Modificação da técnica de incisão para enxertos ósseos em bloco na reconstrução de maxilas atróficas

Introduction: The study of graft donor sites, whether from the anatomical, physiological or morphological point of view, has become a topic of current interest, due to the increasing number of patients needing facial bone reconstruction for various reasons. Purpose: In view of the need to constantly improve surgical techniques for autogenous bone graft harvesting, still considered the best choice for facial bone reconstruction, this paper describes an anatomical study on dry skulls in order to evaluate the average thickness of the parietal bone. Material and Methods: Measurements of this bone were taken with a goniometer, at four previously defined points, in the region that is often used as a donor site, in 49 dry skulls (98 parietal bones). The results were evaluated using the T test. Results: Thickness was measured at four predetermined points. The mean values (Point A = 4898mm, B = 4517mm, C = 6185mm, D = 4280mm) show that the bone can be even thinner than previously reported in the literature in other studies of the same nature. The largest bone thickness is in the medial and posterior region. Conclusion: A knowledge of these anatomical characteristics is helpful in preventing possible surgical complications, as well as making it safer for the surgeon to remove this graft and providing more information on whether or not to indicate this region as a bone graft donor site.

Introdution: Tooth extraction results in alveolar ridge resorption due to the progressive reabsorption, which frequently is a limitating factor of dental implants treatment, in function of the insufficient bone height for execution, could be indicated the use of grafts for increase of the alveolar edge. However, the success of the bone graft requests the meticulous attendance of the clinical stages. Proposition: The objective of this research was to evaluate the postoperative complications associated to the autogenous bone grafts. Materials and Methods: Through a random retrospective analysis, 90 file records of patients submitted to the surgical procedure of autogenous bone graft in the period of January from 2000 to the March of 2008. A clinical record was elaborated with base in the necessary data for this evaluation. Results: About the file data analysis, 59 female with average of 49,42 years and 31 male with average of 47,90 years. The previous diseases most related were arterial hypertension, stomachache and diabetis. The smokers was noticed in 13,3% of patients. The donor site most used was a mandibular ramus. The postoperative complications in receiving area represented 17,8% of file datas analysed and in the donor site only one patient showed parestesis. Conclusion: Among the total analyzed files it was observed that the postoperative complications associated with the autogenous bone grafts represented 18,9% of the patients submitted to the procedure, more frequently affecting the receiving area, obtaining partial exposure of the bone graft and absence of inserted gingiva on vestibular cortical bone.

Background: The principles of tissue regeneration to repair alveolar bone defects are based on the fabrication of a biologic barrier with different biomaterials. Therefore, the present study aimed to investigate the guided bone regeneration (GBR) by using membrane of demineralized bovine bone matrix (DBBM) on experimental defects in tibia of dogs. Methods: Four dogs were used and after anesthesia, shaving and antisepsis, two standardized bone defects were created on the right tibia of each animal with trephine drill. In the Control Group, the defects were filled with blood coagulum, while in the Treated Group, a membrane of DBBM was used to cover the defects. After 90 days, animals were sacrificed. Results: In the Treated Group, 67.4% of new bone formation was observed and, in the Control Group, 32.6% of the bone tissue reabsorbed when compared with initial bone volume. The membrane remained intact and no inflammatory tissue was observed on membrane/ bone interface. Conclusion: It was concluded that the use of DBBM is an osteoconductive material, presents biocompatibility and may be promise option to repair bone defects.

Allogeneic, fresh-frozen bone has been used in order to replace bone autografts. However, its osteoinduction and osteoconduction properties are not well-defined in the scientific literature. This work aimed to evaluate samples of homogenous bone grafts in humans by qualitative histological and immunohistochemical analysis. For this, ten pre-selected patients underwent surgical augmentation of bone defects. The homogenous fresh frozen block bone graft was stabilized and fixed by bicortical screws. After six months, the reopening procedure was performed for installation of osseointegrated implants. At this time surgical bone graft samples were removed by means of drill trephine. The samples were fixed in 10% formalin, processed with decalcified paraffin, and stained with hematoxylin and eosin. Immunohistochemistry was performed for the expression of Caspase 3 enzyme. The slides were brought to light microscopy for qualitative histology and immunohistochemistry. The results showed non-vital bone tissue, with few areas of deposition of new bone formation on the amorphous matrix, presence of chronic inflammatory infiltrate with areas of osteomyelitis, and expressive immunolabeling of Caspase 3. Given the methods employed and the results it was concluded that the allograft fresh-frozen block is not incorporated into the recipient bed after a healing period of six months.

OBJETIVO: Avaliar o valor da avaliação radiográfica pré-operatória nas revisões de artroplastias totais do joelho. MÉTODOS: Trinta e um joelhos operados no período de 2006 a 2008 em uma série consecutiva de casos de cirurgia de revisão de artroplastia total de joelho foram analisados retrospectivamente. CRITÉRIOS ANALISADOS: número de cunhas ou enxertos ósseos estruturados utilizados para preenchimento dos defeitos ósseos; localização das cunhas e enxertos ósseos utilizados e espessura média do polietileno utilizada. A classificação AORI era estabelecida previamente através de radiografias pré-operatórias segundo critérios preestabelecidos. Após a análise, os joelhos foram divididos em quatro grupos (I, IIA, IIB e III). RESULTADOS: O número médio de cunhas ou enxertos ósseos utilizados em cada joelho variou de maneira crescente entre os grupos (grupo I: 1,33; grupo IIA: 2; grupo IIB: 4,33; grupo III: 4,83) (P = 0,0012). As localizações mais comuns foram: medial na tíbia e posteromedial no fêmur. Não houve diferença estatisticamente significante na espessura do polietileno utilizado. CONCLUSÃO: A classificação AORI para defeitos ósseos no joelho baseada em radiografias pré-operatórias mostrou correlação crescente com a necessidade de utilização de cunhas e/ou enxertos estruturados na revisão de artroplastia total do joelho. Porém, até 46% dos joelhos dos grupos I e IIA apresentaram falhas ósseas de até 5mm não diagnosticadas através das radiografias pré-operatórias.

OBJETIVO: A análise morfológica da estrutura óssea de C2, em pacientes com artrite reumatoide, com o objetivo de aumentar a segurança de procedimentos de estabilização desta vértebra. MÉTODOS: Analisamos retrospectivamente 20 tomografias de coluna cervical realizadas em pacientes com artrite reumatoide, foram medidos os seguintes parâmetros: o ângulo espinolaminar, espessura e comprimento da lâmina de C2. RESULTADOS: Os valores médios encontrados são: espessura das lâminas direita 5,92mm e esquerda 5,87mm; comprimento das lâminas direita 27,75mm e esquerda 27,94mm e ângulo espinolaminar 44,7(0). CONCLUSÃO: Os valores obtidos são compatíveis com os de estudos realizados em indivíduos normais publicados por outros autores, não havendo aparente necessidade de alteração na técnica para colocação dos parafusos. OBJETIVO: Avaliar a posição das estruturas anatômicas em risco durante a inserção de parafusos pediculares na coluna torácica e sua relação com a variação do ângulo de Cobb. MÉTODOS: Os parâmetros estudados foram: a medida do ângulo de Cobb nas radiografias e a posição da medula espinhal, da cavidade pleural e aorta na ressonância nuclear magnética em relação a uma linha de 40mm criada para simular o parafuso pedicular nas cinco vértebras apicais. RESULTADOS: A distância da aorta ao corpo vertebral e o ângulo de segurança do lado convexo apresentaram diferença estatística quando relacionados com a variação do ângulo de Cobb medido. CONCLUSÃO: Os resultados apresentados sugerem maior risco de lesão da artéria aorta com o aumento do ângulo de Cobb e aumento do risco na inserção de parafusos pediculares no lado convexo da curvatura, quando se considera o ângulo de segurança.
