Objetivo: estudar os efeitos hemodinâmicos da solução salina hipertônica/dextran, comparada com solução salina normal, em pacientes com sepse grave. Modelo: ensaio clínico randomizado, prospectivo, duplo-cego, controlado. Local: Unidade de Terapia Intensiva de um hospital universitário. Pacientes: 29 pacientes com sepse grave, admitidos na UTI com pressão de oclusão da artéria pulmonar (POAP) menor que 12 mmHg. Intervenções: os pacientes foram randomizados para receber 250 ml da solução salina normal [NaCl 0,9%] (Grupo SS, n=16) ou solução salina hipertônica [NaCl 7,5%]/dextran 70 8% ( Grupo SSH, n=13). Medidas e resultados: para cada grupo foram coletadas medidas hemodinâmicas, gasometrias (arterial e venosa), lactato e sódio séricos nos tempos 0, 30 minutos, 60 minutos, 120 minutos e 180 minutos. Durante o período do estudo não foi permitida qualquer alteração na infusão tanto de fluidos quanto das drogas vasopressoras. A POAP foi maior no grupo SSH, com a diferença sendo maior em 30 minutos (10,7±3,2 mmHg vs. 6,8±3,2 mmHg) e 60 minutos (10,3±3 mmHg vs. 7,4±2,9 mmHg); p<0,05. O índice cardíaco aumentou apenas no grupo SSH, sendo que as diferenças foram maiores em 30 minutos (6,5±4,7 l min-1 m-2 vs. 3,8±3,4 l min-1 m-2), em 60 minutos (4,9±4,5 l min-1 m-2 vs. 3,7±3,3 l min-1 m-2) e em 120 minutos (5,0±4,3 l min-1 m-2 vs. 4,1±3,4 l min-1 m-2); p<0,05. O índice sistólico seguiu o mesmo padrão e foi maior em 30 minutos (53,6[39,2-62,8] ml m-2 vs. 35,6[31,2-49,2] ml m-2) e em 60 minutos (46,8[39,7-56,6] ml m-2 vs. 33,9[32,2-47,7] ml m-2); p<0,05. A resistência vascular sistêmica diminuiu no grupo SSH e foi menor nos tempos 30 minutos (824±277 dyne s-1 cm-5 m-2 vs. 1139±245 dyne s-1 cm-5 m-2), em 60 minutos (921±256 dyne s-1 cm-5 m-2 vs. 1246±308 dyne s-1 cm-5 m-2) e em 120 minutos (925±226 dyne s-1 cm-5 m-2 vs. 1269±494 dyne s-1 cm-5 m-2); p<0,05. O sódio sérico aumentou no grupo SSH e foi maior do que o grupo SS em 30 minutos (145±3 mEq l-1 vs. 137±7 mEq l-1), em 60 minutos (143±4 mEq l-1 vs. 136±77 mEq l-1), em 120 minutos (142±5 mEq l-1vs. 136±7 mEq l-1) e em 180 minutos (142±5 mEq l-1 vs. 136±87 mEq l-1); p<0,05. Conclusão: Solução salina hipertônica/dextran pode melhorar a performance cardiovascular na ressuscitação de pacientes com sepse grave. Os efeitos hemodinâmicos parecem estar relacionados tanto ao efeito no volume quanto a melhora da função cardíaca. A SSH/dextran podem ajudar a restaurar rapidamente a estabilidade hemodinâmica em pacientes sépticos, hipovolêmicos, sem apresentar efeitos indesejáveis significativos.

Radiobiocomplexes are used to obtain images in nuclear medicine and employed in basic research. Blood constituents labeled with technetium-99m (99mTc) have also been employed as radiobiocomplexes and used also experimental model for evaluation of the biological effects of natural or synthetic drugs. The analysis of the morphology and the morphometrics parameters (perimeter/área ratio) can be used to evaluate the effects of drugs upon the structure of the membrane of red blood cells. Cinnamomum zeylanicum (cinnamon) is a spice used as herbal medicine to treat diseases. The aim of this study was to evaluate the effect of in vitro and in vivo treatment with an aqueous cinnamon extract on the labeling of blood constituents with 99mTc and on the morphology of red blood cells from Wistar rats. In the in vitro treatment, isolated blood sample from animals were incubated with cinnamon extract. In the in vivo treatment, blood samples were also withdrawn from animals treated with cinnamon extract. In both cases, the radiolabeling of blood constituents was done. The morphological analysis of red blood cells was also done. As control, blood or animals treated with NaCl 0.9%. The data obtained on the labeling of blood constituents with 99mTc experiments indicated that the in vitro treatment with cinnamon extract was capable to decrease signiicantly (p<0.05) the percentage of radioactivity in cellular compartments and on the fixation of cellular and plasma proteins. These effects were not observed on the in vivo treatment. The results obtained for the morphology of red blood cells suggest that the in vitro and in vivo treatments did not alter the morphology and the perimeter/area ratio. The in vitro treatment with aqueous cinnamon extract could affect the membrane structures related with the oxidation status of the stannous ion pertechnetate ion, altering the labeling of blood constituentes with 99mTc. This study was a multidisciplinary experimental research. Effects of a Cordia salicifolia (porangaba) extract on the labeling of blood cells (BCs) with technetium-99m ((99m)Tc) and on the morphology of red BCs were evaluated. Labeling of cellular and molecular structures with (99m)Tc depends on a reducing agent. Some physical characteristics, as visible absorbance spectrum, electric conductivity, and refractive index of this porangaba extract, were also determined. Blood samples from Wistar rats were incubated with porangaba extract or with 0.9% NaCl (control). Labeling of blood constituents with (99m)Tc was performed. Plasma (P) and BCs, both soluble (SF-P and SF-BC) and insoluble (IF-P and IF-BC) fractions, were separated. The radioactivity in each fraction was counted, and the percentage of radioactivity incorporated (%ATI) was calculated. Blood smears were prepared, fixed, and stained, and the morphology of the red BCs was evaluated. Data showed an absorbance peak at 480 nm and electric conductibility and refractive index concentration-dependent. Porangaba extract decreased significantly (P < .05) the BC, IF-P, and IF-BC %ATI, and no modifications were verified on the shape of red BCs. Analysis of the results reveals that some physical parameters could be useful to aid in characterizing the extract studied. Moreover, it is possible that chemical compounds of this extract could have chelating/redox actions or be capable of binding to plasma and/or cellular proteins

Clinical evaluations have been made possible with radiobiocomplexes marked with tecnecium-99m (99mTc). Natural or synthetic drugs are able to interfere in the marking of blood structures with 99m Tc. Also, the toxicity of several natural products has been described. The aim of this study was evaluating the effect of an extract of Ganoderma lucidum (Reishi) in the marking of blood constituents with 98m Tc and in the survival of Escherichia coli. Blood samples from Wistar rats were treated with reishi extract. Radiomarking procedure was performed. Samples of plasma (P), blood cells (CS), and insoluble (FI) and soluble (FS) fractions of P and CS were separated and the radioactivity was counted to determine radioactivity percentages (%ATI). Escherichia coli AB1157 cultures were treated with stannous chloride in the presence and absence of the reishi extract. Blood samples and bacterial cultures treated with NaCl 0.9% were used as controls. Data indicated that the reishi extract has significantly altered (p<0,05) the %ATI of P, CS, FI-P, FS-P, FI-CS e FS-CS, as well as it has increased survival of bacterial cultures treated with stannous chloride. Our results suggest that the Reishi extract would be able to present a redox/ chelant action by altering blood constituent marking with 99mTc and by protecting bacterial cultures against stannous chloride-induced oxydating lesions. Quinze eqüinos machos, da raça Mangalarga, com idades entre dois e três anos, foram utilizados para se avaliar os possíveis efeitos clínicos benéficos da administração de dexametasona ou diclofenaco sódico durante a endotoxemia experimental em eqüinos. Os animais foram divididos em três grupos de cinco animais cada: controle (C), diclofenaco sódico (SD) e dexametasona (DM). Todos os grupos receberam 0,1µg/kg de lipopolissarídeo de Escherichia coli 055:B5, durante 15 minutos, por via intravenosa mais: grupo SD - 2,2mg/kg de SD, por via oral, 60 minutos antes da infusão da endotoxina; grupo DM - 1,1mg/kg, por via intravenosa, 30 minutos antes da endotoxina; grupo C - 20ml de NaCl 0,9%, por via intravenosa, 30 minutos antes da endotoxina. O SD não preveniu a leucopenia, neutropenia e linfopenia ocorridas três horas após a indução da endotoxemia, porém a DM atenuou essas alterações. As taxas de proteínas plasmática e peritoneal, a concentração de glicose e de fósforo inorgânico e a contagem de células nucleadas totais peritoneais mantiveram-se inalteradas. O diclofenaco foi eficaz na prevenção da febre e alterações nos borborigmos intestinais enquanto que a dexametasona bloqueou as alterações no número de células inflamatórias em relação ao grupo controle.

Two alkaloids, erysodine (1) and erysothrine (2) were isolated from the flowers of a Pakistani medicinal plant, Erythrina suberosa. These compounds were investigated for anxiolytic properties, and the results showed significant effect, in an acute oral treatment with 1-2, which were suspended in saline (NaCl 0.9%) plus DMSO 1%, and evaluated in 122 Swiss male mice exposed to two tests of anxiety - the elevated plus-maze (EPM) and the light/dark transition model (LDTM).

To study the effects of diclofenac, a nonselective nonsteroidal anti-inflammatory drug (NSAID), on lipid profile, oxidized low-density-lipoprotein (Ox-LDL), serum antioxidant defenses and markers of oxidative stress, male Wistar rats were divided into two groups (n = 10): (C) receiving intramuscularly a single daily dose of saline (NaCl 0.9%), and (AI) receiving intramuscularly a single daily dose of 10 mg/kg diclofenac sodium (C14H10C12NNaO2). After 28 days diclofenac-treated rats had lower Ox-LDL, apoprotein B (apo-B), apo-B/LDL-cholesterol and lipid hydroperoxide than C. Total antioxidant substances and superoxide dismutase were increased in diclofenac-treated rats, while no significant changes were observed in catalase, glutathione peroxidase and nitric oxide. A perincubation test done to examine the possibility of mechanism-based activation showed that diclofenac had no effect on maximal superoxide dismutase velocity, but significantly reduced the Michaelis-Menten (K-M) constant, indicating that diclofenac induced SOD activation increasing substrate linkage affinity to the enzyme-catalytic site. In conclusion, diclofenac had beneficial effects decreasing Ox-LDL and improving antioxidant defense. It appears that the application of this agent may be feasible and beneficial for serum antioxidant protection, which certainly would bring new insights on dyslipidemia control. (C) 2008 Elsevier B.V. A dopamina possui um amplo espectro de ação no sistema urinário. Aumento da taxa de filtração glomerular, do fluxo sanguíneo renal e da excreção fracionada de sódio e fósforo é um efeito renal esperado em indivíduos normais. Este estudo foi realizado com o propósito de testar a hipótese de que a dopamina é capaz de aumentar a excreção fracionada de fósforo em cães nefropatas. Cinco cães sadios e quatro cães nefropatas, com doença predominantemente túbulo-intersticial, foram submetidos à infusão de solução controle (NaCl 0,9%) e solução de dopamina em duas taxas de infusão diferentes (1µg kg-1 min-1 e 3µg kg-1 min-1), sendo realizadas avaliações antes, durante e 30 minutos após a infusão. Os cães sadios apresentaram aumento significativo (P≤0,05) na excreção fracionada e excreção renal de fósforo durante a infusão de 3µg kg-1 min-1, porém a concentração sérica permaneceu sem alterações durante o tratamento. Já os cães nefropatas apresentaram aumento significativo (P≤0,05) na excreção fracionada e excreção renal de fósforo, tanto na dose de 1µg kg-1 min-1, como na de 3µg kg-1 min-1. Além disso, após a infusão de 1µg kg-1min-1, a concentração sérica de fósforo apresentou redução significativa. Os resultados são indicativos de que a dopamina nas doses de 1µg kg-1 min-1 e 3µg kg-1 min-1 podem ser incluídas na terapia de cães nefropatas para melhorar a homeostase de fosfato.

Objective To compare the effect of intraperitoneal (IP) or incisional (INC) bupivacaine on pain and the analgesic requirement after ovariohysterectomy in dogs.Study design Prospective, randomized clinical study.Animals Thirty female dogs undergoing ovariohysterectomy (OHE).Methods Dogs admitted for elective OHE were anesthetized with acepromazine, butorphanol, thiopental and halothane. Animals were randomly assigned to one of three groups (n = 10 per group). The treatments consisted of preincisional infiltration with saline solution (NaCl 0.9%) or bupivacaine with epinephrine and/or IP administration of the same solutions, as follows: INC and IP 0.9% NaCl (control group); INC 0.9% NaCl and IP bupivacaine (5 mg kg(-1), IP group); INC bupivacaine (1 mg kg(-1)) and IP 0.9% NaCl (INC group). Postoperative pain was evaluated by a blinded observer for 24 hours after extubation by means of a visual analog scale (VAS) and a numeric rating scale (NRS). Rescue analgesia (morphine, 0.5 mg kg(-1), IM) was administered if the VAS was > 5/10 or the NRS > 10/29.Results At 1 hour after anesthesia, VAS pain scores were [medians (interquartile range)]: 6.4 (3.1-7.9), 0.3 (0.0-2.6) and 0.0 (0.0-7.0) in control, IP and INC groups, respectively. VAS pain scores were lower in the IP compared to the control group. Over the first 24 hours, rescue analgesia was administered to 7/10, 5/10 and 3/10 dogs of the control, INC and IP groups, respectively. Total number of dogs given rescue analgesia over the first 24 hours did not differ significantly among groups.Conclusions and clinical relevance Intraperitoneal bupivacaine resulted in lower pain scores during the first hour of the postoperative period and there was a trend towards a decreased need for rescue analgesia after OHE in dogs.
