A partir da década de 60, com a utilização do transplante renal em larga escala como terapia substitutiva para pacientes com falência do órgão, surgiu a preocupação quanto ao desenvolvimento do processo de rejeição do enxerto. Tal intercorrência, em geral, cursa com sinais e sintomas clínicos apenas quando o evento está bem estabelecido, ou mesmo quando lesões irreversíveis já se instalaram. Assim, é fundamental um acompanhamento rigoroso, visando detectar os casos subclínicos. O presente trabalho, a fim de fornecer novas ferramentas que auxiliem o diagnóstico precoce de rejeição do enxerto, avaliou a expressão imuno-histoquímica dos anticorpos CD3, CD5, CD20, CD68, CD25, FoxP3 e C4d em biópsias renais realizadas entre os anos de 2007 e 2009 em pacientes transplantados acompanhados pelo Serviço de Nefrologia do Hospital Universitário Pedro Ernesto, UERJ - RJ, correlacionando os resultados obtidos com o diagnóstico histológico. Para tal, as biópsias foram reavaliadas por três médicos patologistas que as classificaram, segundo Critérios de Banff 2007, quanto à presença ou não de rejeição do enxerto e seu tipo, aguda ou crônica. A partir de então, os blocos de parafina foram processados pela técnica Tissue Microarray for all (Pires, ARC. e cols.) e submetidos à imuno-histoquímica. A positividade dos marcadores foi avaliada e graduada e os resultados encontrados foram correlacionados, em um primeiro momento, com a presença ou ausência de rejeição. Posteriormente, os casos com diagnóstico histológico de rejeição tiveram seu perfil imuno-histoquímico analisado em função da positividade para C4d, marcador definidor de rejeição humoral. Neste momento, buscou-se averiguar se os anticorpos estudados seriam úteis em detectar, neste grupo, rejeição humoral e celular. Após a análise estatística, realizada pelo Teste Exato de Fisher, pode-se, então, concluir que o comportamento do marcador CD3 é capaz de inferir a presença de rejeição e que os anticorpos CD5 e CD25 permitem sugerir rejeição celular e humoral, respectivamente. Foi observado também que casos sem diagnóstico histológico de rejeição podem apresentar marcação para C4d em mais de 10% de seus capilares peritubulares.

The study of a score by a serious performer is a fundamental step in the process of arriving at a knowledgeable and deeply informed approach to performing a piece of music. In order to obtain this knowledge numerous aspects of the score must be taken into consideration. It is the intent of this dissertation to gather and analyze the information concerning Naturale, a work written by Luciano Berio in 1985 for viola, percussion and recorded voice, based on Sicilian folk songs. All the aspects surrounding Naturale’s existence are taken into consideration in this study. First, it is important to reflect on Berio’s compositional style and traits, the manner in which he relates his works one to another, what he sees in folk music and his own personal desire to intertwine art music and folk music. For Berio Naturale is not an isolated venture into the realm of mixing folk music and his own avant-garde style; it is instead one of many works resulting from his long-standing relationship with folk music. Another essential aspect in this case is the study of Sicilian folk music itself, and the sources used by Berio to find the songs by which he was inspired. The work is examined section by section with figures showing both excerpts of Naturale as well as the original songs with their translations. An analysis containing harmonic, thematic and formal aspects of the score was developed in order to arrive at a better understanding of the structure and pacing of the piece. For this research the author went to Italy to conduct an interview with Maestro Aldo Bennici, the Sicilian violist for whom Naturale was composed. This interview helped in the discovery of two more songs used by Berio that have not to this point been identified in any other document. Bennici’s outstanding testimony portrayed the expressive character of this music and the evocative imagery behind this score. I hope to bring this knowledge to other performers, that they may fully understand and appreciate the unique beauty and power of Berio’s Naturale.

For physicians facing patients with organ-limited metastases from colorectal cancer, tumor shrinkage and sterilization of micrometastatic disease is the main goal, giving the opportunity for secondary surgical resection. At the same time, for the majority of patients who will not achieve a sufficient tumor response, disease control remains the predominant objective. Since FOLFOX or FOLFIRI have similar efficacies, the challenge is to define which could be the most effective targeted agent (anti-EGFR or anti-VEGF) to reach these goals. Therefore, a priori molecular identification of patients that could benefit from anti-EGFR or anti-VEGF monoclonal antibodies (i.e. the currently approved targeted therapies for metastatic colorectal cancer) is of critical importance. In this setting, the KRAS mutation status was the first identified predictive marker of response to anti-EGFR therapy. Since it has been demonstrated that tumors with KRAS mutation do not respond to anti-EGFR therapy, KRAS status must be determined prior to treatment. Thus, for KRAS wild-type patients, the choices that remain are either anti-VEGF or anti-EGFR. In this review, we present the most updated data from translational research programs dealing with the identification of biomarkers for response to targeted therapies.

Hemorrhage and resuscitation (H/R) leads to phosphorylation of mitogen-activated stress kinases, an event that is associated with organ damage. Recently, a specific, cell-penetrating, protease-resistant inhibitory peptide of the mitogen-activated protein kinase c-JUN N-terminal kinase (JNK) was developed (D-JNKI-1). Here, using this peptide, we tested if inhibition of JNK protects against organ damage after H/R. Male Sprague-Dawley rats were treated with D-JNKI-1 (11 mg/kg, i.p.) or vehicle. Thirty minutes later, rats were hemorrhaged for 1 h to a MAP of 30 to 35 mmHg and then resuscitated with 60% of the shed blood and twice the shed blood volume as Ringer lactate. Tissues were harvested 2 h later. ANOVA with Tukey post hoc analysis or Kruskal-Wallis ANOVA on ranks, P < 0.05, was considered significant. c-JUN N-terminal kinase inhibition decreased serum alanine aminotransferase activity as a marker of liver injury by 70%, serum creatine kinase activity by 67%, and serum lactate dehydrogenase activity by 60% as compared with vehicle treatment. The histological tissue damage observed was blunted after D-JNKI-1 pretreatment both for necrotic and apoptotic cell death. Hepatic leukocyte infiltration and serum IL-6 levels were largely diminished after D-JNKI-1 pretreatment. The extent of oxidative stress as evaluated by immunohistochemical detection of 4-hydroxynonenal was largely abrogated after JNK inhibition. After JNK inhibition, activation of cJUN after H/R was also reduced. Hemorrhage and resuscitation induces a systemic inflammatory response and leads to end-organ damage. These changes are mediated, at least in part, by JNK. Therefore, JNK inhibition deserves further evaluation as a potential treatment option in patients after resuscitated blood loss.

Depuis les débuts des études sur la chanson, plusieurs chercheurs ont pu déterminer que l’âge d’or de la chanson québécoise comprenait la production chansonnière des années soixante et soixante-dix. C’est en filiation avec cette époque culturellement engagée et collective que la chanson du tournant du millénaire s’établit. En effet, après le creux musical des années quatre-vingt au Québec, un groupe reprit le collier de la chanson engagée au début des années quatre-vingt-dix. En défendant les marginalisés de la société et en se faisant le porte-parole de ceux-ci, les Colocs ont contribué à l’acceptation sociale des chômeurs, des sans-abris, des sidéens, des dépressifs, et autres laissés pour compte de la société. En s’attaquant à des problématiques socio-politiques graves, le groupe permet de les voir sous un autre angle et de créer une mise en perspective chez les victimes. Par l’utilisation de procédés ironiques, les Colocs parvinrent à installer une distance entre les problèmes et leurs victimes. En majorité auto-ironiques, les chansons du groupe révèlent en plus une ironie de situation, aussi appelée ironie du sort. En parallèle aux Colocs, d’autres artistes des années quatre-vingt-dix et deux mille offrent des chansons engagées au caractère ironique, et permettent une comparaison fructueuse. Dans l’optique de dédramatiser des situations potentiellement catastrophiques et d’élaborer une forme de critique sociale engagée, les Colocs ont créé une production chansonnière où l’ironie primait.

Dans le contexte urbain du Burkina Faso, la chanson de sensibilisation se situe au coeur des pratiques sociales traditionnelles, des nouvelles économies culturelles et des approches participatives en matière de développement. Cette pratique répond à des enjeux particuliers au contexte local et aux relations internationales. La musique comme outil commun de communication sociale artistique rejoint des stratégies professionnelles différentes chez les artistes-musiciens et les associations locales. Pour les premiers, nous pouvons porter un regard sur la place du musicien dans la société contemporaine burkinabè, entre paupérisation et mondialisation. Pour les seconds, la chanson de sensibilisation s’inscrit comme une tentative d’innover dans les approches d’Information, Éducation et Communication (IEC) pour la santé et la Communication pour le Changement de Comportement (CCC). Ce mémoire présente les modes de collaboration entre les artistes-musiciens et les associations locales qui oeuvrent en santé au Burkina Faso. Il décrit l’état respectif de leur champ puis analyse les négociations sociales issues de leur rencontre. La problématique de recherche s’inscrit dans des courants de recherche en anthropologie et établit un pont avec la praxis de l’animation sociale et culturelle. La chaîne de création (production-diffusion-consommation) permet de poser un regard historique sur les concepts de culture et de développement dans le contexte particulier de l’Afrique de l’Ouest francophone.

L’hypoalbuminémie est une trouvaille fréquente chez le patient brulé mais sa relation avec la morbidité et mortalité n’a pas été bien établie. Objectif : Déterminer si l’hypoalbuminémie dans les premières 24 heures suivant l’admission est associée avec la dysfonction d’organes (mesurée avec le score SOFA) chez les patients présentant des brûlures graves. Méthodologie : Nous avons révisé les dossiers médicaux des patients adultes avec de brûlures de 20% ou plus de surface corporelle admis pendant les premières 24 heures à l’unité de grands brulés du CHUM entre les années 2008 et 2009. Nous avons utilisé un modèle de régression linéaire multivariée pour déterminer si l’hypoalbuminémie était un prédicteur indépendant de la dysfonction d’organes. Résultats : 56 sujets ont été analysés. L’analyse de régression linéaire multiple a montré qu’en contrôlant pour l’âge, le sexe, la surface corporelle brûlée et les brûlures par inhalation, l’hypoalbuminémie pendant les premières 24 heures suivant l’admission est un prédicteur indépendant de la dysfonction d’organes. Une concentration d’albumine ≤30g/L est aussi associée à une augmentation de la dysfonction d’organes [score SOFA au jour 0 (p = 0.005), jour 1 (p = 0.005), moyenne de la première semaine (p = 0.004)], mais n’est pas associée avec la mortalité (p = 0.061). Conclusions : L’hypoalbuminémie est associée avec la dysfonction d’organes chez les patients brulés. À la différence de facteurs non modifiables comme l’âge, le sexe, la surface corporelle brûlée et la présence de brûlures par inhalation, la correction de l’hypoalbuminémie peut être un objectif intéressant pour un futur essai clinique.

Introducción:Los individuos en quienes se realiza el sugarbaker tienen diagnósticos tumorales bien caracterizados. Actualmente no se dispone en la literatura médica universal de comparadores previos que permitan estimar la morbilidad relacionada específicamente con procesos infecciosos en pacientes sometidos al procedimiento, por tanto se presenta la caracterización de procesos febriles e infecciosos en el postoperatorio de la cohorte de pacientes intervenidos en la FSFB y de los factores de riesgo asociados a su manifestación. Métodos:Estudio descriptivo con componente analítico de una cohorte ambidireccional compuesta por pacientes intervenidos en la FSFB mediante el procedimiento de Sugarbaker. Resultados:En total se incluyeron en el estudio 53 pacientes consecutivos (37mujeres y 16hombres), quienes fueron llevados al procedimiento de peritonectomía radical más quimioterapia hipertérmica intraperitoneal entre el mes de nov/2007 y jun/2012 en el Hospital-Universitario Fundación Santa Fe de Bogotá. Los desenlaces de morbilidad asociada al procedimiento fueron caracterizados, indicando que las principales causas de morbilidad son los eventos tromboticos y las infecciones. Se caracterizaron como estadísticamente significativos para estancia hospitalaria el requerimiento transfusional (r=0,451, p=0,001), colecistectomía (p=0,016), el riesgo anestésico ASA≥3 (p=0,03), entre otros. El perfil de infección mostró relación estadísticamente significativa con resecciones de órganos específicas (p<0,05 para colectomía derecha [OR=5,3], colecistectomía [OR=21,8] y esplenectomía [OR=4,2]), el riesgo anestésico ASA≥3 (OR=1,2, p=0,036), anemia (OR=7,1, p=0,004), fístula (OR=5,2, p=0,036), entre otros. Conclusiones: El procedimiento de Sugarbaker es eficaz y seguro en nuestra institución. Se requiere de más estudios en poblaciones diversas que permitan comprender el comportamiento de las infecciones en la población sometida al procedimiento.

This study aimed at examining the representation and the level of knowledge as well as getting acquainted whether there had been significant divergence among three social groups of 3rd year Law students, 7th period Medical students at UFPB and a group of people from the Catholic Church in vila dos pescadores in João Pessoa - about organ donation, transplant law and ethical issues that raise questions. In order to accomplish the qualitative analysis, Bardin´s content analysis technique was applied in conjunction with the Chisquare test which was applied with significance level of 5% to quantitative data. The data revealed that most informants agree with organ donation, Although they are not acquainted with the law of transplants, and with the lack of confidence in the single list of recipients. The problem is that there is an encouragement to trades with organs and the possibility of any person legally authorized to donate organs in life. The statistically significant difference was observed in only two questions, ie, in response to the confidence in the diagnosis of brain death: 64% of 7th period Medical students at UFPB trust this diagnosis versus 12% of the evangelizing group of vila dos pescadores. The other difference refers to the answer about the confidence in the single list of recipients: 36% of the 7th period Medical students of UFPB said to trust the list versus 12% of the 3rd law students of UFPB. Isolates of Paracoccidioides brasiliensis may vary in virulence according to time of in vitro subcultivation. The present study compared the morphology and pathogenicity to hamsters of two P. brasiliensis isolates: one obtained from human lesions and maintained in the laboratory for several years (Pb-18) and the other isolate recovered from hamsters inoculated with organ homogenates from armadillos (Pb-T). The microscopic morphology of Pb-18 and Pb-T showed yeast cells with similar diameter. However, Pb-T produced a significantly higher number of buds per mother cell than Pb-18. Besides, the mycelial form of Pb-T developed abundant sporulation during 8 weeks of culture which was absent in the Pb-18 isolate. Virulence studies demonstrated that mortality rates, antibody levels, fungal load and extent of lesions in the organs were significantly higher in animals infected with Pb-T. The results demonstrated that Pb-T recently isolated from an animal was more virulent than Pb-18. These differences between the two P. brasillensis isolates may be indicators of virulence attenuation in this fungal species.

Sabe-se que as regiões cerebrais envolvidas no controle do canto são sexualmente dimórficas em muitas espécies de pássaros adultos de regiões temperadas como nos zebra finches em que os machos cantam e as fêmeas não cantam. Em diversas espécies de pássaros canoros dos trópicos, contudo, tanto os machos quanto as fêmeas são capazes de cantar. Porém, os mecanismos envolvidos na produção do canto em fêmeas ainda é pouco compreendido. Com o intuito de identificar diferenças que possam explicar o canto em fêmeas, nós estudamos a morfologia do sistema do canto de pássaros machos e fêmeas da espécies Uraegynthus cyanocephalus, espécie esta em que tanto machos quanto fêmeas cantam. Como primeiro passo para a análise e estabelecimento de diferenças anatômicas quanto ao sexo, nós quantificamos alterações de volume de áreas prosencefálicas relacionadas ao cantos, através de marcação com Nissl e de marcação de receptores andrógenos (RA) por meio de hibridização in situ radioativa. Nós verificamos que, tanto em machos quanto em fêmeas, o volume do centro vocal superior (HVC) não sofre alteração estatisticamente significativa ao longo do desenvolvimento. Observamos, ainda, que o volume do HVC em machos é sempre superior ao das fêmeas, inclusive na fase adulta, quando esta diferença se torna significativa, existindo portanto, dimorfismo sexual. Contrariamente ao desenvolvimento do HVC, o núcleo robusto do arcopalio (RA) de machos aumenta de modo significativo gradualmente com a idade, atingindo o seu pico de crescimento na fase adulta. O volume do RA aumentou em 2,21 vezes no macho (0,104 mm3 em 20 dias para 0,236 mm3 na idade adulta). Nas fêmeas, as alterações volumétricas de RA observadas ao longo do crescimento não foram significativas.

O Curió Oryzoborus angolensis (Aves, Passeriformes, Oscines, Emberizinae) é uma espécie Neotropical muito conhecida e apreciada entre os criadores de pássaros no Brasil, por possuir canto melodioso e variado. O canto tem como função biológica o reconhecimento específico. A presença de repertórios vocais longos, variações populacionais e individuais pode ser um indicativo de aprendizagem vocal, que é o caso do Oryzoborus angolensis. O repertório representa os diferentes tipos de unidades que constituem o canto, as notas, emitidas durante a vocalização. No caso do canto do Curió a frase pode ser caracterizada como uma sequência de notas que se repetem. Analisamos as amostras de gravações de 26 indivíduos, sendo dezesseis de cativeiro e dez selvagens provenientes de diversas localidades do país. Identificamos e denominamos cada uma das 2414 notas gravadas com uma letra do alfabeto. As medidas dos parâmetros físicos das notas (duração, intervalo entre as notas, ritmo, frequência máxima e frequência mínima) apresentaram diferenças globais significativas, podendo ser um dos caracteres responsáveis pela função de reconhecimento específico. Observamos que as notas são constituídas principalmente por sons puros e amplamente moduladas, distribuídas de forma homogênea na amostra. Descrevemos a estrutura do canto, com a análise do repertório dos indivíduos de cativeiro e selvagem, apresentado diferenças estatisticamente significativas entre si. Considerando a distribuição dos diferentes tipos de notas emitidas por indivíduos,calculamos a entropia informacional, que forneceu um índice que correspondente à previsibilidade do canto individual.

Fin whale (Balaenoptera physalus) is the only misticeto commonly observed in the Mediterranean. The males emit sounds, classic pulse products in sequences called songs, at 20Hz for sexual purposes: sounds are produced during the spring for migration to the Tirreno-Ligurian-Provençal basin, the summer feeding area, and during the autumn, when there is a migration to the south to meet the winter breeding season. This area in the Mediterranean sea is unknown. The east coast of the Iberian Peninsula is a migration area. The study was conducted by analyzing through Adobe Audition 3.0 and XBAT softwares files audio of 30 minutes recorded in 2006, in 2011 and 2012 at the level of the Columbretes Islands, in the western Mediterranean sea, using two hearing aids: the MARU, used in 2006 and the EAR, used in 2011 and 2012. From the analysis have emerged that, in addition to songs with pulses of 20 Hz, there are new sounds of fin whale never previously recognized: the VFPs (Variable Frequency Pulses), higher-frequency pulses emitted, between 50 and 120Hz and the ramps, a set of 7-8 pulses, pertaining to a particular song, of increasing frequency. Further studies are needed to understand the importance of these new sounds.
