Background: Increasing resistance to clarithromycin and nitroimidazole is the main cause of failure in the Helicobacter pylori eradication. The ideal retreatment regimen remains unclear, especially in developing countries, where the infection presents high prevalence and resistance to antibiotics. The study aimed at determining the efficacy, compliance and adverse effects of a regimen that included furazolidone, levofloxacin and lansoprazole in patients with persistent Helicobacter pylori infection, who had failed to respond to at least one prior eradication treatment regimen. Methods: This study included 48 patients with peptic ulcer disease. Helicobacter pylori infection was confirmed by a rapid urease test and histological examination of samples obtained from the antrum and corpus during endoscopy. The eradication therapy consisted of a 7-day twice daily oral administration of lansoprazole 30 mg, furazolidone 200 mg and levofloxacin 250 mg. Therapeutic success was confirmed by a negative rapid urease test, histological examination and 14C- urea breath test, performed 12 weeks after treatment completion. The Chi-square method was used for comparisons among eradication rates, previous treatments and previous furazolidone use. Results: Only one of the 48 patients failed to take all medications, which was due to adverse effects (vomiting). Per-protocol and intention-to-treat eradication rates were 89% (95% CI-89%-99%) and 88% (88-92%), respectively. Mild and moderate adverse effects were reported by 41 patients (85%). For patients with one previous treatment failure, the eradication rate was 100%. Compared to furazolidone-nave patients, eradication rates were lower in those who had failed prior furazolidone-containing regimen(s) (74% vs. 100%, p = 0.002). Conclusion: An empiric salvage-regimen including levofloxacin, furazolidone and lansoprazole is very effective in the eradication of Helicobacter pylori, particularly in patients that have failed one prior eradication therapy.

This study deals with two innovative brewing processes, high gravity batch and complete continuous beer fermentation systems. The results show a significant influence of the variables such as concentration and temperature on the yield factor of the substrate into ethanol and consequently on the productivity of the high gravity batch process. The technological feasibility of continuous production of beer based on yeast immobilization on cheap alternative carriers was also demonstrated. The influence of process parameters on fermentation performance and quality of the obtained beers was studied by sensorial analysis. No significant difference in the degree of acceptance between the obtained products and some traditional market brands was found. (c) 2008 Institute of Chemistry, Slovak Academy of Sciences.

We describe administrative reform involving management innovation undertaken at the Superior Tribunal of Justice, Brazil`s highest appellate court for infra-constitutional cases. The innovation is the introduction of a new management model based on strategic planning and a process management approach to work processes. Introduction of the new model has been supported by the use of information technology and project management techniques. Qualitative methods were used for data collection and analysis. Findings reveal that the innovation is contributing to the development of a systemic overview of key processes, reducing the fragmenting effects of the division of work activities within the Tribunal. At least three new organizational routines or capabilities have been developed as a result of the innovation studied: Electronic Court Management, Project Management, and Process Management. The paper contributes to knowledge about court management, a field that has received little research attention in the public administration literature.

Background: The Helicobacter pylori reinfection seems to be higher in developing countries, than in developed ones. The aim of the study was to determine the annual recurrence rate of H. pylori, in Brazilian patients with peptic ulcer disease, in a 5-year follow-up. Methods: Patients, with peptic ulcer disease diagnosed by upper digestive endoscopy (UDE) and H. pylori infection verified by histological analysis, rapid urease test, polymerase chain reaction, and urea breath test (UBT), were treated for bacterial eradication. The cure of the infection was verified using the same tests, 3 months after. Clinical evaluation and UBT were performed after sixth and ninth month. After 1 year of follow-up, UBT and UDE were repeated. Up to the fifth year, patients were assessed twice a year and an UBT was performed annually. The patients included and all the reinfected were tested for 15 different genes of the H. pylori. Results: One hundred and forty-seven patients were followed: 19 for 1 year, eight for 2 years, four for 3 years, five for 4 years, and 98 for 5 years, totaling 557 patients/years. Recurrence did not occur in the first year. In the second year, two patients were reinfected; in the third, four patients; in the fourth, three patients; and in the fifth, one patient. The total of reinfected patients was 10. The annual reinfection rate was 1.8%. Conclusion: Brazil presents a low prevalence of H. pylori reinfection, similar to the developed countries.

Aims Neurally meditated reflex or neurocardiogenic or vasovagal syncope (NMS) is usually mediated by a massive vagal reflex. This study reports the long-term outcome of NMS therapy based on endocardial radiofrequency (RF) catheter ablation of the cardiac vagal nervous system aiming permanent attenuation or elimination of the cardioinhibitory reflex (cardioneuroablation). Methods and results A total of 43 patients (18F/25M, 32.9+/-15 years) without apparent cardiopathy (left ventricular ejection fraction=68.6+/-5%) were included. All had recurrent NMS (4.7+/-2 syncope/patient) with important cardioinhibition (pauses=13.5+/-13 s) at head-up tilt test (HUT), normal electrocardiogram (ECG), and normal atropine test (AT). The patients underwent atrial endocardial RF ablation using spectral mapping to track the neurocardiac interface (AF Nest Mapping). The follow-up (FU) consisted of clinical evaluation, ECG (1 month/every 6 months/or symptoms), Holter (every 6 months/or symptoms), HUT (>= 4 months/or symptoms), and AT (end of ablation and >= 6 months). A total of 44 ablations (48+/-9 points/patient) were performed. Merely three cases of spontaneous syncope occurred in 45.1+/-22 months (two vasodepressor, one undefined). Only four partial cardioinhibitory responses occurred in post-ablation HUT without pauses or asystole (sinus bradycardia). Long-term AT (21.7+/-11 months post) was negative in 33 (76.7%, P<0.01), partially positive in 7(16.3%), and normal in three patients only (6.9%) reflecting long-term vagal denervation (AT-Delta% HR pre 79.4% x 23.2% post). The post-ablation stress test and Holter showed no abnormalities. No major complications occurred. Conclusion Endocardial RF catheter ablation of severe neurally meditated reflex syncope prevented pacemaker implantation and showed excellent long-term results in well selected patients. Despite no action in vasodepression it seems to cause enough long-term vagal reflex attenuation, eliminating the cardioinhibition, and keeping most patients asymptomatic. Indication was based on clinical symptoms, reproduction of severe cardioinhibitory syncope, and normal atropine response.

This is a study in the rat of the distribution of specific neurotransmitters in neurones projecting from the substantia nigra reticulata (SNR) to the ventrolateral (VL) and ventromedial (VM) thalamic nuclei. Individual axons projecting from the SNR to these thalamic nuclei have also been reconstructed following small injection of the anterograde tracer dextran biotin into the the SNR. Analysis of reconstructions revealed two populations of SNR neurones projecting onto the VL and VM thalamic nuclei. One group projects directly onto the VM and VL, and the other projects to the VM/VL and to the parafascicular nucleus. In another set of experiments Fluoro-Gold was injected into the VL/VM to label SNR projection neurones retrogradely, and immunohistochemistry was performed to determine the distribution of choline acetyltransferase (ChAT), vesicular acetylcholine transporter (VAChT), gamma -aminobutyric acid (GABA), and glutamate in Fluoro-Gold-labelled SNR projection neurones. Most SNR-VL/VM thalamic projection neurones were immunoreactive to acetylcholine or glutamate, whereas only 25% of the projection neurones were found to be immunoreactive to GABA. Flavonoids, phenolic acids and abscisic acid of Australian and New Zealand Leptospermum honeys were analyzed by HPLC. Fifteen flavonoids were isolated in Australian jelly bush honey (Leptospermum polygalifolium), with an average content of 2.22 mg/100 g honey. Myricetin (3,5,7,3',4',5'-hexahydroxyflavone), luteolin (5,7,3',4'-tetrahydroxyflavone) and tricetin (5,7,3',4',5'-pentahydroxyflavone) were the main flavonoids identified. The mean content of total phenolic acids in jelly bush honey was 5.14 mg/100 g honey, with gallic and coumaric acids as the potential phenolic acids. Abscisic acid was quantified as twice the amount (11.6 mg/100 g honey) of the phenolic acids in this honey. The flavonoid profile mainly consisted of quercetin (3,5,7,3',4'-pentahydroxyflavone), isorhamnetin (3,5,7,4'-tetrahydroxyflavone 3'-methyl ethyl), chrysin (5,7-dihydroxyflavone), luteolin and an unknown flavanone in New Zealand manuka (Leptospermum scoparium) honey with an average content of total flavonoids of 3.06 mg/100 g honey. The content of total phenolic acids was up to 14.0 mg/100 g honey, with gallic acid as the main component. A substantial quantity (32.8 mg/100 g honey) of abscisic acid was present in manuka honey. These results showed that flavonoids and phenolic acids could be used for authenticating honey floral origins, and abscisic acid may aid in this authentication. A frontoplastia endoscópica (FE) representa um significativo avanço para a elevação das sobrancelhas, tendo substituído a técnica coronal clássica. OBJETIVO: Demonstrar a casuística e avaliar os resultados e complicações com a técnica de FE no Serviço de Otorrinolaringologia da Universidade Federal de Uberlândia. PACIENTES E MÉTODOS: De janeiro de 2001 a janeiro de 2004, 67 pacientes foram submetidos à FE, sendo que 7 à "técnica dos triângulos". As idades variaram entre 38 a 59 anos (média de 48,3 anos), sendo 65 (97%) do sexo feminino. RESULTADOS: Destes, 56 pacientes apresentaram resultado satisfatório, 2 apresentaram déficit estético notado pelo cirurgião e pelo paciente. Dos 7 pacientes submetidos à "técnica dos triângulos", todos apresentaram resultado satisfatório. Todos apresentaram melhora da ptose lateral do terço lateral e glabelar das sobrancelhas e diminuição das rugas verticais e frontais. A cirurgia revisional foi necessária em 2 pacientes nos quais ocorreu recorrência da ptose. Houve um paciente que apresentou paralisia do ramo frontal do nervo facial unilateral. Com a "técnica dos triângulos", 5 pacientes apresentaram cicatriz visível no local das incisões. CONCLUSÃO: A FE é uma técnica que produz resultados satisfatórios na grande maioria dos casos, com baixo índice de complicações.

A dor no pós-operatório imediato apresenta-se como um grave problema, requerendo do médico uma adequada assistência. Na Otorrinolaringologia, merece atenção especial a dor após uvulopalatofaringoplastia (UPFP). OBJETIVO: Comparar a eficácia na analgesia pós-operatória do cetorolaco com o cetoprofeno em UPFP. PACIENTES E MÉTODOS: Estudo prospectivo, randomizado, duplo-cego com 24 pacientes submetidos à UPFP, divididos em 2 grupos, sendo que 14 receberam cetorolaco e 10 cetoprofeno. Avaliação da intensidade da dor através de escala visual analógica e necessidade do uso associado de opióide (tramadol). RESULTADOS: Dos 14 pacientes que receberam cetorolaco, apenas 3 (21%) necessitaram uso complementar de opióide, enquanto que 7 (70%) do grupo do cetoprofeno o fizeram. Após 12 horas de cirurgia, houve um predomínio de 71% dos pacientes que receberam cetorolaco, com dor leve ou até ausência desta, enquanto 70% dos do cetoprofeno referiram dor moderada ou incômoda. Após 24 horas de cirurgia, 60% dos pacientes que fizeram uso de cetoprofeno referiam dor moderada a incômoda, ao passo que 86% dos do cetorolaco referiram dor leve à ausência. CONCLUSÃO: Conclui-se que o cetorolaco é mais eficaz em relação ao cetoprofeno no tratamento da dor pós-operatória imediata de UPFP, pois houve dor de menor intensidade e menor uso de opióide.

O artigo descreve a percepção de profissionais de saúde a respeito do prontuário eletrônico do paciente na gestão de hospitais e o impacto dessa tecnologia no trabalho. A coleta de dados ocorreu em 2009 e 2010 com 336 usuários deste prontuário, em dois hospitais universitários, sendo um no Brasil e outro na Espanha. Os participantes da pesquisa preencheram um questionário contendo respostas padronizadas em escala de concordância do tipo Likert. Foram realizadas análises estatísticas descritivas e inferenciais e os resultados mostram que: a) esta tecnologia é percebida como um mecanismo que contribui com a gestão e a assistência hospitalar mediante mecanismos de controle; b) os respondentes com maior domínio de informática concordam mais do que aqueles com menor domínio a respeito do impacto desse prontuário no respectivo desempenho; c) há diferenças estatisticamente significativas nas médias de percepção do papel desse prontuário no desempenho quando considerado o tempo de serviço e o hospital (ou o país) onde o participante da pesquisa trabalha. Estudos adicionais são sugeridos no final.

En la década del 1980, un nuevo fenómeno: las tomas de tierras o "asentamientos" transforma los procesos de urbanización en el Area Metropolitana de Buenos Aires. Los autores, miembros de un equipo de investigación de la Universidad de General Sarmiento (Argentina), entrevistaron a habitantes de estos asentamientos, y se sorprendieron con las frecuentes menciones a los saqueos y ollas populares ocurridos doce años atrás en un contexto de hiperinflación. En este artículo, estos hechos son considerados como constitutivos de una experiencia formativa iniciada con la organización para la toma de tierras y seguida poco después por los saqueos de supermercados y otros negocios durante la hiperinflación de 1989. Esto da origen, posteriormente, a las ollas populares que paliaron el hambre de ese momento, y que luego se transformaron en comedores comunitarios, guarderías y otras organizaciones que hoy persisten. Esta indagación se apoya en los relatos de los entrevistados y en la recopilación de diferentes periódicos de ese momento.

O presente estudo, de características exploratório-descritivas, teve como objectivo geral avaliar a percepção dos profissionais de saúde sobre o exercício físico em transplantados renais e, mais especificamente, avaliar o grau de informação dos profissionais de saúde sobre o tema; analisar a necessidade de informação sobre o tema e avaliar a relevância atribuída à promoção do exercício físico na rotina dos cuidados prestados aos transplantados renais. A metodologia adaptada foi a aplicação do método de inquérito por questionário à amostra constituída por médicos, enfermeiros e fisioterapeutas (n=89) de sete hospitais que realizam transplante renal.

This paper describes an assessment of the impact of the enforcement of the European carbon dioxide (CO2) emissions trading scheme on the Portuguese chemical industry, based on cost structure, CO2 emissions, electricity consumption and allocated allowances data from a survey to four Portuguese representative units of the chemical industry sector, and considering scenarios that allow the estimation of increases on both direct and indirect production costs. These estimated cost increases were also compared with similar data from other European Industries, found in the references and with conclusions from simulation studies. Thus, it was possible to ascertain the impact of buying extra CO2 emission permits, which could be considered as limited. It was also found that this impact is somewhat lower than the impacts for other industrial sectors.
