A Pseudomonas aeruginosa aparece como um patógeno oportunista na fibrose cística, sendo a maior fonte de infecção pulmonar e é responsável por altas taxas de mortalidade nestes pacientes. O objetivo deste trabalho é de mostrar as características espirométricas e clínicas dos pacientes com fibrose cística atendidos no Hospital Universitário João de Barros Barreto (HUJBB), no Pará. Foi realizado um estudo retrospectivo nos prontuários de 44 pacientes acompanhados no HUJBB, no período de 1997-2007, que atenderam aos critérios de inclusão, sendo 14 colonizados por P. aeruginosa, com a ocorrência de 9 óbitos. Com nível de significância de 5%. No grupo colonizado 10 eram do sexo feminino, a idade média dos sintomas iniciais foi de 0.3±0.6 anos, com diferença significativa quando comparado com pacientes não colonizados (p<0.05). A idade média ao diagnóstico foi de 13.1±10.8 nos colonizados todos apresentavam sintomas respiratórios persistentes ao diagnóstico. A média dos valores percentuais preditos das espirometrias, referentes à avaliação inicial e final, do grupo colonizado foi VEF1(60.0± 25.0%) e (47,82±16.1%) e FEF25-75%(42.5± 22.9%) e (26,5±17.9%) e no grupo não colonizado foi VEF1(79.2± 21.0%) e (79,6±18.0%) e FEF25-75%(69.2± 26.7%) e (68,9±26.8%), respectivamente (p<0.005). A média do escore de Shwachman inicial nos colonizados foi de 42.9±13.5 e nos não colonizados foi de 68.4±15.1(p<0.0001) e na avaliação final foi de 36.6±18.7 e 73.6±12.3 (p<0.0001), respectivamente, com diferença significativa entre os grupos. Os fatores relacionados aos óbitos encontrados foram a colonização por P. aeruginosa, estado nutricional deficiente e VEF1 reduzido. Na casuística estudada foi encontrado maior comprometimento da função pulmonar e idade média mais elevada ao diagnóstico do que em outros estados brasileiros. Estes dados reforçam a necessidade de ações para um diagnóstico precoce no estado do Pará, propiciando uma abordagem terapêutica eficaz e direcionada, buscando aumento de sobrevida e melhoria da qualidade de vida destes indivíduos, objetivo principal das ações dos profissionais envolvidos nesta assistência.

PURPOSE: To analyze the changes in both respiratory function and cardiopulmonary exercise tests results in patients subjected to laparoscopic cholecystectomy. METHODS: Fifty patients were evaluated (76% women) and the average age was 47.8±14.2 years. All individuals underwent the measurement of spirometry, manovacuometry, 6-minute walk test (6MWT) and stair-climbing test (SCT). All tests were performed at the first (PO1), fifth (PO5) and thirtieth (PO30) postoperative days. RESULTS: BMI average was 28.8±4.8 kg/m2. Sample comprised 68% non-smokers, 20% current smokers, and 12% former smokers. There was no incidence of postoperative complication whatsoever. There was a significant decrease in spirometric values at PO1, but values were similar to the ones of PRE at PO30. Manovacuometry showed alterations at PO1 displaying values that were similar to the ones of PRE at PO30. 6MWT was significantly shorter at until PO5, but at PO30 values were similar to ones of PRE. As for SCT, values were significantly compromised at PO5 and PO30 since they were similar to the ones of PRE. CONCLUSION: Patients submitted to laparoscopic cholecystectomy present a decrease in cardiorespiratory function on the first postoperative moments but there is a rapid return to preoperative conditions.

Ivabradine (IVB) is a heart rate lowering agent that acts via selective inhibition of the pacemaker funny current in sinoatrial nodal P cells, thus, reducing heart rate at rest and during exercise with minimal effect on myocardial contractility, blood pressure, and intracardiac conduction. IVB exerts no effect on external respiratory function parameters and it may also play a role in patients with concurrent chronic obstructive pulmonary disease. This property constitutes an important advantage over β-blockers. IVB acts by reducing the heart rate in a mechanism different from β-blockers, calcium channel blockers or late sodium channel blockers, three commonly prescribed antianginal drugs. As clinical trials have shown, it is remarkably well-tolerated and offers an alternative for patients who cannot take β-blockers. The combination of IVB and atenolol at commonly used doses in patients with chronic stable angina produced additional efficacy with no untoward effect on safety or tolerability. Additionally, side effects are rare and largely limited to a luminous phenomenon or phosphenes. This sensation is thought to be due to a block of Ih in the retina, a current very similar to cardiac If channels. IVB is contraindicated in patients with sick sinus syndrome or sinus node dysfunction and in patients taking hepatic inhibitors of Cytochrome P450 family 3, subfamily A, polypeptide 4 (abbreviated CYP3A4), with exception of omeprazole or lansoprazole. This review briefly summarizes the main studies regarding this drug.

The research discusses the pulmonary function of the students of SORRI that are part of the Project for the extension of the Department of Physical Education in Bauru Swimming for People with Disabilities during the aquatic activities, noting the changes in efficiency class of breathing with the practice of aquatic activity, by the analysis of the volume and respiratory capacity. The respiratory control is essential in the process of adaptation to the liquid medium, because a student who fails to immerse the face in the water will not be able to adopt a horizontal position sufficiently stable. The practice of aquatic activities requires a great effort of breathing. Therefore, the practice of aquatic activities influence favorably the breathing, because the movements performed in the water tone the diaphragm, which is the essential muscle of the breathing, allowing an improvement in pulmonary ventilation. For the purposes of this study, the sample was composed of 10 students of the SORRI of the city of Bauru forming part of the project Swimming for People with Disabilities, practicing swimming once a week for 1 hour. For the collection of lung volumes and capacities was used a transducer of air flow and a unit of data collection, model MP36, both of the brand Biopac connected to a computer, where the data were collected and recorded for later analysis. Two tests were carried out in a day with each participant in the project of swimming, and a test at the beginning of the lesson, at rest, and another after a series of 10 breaths carried out within the swimming pool. These tests were performed in the months of March and April. The analysis of the data was through the medium of the figures and also individually, noting the changes in respiratory function of students practicing aquatic activities, without which there would be no comparison between the participants. Twenty-four bitches which had been in labour for less than 12 hours were randomly divided into four groups of six. They all received 0(.)5 mg/kg of chlorpromazine intravenously as premedication, followed 15 minutes later by either 8 mg/kg of thiopentone intravenously (group 1), 2 mg/kg of ketamine and 0-5 mg/kg of midazolam intravenously (group 2), 5 mg/kg of propofol intravenously (group 3), or 2(.)5 mg/kg of 2 per cent lidocaine with adrenaline and 0(.)625 mg/kg of 0(.)5 per cent bupivacaine with adrenaline epidurally (group 4). Except for group 4, the bitches were intubated and anaesthesia was maintained with enflurane. The puppies' heart and respiratory rates and their pain, sucking, anogenital, magnum and flexion reflexes were measured as they were removed from the uterus. The puppies' respiratory rate was higher after epidural anaesthesia. in general the puppies' neurological reflexes were most depressed after midazolam/ketamine, followed by thiopentone, propofol and epidural anaesthesia.

Objective: To investigate the significance of cellular immune markers, as well as that of collagen and elastic components of the extracellular matrix, within granulomatous structures in biopsies of patients with pulmonary or extrapulmonary sarcoidosis. Methods: We carried out qualitative and quantitative evaluations of inflammatory cells, collagen fibers, and elastic fibers in granulomatous structures in surgical biopsies of 40 patients with pulmonary and extrapulmonary sarcoidosis using histomorphometry, immunohistochemistry, picrosirius red staining, and Weigert's resorcin-fuchsin staining. Results: The extrapulmonary tissue biopsies presented significantly higher densities of lymphocytes, macrophages, and neutrophils than did the lung tissue biopsies. Pulmonary granulomas showed a significantly higher number of collagen fibers and a lower density of elastic fibers than did extrapulmonary granulomas. The amount of macrophages in the lung samples correlated with FVC (p < 0.05), whereas the amount of CD3+, CD4+, and CD8+ lymphocytes correlated with the FEV1/FVC ratio and VC. There were inverse correlations between TLC and the CD1a+ cell count (p < 0.05), as well as between DLCO and collagen/elastic fiber density (r = -0.90; p = 0.04). Conclusions: Immunophenotyping and remodeling both showed differences between pulmonary and extrapulmonary sarcoidosis in terms of the characteristics of the biopsy samples. These differences correlated with the clinical and spirometric data obtained for the patients, suggesting that two different pathways are involved in the mechanism of antigen clearance, which was more effective in the lungs and lymph nodes.

Extracellular matrix (ECM) composition has an important role in determining airway structure. We postulated that ECM lung composition of chronic obstructive pulmonary disease (COPD) patients differs from that observed in smoking and nonsmoking subjects without airflow obstruction. We determined the fractional areas of elastic fibres, type-I, -III and -IV collagen, versican, decorin, biglycan, lumican, fibronectin and tenascin in different compartments of the large and small airways and lung parenchyma in 26 COPD patients, 26 smokers without COPD and 16 nonsmoking control subjects. The fractional area of elastic fibres was higher in non-obstructed smokers than in COPD and nonsmoking controls, in all lung compartments. Type-I collagen fractional area was lower in the large and small airways of COPD patients and in the small airways of non-obstructed smokers than in nonsmokers. Compared with nonsmokers, COPD patients had lower versican fractional area in the parenchyma, higher fibronectin fractional area in small airways and higher tenascin fractional area in large and small airways compartments. In COPD patients, significant correlations were found between elastic fibres and fibronectin and lung function parameters. Alterations of the major ECM components are widespread in all lung compartments of patients with COPD and may contribute to persistent airflow obstruction.
