Background: An important role for parents and caregivers in the prevention of dental caries in children is the early establishment of health promoting behaviours. This study aimed to examine mothers' views on barriers and facilitators to promoting child and family oral health. Methods: Semi-structured interviews were undertaken with a purposive sample of mothers (n = 32) of young children. Inductive thematic analysis was conducted. Results: Parental knowledge and beliefs, past experiences and child behaviour emerged as major influences on children's oral health. Child temperament and parental time pressures were identified as barriers to good oral health with various strategies reported for dealing with uncooperative children at toothbrushing time. Parental oral health knowledge and beliefs emerged as positive influences on child oral health; however, while most mothers were aware of the common causes of dental caries, very few knew of other risk factors such as bedtime feeding. Parents' own oral health experiences were also seen to positively influence child oral health, regardless of whether these were positive or negative experiences. Conclusions: Understanding parental oral health beliefs is essential to overcoming barriers and promoting enablers for good child oral health. Improving child oral health also requires consideration of child behaviour, family influences, and increasing awareness of lesser-known influencing factors.

Os implantes osteointegráveis assumiram condição prioritária na reabilitação da perda dentária unitária ou múltipla em função das elevadas taxas de sucesso e previsibilidade no tratamento e vêm sendo cada vez mais utilizados por especialistas e clínicos. Atualmente existe a preocupação com a manutenção dos tecidos moles periimplantares, principalmente em áreas estéticas. De modo geral, um ano após a instalação dos implantes osteointegráveis ocorre uma perda óssea proximal de 1,5 mm e em média 0,1 mm durante os anos subsequentes. Nos últimos anos, achados clínicos evidenciaram menor perda óssea inicial associada a intermediários de diâmetro reduzido em relação à plataforma dos implantes. Com o objetivo de comparar, por meio de imagens radiográficas o comportamento ósseo proximal ao redor de implantes osteointegráveis com plataformas convencionais e plataformas de diâmetro intermediário reduzido, foi estabelecido o seguinte desenho de estudo clínico prospectivo: em 08 pacientes totalmente edentados, foram instalados 40 implantes, 5 implantes mandibulares por paciente. Cada paciente recebeu 3 implantes com plataforma convencional e 2 com plataforma associada aos intermediários de diâmetro reduzido (cone morse). Foram confeccionadas próteses em resina acrílica e fixadas precocemente aos implantes por intermédio de parafusos, seguindo o modelo protocolo Bränemark. Foram feitas radiografias periapicais padronizadas em intervalos de 21 dias, 3, 6 e 12 meses, após a instalação dos implantes. As imagens radiográficas foram digitalizadas e realizada a subtração radiográfica digital pelo programa emago, sendo comparadas com a radiografia inicial. Os resultados obtidos neste estudo mostraram uma regularidade no remodelamento ósseo ao longo do tempo para todos os implantes, não tendo sido encontradas diferenças significativas entre os diferentes implantes analisados.

目的 探讨颅颌运动仿真系统的骀接触模拟精度,为该仿真系统的应用提供依据.方法 制作10副石膏模型并上(牙合)架.用(牙合)架模拟侧方运动,三维扫描侧方运动终点(牙合)架上的上下颌模型,重建数字化上下颌模型作为对照组.运用仿真系统模拟耠架的侧方运动,以仿真系统输出的侧方运动终点的数字化上下颌模型为试验组.通过比较试验组与对照组下颌牙列之间的位置差异评价仿真系统的(牙合)接触模拟精度.结果 仿真系统模拟的下颌牙列与对照组下颌牙列之间的绝对平均距离为(0.18±0.05)mm;在前后左右四个分区中,两组右后牙区之间的绝对平均距离最大,为(0.19±0.07)mm.结论 该仿真系统的体外胎接触模拟精度为0.19mm.

The role of microorganisms in the development and maintenance of pulpal and periapical inflammation have been well documented. The success of root canal treatment largely depends on the elimination of microbial contamination from the root canal system. Although mechanical instrumentation of root canals can reduce bacterial population, effective elimination of bacteria cannot be achieved without the use of antimicrobial root canal irrigation and medication. This review will discuss the antimicrobial effects of the known root canal irrigants and medicaments and explore future developments in the field. © 2007 Mosby, Inc. All rights reserved.

Human Papillomavirus (HPV) related oropharyngeal squamous cell carcinomas (OPSCCs) are reported to have improved prognosis and survival in comparison to other head and neck squamous cell cancers (HNSCCs). This systematic review and meta-analysis examines survival differences in HPV-positive HNSCC and OPSCC subtypes including tonsillar carcinoma in studies not previously investigated. Four electronic databases were searched from their inception till April 2011. A random effects meta-analysis was used to pool study estimates evaluating disease-specific (death from HNSCC), overall (all-cause mortality), progression-free and disease-free (recurrence free) survival outcomes in HPV-positive vs. HPV-negative HNSCCs. All statistical tests were two-sided. Forty-two studies were included. Patients with HPV-positive HNSCC had a 54% better overall survival compared to HPV-negative patients HR 0.46 (95% CI 0.37-0.57); the pooled HR for tonsillar cancer and OPSCC was 0.50 (95% CI 0.33-0.77) and HR 0.47 (95% CI 0.35-0.62) respectively. The pooled HR for disease specific survival was 0.28 (95% CI 0.19-0.40); similar effect sizes were found irrespective of the adjustment for confounders, HPV detection methods or study location. Both progression-free survival and disease-free survival were significantly improved in HPV-positive HNSCCs. HPV-positive HNSCCs and OPSCCs patients have a significantly lower disease specific mortality and are less likely to experience progression or recurrence of their cancer than HPV-negative patients; findings which have connotations for treatment selection in these patients.
