination, Tre forehead sweating cessation increased (P<0.05) in trial 2 irrespective of group (1: 37.55±0.39°C; 2: 37.90±0,46°C). Tre shivering onset did not increase (P>0.05) in trial 2 (1: 36.91±0.50°C; 2: 37.07±0,45°C). The widths of the interthreshold zone increased (P<0.05) in trial 2 (1: 0.64±0.22°C; 2: 0.82±0.37°C) due to the increased sweating threshold only. HCG cooled quicker (1: -l.15±0,43°C; 2: -1.00±0.50°C) than NCG participants (1: - 0.58±0.22°C; 2: -0.52±O.29°C), and tympanic (Tty) sweat thresholds were significantly (P<0.05) decreased (1: 34.76±0.54°C; 2: 35.39±0.61°C) versus NCG (l: 35.57±0.77°C; 2: 35.89±1.04°C). Lastly, Tre and Tty thresholds were significantly different (Psweating is altered by menstrual phase, contraceptive users have enhanced thermal sensitivities, and Tty opposed to Tre provides different measures of core temperature.

Type 1 Diabetes Mellitus (T1DM) is an autoimmune disease that destroys pancreatic beta cells, affecting glucose homeostasis. In T1DM, glucoregulation and carbohydrate oxidation may be altered in different ambient temperatures; however, current literature has yet to explore these mechanisms. This study examines the effects of 30 minutes of exercise at 65% VO2max in 5ºC, 20ºC and 35ºC in individuals with T1DM. No significant differences were observed for blood glucose across the 3 conditions (p = 0.442), but significance was found for core temperature, heat storage, and sweat rate (p < 0.01). Blood glucose was also shown to vary greatly between individuals among conditions. The mechanisms behind the differences in blood glucose may be due to the lack of significant glucagon production among conditions. These findings suggest that T1DM individuals may exercise submaximally for 30 minutes in different ambient temperatures without significant differences in glucoregulation.

Les principaux substrats oxydés à l’exercice, soit les glucides, les lipides et les pro- téines ne contribuent pas tous au même niveau à la fourniture d’énergie lors de l’effort prolongé. De plus, le glucose peut provenir de différentes sources endogènes (muscle, foie) et exogènes. Plusieurs facteurs peuvent influencer leur contribution respective incluant : la masse musculaire impliquée et l’entraînement préalable, le sexe, l’état nutritionnel et les conditions environnementales. L’utilisation d’isotopes stables, tels que le carbone 13 (13C), combinée à la calorimétrie indirecte respiratoire corrigée pour l’excrétion d’urée dans l’urine et la sueur, permet de différencier les substrats endogènes et exogènes et d’évaluer la contribution de leur oxydation à la fourniture d’énergie. Ces méthodes d’investigation permettant d’apprécier la sélection des substrats lors de l’exercice prolongé avec ingestion de glucose ont permis d’effectuer les comparaisons qui ont fait l’objet des trois études de cette thèse. Dans la première étude, la sélection des substrats au cours d’un effort prolongé effectué avec les membres inférieurs ou les membres supérieurs a été comparée avec et sans ingestion de glucose. Une différence modeste fut observée entre la sélection des substrats selon le mode d’exercice avec l’ingestion d’eau, celle-ci favorisant légèrement l’oxydation des glucides lors de l’effort avec les membres supérieurs. La quantité de glucose exogène oxydée était plus faible lors de l’exercice avec les membres supérieurs qu’avec les membres supérieurs, mais sa contribution plus importante, conséquence d’une dépense énergétique plus faible. Dans la deuxième étude, on a comparé la sélection des substrats chez des sujets mas- culins et féminins et les effets d’une alimentation enrichie en glucides ou de l’ingestion de glucose, au cours d’un exercice prolongé d’une durée de deux heures. On reconnaît généralement que, pour une même puissance relative, les femmes utilisent moins de glucides et davantage de lipides que les hommes. Les effets séparés d’une alimentation riche en glucides ou de l’ingestion de glucose pendant l’exercice sur la sélection des substrats furent pourtant similaires chez les deux sexes. L’effet combiné des deux procédures de supplémentation est toutefois plus important chez la femme que chez l’homme, soutenant l’hypothèse qu’un léger déficit en glucides soit présent chez les femmes. Dans la troisième étude, l’oxydation des substrats et particulièrement celle d’amidon exogène au cours d’une marche prolongée à une faible puissance de travail a été décrite. Les individus qui pratiquent des activités physiques prolongées à des intensités faibles (< 40 %VO2max) sont encouragés à ingérer des glucides et de l’eau pendant l’effort, mais la contribution de leur oxydation à la fourniture d’énergie est relativement peu connue. Nous avons montré que, contrairement aux observations précédemment effectuées à jeun sans ingestion de glucides pendant l’effort, les glucides (incluant de source exogène) peuvent fournir une très grande partie de l’énergie lorsqu’ils sont ingérés à des intervalles réguliers au cours de l’exercice prolongé. Dans l’ensemble, les résultats des études expérimentales présentées dans cette thèse montrent que les glucides ingérés peuvent fournir une grande proportion de l’énergie pendant l’exercice prolongé. Toutefois, le mode d’exercice, le sexe et la puissance de travail mènent à des variations qui sont en grande partie liées à une dépense énergétique variable selon les conditions et les groupes d’individus ayant des caractéristiques différentes.

Depuis la fin des années 1980, le phénomène de revitalisation culturelle amérindienne observé à l’échelle continentale s’est enraciné au Québec. Ce phénomène panindien, qui se définit entre autres par un mouvement de guérison dit communautaire – c’est- à-dire qui s’organise à l’intérieur-même des communautés (par opposition à ce qui vient de l’extérieur) – est caractérisé par la prise en charge des problèmes sociaux rencontrés par les populations amérindiennes. Par l’analyse du rite de la tente à sudation, une pratique emblématique de la spiritualité panindienne et du mouvement de guérison, ce mémoire explore la dualité des stratégies de relation d’aide qui y sont déployées. Pour ce faire, l’expérience en milieu carcéral et en communauté d’aînés et d’intervenants autochtones a été prise à témoin. L’enquête de terrain révèle ainsi qu’en parallèle avec la fonction de mobilisation sociale et politique associée à la revitalisation culturelle amérindienne, on assiste à une instrumentation du rituel à des fins psychothérapeutiques. Tout en s’inscrivant dans la structure cosmologique commune à plusieurs traditions orales algonquiennes, cet usage particulier de la symbolique du rituel met à jour une vision plus clinique, plus individualisée et plus dépolitisée de la guérison autochtone habituellement revendiqué dans le discours panindien.

La Fibrosis Quística es la enfermedad autosómica recesiva mas frecuente en caucásicos. En Colombia no se conoce la incidencia de la enfermedad, pero investigaciones del grupo de la Universidad del Rosario indican que podría ser relativamente alta. Objetivo: Determinar la incidencia de afectados por Fibrosis Quística en una muestra de recién nacidos de la ciudad de Bogotá. Metodología: Se analizan 8.297 muestras de sangre de cordón umbilical y se comparan tres protocolos de tamizaje neonatal: TIR/TIR, TIR/DNA y TIR/DNA/TIR. Resultados: El presente trabajo muestra una incidencia de 1 en 8.297 afectados en la muestra analizada. Conclusiones: Dada la relativamente alta incidencia demostrada en Bogotá, se justifica la implementación de Tamizaje Neonatal para Fibrosis Quística en Colombia.

La exposición a altas temperaturas en ambientes laborales conlleva a cambios fisiológicos que se manifiestan como mecanismos de compensación a la alteración del equilibrio homeostático corporal. El propósito del presente estudio fue determinar los cambios y el comportamiento de variables fisiológicas a través de frecuencia cardiaca, densidad urinaria, temperatura corporal y tasa de sudoración, en dos escenarios con condiciones térmicas ambientales diferentes definidas por la exposición (grupo expuesto y no expuesto). Adicional, en dos áreas de trabajo diferentes correspondientes al proceso de fundición del acero, una de ellas, Horno electrico donde se hace la fusión de la chatarra y demás materias primas, obteniendo así el acero liquido, el cual se vuelca en el Horno Cuchara y en este, libre ya de escoria se realiza el afino y ajuste definitivo de la composición química del acero. Objetivos: Identificar la relación de las respuestas fisiológicas a carga física y térmica, comparar las respuestas funcionales registradas en el grupo expuestos y no expuestos y contribuir a la introducción de nuevos indicadores para evaluar carga e intensidad de trabajo con fines de normalización ergonómica. Método: Investigación experimental en una muestra de 30 trabajadores evaluados en dos condiciones ambientales diferentes. La temperatura oral se registró al inicio de la jornada y con intervalos de toma de 3 horas. La frecuencia cardiaca (HR) se registró durante las 8 horas de trabajo continuas con pulsometría. Igualmente, se estimó la sudoración por pérdida de masa corporal entre el inicio y el final de la jornada laboral teniendo en cuenta ingestas y perdidas. El procesamiento estadístico se realizó con el programa SPSS v. 20.0, calculándose medidas de tendencia central y dispersión, prueba de wilconxon para las variables dependientes y correlación para identificar asociaciones. Para todos los cálculos se asumió p <0,05. Resultados: No se observaron diferencias significativas frente a la variación de la frecuencia cardiaca (media y máxima), la tasa de sudoración y la densidad urinaria. A pesar de que no hubo diferencias significativas en la variación de la temperatura corporal en horno cuchara, si se observó una diferencia significativa en el horno eléctrico Conclusión: Aunque no se encontraron diferencias estadísticamente significativas en la mayoría de las variables, es un hecho que la exposición a temperaturas elevadas extremas tiene un impacto en el comportamiento fisiológico del organismo. Futuros estudios deben considerar la posibilidad de estandarizar protocolos que permitan la exposición térmica basada en el perfil particular de cada trabajador.

Luego, utilizando materiales simples, se tendrá la oportunidad de experimentar con esta ilusión.

Con materiales de uso corriente, los niños pueden realizar más de cuarenta juegos y experimentos sencillos que les ayudaran a comprender mejor qué es el tiempo y cómo se mide. Las actividades son fáciles de realizar y están dirigidas a niños, a partir de diez años, que ya han alcanzado un nivel aceptable de lectura comprensiva. Estos experimentos no entrañan ningún peligro y no requieren el uso de sustancias ni materiales especiales.

A simple formulation relating the L-band microwave brightness temperature detected by a passive microwave radiometer to the near surface soil moisture was developed using MICRO-SWEAT, a coupled microwave emission model and soil-vegetation-atmosphere-transfer (SVAT) scheme. This simple model provides an ideal tool with which to explore the impact of sub-pixel heterogeneity on the retrieval of soil moisture from microwave brightness temperatures. In the case of a bare soil pixel, the relationship between apparent emissivity and surface soil moisture is approximately linear, with the clay content of the soil influencing just the intercept of this relationship. It is shown that there are no errors in the retrieved soil moisture from a bare soil pixel that is heterogeneous in soil moisture and texture. However, in the case of a vegetated pixel, the slope of the relationship between apparent emissivity and surface soil moisture decreases with increasing vegetation. Therefore for a pixel that is heterogeneous in vegetation and soil moisture, errors can be introduced into the retrieved soil moisture. Generally, under moderate conditions, the retrieved soil moisture is within 3% of the actual soil moisture. Examples illustrating this discussion use data collected during the Southern Great Plains '97 Experiment (SGP97).
