Visceral leishmaniasis is a severe and potentially fatal vector-borne disease. The most typical symptoms are fever, hepatosplenomegaly, weight loss, bleeding and bacterial infections. Neurological changes are rarely reported. This paper describes a child who presented with neurological signs as the first symptoms of leishmaniasis; tone was diminished and tremors in the extremities were observed. A diagnosis of visceral leishmaniasis was confirmed by parasite detection in the bone marrow. Symptoms were reversed by specific treatment. The nature of a possible mechanism of neurological involvement in visceral leishmaniasis remains unexplained.

INTRODUCTION: White piedra is a superficial mycosis caused by the genus Trichosporon and characterized by nodules on hair shaft. METHODS: The authors report a family referred to as pediculosis. Mycological culture on Mycosel® plus molecular identification was performed to precisely identify the etiology. RESULTS: A Trichosporon spp. infection was revealed. The molecular procedure identified the agent as Trichosporon inkin. CONCLUSIONS: White piedra and infection caused by T. inkin are rarely reported in Southern Brazil. The molecular tools are essentials on identifying the Trichosporon species.

INTRODUCTION: Persistence of the hepatitis B virus (HBV) genome in individuals negative for the HBV surface antigen (HBsAg) reflects occult infection. The aim of this study was to identify occult HBV infection among hemodialysis patients at 5 clinics in Recife, State of Pernambuco, Brazil, between August 2006 and August 2007. METHODS: Serum samples underwent enzyme-linked immunosorbent assay to investigate total antibodies against HBcAg (anti-HBc), HBsAg, and antibodies against HBsAg (anti-HBs). Samples that were HBsAg-negative were tested for total anti-HBc, and those that were positive for total anti-HBc were tested for anti-HBs. HBV DNA was investigated with an in-house PCR technique to identify samples positive for total anti-HBc. Subsequently, the samples positive for HBV DNA were sequenced to identify the genotype and mutations. RESULTS: The study population (n = 752) had a mean age of 50 15.1 years and included both sexes. All samples analyzed were negative for HBsAg. The seroprevalence of total anti-HBc was 26.7% (201/752), while that of anti-HBs was 67.2% (135/201). Total anti-HBc alone was detected in 5.7% of the patients. Occult infection was found in 1.5%, comprising genotypes A (33.3%, 1/3) and D (66.7%, 2/3). No mutations were found. CONCLUSIONS: The study detected occult hepatitis B virus infection in hemodialysis patients. Molecular studies on HBV are of fundamental importance because they identify patients that had been considered virus-negative but who, in reality, host the virus and have the ability to transmit it to other patients and staff.

The treatment of chronic hepatitis C has frequent side effects such as cytopenias and neuropsychiatric symptoms. However, pulmonary toxicity associated with interferon is rarely described. This paper describes the clinical case of a 67-year-old female patient with chronic hepatitis C who presented an acute onset of dry cough, dyspnoea, and fever 36 weeks after the use of pegylated interferon alfa-2a and ribavirin. The lung biopsy confirmed the diagnosis of a bronchiolitis obliterans organizing pneumonia (BOOP). Corticotherapy was initiated, with clinical and radiological improvement. This paper aims to advise physicians to this occasional, though severe, adverse event related to hepatitis C virus (HCV) treatment.

Introduction Arboviruses are an important public health problem in Brazil, in especially flaviviruses, including the Saint Louis encephalitis virus (SLEV) and the Rocio virus (ROCV), are especially problematic. These viruses are transmitted to humans or other vertebrates through arthropod bites and may cause diseases with clinical manifestations that range from asymptomatic infection, viral hemorrhagic fever to encephalitis. Methods A serological survey of horses from various regions of Brazil using an enzyme-linked immunosorbent assay (ELISA) with recombinant SLEV domain III peptides and ROCV E protein as antigens. Results Overall, 415 (55.1%) of the 753 horses that were screened were seropositive for flavivirus and, among them, monotypic reactions were observed to SLEV in 93 (12.3%) and to ROCV in 46 (6.1%). These results suggested that these viruses, or other closely related viruses, are infecting horses in Brazil. However, none of the studied horses presented central nervous system infection symptoms. Conclusions Our results suggest that SLEV and ROCV previously circulated among horses in northeast, west-central and southeast Brazil.

Introduction The present study shows the colonization of Aedes mosquitoes in breeding sites specific for Culex quinquefasciatus in neighborhoods in the municipality of Olinda. Methods Samples were collected between May 2011 and June 2012 from breeding sites positive for Cx. quinquefasciatus by using a ladle and manual suction pump. Results Aedes aegypti (0.12%), Aedes albopictus (0.03%), and Cx. quinquefasciatus (99.8%) were found across the breeding sites. Conclusions The presence of Aedes ssp. in several Cx. quinquefasciatus breeding sites with a heavy load of organic material demonstrates the need to review the concepts and methods used for treatment, as the use of specific larvicide for breeding sites of Culex.

Introduction Leishmaniasis and toxoplasmosis are important to public health. Methods Antibodies for Toxoplasma gondii and Leishmania spp. were evaluated in cats from Campo Grande, State of Mato Grosso do Sul, Brazil, a region endemic for canine visceral leishmaniasis. Serum samples from 50 asymptomatic cats were titrated for T. gondii by the immunofluorescence antibody test and modified agglutination test and for Leishmania spp. by the immunofluorescence antibody test. Results These two agents coinfected two (4%) of the 50 tested animals. Conclusions These findings demonstrate the concomitant presence of two important zoonoses in cats from Brazilian endemic regions for canine visceral leishmaniasis.

INTRODUCTION : We hypothesized higher mannose-binding lectin level and classic factors (i.e., age, sex, alcohol consumption, exposure, and specific treatment) are associated with the severity of periportal fibrosis in schistosomiasis. METHODS : This cross-sectional study involved 79 patients infected with Schistosoma mansoni with severe or mild/moderate periportal fibrosis. Serum concentrations of mannose-binding lectin were obtained by enzyme-linked immunosorbent assay (ELISA). RESULTS: Higher serum level of mannose-binding lectin was significantly associated with advanced periportal fibrosis. CONCLUSIONS: Mannose-binding lectin may contribute to liver pathology in schistosomiasis and may represent a risk factor for advanced periportal fibrosis in the Brazilian population studied.

184 machos da Carcharchinus porosas foram capturados com rede de emalhar nas Reentrâncias Maranhenses, entre Cururupu (1º 20' S e 45º W) e São Luis (2º 241 30'' S e 44º W) no período de outubro de 1983 à janeiro de 1984 e de junho à setembro de 1984. O desenvolvimento dos machos desta espécie foi estudado através da relação peso-comprimento e do crescimento: dos claspers, dos testículos, dos epidídimos e do fígado. Com exceção dos claspers e do fígado, o crescimento dos órgãos em questão é contínuo. No crescimento dos claspers evidenciou-se 3 fases que correspondem: a fase juvenil, a fase de adolescência e a fase adulta. Demonstrou-se, também, que existe uma descontinuidade no crescimento do fígado no momento da passagem da fase de adolescência à fase adulta, e que esta se verifica quando se relaciona o peso deste órgão com o peso corporal. Esfregaços do conteúdo da visívula seminal mostraram que o líquido branco presente neste órgão é livre de espermatozóides até 70 cm. Com base no crescimento dos claspers e na produção de espermatozóides normais, estabeleceu-se o tamanho de primeira maturidade em 71 cm (fim da fase de adolescência) e o tamanho de 100% de maturidade em 75 cm (início da fase adulta).

This study investigated the efficiency of Moringa oleifera (MO) seeds as natural coagulant in coagulation/flocculation/dissolved air flotation (C/F/DAF), followed by nanofiltration (NF) for Microcystis protocystis and microcystin-LR removal. The methodology adopted in this work was performed in two steps: 1) coagulation/flocculation/dissolved air flotation (C/F/DAF) process using the MO extracted in saline solution of potassium chloride (KCl-1M) and sodium chloride (NaCl-1M) in optimum dosage 50 mg·L-1; 2) nanofiltration process using NF90 and NF270 membrane provided Dow Chemical Company®. A working pressure of 8 bar was applied. In all samples were analyzed color, turbidity, pH, cyanobacterial cells count and microcystin concentration. The use of MO seeds as natural coagulant, obtained satisfactory results in the M. protocystis, color and turbidity removal. NF was able to completely remove cyanobacterial cells and microcystins (100 %) from M. protocystis (always under the quantification limit). Therefore, C/F/DAF+NF sequence is a safe barrier against M. protocystis and microcystins in drinking water.

A vegetação secundária tem funções relevantes para os ecossistemas, tais como a fixação de carbono atmosférico, a manutenção da biodiversidade, o estabelecimento da conectividade entre remanescentes florestais, manutenção dos regime hidrológico e a recuperação da fertilidade do solo. O objetivo deste trabalho é, através de uma abordagem amostral, estimar a área ocupada por vegetação secundária na Amazônia Legal Brasileira (AML) em 2006. A amostragem se baseia em uma abordagem estratificada pelo grau de desflorestamento das cenas LANDSAT-TM que recobrem a AML. Foram selecionadas 26 cenas para o ano de 2006, distribuídas em sete estratos conforme o percentual de desflorestamento, nas quais foram mapeadas as áreas de vegetação secundária a partir de técnicas de classificação de imagens. Foi desenvolvido um modelo multivariado de regressão para estimar a área de vegetação secundária utilizando como variáveis independentes a área de desflorestamento, a área de hidrografia, a estrutura agrária, e área das unidades de conservação. A análise de regressão encontrou um R2 ajustado de 0,84 , e coeficientes positivos para a proporção de hidrografia na imagem (2,055) e para a estrutura agrária (0,197), e coeficientes negativos para o grau de desflorestamento na imagem (-0,232) e para a proporção de Unidades de Conservação na imagem (-0,262). O modelo de regressão estimou uma área de 131.873 km² de vegetação secundária para o ano de 2006. Aplicando uma simulação Monte Carlo foi estimada uma incerteza de aproximadamente 12.445 km² para a área.

A palmeira babaçu (Attalea speciosa C.Martius, Arecaceae) tem grande importância socioeconômica e ecológica em grande parte da área tropical brasileira, especialmente em áreas degradadas por queimadas freqüentes na Amazônia. No entanto, ainda pouco se sabe sobre as características ecológicas desta espécie-chave. Este estudo investiga a alometria do babaçu com o objetivo de estabelecer uma metodologia eficiente na estimativa da biomassa aérea de palmeiras juvenis e adultas e para um melhor entendimento da sua arquitetura. A biomassa de palmeiras juvenis pode ser estimada facilmente e com precisão com o diâmetro mínimo das ráquis das folhas a 30 cm de extensão. A biomassa de palmeiras adultas pode ser estimada com base na altura do tronco lenhoso, também relativamente de fácil medição em campo. A biomassa foliar das palmeiras adultas foi em media 31,7% da biomassa aérea, porém houve uma alta variação e, portanto, somente pode ser estimada indiretamente através da relação entre a razão madeira:folha e biomassa aérea total. Os teores de carbono no babaçu apresentaram baixa variação, sem diferenças sistemáticas em relação ao tamanho ou estágio de crescimento, o que aponta à aplicabilidade geral dos valores 42.5% C para troncos, 39.8% C para folhas. Em conseqüência do limitado crescimento secundário do diâmetro inerente de palmeiras, não houve relação do diâmetro de tronco com a altura e a biomassa das palmeiras adultas. Observou-se que o afilamento do caule diminui com o aumento da altura das palmeiras, o que é parcialmente compensado pelo incremento da densidade de madeira em troncos quase-cilíndricos. No entanto, a altura máxima do babaçu, de cerca de 30 metros, aparentemente está definida por limitações na estabilidade mecânica. Todas as relações alométricas aqui descritas são independentes da idade da vegetação, indicando a aplicabilidade geral das relações encontradas.

Criança com dois anos e seis meses de idade, apresentando cardiomiopatia dilatada idiopática, classe funcional III-IV (New York Heart Association - NYHA) e indicação de transplante cardíaco, foi submetida à ventriculectomia parcial, pela técnica de Batista. A avaliação pré-operatória com ecocardiograma (ECO) e ventriculografia radioisotópica (Gated Blood Pool) revelaram fração de ejeção de ventrículo esquerdo (FEVE) de 13% e 20%, respectivamente. Após a cirurgia, que consistiu na remoção de uma parte do músculo da parede lateral do ventrículo esquerdo, a FEVE foi para 50% pelo ECO e 30% pela ventriculografia radioisotópica. A criança recebeu alta hospitalar no 16º pós-operatório em classe funcional I (NYHA).

OBJETIVO: Em ausência de alterações estruturais miocárdicas (AEM), avaliar se o bloqueio de ramo direito (BRD) gera potenciais fragmentados (PF) e turbulência espectral (TE) no eletrocardiograma de alta resolução (ECGAR). MÉTODOS: Doze crianças com comunicação inter-atrial (CIA) e bloqueio incompleto do ramo direito (BIRD) sem AEM (Grupo I), foram comparadas com 17 crianças com tetralogia de Fallot (TF) operada, BCRD e AEM, 5 com extra sístoles ventriculares e 2 com taquicardia ventricular sustentada (Grupo II). Todas fizeram ECGAR nos domínios do tempo (DT) e da freqüência (DF), com cinco variáveis analisadas. RESULTADOS: Os pacientes do grupo I tiveram as variáveis normais apesar do BIRD. No grupo II, 4 das cinco variáveis foram anormais, sugerindo a presença de PF e TE atribuíveis a AEM inerentes à malformação e ao ato cirúrgico. CONCLUSÃO: Na CIA o BIRD não complicado de AEM não gera PF e TE, não constituindo fator de risco para taquicardia ventricular sustentada.

OBJECTIVE: To identifity characteristics associated with complications during pregnancy and puerperium in patients with rheumatic mitral stenosis. METHODS: Forty-one pregnant women (forty-five pregnancies) with mitral stenosis, followed-up from 1991 to 1999 were retrospectively evaluated. Predictor variables: the mitral valve area (MVA), measured by echocardiogram, and functional class (FC) before pregnancy (NYHA criteria).Maternal events: progression of heart failure, need for cardiac surgery or balloon mitral valvulotomy, death, and thromboembolism. Fetal/neonatal events: abortion, fetal or neonatal death, prematurity or low birth weight (<2,500g), and extended stay in the nursery or hospitalization in newborn ICU. RESULTS: The mean ± SD of age of the patients was 28.8±4.6 years. The eventful and uneventful patients were similar in age and percentage of first pregnancies. As compared with the level 1 MVA, the relative risk (RR) of maternal events was 5.5 (95% confidence interval (CI) =0.8-39.7) for level 2 MVA and 11.4 (95% CI=1.7-74.5) for level 3 MVA. The prepregnancy FC (FC > or = II and III versus I) was also associated with a risk for maternal events (RR=2.7; 95% CI=1.4-5.3).MVA and FC were not importantly associated with these events, although a smaller frequency of fetal/neonatal events was observed in patients who had undergone balloon valvulotomy. CONCLUSION: In pregnant women with mitral stenosis, the MVA and the FC are strongly associated with maternal complications but are not associated with fetal/neonatal events. Balloon mitral valvulotomy could have contributed to reducing the risks of fetal/neonatal events in the more symptomatic patients who had to undergo this procedure during pregnancy.
