Le Programme cantonal diabète (PCD) du canton de Vaud a pour objectifs de réduire durablement l'impact du diabète sur la population vaudoise. Il a été élaboré à l'initiative du Service de la santé publique du canton de Vaud et la stratégie du programme a été construite sur la base des propositions de plusieurs groupes de travail. Ce programme inclut divers projets complémentaires qui ont été progressivement mis sur pied à partir de 2010. 2012 marque la fin de la première phase du programme et une évaluation-bilan du programme est demandée. Cette évaluation-bilan fait suite à une première phase de travaux qui ont consisté en une évaluation de l'évaluabilité du programme. Les résultats de cette première phase ont été délivrés en décembre 2011 au groupe de pilotage et consignés dans un rapport en février 2012. L'IUMSP propose donc pour 2012 des travaux en continuité de ce qui a été fait en 2011 et qui vont dans le sens d'un bilan critique et de propositions pour un système de monitoring à long terme du programme, comportant des indicateurs d'activité (à harmoniser lorsque c'est possible dans des projets similaires), des indicateurs de couverture et de résultats. Dans le cadre de l'élaboration du Programme cantonal Diabète du canton de Vaud, nous avons mis en place une étude qualitative visant à connaître l'opinion des divers acteurs du système de santé du canton, sur la prise en charge actuelle du diabète et le développement du Programme cantonal Diabète. Nous avons recruté des patients diabétiques et des professionnels de la santé dans le but d'organiser huit focus-groupes (entretiens de groupe) : un focus-groupe de patients diabétiques et un focus-groupe de professionnels de la santé, dans chacune des quatre régions sanitaires du canton de Vaud. Background: Previous work examining differences in hypertension across ethnic groups employ race as the principal variable. While differences in hypertension have been identified across racial groups, there is great variation between ethnic groups amongst racial groupings that could mask differences in hypertension and cardiovascular disease (CVD) risk. In light of Canada's ethnic diversity, research aimed at identifying specific groups that are at a health disadvantage is essential for understanding the health of the overall population. In addition, this research would be beneficial for creating programs and policies aimed at reducing or eliminating these disparities. Since CVD is the leading cause of mortality in Canada and hypertension is one of the most significant and modifiable risk factors for CVD, it is important to move past crude classifications based on race and examine ethnic group differences. The purpose of this study is to examine the relationship between ethnicity and hypertension in Canada, while employing more narrow classifications for ethnicity than previous studies. In addition, because ethnicity has been shown to be representative of an individual's social experience, this study also aims to investigate whether this relationship can be explained by one or all of the following variable: socioeconomic status, physical activity, body mass index, smoking status, daily alcohol consumption or acculturation. Methods. This study used the 2004 Canadian Community Health Survey, cycle 2.1 to compare 29 different ethnic groups in Canada on whether they had high blood pressure that had been diagnosed by a health professional. Associations were examined using logistic regression. Subsequent logistic regression analyses included socioeconomic status, physical activity, body mass index, smoking status, daily alcohol consumption and acculturation to test for the effect of each of these variables on the relationship between ethnicity and hypertension. Results. Ukrainians, Chinese, Portuguese, South Asians, Aboriginals, Blacks, Filipinos and South East Asians were found to have significantly higher odds of having high blood pressure than Canadians (OR's = 1.50, 1.56, 2.72, 1.38, 1.36, 1.66, 2.21 & 2.24 respectively, p<.001). In addition, the only significant mediating effects were between SES and Aboriginals as well as obesity and Aboriginals. None of the other independent variables accounted for >10% of the risk experienced by the ethnic groups that were significantly associated with hypertension. Interpretation: The odds of having high blood pressure in Canada varies considerably across ethnic groups within racial groups indicating previous research is not specific enough to inform policy and program development. Because this study was not able to explain this relationship using the sociodemographic and lifestyle factors mentioned above, future research should be done to determine what places certain ethnic groups at a greater risk in order to tailor interventions aimed at reducing high blood pressure that are suited to the specific needs of each cultural group.

This is a study which examines the roles and responsibilities of Deans, specifically focussing on the Deans in the Faculties of Education at three Ontario Universities - Brock University, the University of Western Ontario, and the University of Windsor. This study examines the roles of Deans in the context of leadership and as a management position. The initial belief of the researcher was that Deans acted as middle managers at their institution besides being role models, scholars and leaders. Data were collected through interviews with the various participants and through the examination of the official job descriptions at each institution. Concepts such as leadership, motivation, empowerment, and management are discussed in relation to the position of Dean. The research concludes that a Dean is a leader in higher education who is responsible for a variety of issues. Besides academic related responsibilities such as faculty development, program development and research, a Dean is also responsible for a wide range of administrative tasks including financial management and obligations to external groups. As a role model and scholar, the Dean must ensure that all areas have sufficient energies devoted to them. This creates a heavy burden on Deans as they have a great deal of responsibilities to manage while still maintaining their role as a scholar. The researcher concludes that the position of Dean requires additional support from the institution. This support could be in an Associate Dean or an Executive Assistant with training and support mechanisms on an ongoing basis.

El giro que el sistema internacional dio tras los atentados perpetrados el 11 de septiembre de 2001 fue de enorme importancia para la política exterior de Irán y su posicionamiento en el sistema internacional. En efecto, la lucha guerra contra el terrorismo que Estados Unidos inició y que finalmente desembocaría en las incursiones armadas a Afganistán e Irak -ambos países limítrofes de Irán, en 2001 y 2003 respectivamente, así como la inclusión de Irán por parte de la administración Bush en 2002 como parte del denominado Eje del mal, generaron en el imaginario iraní una idea de amenaza internacional y por lo mismo, la necesidad de hacerse fuerte con el fin de mantener su integridad territorial y gubernamental. Así pues, una compleja situación económica y social sumada a una fuerte coalición del partido conservador iraní -uno de los más fuertes del país, soportado por el Ayatolá Alí Jamenei, autoridad suprema de la nueva República, un nuevo líder de corte conservador, llegaría a la presidencia: Mahmoud Ahmadinejad, un líder controversial y problemático que desde su primera campaña presidencial en 2002 defendería el derecho de la República a desarrollar armas nucleares para fines pacíficos. El verdadero inconveniente surge cuando occidente, en cabeza de los Estados Unidos –en compañía de algunas potencias europeas como lo son Alemania, Francia y Gran Bretaña, duda sobre esta última afirmación. Esta investigación pretende profundizar cada uno de los elementos anteriormente mencionados así como establecer la forma en la cual China y Rusia –países que ven de forma diferenciada el manejo de arsenal nuclear, logran configurarse y acreditar este desafío ante la comunidad internacional.

Esta monografía se interroga sobre los factores que, en el caso del Programa de Desarrollo y Paz del Magdalena Medio (PDPMM), llevaron a la emergencia de una forma de diplomacia paralela. Dicho análisis se hará tomando como referencia el periodo de formulación y entrada en vigencia del Plan Colombia. Los propósitos particulares de este trabajo son los siguientes. En primer lugar, caracterizar al Programa de Desarrollo y Paz teniendo en cuenta el contexto nacional y regional en el cual emerge esta iniciativa, con el objeto de explicar por qué esta tiene desde el comienzo una conexión muy fuerte con el ámbito internacional. En segundo lugar, analizar las posiciones del programa frente al Plan Colombia y explicar de qué manera y por qué razones en un momento específico sus dirigentes decidieron lanzar una iniciativa de diplomacia paralela, tratando así de contrarrestar los efectos negativos que le traería al Magdalena Medio la entrada en vigencia del Plan Colombia.

One quarter of children and young people (CYP) experience anxiety and/or depression before adulthood, but treatment is sometimes unavailable or inadequate. Self-help interventions may have a role in augmenting treatment and this work aimed to systematically review the evidence for computerised anxiety and depression interventions in CYP aged 5–25 years old. Databases were searched for randomised controlled trials and 27 studies were identified. For young people (12–25 years) with risk of diagnosed anxiety disorders or depression, computerised CBT (cCBT) had positive effects for symptoms of anxiety (SMD −0.77, 95% CI −1.45 to −0.09, k = 6, N = 220) and depression (SMD −0.62, 95% CI −1.13 to −0.11, k = 7, N = 279). In a general population study of young people, there were small positive effects for anxiety (SMD −0.15, 95% CI −0.26 to −0.03; N = 1273) and depression (SMD −0.15, 95% CI −0.26 to −0.03; N = 1280). There was uncertainty around the effectiveness of cCBT in children (5–11 years). Evidence for other computerised interventions was sparse and inconclusive. Computerised CBT has potential for treating and preventing anxiety and depression in clinical and general populations of young people. Further program development and research is required to extend its use and establish its benefit in children.

This book is focused on ten action research and evaluative case studies in environmental education carried out by teacher educators and teachers. The case studies range across five European countries: Austria, Hungary, Italy, Sweden and Switzerland. They are followed by cross-case comparisons which explore issues emerging from the documented reflective practice: aims of environmental education in the educational policy context of the countries, their relationship to the disciplines and the traditional knowledge transmission position, the role of action research approaches for innovation and reflection, and institutional conditions of collaboration in teacher education. This international case study project is research based in adopting professional development approaches that are informed by action research principles. It represents examples of innovation that challenge established practice in schools and teacher education institutions. It provides study material for all who attempt to describe, change and improve their own education practices and who want to adopt an action research approach to professional or program development.

This paper presents findings from a study on perceptions of e-commerce as an academic discipline in Australian universities. The study examined Australian universities' perceptions on whether e-commerce should be regarded as a business-oriented discipline or a technology-oriented discipline and further whether e-commerce should be considered as a distinctive discipline. Data was collected from official websites of all Australian universities and was categorized in accordance to award titles, host faculty and program structures. Findings showed that most Australian universities perceived e-commerce as a business-oriented discipline. However, there was no consensus on whether e-commerce should be considered as a distinctive discipline.

Physical activity participation can provide benefits for both the health and well-being of the community. Many people, however, are not active enough to achieve these health and well-being outcomes. The Victorian Health Promotion Foundation’s (VicHealth) 2001 Active Recreation Scheme funded 27 projects designed to provide recreation programs to address the active recreation needs of disadvantaged groups who are often the least physically active members of Victorian communities. This research aimed to identify themes and strategies of these projects that increased participation or reduced barriers to participation in active recreation. Site visit interviews were conducted with representatives of 11 projects to become familiar with successful program strategies and barriers to recreation program development. Following the site visits a focus group discussion with representatives from all 27 funded projects was conducted to explore ways in which barriers to recreation participation could be minimized and what strategies were effective in increasing recreation participation. Nine general dimensions that were identified as strategic approaches to increase the participation of disadvantaged groups in recreation programs were relationships, resources, community values/attitudes, communication (promotion and education), participant awareness/motivation, autonomy supportive, planning, program design, and mentors/role models. It was found that a focus on a community coalitions and complementary policy developments had positive effects on creating active communities. Four themes that guided the community and policy developments were i) community partnerships, ii) community support, iii) focus on the participants and iv) program design elements. The study also showed that the management and manipulation of these themes assisted agencies to develop programs that increased recreation participation.

Electronic Commerce (EC) / Electronic Business (EB) has been (and is expected to continue to be) a dynamic, rapidly evolving area of technology, requiring skilled people with up-to-date knowledge and skills. The global community has required (and still requires) tertiary academic programs to prepare and train these people quickly. In the late nineties, following a tidal wave of tertiary EC program development in the United States, new tertiary programs began to appear in the Asia-Pacific (AP) region to satisfy this need, over a very short period of time. This research project aims to examine whether the development and effectiveness of tertiary EC/EB educational programs can be enhanced through employing a particular marketing paradigm. Four regions - Australia, New Zealand, Hong Kong SAR and the Republic of Singapore — were selected from the AP region, for this study. Based on a review of marketing literature, an inductive approach is adopted to build a model for new educational service product offerings. I also provide a description and comprehensive analysis of EC/EB education, and explore the model empirically, examining how it applies to the way EC education programs have been developed, to date. Essentially, this project consists of two major activities: theory building and theory testing – and is divided into three parts. Part 1: Preliminary study – literature review for theory building. This section of the thesis provides a literature review of the domains of curriculum development, EC/EB program development and management, EC/EB component models and new service product development. Part 2 : Understanding the marketplace – quantitative analysis. This section comprises five major surveys which provide an understanding of EC/EB education. Part 3 : In-depth analysis – qualitative research for theory testing. This section discusses the results of the multiple case studies of EC/EB degree programs undertaken over a five year period. The results of this project highlight both theoretical and practical aspects of the topic. In terms of the theoretical aspect, I provide a contribution to existing theory concerning the planning and development of new tertiary education programs. Research into academic course development in the past has tended to assume that all program development is pedagogically based and influenced. There is an assumption that people only develop academic programs and academic courses for pedagogic reasons. What this research project has done is to suggest that there are, in fact, many possible reasons for developing new programs and that, although these reasons might be pedagogic in nature, they can also be industry-focased, and market-oriented in the following ways: -the university is shaping the way it is perceived by the public – that is, the market; -the university is highlighting where its expertise lies. This led me to a form of new service product development consistent with the new image of the university. There is a clear need for diverse models for program development which accommodate the dynamic roles of modern universities. My research project develops such a model based on conditions in the Asia-Pacific region, and discusses findings arising from the overall project, which can be used to improve new educational program offerings in future, in both the Asia-Pacific and, I suggest, in other regions. This potential use of my findings highlights the practical contribution made by the research Project.

The aim of the research was to generate a cyclonic model for understanding the influences and processes of continuously improving management education in an environment rich in online learning technologies. The research questions were:
1. What is the nature of the cyclonic interactions observed in the transactions of a team of online management educators?
2. How might an understanding of cyclonic interactions
a. help refine action research, and
b. generate rich insight for online management education?
The methodology was an action research project. The research team worked in an online Master of Business Administration (MBA) to continuously develop teaching practice in one unit of the MBA. The methodology matched the objectives of the project, and the appropriate rigour associated with qualitative, interpretive research. The results showed that theories of systems and relational dynamics, adapted to hermeneutics and aligned with other learning theories, can be framed by the metaphor of a cyclone to conduct research into teaching practice and build upon the theory base in the field of online education.
Online management education is subject to reinterpretations. The cyclonic framework explains some of the changes. The project showed that a chaotic but organised cyclonic program development process in one particular MBA course was informative for and informed by the chaotic and cyclonic globalized business world. For the education of managers the cyclonic view was relevant. The approach was metaphorical and, therefore, opened new ways of seeing and speaking. Findings pertained to the nature of the cyclonic interactions, how an understanding of cyclonic interactions helped to refine action research, and how an understanding of cyclonic interactions helped generate rich insight for online management education.
It was found that it was the asymmetrical impetus of imperfection that created the examples of cyclonic learning spirals formed as double feedback loops for improved understanding. Online education in the action research required cyclical enhancement of connectedness by teachers, stronger emphasis on relational considerations in learning, and heightened expectations of collaboration by educators. It became possible to correlate earlier conceptions of action research with cyclonic categories and analyse the parallels with events in this action research project. Models were developed and presented to explain 3 cyclonic connections with hermeneutics, collaborative teaching, online resource
development, and the environment of online management.

The paper discusses the contrast between what is being done and what ought to be done about entrepreneurship education at university level. Research from the Global Entrepreneurship Monitor program demonstrates that entrepreneurship education is an issue of worldwide economic and social significance. It has major policy implications for every nation. The paper briefly discusses the network of entrepreneurship education and experiential learning of which the university is only one component – and not necessarily the most important. The strategic and organizational context of the movement towards more entrepreneurial universities is distinguished from the purely content issue of curriculum designed for aspiring entrepreneurial practitioners. An overview of actual curricula, worldwide, is contrasted with the normative entrepreneurship education framework posited by McMullan and Long (1987). Following consideration of the problems involved in measuring the success of entrepreneurship education programs, a broad, generic template for integrated program development is presented and compared with the approach usually employed in most MBA programs at university business schools. The hierarchical, functionalist approach, symbolized by a pyramid, is contrasted with a more fluid, organic and boundary-crossing approach, symbolized by a wheel. Its central hub is a ‘plus zone’ where lies the deepest challenge for development of entrepreneurship education in a university context.

"Joined-up' government and 'whole-of-government' approaches have evolved over the past two decades from the simple 'one-stop-shop' concept to much more formal organisational structures mandated at the highest levels. In many cases, the participants in these developments were learning on the job, as they responded to community and political demands for better service delivery and more accountability. This paper looks back at some of those developments and proposes a schema to assess and place policies, strategies and programs.
