OBJECTIVES. To evaluate the clinical characteristics and co-morbidities associated with development of Acute kidney injury (AKI) after cardiac surgery in patients of a predominantly tertiary care university hospital.METHODS. One hundred and fifty cardiac surgery patients were prospectively studied at the Botucatu School of Medicine, University Hospital- UNEP, using a protocol which started 48h before and finished 48h after surgery, AKI was defined as a 30% increase in baseline serum creatinine.RESULTS. Were expressed as mean +/- standard deviation or median, with a significance level of 5%. Results: There was a slight male predominance (57%); mean age of the studied population, was 56 +/- 14 years. Sixty-five per cent of patients underwent myocardial revascularization. AKI was diagnosed in 34% of cases, Multivariate analysis showed that age over 60 years and peripheral vascular disease were significant associated with AKI developmentCONCLUSION. AKI was a common complication in cardiac surgery patients at this hospital, AKI was associated with age over 60 years and presence of peripheral vascular disease.

O presente trabalho foi desenvolvido no Hospital Maternidade Santa Izabel da cidade de Bauru, São Paulo, Brasil, e teve como objetivo investigar o abortamento na adolescência e compará-lo com o das mulheres adultas. Os indicadores buscaram traçar o perfil epidemiológico das mulheres internadas, por meio de dados coletados em prontuários, entre aquelas com diagnóstico de aborto, independente da forma clínica, entre os anos de 2000 a 2003. O total de abortamentos registrados foi de 2.286, sendo 459 (20,08%) na faixa etária da adolescência e a forma completa foi significativamente maior nas adolescentes quando comparada às das mulheres adultas; segundo o ano de análise, constatou-se decréscimo de ocorrência entre as adolescentes, com maior incidência em 2000 (30,50%), seguida dos anos de 2001 (25,05%), 2002 (23,53%) e 2003 (20,92%), sendo que as adolescentes apresentaram tendência a permanecer dois ou mais dias hospitalizadas. Enfatiza-se a implementação de políticas e programas direcionados à saúde sexual e reprodutiva e suas co-morbidades.

Chagas disease (CD), caused by the protozoan Trypanossoma cruzi, affects approximately 18 million individuals in the Americas, 5 million of which five in Brazil. Most chronic sufferers have either the indeterminate form of the disease, without organic compromise, or the cardiac or digestive forms. Despite the importance of this disease, there is no information on the effect of nutrition on CD evolution. We evaluated the clinical-nutritional profile of individuals with CD treated at the Tropical Diseases Nutrition Out-Patient Clinic of the Botucatu School of Medicine, UNESP.A retrospective cohort study was performed between 2002 and 2006, on 66 patients with serum and parasitological diagnosis of CD. Epidemiological, clinical, nutritional, and biochemical data were collected, including gender, age, skin color, smoking, alcoholism, physical activity, weight, stature, body mass index, abdominal circumference, glycemia, and lipid profile. Fifty-three percent were mate and 47% female; 96% were white skinned. Mean age was 49.6±6.36 years. The predominant form was indeterminate in 71 %; smoking and drinking were recorded in 23% and 17%, respectively. Sedentariness predominated in 83%, and 55% presented increased abdominal circumference. Most, 94%, were overweight or obese. The biochemical exam revealed hyperglycemia in 12% and dyslipidemia in 74%. These findings suggest that the Chagas population presents co-morbidities and risk factors for developing chronic non-transmissible diseases, including cardiovascular diseases, making CD evolution even worse. © 2007 by The Brazilian Journal of Infectious Diseases and Contexto Publishing. Objective: to investigate the quality of life among physically active post-menopausal women with and without a diagnosis of osteoporosis. Methods: a cross-sectional descriptive study was carried out through interview. All the participating women volunteered to take part of this study through previous contact by telephone. The characteristics and objectives of the interview were explained in addition to confirmation of their post-menopausal status. There were two groups of 21 volunteers each: group 1 were non-osteoporotic women (64.38 ± 4.24 years-old) and group 2 were osteoporotic women (67.81 ± 4.19 years-old). Each volunteer was asked to fulfill a preliminary form in order to register personal information, clinical history, co-morbidities and health care. Following, the SF-36 questionnaire was applied. The Wilcoxon rank-sum test was used to assess differences between the two groups. Results: there was a significant difference (p < 0.05) between both groups only with regard to the Role Physical and General Health components of the SF-36 form showing a better performance to group 1. Condusion: post-menopausal women with a diagnosis of osteoporosis that did not sustain a fracture may present a similar quality of life, as compared to non-osteoporotic post-menopausal women.

The Japanese Brazilian population has one of the highest prevalences of diabetes worldwide. Despite being non-obese according to standard definitions, their body fat distribution is typically central. We investigated whether a subset of these subjects had autoantibodies that would suggest a slowly progressive form of type 1 diabetes. A total of 721 Japanese Brazilians (386 men) in the 30- to 60-year age group underwent clinical examination and laboratory procedures, including a 75-g oral glucose tolerance test and determinations of serum autoantibodies. Antibodies to glutamic acid decarboxylase (GADab) were determined by radioimmunoassay and to thyroglobulin (TGab) and thyroperoxidase (TPOab) by flow-cytometry assays. Mean body mass index was 25.2 ± 3.8 kg/m2, but waist circumference was elevated according to the Asian standards. Diabetes, impaired glucose tolerance, and impaired fasting glycemia were found in 31%, 22%, and 22%, respectively, and 53% of the subjects had metabolic syndrome. Glutamic acid decarboxylase (GADab) was positive in 4.72%, TGab in 9.6%, and TPOab in 10% of the whole sample. When participants were stratified according to the presence of thyroid antibodies, similar frequencies of GADab were found in positive and negative groups. The prevalence rates of glucose metabolism disturbances did not differ between GADab positive and negative groups. Our data did not support the view that autoimmune injury could contribute to the high prevalence of diabetes seen in Japanese Brazilians, and the presence of co-morbidities included in the spectrum of metabolic syndrome favors the classification as type 2 diabetes.

A hanseníase é doença infecto-contagiosa crônica causada pelo Mycobacterium leprae. Caracteriza-se por acometimento dermatoneurológico, variando em espectro entre dois polos estáveis (tuberculoide e virchoviano), com formas intermediárias instáveis. Uma classificação operacional, para fins de tratamento, reúne os doentes em dois grupos: paucibacilares (PB) que correspondem a formas clínicas que possuem 1-5 lesões e baciloscopia negativa; multibacilares (MB) que correspondem a formas clínicas com mais de 5 lesões e com ou sem baciloscopia positiva. Apesar de curável, a hanseníase ainda representa relevante problema de saúde pública. Sua maior morbidade associa-se aos estados reacionais e ao acometimento neural que podem causar incapacidades físicas e deformidades permanentes, comprometendo significativamente a qualidade de vida dos pacientes. Consequências clínicas, no que diz respeito as alterações oftalmológicas, endócrinas e cardiovasculares podem advir da etiopatogenia do processo infeccioso e imunopatológico, assim como dos efeitos adversos medicamentosos, desse modo tais eventos necessitam de esclarecimento afim de que o planejamento em saúde possa minimizar tais agravos. Um pronto diagnóstico, possibilita um tratamento precoce e eficaz, evitando com isso alterações clínicas importantes e sequelas. Foi realizado um estudo descritivo do tipo série de casos com 68 pacientes com alta terapêutica da hanseníase maior ou igual a 2 anos e com tratamento dos estados reacionais, tendo como objetivo descrever os aspectos clínicos, epidemiológicos e laboratoriais em pacientes multibacilares após alta com enfoque no diagnóstico de hipertensão, diabetes, osteoporose e discutir possíveis relações com tratamento dos episódios reacionais hansênicos. Observamos que a maioria dos pacientes eram do sexo masculino, com faixa etária acima de 45 anos, procedente de Belém, baixa escolaridade e de baixa renda familiar, o tipo de hanseníase predominante foi a forma virchowiana, a reação reversa foi o estado reacional mais prevalente, o corticoide foi o medicamento mais utilizado para o tratamento nos estados reacionais, os pacientes que não usaram corticoide apresentaram maior percentagem de densitometria normal, as comorbidades diabetes, hipertensão e osteoporose foram mais frequente em pacientes que usaram corticoide e com idade acima de 45 anos. O grau 2 foi o grau de incapacidade mais prevalente.
